Do chronické pankreatitidy nepřechází pouze akutní pankreatitida alkoholická, ale nalezli jsme tento přechod i u pankreatitidy biliární etiologie., Introduction: One of the diseases involving a potential risk of developing chronic pancreatitis is acute pancreatitis. Material: Of the overall number of 231 individuals followed with a diagnosis of chronic pancreatitis, 56 patients were initially treated for acute pancreatitis (24.2 %). Within an interval of 12- 24 months from the first attack of acute pancreatitis, their condition gradually progressed to reached the picture of chronic pancreatitis. The individuals included in the study abstained (from alcohol) following the first attack of acute pancreatitis and no relapse of acute pancreatitis was proven during the period of their monitoring. Results: The etiology of acute pancreatitis identified alcohol as the predominant cause (55.3 %), biliary etiology was proven in 35.7 %. According to the revised Atlanta classification, severe pancreatitis was established in 69.6 % of the patients, the others met the criterion for intermediate form, those with the light form were not included. Conclusion: Significant risk factors present among the patients were smoking, obesity and 18 %, resp. 25.8 % had pancreatogenous diabetes mellitus identified. 88.1 % of the patients with acute pancreatitis were smokers. The majority of individuals with chronic pancreatitis following an attack of acute pancreatitis were of a productive age from 25 to 50 years. It is not only acute alcoholic pancreatitis which evolves into chronic pancreatitis, we have also identified this transition for pancreatitis of biliary etiology., and Martina Bojková, Petr Dítě, Magdalena Uvírová, Nina Dvořáčková, Bohuslav Kianička, Tomáš Kupka, Pavel Svoboda, Pavel Klvaňa, Arnošt Martínek
Popisujeme případ onemocnění 25letého muže s nespecifickými příznaky a hepatomegalií, u kterého jsme diagnózu stanovili na základě výsledků zobrazovacích metod a laboratorních vyšetření. Vzhledem k inoperabilitě je pacient v současnosti dispenzarizován infektologem, dlouhodobě léčen albendazolem a je klinicky stabilní, zařazen v čekacím listu k transplantaci jater., Alveolar echinococcosis is a rare parasitic disease, especially of liver, caused by larval stage of tapeworm Echinococcus multilocularis. At the end of the last century France, Germany, Austria and Switzerland were the most often regions with this disease, these days is this infection diagnosed also in our territory. We describe the case of the disease of the twenty-five years old male with nonspecific signs and hepatomegaly, who was diagnosed on the basis of imaging and laboratory sampling. Due to inoperability the patient is now in infectologist follow-up on a long-term treatment with albendazole. He is clinically stable, included in waiting list for liver transplantation., and Tomáš Kupka, Peter Baľa, Lubomíra Hozáková, Jaroslav Havelka, Martina Bojková, Bohdana Břegová, Arnošt Martínek, Petr Dítě
Metody: U 76 pacientů s Crohnovou chorobou a minimální délkou trvání nemoci 5 let byly testovány tři sekvenční varianty NOD2/CARD15 genu (R702W, G908R, 3020insC). Fenotyp onemocnění signifikantně nesouvisel s přítomností vyšetřovaných sekvenčních variant., Introduction: Three NOD2/CARD15 gene variants (3020insC, R702W, G908R) have been identified as genetic risk factors for Crohn’s disease patients. However the diagnostic and therapeutic relevance for clinical practice remains limited. The aim of this study was to evaluate the association between these variants, the risk of reoperation and disease phenotype. Methods: In 76 Crohn’s disease patients (41 female, 35 male) with a minimum 5 year follow-up, three polymorphisms of the NOD2/CARD15 gene (R702W, G908R, 3020insC) were tested. Detailed clinical and medical history including surgical procedures and reoperations were obtained by reviewing the medical charts and completed prospectively. Association between the need for reoperation, disease phenotypes and gene variants were analyzed. Results: 24 patients (32%) showed at least one NOD2/CARD15 mutation. 25 patients (33%) required reoperation, 51 (67%) represented the control group. The expected trend that patients with NOD2/CARD15 variants have a higher frequency of reoperations was not confirmed to a level of statistical significance (p=0.2688). Two of the four patients (50%) with the 3020insC variant required further surgery. We did not confirm any association between NOD2/CARD15 mutations and age at diagnosis (p=0.4356), behavior (p=0.6610), or localization (p=0.4747) according to the Montreal classification. Conclusion: NOD2/CARD15 polymorphisms did not significantly affect the reoperation rate. Homozygosity for the 3020insC variant in the NOD2/CARD15 gene is associated with a high risk of reoperation. NOD2/CARD15 gene variants are not significantly associated with specific disease phenotypes., and L. Martínek, T. Kupka, J. Šimová, P. Klvaňa, M. Bojková, M. Uvírová, P. Dítě, J. Dvořáčková, J.
