Přehledové sdělení se věnuje diagnostickému zobrazení akutních aortálních syndromů s důrazem na multidetektorovou výpočetní tomografii a na její význam pro volbu léčby. Diskutovány jsou jednak techniky CT zobrazení včetně CT s EKG synchronizací a dále diagnostické nálezy u disekce aorty, intramurálního hematomu a penetrujícího vředu aorty. Nádory měkkých tkání jsou rozsáhlou a značně heterogenní skupinou onemocnění vyžadující multimodalitní diagnostický přístup. Magnetická rezonance se stala díky vysoké senzitivitě a kontrastnímu rozlišení nejefektivnější metodou při jejich detekci a lokálním stagingu. Její možnosti při stanovení histologického typu nádoru a často i při rozhodnutí, zda je leze benigní či maligní jsou značně omezené. Z tohoto důvodu musí být až na výjimky kombinována s histologickým vyšetřením, které je rovněž nezbytné pro kompletní staging. Článek se zabývá technikou vyšetřování nádorů měkkých tkání pomocí magnetické rezonance a využitím metody při stagingu a diferenciální diagnóze. Krátce jsou zmíněny i možnosti dalších zobrazovacích modalit., Soft tissue tumors are large and heterogenous group of diseases requiring multimodality diagnostic approach. Magnetic resonance is, thanks to its high sensitivity and contrast resolution, the most effective method for their detection and local staging. Its possibilities in determining the histological type of tumor and in the decision whether the lesion is benign or malignant are quite limited. For this reason it MRI must be, with few exceptions, combined with histological examination, which is also necessary for complete staging. This article deals with MRI examination technique and the use of this method for staging and differential diagnosis. Přehledové sdělení podává souhrn současných postupů v zobrazovací diagnostice neurodegenerativních onemocnění, která se projevují kognitivním deficitem. Základním postupem je zobrazení magnetickou rezonancí, která zahrnuje zobrazení pomocí T2-vážených sekvencí včetně zobrazení roviny kolmé na dlouhou osu spánkového laloku a izotropní zobrazení T1-váženými obrazy gradientního echa. K hodnocení kortikální atrofie je používáno mediotemporální skóre atrofie (MTA, Scheltens) a skóre zadní atrofie (PA, Koedam), posouzení vlivu ischemie na vznik kognitivního deficitu pomáhá určit Fazekasovo skóre. Hybridní zobrazení pomocí PET/MR dovoluje současně hodnotit morfologické změny zejména v temporálním laloku, ale také při použití 18F-FDG typické snížení úrovně oxidativní glykolýzy v temporálních lalocích a temporoparietálně. Cílenou metodikou v diferenciaci Alzheimerovy choroby je použití radiofarmaka, které se specificky váže na amyloid beta v mozkové tkáni, prokázání akumulace radiofarmaka florbetabenu nebo flutemetamolu v šedé hmotě významně přispívá ke stanovení vysoké pravděpodobnost diagnózy Alzheimerovy choroby. V diferenciální diagnostice některých tzv. Parkinson plus syndromů, jakými je například demence s Lewyho tělísky, se může uplatnit i průkaz deficitu dopaminergního systému pomocí podání 18F-dihydroxyfenylalaninu (FDOPA). Magnetic resonance imaging plays an essential role, it is including the T2 weighted imaging in the planes perpendicular to the longitudinal axes of the temporal lobes, and isotropic T1 weighted images with isotropic resolution. In the assessment of the cortical atrophy, the mediotemporal atrophy score (MTA, Scheltens) and posterior atrophy score (PA, Koedam) are used. Fazekas score assesses the hypoxemic-ischemic influences. When 18F-FDG is used hybrid imaging PET/MRI enables to evaluate the morphological changes, especially within the temporal lobe, with the typical changes of decreased oxidative glycolysis in the temporal lobe ant in the temporoparietal cortex. Targeted method of the Alzheimer disease differentiation is the application of the radiopharmaceutical with the ability to be bind to the amyloid beta. Confirmation of the accumulation such as molecule - florbetaben or flutemetamol to the amyloid beta within the gray matter supports the setting of the highest probability of Alzheimer disease diagnose. In the differential diagnosis of some so-called Parkinson plus syndromes, such as dementia with Lewy bodies, could play the role also the application of 18F-dihydroxyphenylalanine (FDOPA). Renální karcinom jako nejvýznamnější solidní nádor ledvin dosahuje v České republice nejvyšší incidence v celosvětovém měřítku. Odlišit jej mezi ostatními solidními a cystoidními lézemi je nejdůležitější úlohou zobrazovacích metod. Základní metodou je ultrasonografie, kterou je možné rozšířit pomocí aplikace kontrastní látky na metodu vysoce spolehlivou v diferenciální diagnostice solidních útvarů. K předoperačnímu zobrazení nádorů ledvin je nezbytné zobrazení cévního zásobení ledviny i vlastního tumoru pomocí CT angiografie nebo MR angiografie. Magnetickou rezonancí lze blíže specifikovat především hypovaskularizované nádory, u kterých se difuzní vážení ukazuje jako velmi důležité při odlišení papilárních renálních karcinomů. PET/CT s podáním fluorodeoxyglukózy je významnou metodou umožňující na základě míry akumulace fluorodeoxyglukózy posoudit agresivity renálního karcinomu., Renal cell carcinoma reaches the highest incidence in the Czech Republic world wide. The most important role of the imaging methods is to distinguish it in between the all solid and cyst-like renal lesions. Even the ultrasound (sometimes including application of contrast material) remains the basic imaging method, CT-angiography and MR-angiography is the most important recent methods to display its own tumorous tissue including the vessels supplying the tumor and the whole kidney. Magnetic resonance is able to improve the differential diagnosis in hypo-vascular (especially papillary renal cell carcinoma) when the diffusion weighted imaging is used.
