Cíl: Cílem studie bylo zjistit spokojenost pacientů s poskytovanou ošetřovatelskou péčí na vybraných odděleních dvou nemocnic Moravskoslezského kraje. Porovnat spokojenost pacientů podle zdravotnického zařízení a oddělení, kde byli hospitalizováni. Zjistit, zda existuje souvislost mezi demografi ckými a klinickými charakteristikami pacientů a jejich spokojeností s ošetřovatelskou péčí. Metodika: Zkoumaný soubor deskriptivní korelační studie tvořilo 507 pacientů hospitalizovaných v období od února do března 2011 na 13 lůžkových odděleních v nemocnicích Karviná-Ráj a Orlová. Ke sběru dat byl použit standardizovaný dotazník Patient Satisfaction Scale (PSS), který měří spokojenost pacientů v jedenácti položkách třech oblastí – technické/racionální aspekty, podávání informací a oblast interakce a podpory. Výsledky: Pacienti byli nejvíce spokojeni s technickými aspekty ošetřovatelské péče a nejméně s poskytováním informací. Demografi cké charakteristiky pacientů na jejich spokojenost zásadní vliv neměly. Pouze v jedné nemocnici (NsP Karviná) byli muži ve všech faktorech spokojenější než ženy a naopak nejmenší spokojenost udávali pacienti, kteří popisovali svůj zdravotní stav jako uspokojivý. V NsP Orlová udávali vyšší spokojenost s podporou a interakcí pacienti hospitalizovaní delší dobu. Závěr: Výsledky studie neprokázaly statisticky významné rozdíly v hodnocení spokojenosti pacientů mezi nemocnicemi ani mezi sledovanými odděleními., Aims: The aim of this study was to determine patient satisfaction with nursing care provided in selected wards of two hospitals of Moravian-Silesian region, to determine whether there is a connection between demographic and clinical characteristics of patients and their satisfaction and to compare patient satisfaction according to health facility and department, where they were hospitalized. Methods: The sample of descriptive correlation study consisted of 507 patients hospitalized in the period from February to March 2011 on 13 inpatient wards in hospitals Karvina-Raj and Orlova. Data were collected using a standardized questionnaire Patient Satisfaction Scale (PSS), which measures patient satisfaction in eleven items of three areas – technical/rational aspects, provision of information and area of interaction and support. Results: Patients were most satisfi ed with the technical aspects of nursing care and least with provision of information. Demographic characteristics of the patients did not have a major impact on their satisfaction. Only in one hospital (NsP Karvina) were men in all factors happier than women and on the contrary the smallest satisfaction reported patients who described their health status as satisfactory. In NsP Orlova they reported higher satisfaction with the support and interaction patients hospitalized longer period of time. Cíl: Cílem studie bylo zjistit potencionální riziko hypertenze u dětí ve věku 14 – 15 let dvojím standardním měřením krevního tlaku a zmapovat moţný vztah mezi hodnotami krevního tlaku dětí a jejich vybranými sociodemografickými faktory a tělesnými parametry. Metodika: Výzkumný soubor tvořilo 143 ţáků 8. a 9. tříd základních škol v Ostravě zapojených do projektu Zdravá škola. Data byla získána měřením fyziologických funkcí a antropometrických dat a doplňujícími otázkami na vybrané rizikové faktory kardiovaskulárních chorob. Výsledky: Měření krevního tlaku (TK) proběhla opakovaně, při druhém měření byla prokázána odlišnost naměřených hodnot TK mezi pohlavími, kdy u chlapců byly hodnoty TK vyšší neţ u děvčat. Bylo potvrzeno, ţe na hodnoty TK má vliv pohybová aktivita, a to zejména u dětí s vysokými hodnotami BMI. U normálních hodnot BMI korelace mezi hodnotou krevního tlaku a pohybovou aktivitou nebyla statisticky významná. Z rizikové skupiny 14 dětí s rodinnou predispozicí hypertenze bylo evidováno 29 % dětí s průměrnou hodnotou TK v pásmu pro 95. percentil (hypertenzní hodnoty) vzhledem k danému věku, pohlaví a výšce. Závěr: Výskyt vysokého TK byl četnější u dětí s nadváhou a obezitou. Pohybová aktivita ovlivňovala hodnotu krevního tlaku, zvláště pak právě u dětí s nadváhou a obezitou, kdy hodnoty TK u těchto dětí, které necvičily, byly vyšší neţ u dětí cvičících. Pravidelným měřením a správným zhodnocením TK můţe sestra včas zachytit děti a adolescenty s rizikovými parametry., Aim: The aim of the thesis is to examine potential risk of hypertension in children aged 14 - 15 years by two standard measurements of blood pressure and to find out possible relation between values of blood pressure in children and their selected socio-demographic factors and physical parameters. Methods: The research was carried out on 143 pupils from elementary schools in Ostrava, which participate in the project „Zdravá škola“(Healthy School). The data were obtained from measuring of physiological functions, survey of anthropometric data and further questions focused on particular risk factors for cardiovascular diseases. Results: The second measurement revealed a difference in values of blood pressure between sexes, when boys had higher blood pressure than girls. The research confirmed that blood pressure is influenced by physical activity, especially in children with high value of BMI. In children with lower values of BMI, the correlation between the value of blood pressure and physical activity was not statistically relevant. From a high-risk group of 14 children with genetic predisposition, 29% of children had average value of blood pressure in range of 95. percentile (hypertension) given age, sex and height. Conclusion: The occurrence of high blood pressure is common in children who suffer from overweight and obesity. The value of blood pressure is influenced by physical activity, especially in overweight and obese children, when value of blood pressure in the children who did not practice, were higher in children than exercisers. Regular check-ups and proper evaluation of measured blood pressure in children and adolescents can contribute to early revelation of children and adolescents with high-risk parameters of blood pressure., Alena Petzuchová, Darja Jarošová, and Literatura 16
Cíl: Cílem studie bylo ověřit využitelnost ošetřovatelských intervencí NIC v ošetřovatelské praxi domácí péče a porovnat záznam aktuálních ošetřovatelských intervencí evidovaných v ošetřovatelské dokumentaci agentury domácí péče s navrhovaným souborem ošetřovatelských intervencí NIC. Metodika: Pilotní soubor o velikosti 110 ošetřovatelských intervencí a 2150 ošetřovatelských aktivit byl sestaven podle taxonomie NIC (Nursing Intervention Classification), jeho ověřování proběhlo u 30 klientů agentury domácí péče v roce 2011. Analýzou ošetřovatelské dokumentace 200 klientů stejné agentury byly pak sledovány reálné intervence a aktivity sester zaznamenané v ošetřovatelském plánu. Výsledky: Z pilotního souboru intervencí NIC využily sestry celkem 39 ošetřovatelských intervencí pro 221 potvrzených ošetřovatelských diagnóz NANDA-I. Nejvíce intervencí (73 %) bylo indikováno pro ošetřovatelské diagnózy Zhoršená pohyblivost, Riziko infekce, Riziko deficitu tělesných tekutin a Riziko porušení kožní integrity. Mezi nejčetnější ošetřovatelské intervence, které sestry ze souboru NIC vybíraly, patřily: Hospodaření s energií, Prevence pádu, Péče o ránu, Učení: předepsané činnosti/cvičení a Podpora cvičení. Ošetřovatelské činnosti reálně vykonávané sestrami u klientů ADP a zaznamenané v ošetřovatelském plánu (44 různých ošetřovatelských intervencí, 1406 provedených výkonů) se lišily od ošetřovatelských intervencí, které byly potvrzeny při pilotním ověřování. Pojmenování ošetřovatelských intervencí sestrami agentury domácí péče nereflektovalo terminologii NIC, proto nebyla data vzájemně porovnávána. Závěr: Používání jednotné ošetřovatelské terminologie je důležité pro přesnou a jednoznačnou komunikaci ošetřovatelských profesionálů, pro popis a evidenci práce sester a doložitelné vykazování ošetřovatelských činností., Aim: The aim of this study was to verify the utilization of nursing interventions NIC in home care and to compare the report of current nursing interventions recorded in nursing documentation in home care agency with proposed set of nursing interventions NIC. Methods: The sample of 110 nursing interventions and 2150 nursing activities was compiled according to taxonomy NIC (Nursing Intervention Classification), its verification was carried out in 30 home care agency clients in 2011. Using the analysis of nursing documentation in 200 clients of the same agency, were then monitored real interventions and activities by nurses recorded in nursing care plan. Results: From the pilot set of NIC interventions nurses have used 39 of nursing interventions for 221 confirmed nursing diagnoses NANDA-I. The most of interventions (73%) were indicated for nursing diagnoses: Impaired physical mobility, Risk for infection, Risk for deficient fluid volume and Risk for impaired skin integrity. Among the most frequent nursing interventions, which nurses have selected from NIC file, were: Energy management, Fall prevention, Wound care, Teaching: prescribed activities/exercise. Nursing activities actually performed by nurses in home care clients and recorded in nursing care plan (44 different nursing interventions, 1406 performed procedures) differed from nursing interventions that were confirmed during the pilot verification. Naming of nursing interventions by the nurses in home care agency did not reflect the NIC terminology, thus the data were not mutually compared. Conclusion: The use of uniform nursing terminology is essential for accurate and unambiguous communication of nursing professionals, for description and record of the work of nurses and for provable reporting of nursing activities., Darja Jarošová, Lucie Sikorová, and Literatura 18
Cíl: Zmapovat vliv preventivních strategií na vznik infekce močového traktu v souvislosti se zavedeným močovým katétrem. Metodika: Metodou praxe založené na důkazu bylo v elektronických databázích (PubMed, American Journal of Medicine, Virginie Henderson Library, Google Scholar, PubMed Central, Best Evidence for Nursing, Cochrane Collaboration) nalezeno 25 relevantních studií (meta-analýzy, systematické přehledy, randomizované kontrolované studie, prospektivní kohortové studie, průřezové studie, retrospektivní studie, studie případů a kontrol). Výsledky: Nebyly nalezeny silné důkazy, které by jednoznačně potvrdily nebo vyvrátily vliv používaných preventivních strategií (močový katétr potažený stříbrem, zvýšená hygiena rukou, stanovení optimální délky zavedení močového katétru) na snížení výskytu infekce močových cest u pacientů se zavedeným močovým katétrem. Závěr: Silné důkazy potvrzující vliv preventivních strategií nalezeny nebyly, avšak validní a relevantní výstupy a doporučení zahraničních studií mohou být přínosné v péči o katetrizované nemocné v podmínkách českého ošetřovatelství., Objective: To find out the influence of preventive strategies on the occurrence of catheter-associated urinary tract infection. Methods: Using the method of evidence based practice were found in electronic databases (PubMed, American Journal of Medicine, Virginie Henderson Library, Google Scholar, PubMed Central, Best Evidence for Nursing, Cochrane Collaboration) 25 relevant studies (meta-analysis, systematic reviews, randomized controlled trials, prospective cohort studies, cross-sectional studies, retrospective studies, case-control studies). Results: We did not found any strong evidence, which could clearly validate or refute the influence of prevention strategies used (silver-impregnated urinary catheter, intensive hand hygiene practices, determination of the optimal length of the inserted urinary catheter) on reducing the incidence of catheter-associated urinary tract infection. Conclusion: Strong evidence confirming the influence of preventive strategies were not found, however valid and relevant outcomes and recommendations of international studies may be useful in the care of catheterised patients in conditions of Czech nursing., Jana Bizoňová, Darja Jarošová, and Literatura
