INTRODUCTION: Autologous stem cell transplantation (ASCT) became standard of care for patients with multiple myeloma (MM) under the age of 65 years. We routinely perform ASCT for newly diagnosed MM since 1996 in our department. PATIENTS AND METHODS: We retrospectively analyzed all 285 transplants in 185 patients done for MM from January 1996 till December 2010. We analyzed overall survival (OS) and progression-free survival (PFS) regarding conditioning, stage, complete or very good partial remission (CR, VGPR) achievement, renal impairment, single vs. double transplant. RESULTS: Estimated 10-years survival of the whole set of patients is 39% (median survival 95 months). Patients with renal impairment show same OS as those without (p = 0.22). Patients show similar overall survival and event free survival regardless of type of transplant. We observed better outcome in terms of overall survival in patients treated with new drugs (p = 0.0014). Reaching CR or VGPR was not translated into better OS (p = 0.30) and EFS (p = 0.10). Also stage of the disease and whether single or double transplant was used did not make any significant difference in the outcome. CONCLUSION: Stem cell transplantation greatly improved outcome of patients with MM. Poor outcome of allogeneic transplantation in our group of patients is related to high transplant related mortality (20% vs. 0%) and unexpected high relapse rate. There is a trend towards better survival, when new drugs are incorporated at any time in the course of the disease. Úvod: Biklonální gamapatie nejistého významu (BGUS) je méně obvyklé onemocnění charakterizované přítomností dvou monoklonálních gradientů a relativně příznivým biologickým chováním. Cíl: Cílem práce bylo posouzení možného přínosu či naopak limitací obvyklých parametrů používaných u monoklonální gamapatie nejistého významu (MGUS) ve skupině jedinců s BGUS. Metody: Analyzovaná sestava čítala 18 vzorků sér od jedinců s BGUS (9x IgG–IgA, 7x IgG–IgM a 2x IgG–IgG typ). Pro analýzu byl použit turbidimetr SPA Plus a stanovované parametry zahrnovaly sérové hladiny volných lehkých řetězců imunoglobulinu (FreeLite™), hladiny polyklonálních imunoglobulinů a hladiny párů těžkých/lehkých řetězců imunoglobulinu (HevyLite ™). Výsledky a závěr: Pilotní analýza a zjištěné výsledky potvrzují skutečnost, že u velké části BGUS dochází k ovlivnění některých parametrů užívaných ve stratifikaci a sledování jedinců s MGUS, zejména v případě odlišnosti lehkých řetězců v obou molekulách monoklonálních imunoglobulinů. Jako velmi perspektivní se jeví analýza párů těžkých/lehkých řetězců imunoglobulinu (HLC), zohledňující možnost izotypové suprese alternativního páru s ovlivněním HLC indexu. Avšak v podmínkách běžné klinické praxe je zřejmé, že zásadní pro určení prognózy BGUS bude nutná izolace a analýza klonálních populací buněk na molekulárně-cytogenetické úrovni., Introduction: Biclonal gammopathy of undetermined significance (BGUS) is a less frequent disease characterized by the presence of two monoclonal gradients and a relatively favourable biological development. Objective: The objective was to assess potential benefits and/or limitations of laboratory parameters typically used with monoclonal gammopathy of undetermined significance (MGUS) when applied to a group of BGUS patients. Methods: The analysed batch comprised 18 serum samples from BGUS patients (9x IgG–IgA, 7x IgG–IgM a 2x IgG–IgG type). The assays were performed with a SPA Plus turbidimeter. Serum levels of polyclonal immunoglobulins, free light immunoglobulin chains (FreeLite™), and pairs of heavy/light immunoglobulin chains (HevyLite™) were assayed. Results and conclusion: The results of the pilot analyses confirm that a significant portion of BGUS patients exhibits changes of certain parameters typically applied to the stratification and monitoring of MGUS patients, particularly when the light chains in both molecules of monoclonal immunoglobulins differ. The analysis of heavy/light immunoglobulin chain pairs (HLC) appears very promising, indicating the possibility of an isotype suppression of the alternative pair affecting the HLC index. Monoklonální gamapatie nejasného významu (MGUS) je definována jako klinicky němý bezpříznakový stav, při kterém nejsou splněna diagnostická kritéria mnohočetného myelomu (MM), Waldenströmovy makroglobulinémie, AL-amyloidózy nebo jiného zhoubného lymfoproliferativního onemocnění. Dlouhodobým sledováním bylo prokázáno, že jde o možnou prekancerózu s přechodem v maligní monoklonální gamapatií v přibližně 1 % pozorování za rok, pravděpodobně všechny případy MM vznikají transformací z MGUS. V rámci přehledného sdělení se autor snaží odpovědět na otázku, zda je v současnosti možné nějakým způsobem transformaci MGUS v MM zabránit nebo ji alespoň zpomalit. Odpovědí u MGUS pacientů je zatím jednoznačné NE. Vzhledem k mnohem vyšší míře rizika transformace u nemocných s asymptomatickým mnohočetným myelomem než je tomu u pacientů s MGUS, budou další analýzy CMG (Česká myelomová skupina) zaměřeny na tuto skupinu nemocných s cílem vytipovat nejrizikovější skupinu nemocných, a tu poté podrobit časné a pokud možno netoxické léčbě., Monoclonal gammopathy of undetermined significance (MGUS) is defined as a clinically silent symptomless condition where not met diagnostic criteria for multiple myeloma, Waldenström macroglobulinemia, AL-amyloidosis or other malignant lymphoproliferative disease. Long-term monitoring has shown that it is possible to premalignant progression to malignant monoclonal gammopathy in approximately 1% of cases a year, probably all cases of MM arise transformation from MGUS. Within an easy communication, the author tries to answer the question whether it is currently possible to somehow transform MGUS in MM to prevent or at least slow down. The response in MGUS patients is not for now. Given the much higher level of risk of transformation in patients with asymptomatic multiple myeloma than in MGUS patients, further analysis CMG (Czech myeloma group) is focused on this group of patients with the aim to identify the highest risk patients and then subject them to early and, if possible, non-toxic treatment., Maisnar V., and Literatura
