Video prezentuje naši techniku provádění LESS nefrektomie (NE). Materiál: Od 8/2011 do 10/2014 jsme provedli 262 laparoskopických NE, z toho 62 LESS (23,7 %), které jsou dále hodnoceny. Celkem 26 mužů a 36 žen, 33 vlevo, 29 vpravo. Průměrné BMI 26,9±3,9 (16–33,2). Většina pomocí Quadport+®, pouze osm pomocí GelPoint®. Z nich 45 pro nádor, 17 pro nenádorové onemocnění. Indikace pro LESS nebyla dána žádnými striktními kritérii, záleželo na subjektivním hodnocení proveditelnosti operujícím týmem. Operace prováděli čtyři operatéři. Jednalo se zejména o neobézní pacienty s méně pokročilým nádorem. Popis metody: Video prezentuje typický případ. Poloha na boku, transumbilikální minilaparotomie, multikanálový port (Quadport+®). Kamera 10 mm přímohledná, jeden dvakrát zahnutý grasper, pečetící nástroj. Uvolněn dolní pól ledviny, ureter a s ním celý hilus ledviny elevovány vláknem zavedeným přes stěnu břišní. Hilus přerušen en bloc staplerem. Douvolněna ledvina a odstraněna původní minilaparotomií v sáčku (Endocatch®). Minilaparotomie uzavřena bez drénování. Výsledky: U 27 LESS-NE (43,5 %) byl užit transumbilikální přístup (u posledních 19 jen takto), u zbylých pararektální incize. Přídatný port užit ve 27,4 % (17) – 2/33 (6,1 %) vlevo, 15/29 (51,7 %) vpravo. U tří z nich (4,8 %) ještě další port (= konverze na LNE). U dvou (3,2 %) provedena otevřená konverze. Průměrný čas operace byl 96,7±40,0 (28– 230). U nejzkušenějšího operatéra (34 výkonů) to bylo 73,4±20,0 (28–104). Hilové cévy byly přerušeny u 14 (22,6 %) separovaně pomocí uzamykatelných klipů, u 48 (77,4 %) en bloc staplerem. Dimise 5,9±4,8 (2–40) pooperační den. Komplikace dle Claviena 3 x 2, 2 x 3b (operační revize pro lézi tenké kličky zavzaté do stehu minilaparotomie resp. krvácení z jater) a 1 x 4a (pooperační kardiopulmonální resuscitace). Závěr: LESS NE prováděnou zkušeným laparoskopistou je relativně bezpečná a efektivní metoda nefrektomie pro benigní i maligní onemocnění u vybraných případů (BMI<33 a méně pokročilé tumory)., The video presents our technique of LESS nephrectomy. Material: From 8/2011 to 10/2014 we performed 262 mini-invasive nephrectomies including 62 LESS procedures (23.7 %) that are evaluated below. There were 26 men, 36 women, 33 left sided and 29 right sided. The mean BMI was 26.9±3.9 (16–33.2). We used mostly Quadport+® and GelPoint® in 8 cases. There were 45 tumours and 17 benign masses. We had no strict indication criteria for LESS, it depended on subjective evaluation of the feasibility of surgical team. Procedures were performed by 4 surgeons. Nonobese patients with less advanced tumours were included. Method: This video presents a typical case. The surgery begins in a fl ank position using transumbilical minilaparotomy and multichannel port (Quadport+ ®). A 10 mm 0° video camera, one pre-bent grasper and some sealing devices were used. The lower pole of the kidney is deliberated, the ureter and hilum are identifi ed and elevated by using a thread that is introduced through the abdominal wall. The renal hilum is divided en bloc with a stapler. The kidney is totally deliberated without any drainage. Results: We used the transumbilical approach in 27 LESS (43.5 %) nephrectomies (the last 19 only with this procedure) and a pararectal incision in the rest. An additional port was used in 27.4 % (17) – 2/33 (6.1 %) on the left, 15/29 (51.7 %) on the right. In 3 cases of them (4.8 %), another additional port (conventional laparoscopic nephrectomy) was necessary. There were 2 conversions (3.2 %). The mean operation time (OT) was 96.7±40.0 (28–230) minutes. The most experienced surgeon had mean OT 73.4±20.0 (28–104). Hilar vessels were divided separately in 14 (22.6 %) cases using lockable clips, and 48 (77.4 %) cases en bloc with a stapler. The mean hospitalization time was 5.9±4.8 (2–40) days. Complications according to the Clavien-Dindo classifi cation were 3 times grade II and 2 times grade IIIb (revisions for lesion of small intestine sutured to the minilaparotomy and liver bleeding) and one grade IVa (postoperative cardiopulmonary resuscitation). Conclusion: In selected cases (BMI<33 and less advanced tumours). Úvod: Video prezentuje naši současnou standardní/ nejčastější techniku provádění laparoskopické resekce (LR) tumoru ledviny (TL). Materiál: Od 8/2004 do 4/2015 jsme provedli 274 LR. Během let se metodika neustále vyvíjela se zručností operatérů a s využitím nových technických doplňků. Ruku v ruce s tím se rozšiřovalo i spektrum TL ošetřitelných laparoskopicky. Postupně se přidávaly nádory s vyšším RENAL nefrometrickým skórem (větší, centrálněji uložené, lokalizace i dorzálně a i na horním pólu, v kontaktu s cévami). Díky většímu rozsahu laparoskopicky ošetřitelných TL a také díky časnější diagnostice a posunu nově zachycených TL do nižších stadií se každoročně zvyšuje podíl TL ošetřených LR. V roce 2014 to bylo 31,9 % chirurgicky ošetřených TL. V absolutních číslech 48 výkonů za rok. Výsledky – popis metody: Standardní je poloha na boku, přístup transperitoneální ze čtyř portů (10, 2 × 5 a 12 mm). Vpravo někdy pro elevaci jater grasperem je užit další port 5 mm. Je otevřeno zadní peritoneum a Gerotova fascie. Je ozřejměn tumor a verifikován sonograficky laparoskopickou sondou (mj. určujeme ohraničení spodiny oproti parenchymu či sinu ledviny). Je-li tumor verifikován, jsou rozpreparovány hilové cévy. Krátkou laparoskopickou endosvorkou je klampována renální tepna či všechny její větve, či výjimečněji selektivně větev tepny zásobující tumor. Jde-li tumor do hloubky sinu, klampujeme i měkčí svorkou renální žílu. Pouze v případě velmi malých extrarenálně uložených tumorů hilus neklampujeme. U komplexních hilů, kde není možná selektivní preparace hilu, je možno použít dlouhé cévní svorky zavedené přímo přes stěnu břišní. Následuje resekce tumoru nůžkami s lemem zdravé tkáně. Tumor je vložen do sáčku EndoCatch® Gold. Spodina je někdy ošetřena bipolární koagulací či argonovým koagulátorem (bezkontaktní, ale dražší) a to zejména v případech, že resekce byla blízko tumoru. Pomocí Vicryl® či nověji s V-Loc 90™ (oboje vstřebatelné) stehu je provedena sutura spodiny resekce (cévy event. i otevřený dutý systém), stehy jsou kotveny Hem-o-lok™ klipy velikosti ML. Další stejnou suturou jsou k sobě sešity okraje resekované ledviny. Klipy umožňují i u V-Loc™ lepší dotažení tkáně ledviny a vlákno se neprořezává. Hilové cévy jsou uvolněny. Je ošetřeno event. reziduální krvácení dalším stehem, někdy je přiložena tkaná celulóza. S V-Loc 90™ je provedena sutura obalů ledviny (Gerotova fascie, ev. i peritoneum). Laterálním portem je vložen páskový drén. Tumor je v sáčku extrahován lehce rozšířeným portem 12 mm. Spodina tumoru je pro patologa označena černým inkoustem. Závěr: LR je již standardizovaná metoda umožňující ošetřit téměř třetinu nádorů ledvin., This video presents our contemporary standard technique of laparoscopic resection (LR) of kidney tumours (KT), the method used most frequently by our group. Material: Between August 2004 and April 2015, we performed 274 LR. The surgical technique was continually developing along with the surgeons´ dexterity and usage of new technical accessories. At the same time, this surgical technique allowed for the treatment of a broader spectrum of KT, which are now managed using laparoscopic approach. This has gradually led to our management of KT with a higher R.E.N.A.L. nephrometry score (larger, more centrally or dorsally located lesions, and those affecting upper pole or hilus). There is an annual increase of KT managed by LR thanks to earlier diagnosis and an increased rate of detection of early (low) stage KT. In 2014, LR constituted 31.9 % of all surgically treated KT, amounting to 48 procedures. Results – our technique: The standard position is on the side with a transperitoneal approach using 4 ports (10mm, 2x5mm and 12mm). In select 105 Ces Urol 2015; 19(2): 103–105 video cases, an additional 5mm port on the right side is used for liver elevation with a grasper. Posterior peritoneum and Gerota´s fascia are opened. The tumour is detected and verified by a laparoscopic ultrasound probe (we define the borderline between tumour and parenchyma/renal sinus). After tumour verification, dissection of the renal hilar vessels is performed. Next the renal artery/arteries, or in rare instances only selected branch, are clamped using a laparoscopic bulldog endoclamp. In the case of tumour invasion into the sinus, we use a venous bulldog endoclamp to clamp the renal vein. In cases with a small extrarenal tumour we do not clamp the renal hilum. In cases with a complex renal hilum, where it is difficult to selectively dissect the vessel, it is possible to use extracorporeal clamping (inserted through the abdominal wall). This is followed by tumour resection into a healthy surgical margin using scissors. The specimen is inserted into the EndoCatch® Gold. In cases with a close resection margin, the resetected bed is managed by bipolar or Argon coagulation (the latter is contactless, but more expensive). The resected bed (vessels or opened collecting system) is sutured with Vicryl® or more recently V-Loc 90TM (both absorbable), with the sutures tightened with Hem-o-lok clipsTM ML. The same suture is used for closing of the parenchyma. Clips provide (even for V- LocTM) better tightening of kidney tissue and prevent the risk of kidney tearing. Hilar vessels are than released. Possible residual bleeding is managed by another suture or sometimes covered by oxidized cellulose. Suturing of the posterior peritoneum and Gerota´s fascia is done with V-Loc 90TM. An easyflow drain is placed through lateral 5mm port. The specimen is removed through incision of the 12mm port, which is slightly enlarged. The specimen margin is marked black for the pathologist. Angiomyolipom ledviny (AML) je benigní mezenchymový nádor, který se vyskytuje sporadicky nebo v rámci komplexu tuberózní sklerózy (TSC). Celková prevalence AML je 0,44 % a vyskytuje se čtyřikrát častěji u žen. Prevalence v ženské populaci je tedy 0,6 %, zatímco u mužů 0,28 %. Jeho diagnostika je založena na přítomnosti tukové složky v nádoru a lze tedy identifikovat předoperačně pomocí zobrazovacích metod. Morfologie AML je velmi variabilní a někdy tedy tuková složka chybí, což může způsobit diagnostické rozpaky. Zcela zásadní roli má aktivní sledování pacientů s jasnou diagnózou AML, jehož růst je pomalý s minimální morbiditu. Pacient však může přijít ve stadiu komplikací, které zahrnují hematurii, bolest či krvácení do retroperitonea způsobené rupturou nádoru. Mezi hlavní rizikové faktory krvácení jsou velikost nádoru, procento zastoupení cévní složky, kterou tvoří aneuryzmatické cévy a přítomnost TSC. Selektivní arteriální embolizace úspěšně řeší hemoragické komplikace a redukuje objem AML. V rámci dlouhodobých výsledků však selhává díky revaskularizaci a dalšímu růstu AML. Zde pak mají lepší výsledky chirurgické operace, kde se snažíme v maximální míře šetřit funkci ledviny. Slibné dlouhodobé výsledky má též radiofrekvenční ablace, která je ale silně limitována velikostí AML. U objemnějších AML se zdá být dobrou alternativou resekčního výkonu kryoablace, která však vyžaduje hůře dostupné speciální přístrojové vybavení. Novinkou je pak systémová léčba mTOR inhibitory (everolimus, sirolimus), které mají nezastupitelné místo u nemocných s TSC, kde jsou AML mnohočetné a recidivující. Tato terapie může pomoci redukovat celkový objem AML a umožnit případný resekční výkon., Renal angiomyolipoma (AML) is a benign mesenchymal tumor, which occurs sporadically or is associated with tuberous sclerosis complex (TSC). The overall prevalence of AML is 0.44 %, its incidence in women is 4 times higher than in men. Prevalence in the female population is thus 0.6 %, but male rates 0.28 %. Diagnosis is based on the presence of fat in the tumor. In most cases AML can be identified preoperatively. AML morphology is highly variable and in cases where fat is missing, preoperative diagnostic based on imaging methods is not possible. Active monitoring of patients with AML is crucial, because AML grows slowly and with minimal morbidity. Patient can present with haematuria, pain or retroperitoneal bleeding by tumors rupture. The main risk factors for bleeding are tumors volume, percentage of the vascular component in the tumor and the presence of TSC. Selective arterial embolization resolves hemorrhagic complications and reduces the volume of the tumor. The long-term outcomes of embolisation are poor due to tumor revascularization and growth. Nephron sparring surgery is associated with better long term outcomes. Radiofrequency ablation is another option, but strongly restricted by the volume tumor. An additional option is cryoablation, which has better results in cases where tumor is large, but this requires special equipment. Systematic treatment using mTOR inhibitors (everolimus, sirolimus, has recently been introduced, which are essential in patients with TSC, where the disease is often complicated by multiple and recurrent lesion.
