Prezentujeme případ mladé ženy, jež prodělala v dospělém věku operaci vrozené cysty pravého hepatiku, kterou vzhledem k těsnému kontaktu s velkými cévami nebylo možné kompletně resekovat. Byla provedena parciální resekce a cysto-jejuno-anastomóza (CJA) kdrenáži žluči. Pacientka byla 4 roky bez obtíží. Posléze se dostavily bolesti břicha, subikterus a laboratorní známky cholestázy Kvůli těmto potížím a opakovaným cholangoitidám prodělala řadu vyšetření a intervencí, které měly zajistit ozřejmění příčiny obstrukce a obnovení odtoku žluči. Použité zobrazovací metody neprokázaly jednoznačnou příčinu obtíží, proto bylo přistoupeno k operační revizi, při níž byl peroperační ultrasonografií prokázán tumor v oblasti CJA. Nález byl vzhledem k lokalizaci inoperabilní. Pacientka měla následně zjištěnu diseminaci malignity v játrech i v uzlinách a zemřela 5 měsíců od zjištění maligní diagnózy na hepatorenální selhání. Tento poměrně raritní případ dokládá zvýšené riziko maligního zvratu vrozených malformací žlučových cest především v případě jejich diagnózy a léčby v dospělém věku. V dostupné literatuře existuje jen málo dokumentovaných případů metachronního karcinomu po resekci žlučových cest., We present a case of a young woman, who underwent surgery for congenital bile duct cyst in adulthood. Due to location of the cysts it was impossible to completely remove it. The cyst was partially resected and it was performed cyst-jejunal anastomosis (CJA) to drain of bile. After operation the patient was 4 years without any problems. After this interval appeared pain in the right upper quadrant and subicterus with laboratory evidence of cholestasis. The patient suffered from recurrent cholangitis due to which a number of diagnostic tests and interventions were performed. Used imaging metods did not demonstrate an unequivocal cause of problems, therefore subsequently surgical revision was performed and the in-traoperative ultrasonography demonstrated tumor in anastomosis. The finding was inoperable due to inappropriate location. In the following period liver and lymphnode metastases appeared and patient died five months after diagnosis of malignancy. This relatively rare case illustrates an increased risk of malignant transformation of congenital malformations of the biliary tract. Cysta choledochu je vzácné onemocnění, výrazně vyšší výskyt zaznamenáváme u asijské populace. Etiologie není zcela známa, avšak uvažuje se o spojení s anomáliemi v anatomii žlučových cest. Přestože je svojí povahou benigní, jedná se o prekancerózu s nebezpečím zvratu v karcinom žlučových cest. Jediným kurativním řešením je radikální chirurgické odstranění s rekonstrukcí žlučových cest. Autoři prezentují kazuistiku nemocné s cystou ductus hepatocholedochus typu I dle Todaniho klasifikace., Choledochal cyst is a rare disease with a considerably higher incidence found in the Asian population. Although its etiology is not completely known, the disease is believed to be associated with anomalies in the anatomy of the biliary tract. While being a benign unit, it is considered as a precancerosis with the risk of conversion to the biliary tract carcinoma. Autoři prezentují formou kazuistického sdělení případ 57leté ženy s velmi vzácným extragenitálně lokalizovaným maligním Mülleriánským karcinosarkomem retroperitonea infiltrujícím dolní dutou žílu. Je popsána resekce tumoru s částí stěny duté žíly i resekce metastázy v oblasti pánve. Dále autoři diskutují možnosti diagnostiky rozsevu tohoto typu nádoru a doporučenou adjuvantní onkologickou terapii., The authors present the case of a 57-year-old woman with a very rare extragenital malignant retroperitoneal Müllerian carcinosarcoma invading the inferior vena cava. Autoři na dvou předložených kazuistikách představují širokou diferenciální diagnostiku granulomatózních procesů hepatobiliárního traktu. Izolovaný granulomatózní zánět v této oblasti nacházíme velmi zřídka. Diferenciální diagnostika těchto procesů zasahuje od infekční etiologie až po systémová onemocnění či chemickou iritaci. Granulomy často imitují maligní tumory v jaterní krajině, a chirurg tak může být lékařem, který dané onemocnění odhalí jako první při resekčním výkonu či cholecystektomii., The authors present two case reports to demonstrate wide differential diagnosis of granulomatous disorders of the hepatobiliary tract. Isolated granulomatous disorders are very rare in this area. Differantial diagnosis includes infectious and systemic diseases or chemical irritation. Úvod: Přítomnost spolknutých cizích těles v zažívacím traktu patří mezi časté problémy u dětí. Nejčastější výskyt je u dětí od 6 měsíců do 5 let. Většina spolknutých předmětů vyjde per vias naturales, 10–20 % spolknutých těles vyžaduje endoskopické vynětí. Závažné a život ohrožující stavy způsobují spolknutá cizí tělesa asi v 1 % všech případů. Kazuistika: Jedná se o případ dvouleté dívky, u které byla diagnostikována náhlá příhoda břišní, pro niž byla operována. Peroperačně byl nalezen volvulus tenkého střeva a několik střevních píštělí. Příčinou tohoto nálezu byly spolknuté magnetické kuličky a k nim magnetickou silou přitažené spolknuté kovové těleso. Závěr: V případě spolknutí více než jednoho magnetického tělesa je nutné pacienta hospitalizovat a endoskopicky cizí tělesa extrahovat. Neměnící se poloha magnetů v průběhu 24–48 hodin v místě nedostupném pro endoskop je indikací k akutní operaci., Introduction: The occurrence of swallowed foreign bodies in the digestive system is a common problem in children with the highest incidence in children aged six months to five years. Most swallowed objects leave the human body per vias naturales while 10–20% of swallowed foreign bodies need to be removed with an endoscope. Serious and life-threatening situations are caused by the ingestion of foreign bodies in about 1% of all cases. Case report: The authors present a case of a two-year-old girl diagnosed with acute abdomen for which she was operated on. A small bowel volvulus and several intestinal fistulas were found intraoperatively. The cause of this finding was the ingestion of magnetic balls and a swallowed metal body drawn to them by magnetic force. Conclusion: If more than one magnetic body is ingested, it is necessary to admit the patient to hospital and to remove these foreign bodies using an endoscope. U primárně neresekabilních jaterních metastáz může neoadjuvantní konverzní systémová léčba při použití moderních režimů chemoterapie v kombinaci s cílenou biologickou léčbou umožnit radikální chirurgickou resekci metastatických ložisek na játrech. Optimální konverzní režim by měl být co nejméně toxický. Tato kritéria splňuje monoklonální protilátka cetuximab. Následující klinická kazuistika demonstruje využití cetuximabu v konverzní léčbě jaterních metastáz, která vedla k jejich radikální resekci., In primarily unresectable liver metastases, neoadjuvant conversion systemic therapy with the use of modern chemotherapy regimens in combination with targeted biological therapy may allow for a radical surgical resection of metastatic foci in the liver. An optimal conversion regimen should be one with the least toxicity. These criteria are met by the monoclonal antibody cetuximab.
