Úvod: Dynamické metody kvantitativního testování senzitivity (Quantitative Sensory Testing; QST) jsou relativně novou nadstavbovou aplikací QST umožňující objektivizovat funkci centrálních modulačních mechanizmů algické percepce, jejichž individuální nastavení může přispívat k rozvoji chronických bolestivých stavů. Testování těchto mechanizmů umožňuje např. predikci rozvoje pooperační bolesti. K nejčastěji hodnoceným dynamickým QST metodám patří tzv. podmíněná modulace bolesti (Conditioned Pain Modulation; CPM) a časová sumace percepce bolestivých podnětů (Temporal Summation; TS). Soubor a metodika: Vyšetření dQST (dynamic methods of Quantitative Sensory Testing) metod s využitím termoalgických podnětů a modulace bolestivým chladem bylo provedeno u 77 zdravých dobrovolníků (47 žen testovaných v ovulační fázi menstruačního cyklu, 30 mužů, věk 19–73 let). Hodnocen byl také vliv věku a pohlaví na výsledky obou dQST testů. Výsledky: Uspokojivá funkce podmíněné modulace bolesti byla prokázána u 84 % testovaných dobrovolníků. Časová sumace bolestivých podnětů byla zachycena u 92, resp. 98 % jedinců v závislosti na použité teplotě podnětů. Nefunkčnost mechanizmů u zbylé části souboru byla ve většině případů podmíněna nesprávně nastavenými technickými parametry testu. U žen testovaných v ovulační fázi menstruačního cyklu jsme prokázali signifikantně vyšší efektivitu CPM mechanizmu ve srovnání s muži. Funkce TS se mezi oběma pohlavími významně nelišila. Významný vliv věku na efektivitu CPM ani TS mechanizmu nebyl ve sledovaném souboru prokázán. Závěr: Provedená studie ověřila použitelnost zvolených algoritmů dynamického testování senzitivity a prokázala antinociceptivní nastavení žen v ovulační fázi menstruačního cyklu ve srovnání s muži, zatímco významný vliv věku na funkci centrálních modulačních mechanizmů algické percepce nebyl zachycen., Background: So called “dynamic quantitative sensory methods” (dQST) represent relatively new QST application which enables to objectify certain mechanisms of pain processing, e.g. central integration or descending control. Their dysfunction may contribute to the development of chronic painful conditions. dQST methods proved the potential to predict the development of chronic postoperative pain. Conditioned pain modulation (CPM) and temporal summation (TS) are the most commonly used dQST tests. Patients and methods: The CPM and TS testing was performed in a group of 77 healthy volunteers (47 women tested in the ovulatory phase of the menstrual cycle, 30 men, age range 19–73) using thermal pain stimuli. Efficiency of CPM and TS mechanism including age and gender influence was evaluated. Results: Efficient CPM was found in 84% of healthy volunteers. In remaining 16%, the inefficiency was mainly due to low intensity of testing or modulatory stimuli. Based on the temperature used, efficient TS mechanism was found in 92% or 98% of healthy individuals. Women in the ovulatory phase of the menstrual cycle showed significantly more efficient CPM comparing to men. No sex differences of TS testing were found. Age had no significant effect on the CPM or TS mechanism in our group of healthy voluteers. Conclusions: Our study has verified the applicability of CPM and TS algorithms used. Women in ovulatory phase of the menstrual cycle show increased anti-nociceptive setting comparing to men. No significant effect of age on central modulation of pain perception has been found in pain-free healthy individuals. Úvod: Sympatická kožní odpověď (SSR) je jednoduchý a široce dostupný test sudomotorických funkcí. Jedná se o polysynaptický reflex s variabilní aferentací a eferentní částí, zprostředkovanou tenkými nemyelinizovanými vlákny typu C. Cílem práce bylo zhodnocení validity SSR v diagnostice neuropatie s postižením tenkých senzitivních nervových vláken. Metodika: SSR byla vyšetřena na horních a dolních končetinách u 69 pacientů s bolestivou periferní neuropatií. U 33 pacientů této skupiny šlo o izolovanou neuropatii tenkých nervových vláken (NTV). Třicet šest pacientů mělo kombinované postižení vláken tenkých a silných. Nálezy byly srovnány se souborem 89 zdravých kontrol. Hodnocena byla výbavnost odpovědí elektrickým proudem a prudkým inspiriem a jejich reprodukovatelnost, amplituda a latence. Výsledky: Hodnocení latencí ani amplitud odpovědí neprokázalo jasný přínos pro diagnostiku postižení tenkých nervových vláken. Nejspolehlivější typ SSR abnormity je tedy nevýbavnost odpovědí. Postižení tenkých autonomních vláken však bylo v našem souboru pomocí SSR prokazatelné pouze u malé části pacientů se senzitivní neuropatií – senzitivita dosáhla necelých 10 % u pacientů s čistou NTV a 33 % u pacientů s kombinovaným postižením vláken tenkých a silných (u nichž je současně pravděpodobnější těžší stupeň postižení vláken tenkých). Závěr: Při zohlednění nízké senzitivity není SSR optimální metoda využitelná jako jediný diagnostický test neuropatie tenkých vláken., Introduction: Sympathetic skin response (SSR) is a simple and widely available test of sudomotor functions. Afferent part of this polysynaptic reflex is variable, while the efferent direction of the reflex arch is via thin unmyelinated C fibres. No reliable study of the SSR diagnostic validity in patients with sensory small fiber neuropathy (SFN) has so far been published. In clinical practice, however, we repeatedly meet patients, who have been diagnosed with SFN based solely on abnormal SSR. The aim of the study was to evaluate diagnostic validity of SSR in patients with sensory small fibre neuropathy. Methods: SSR was recorded from palms and soles of 69 patients with painful sensory neuropathy (33 of them with pure SFN and 36 with mixed small and large nerve fibre dysfunction) using electrical stimulation and inspiratory gasp stimuli. Small nerve fiber involvement was confirmed by reduced intraepidermal nerve fiber densities in skin biopsy samples of all cases. The results were compared with those of 89 healthy controls. The outcome for health controls were also used to establish age-stratified normative data. We assessed electric stimuli and sudden inspiration response recall end their reproducibility, amplitude and latency. Results: Both the SSR latencies and amplitudes showed very low diagnostic validity in small fibre neuropathy patients. The absence of SSR response represented the most reliable abnormality. However, using this parameter, dysfunction of small autonomic nerve fibres has only been found in a small part of our sensory neuropathy patients: sensitivity did not exceed 10% in pure SFN patients and 33% in those with mixed small and large nerve fibre dysfunction (where more pronounced small sensory nerve fibre involvement has previously been found). Conclusion: Considering its low sensitivity, SSR should not be used as the only test to confirm sensory small fibre neuropathy. Mezi nejnáročnější součásti klinického neurologického vyšetření patří vyšetření citlivosti vyžadující detailní edukaci pacienta a jeho následnou spolupráci. Při správné metodice vyšetření a jeho exaktním hodnocení lze často odhalit i diskrétní poruchy citlivosti, kterých si pacient nemusí být vědom nebo není schopen je validně popsat. Našim cílem je podat přehled nejčastěji používaných metod vyšetření běžně hodnocených senzitivních a algických modalit (taktilní, vibrační a termické čití, termoalgické čití, hodnocení hluboké tlakové bolesti a dále mechanická algická citlivost a hodnocení dynamické mechanické alodynie), včetně interpretace jejich nálezů a využitelných normativních dat (pokud jsou dostupná), a to od nejjednodušších kvalitativních a semikvantitativních metod po složitější metody kvantitativní. Kvalitativní či semikvantitativní metody jsou v běžné klinické praxi využívány podstatně častěji. Jejich výhodou je rychlost a metodická nenáročnost testování a široká dostupnost používaných pomůcek. Nevýhodou je naopak skutečnost, že poskytují pouze orientační informaci o citlivosti pro danou modalitu a spolehlivě detekují většinou až pokročilé poruchy senzitivity. Absence kvantifikace limituje možnosti těchto metod v longitudinálním sledování pacientů i při srovnání nálezů mezi pacienty či jejich skupinami. Kvantitativní metody souhrnně označované termínem kvantitativní testování senzitivity (Quantitative Sensory Testing, QST) jsou časově a metodicky náročnější a vyžadují použití specializovaných pomůcek. Jejich předností jsou kvantifikované výstupy s lepší diagnostickou validitou, které potenciálně diagnostikují i klinicky méně vyjádřené abnormity a jsou lépe využitelné např. pro longitudinální sledování. Tyto metody se častěji uplatňují v klinických studiích a jsou nezastupitelné v diagnostice neuropatie tenkých vláken., Sensory examination represents one of the most complicated parts of clinical neurological examination that requires detailed instructions to patients and their attentive cooperation. Using correct methodology of testing and precise evaluation of the results, sensory tests frequently disclose even very fine sensory abnormities which patients may not be able to describe or even may not be aware of. The aim of this paper is to provide an overview of methods most frequently used to evaluate common sensory and pain modalities (touch, vibration and thermal sensation, thermoalgic sensation, deep pain pressure, mechanical detection threshold and sensitivity, evaluation of dynamic mechanical allodynia), including interpretation of the findings and recommended normal values (if applicable). In each of the tested modalities, most commonly used methods are described from the easiest qualitative and semiquantitative methods (usually more frequently used in clinical practice) up to more complicated quantitative tests. Main advantages of qualitative and semiquantitative methods include their low methodological and time burden and high availability of the equipment used. Quantitative sensory testing (QST) is more demanding and requires the use of specialized, not widely available, equipment. These methods, however, provide quantified results with better diagnostic validity, may disclose even less pronounced sensory abnormities and are more useful in long-term follow-up as well as in inter-individual comparisons, both at individual and group level.
