Hereditární hemoragická teleangiektázie neboli morbus Rendu‑Osler‑Weber je fibrovaskulární dysplazie s autozomálně dominantní dědičností charakteristická přítomností teleangiektázií a arteriovenózních malformací kůže, sliznic a vnitřních orgánů. Plicní arteriovenózní malformace jako zdroj septických embolů mohou zapříčinit vznik mozkového abscesu, který tak bývá prvním a často jediným projevem heriditární hemoragické teleangiektázie. V práci jsou prezentovány tři kazuistiky pacientů trpících touto chorobou, kde prvním a závažným příznakem byl vzniklý mozkový absces. Protože se jedná o relativně vzácné onemocnění, nebývá vždy zvažováno v rámci diferenciální diagnostiky a tím pádem ani diagnostikováno. U každého pacienta s mozkovým abscesem je nezbytné na toto onemocnění pomýšlet a v případě podezření je nutné jej řádně vyšetřit. Při verifikované plicní arteriovenózní malformaci je třeba ji ošetřit a zabránit tak vzniku recidivujících mozkových abscesů. Péče o tyto pacienty je komplexní a multioborová., Hereditary hemorrhagic telangiectasia or Rendu‑Osler‑Weber disease is an autosomal‑dominant disorder that results in fibrovascular dysplasia and is characterized by telangiectases and arteriovenous malformation of the skin, mucosa and viscera. Pulmonary arteriovenous malformations as a source of septic emboli may give rise to brain abscess that so often is the first and only clinical manifestation of hereditary hemorrhagic telangiectasia. Three cases of patients suffering from this disease with brain abscess as the first and serious symptom are presented. Since this is a relatively rare disease, it is not always considered in the differential diagnosis and thus not diagnosed. This disease has to be considered in patients with brain abscess and, in case of a suspicion, it must be properly examined. When pulmonary arteriovenous malformation is verified, it has to be treated in order to prevent recurrent brain abscesses. Care of these patients should be comprehensive and multidisciplinary. Nádory v oblasti tureckého sedla tvoří 10–15 % všech nitrolebních nádorů. Nejčastěji se zde vyskytují adenomy hypofýzy, které tvoří 80 % a jejichž incidence je udávána 1,5–2/100 000/rok, prevalence 20/100 000. Jedná se v naprosté většině o nezhoubné nádory. Komplexní léčba adenomů hypofýzy vyžaduje spolupráci neurochirurga, endokrinologa, radiochirurga, radiologa a oftalmologa. Základní léčebná modalita je s výjimkou prolaktinomů operace. Využití nových neurochirurgických technik umožnilo zvýšit radikalitu operačního výkonu bez zvýšení rizika pro pacienta. Cílem léčby je takové ovlivnění nádoru, které nebrání pacientovi žít normální život bez nutnosti hormonální substituce a bez ovlivnění zraku., Pituitary tumours constitute 10–15 % of all intracranial tumours. Adenomas are the most common and account for 80 % of tumours of the sella turcica. Their incidence is 1.5–2/100,000 / year and prevalence 20/100 000. The vast majority of adenomas have a benign course. Comprehensive treatment of pituitary adenomas requires cooperation among a neurosurgeon, an endocrinologist, a radiosurgeon, a radiologist and an ophthalmologist. The main treatment modality with the exception of prolactinomas is surgery. Using of new neurosurgical techniques have permitted to increase the extent of surgery without increasing the risk to the patient. Cíl: Autoři prezentují výsledky operační léčby tumorů periferních nervů. Operační výkony byly provedeny na Neurochirurgické klinice LF MU a FN Brno v letech 2003–2013. Ve sdělení je prezentován současný přehled histologické klasifikace a její vliv na možnosti chirurgické léčby. Součástí práce je i kazuistika dokumentující operační postup u raritního objemného tumoru n. ischiadicus v hloubce hýžďové krajiny. Soubor a metodika: Operační revizi podstoupilo celkem 24 pacientů, z toho 13 mužů a 11 žen. Průměrný věk pacientů v souboru byl 46,5 roku (min. 22, max. 74 let). Výkony byly provedeny standardní mikrochirurgickou operační technikou, od roku 2007 s užitím peroperační elektrofyziologické monitorace. Retrospektivní analýzou je soubor definován dle postiženého nervu, rozsahu resekce a histologické klasifikace. Je zhodnocen výsledný pooperační neurologický stav. Výsledky: Nejčastěji postiženým periferním nervem byl n. tibialis. V souladu s literárními zdroji se histologicky nejčastěji jednalo o benigní schwannom, a to celkem ve 20 případech. Jednou šlo o neurofibrom, jednou o intraneurální ganglionovou cystu, jedenkrát byla histologie uzavřena jako raritní extraoseální Ewingův sarkom a u jednoho pacienta byl tumor způsoben chronickým zánětlivým procesem. Závěr: V našem souboru bylo dosaženo totální resekce u všech operovaných pacientů, kromě jednoho pacienta. U něj byla provedena pouze revize s otevřenou biopsií. U pacienta s maligním Ewingovým sarkomem byla nutná kompletní rozšířená resekce postižených nervů s příslušným totálním motorickým a senzitivním deficitem. Ve dvou dalších případech došlo po operaci ke zhoršení neurologického stavu, jednou se jednalo o lehkou parézu, jednou o trvalou hypestezii. V ostatních případech klinické obtíže prakticky vymizely., Aim: The authors present surgical treatment results of peripheral nerve tumors. All the surgical procedures were performed between 2003 and 2013 at the Department of Neurosurgery, The University Hospital Brno. The paper provides an overview of current histological classification and its impact on surgical treatment. The paper also includes a case report describing a surgical procedure used for a rare, bulky tumor of the sciatic nerve in the depth of the gluteal region. Methods: In total, 24 patients, 13 men and 11 women, underwent surgical revision. The mean age of the group was 46.5 years (min. 22, max. 74 years). All the surgical procedures were completed using standard microsurgical tools. Perioperative electrophysiological monitoring has been used since 2007. A retrospective analysis according to the localization of the affected nerve, extent of resection and histological classification of the tumor was performed and the final outcome in terms of neurological status was evaluated in the study group. Results: A tumor of the tibial nerve was the most frequent target of the surgery in our study group. Benign schwannoma was the most common tumor in our study group (20 cases) and this is in accordance with previously published data. Other tumors were classified as neurofibroma (one case) and intraneural ganglion cyst (one case). Histology confirmed the presence of a rare extraosseous Ewing’s sarcoma in one case. Finally, one patient suffered from chronic inflammation. Conclusion: In our study group, total resection was performed in all patients except one. Revision with open biopsy was performed in a patient with nerve inflammation. Complete extended resection of the affected nerves was necessary in a patient with malignant Ewing’s sarcoma. This resulted in total motor and sensitive deficit of the corresponding area. Neurological status has also worsened after the surgery in two other cases, one had light paresis and the other had permanent hypoesthesia. The clinical symptoms disappeared after surgery in all other cases. Střelná poranění mozku jsou velmi závažná poranění a přežití závisí na včasné diagnostice a intervenci neurochirurga. Důležitým aspektem je i volba zbraně. V této kazuistice je uváděn případ pacienta hospitalizovaného na Neurochirurgické klinice ve FN Brno. Použitá zbraň je mezi replikami historických zbraní velmi raritní a statistiky poranění dlouhou předovkou neuvádějí., Missile brain injuries are very serious injuries, survival of which depends on early diagnosis and neurosurgical intervention. The choice of a weapon is an important aspect. In this case report, we describe a case treated at the Department of Neurosurgery, University Hospital Brno. The weapon causing this injury was a quite rare replica of a historical weapon and injury statistics do not report injuries caused by a longrifle. Článek přibližuje použitelnost a přínos v praxi využívaných zobrazovacích metod, především funkční magnetické rezonance a difuzní traktografie pro neurochirurgickou operativu intraaxiálních mozkových nádorů. Objasňuje také princip nejčastěji používaných elektrofyziologických metod (vyšetření zvratu fáze somatosenzorických evokovaných potenciálů, monitoraci motorických evokovaných potenciálů, kortikální elektrickou stimulaci při operacích s bdělou fází) sloužících k nalezení funkčně významných oblastí mozku a k monitoraci jejich funkčnosti během operace., The article introduces the applicability and benefit of imaging techniques used in the practice, particularly functional magnetic resonance and diffuse tractography, for neurosurgical treatment of intra-axial brain tumours.
