Cíl: Cílem je zhodnotit zkušenosti s léčbou aneurysmatu podkolenní tepny stentgraftem. Metodika: V období od června 2011 do října 2014 jsme na našem pracovišti léčili 18 aneurysmat podkolenní tepny u 14 pacientů pomocí stentgraftu. Jednalo se o 13 mužů a jednu ženu ve věku od 50 do 90 let. Věkový průměr byl 70 let, medián 69,5 let. Léčili jsme tři aneurysmata o průměru menším než 2 cm, 12 aneurysmat o průměru 2-5 cm a tři výdutě o průměru větším než 5 cm. Výsledky: Pacienti byli sledováni 2-35 měsíců, průměrná doba sledování byla 13, 4 měsíce, medián 10 měsíců. Technická úspěšnost byla 100 %. Primární průchodnost byla 78 %, sekundární průchodnost byla 94 %. Podle Kaplanovy-Maierovy analýzy byly pravděpodobnost primární průchodnosti za 6 a 12 měsíců po implantaci stentgraftu stejná, a to 75,8 % (95 % CI: 54,6-97,0 %), pravděpodobnost sekundární průchodnosti za 6 a 12 měsíců 94,4 % (95 % CI: 83, 8-100 %). Závěr: Endovaskulární léčba stentgraftem je možnou alternativou chirurgické léčby aneurysmatu podkolenní tepny. Endovaskulární léčba je jednoduchá, bezpečná a účinná., Aim: To evaluate the experience in the treatment of popliteal artery aneurysm with stentgraft. Method: From June 2011 to October 2014, we were treated 18 aneurysms of the popliteal artery in 14 patients using the stentgraft at our department. There were 13 men and one woman aged from 50 to 90 years. The mean age was 70 years. We treated three aneurysms with a diameter of less than 2 cm, 12 aneurysms 2-5 cm in diameter and 3 aneurysms with a diameter greater than 5 cm. Results: Patients were followed-up 2-35 months, mean follow-up was 13.4 months. The technical success rate was 100%. Primary patency was 78%, secondary patency was 94%. According to Kaplan-Maier analysis the probability of primary patency at 6 and 12 months after stentgraft implantation was 75.8 % (95% CI: 54.6 to 97.0%), the probability of secondary patency at 6 and 12 months was 94.4% (95% CI: 83,8-100%). Conclusion: Endovascular treatment with stentgraft is a possible alternative of surgical treatment of popliteal artery aneurysm. Hlavní cíl: Zjistit vliv procvičování ošetřovatelského procesu podle OPT modelu klinické rozvahy na erudici studentů oboru všeobecná sestra při využívání pojmů Aliance 3N. Východiska: Motivem k realizaci studie bylo dílo autorek OPT modelu klinické rozvahy Pesut a Herman a výzkumné články k OPT modelu autorů Bartlett et al., Bland et al. a Kautz et al. Metodika: Aplikován byl design přirozeného experimentu u dvou skupin studentů s použitím pretestu a posttestu. Výzkumné nástroje byly nově pro tuto studii připraveny na podkladě publikovaných článků Herman a Bartlett. Jednalo se o pracovní protokoly pro studenty a nástroj k hodnocení erudice klinické rozvahy. Proběhla obsahová validizace protokolů a u hodnotícího nástroje byla ověřena reliabilita. Sběr dat proběhl u souboru šedesáti tří studentů formou záznamů do pracovních protokolů. Data, extrahovaná z protokolů, byla zpracována na hladině signifi kance 0,05 pomocí Studentova párového t-testu a Wilcoxonova párového testu. Výsledky: Medián celkového skóre erudice klinické rozvahy byl, při maximálním dosažitelném počtu 29 bodů, u studentů prezenční formy v pretestu 18,9 a v posttestu 19,5 bodů. U kombinované formy byl před a po cvičení 18,0 a 17,0 bodů. Závěry: U prezenční formy studia byla erudice klinické rozvahy s pojmy Aliance 3N po procvičování zlepšena, ale rozdíl nebyl statisticky signifi kantní. U kombinované formy studia prokázal Wilcoxonův párový test signifi kantní zhoršení. Využití NANDA diagnóz bylo u obou skupin subjektů explicitní, zásadní nedostatky byly shledány ve volbě optimálních NOC a NIC., Main aim: To determine the effect of practicing nursing process by OPT model of clinical reasoning on the expertise of students of General Nursing study in the use of concepts Alliance 3N. Background: The motive for the study was the work of the OPT model of clinical reasoning authors Pesut and Herman and research articles on the OPT model, the authors Bartlett et al., Bland et al. and Kautz et al. Methods: Applied design was a natural experiment on two groups of students using pretest and posttest. Research instruments for this study were new and prepared on the basis Herman and Bartlett’s published articles. Th ese were working protocols for students and a tool to assess erudition of clinical reasoning. Content validation was carried out and evaluation tool was tested on reliability. The data were collected in a group of sixty-three students in the form of records to work protocols. The data extracted from the protocols were processed at 0.05 level of signifi cance using Student’s paired t -test and Wilcoxon paired test. Results: The median of total score sheet clinical erudition was at full-time students in the pretest (before practicing) 18.9 points aft er exercise 19.5 (posttest). The median of total score before and aft er exercise was 18.0 and 17.0 points at part-time study students. Conclusions: For full-time students’ erudition in clinical reasoning with 3N Alliance terms aft er exercises has improved, but the diff erence was not statistically signifi cant. For part-time study students Wilcoxon paired test showed a signifi cant deterioration in student achievement. Úvod: Cílem práce je identifikovat pacienty operované pro vývodový karcinom pankreatu, kteří přežili více než 5 let a analyzovat jejich charakteristiky a faktory s vlivem na přežití. Zkušenosti a výsledky terciárního centra. Metoda: Prospektivně vedená databáze 155 pankreatických resekcí v období leden 2006 až červen 2010 byla prozkoumána se záměrem identifikace pacientů operovaných kurativně pro vývodový karcinom pankreatu. Klinická a patologická data byla analyzována retrospektivně. Kaplan-Meierova analýza byla použita pro hodnocení přežívání, Log-rank test a Coxova regresní analýza pro analýzu faktorů s vlivem na přežití. Testy byly dělány na hladině signifikance 0,05. Jako nezávisle proměnné v Coxově regresní analýze byly vybrány faktory, u nichž byl pomocí Kaplan-Meierovy analýzy prokázán signifikantní vliv na přežívání. Výsledky: V sestavě 155 resekcí pankreatu s kurativním záměrem bylo identifikováno 73 nemocných operovaných pro vývodový karcinom pankreatu, u nichž bylo možné hodnotit 5leté přežití. Více než 5 let přežilo 15 nemocných (20,5 %), z nichž 13 dosud žije. Průměrná doba přežití ve skupině přeživších 5 let je 77,1 měsíce (60–110) a medián 74 měsíce, průměrná doba přežití bez nemoci (relaps free survival) je 63,3 měsíce (14−110), medián 62 měsíce. Ze sledovaných faktorů statisticky významný vliv na přežití měly: nepřítomnost infiltrace lymfatických uzlin (p=0,031), nekomplikovaný pooperační průběh (p=0,025), nepřítomnost vaskulární invaze v tumoru (p=0,017), léčba bez nutnosti podání transfuze krve (p=0,015) a pooperační onkologické léčba – převážně chemoterapie (p=0,009). Významnými nezávislými prediktory přežívání jsou: přítomnost angioinvaze HR=2,239 (95%CI: 1,093−4,590; p=0,028), pooperační chemoterapie HR=2,587 (95%CI: 1,301−5,145; p=0,007) a nutnost podání transfuze HR=2,080 (95%CI: 1,027−4,212; p=0,042). Přítomnost angioinvaze zvyšuje riziko úmrtí pacienta 2,2x, podání transfuze zvyšuje riziko úmrtí 2,1x a nepodání chemoterapie zvyšuje riziko úmrtí 2,6x. Závěr: Vliv na dlouhodobé přežití u nemocných operovaných pro karcinom hlavy pankreatu byl prokázán u těchto faktorů: nepřítomnost infiltrace lymfatických uzlin, nekomplikovaný pooperační průběh, nepřítomnost vaskulární invaze v tumoru, léčba bez nutnosti podání transfuze krve a pooperační onkologická léčba – chemoterapie. Významnými nezávislými prediktory přežívání jsou: vaskulární invaze, chemoterapie a nutnost podání transfuze., Introduction: The purpose was to identify 5-year survivors among a group of radically resected patients with pancreatic cancer and analyse the characteristics and factors associated with their 5-year survival. Single tertiary centre experience. Method: A prospectively maintained database of 155 pancreatic resections from January 2006 to June 2010 was scanned to identify patients after curative radical resections for pancreatic ductal adenocarcinoma. The clinical and pathological data was analysed retrospectively. The outcomes of the PDAC group were evaluated using Kaplan-Meier analysis (survival) with the Log-rank test and Cox regression analysis (evaluation of prognostic factors). Characteristics of the survivors were discussed. Significance level of 0.05 was used. Those factors were used as independent variables for Cox regression analysis whose significant effect on survival was shown based on Kaplan-Meier analysis. Results: Among 155 patients undergoing a curative pancreatic resection, 73 had a pancreatic ductal adenocarcinoma. Fifteen patients (20.5%) after radical surgery survived over 5 years, 13 of whom are still alive. In the group of the survivors, the mean overall survival was 77.1 months (60−110) and the median survival was 74 months. The mean relapse-free interval in the group of the survivors was 63.3 months (14−110) with the median of 65 months. Factors associated with a longer survival included the absence of lymph node infiltration (p=0.031), uncomplicated postoperative course (p=0.025), absence of vascular invasion (p=0.017), no blood transfusions (p=0.015) and the use of postoperative therapy – predominantly chemotherapy (p=0.009). Significant independent predictors of survival included vascular invasion HR=2.239 (95%CI: 1.093−4.590; p=0.028), postoperative chemotherapy HR=2.587 (95%CI: 1.301−5.145; p=0.007) and blood transfusion HR=2.080 (95%CI: 1.027−4.212; p=0.042). The risk of death was increased 2.2 times in patients with vascular invasion, 2.1 times in patients with transfusions, and finally 2.6 times in those with no chemotherapy. Conclusion: Factors associated with an improved overall survival included: the absence of lymph node infiltration, an uncomplicated postoperative course, absence of vascular invasion, no need of blood transfusions, and finally the use of postoperative chemotherapy.
