BACKGROUND: Human immunodeficiency virus (HIV) continues to be a serious health issue and one of the world most devastating epidemics. An estimated 1.5 million people died from AIDS-related illnesses in 2013, and an estimated 37 million people with AIDS have died worldwide since the epidemic has begun. HIV infection is known for its oral manifestations which causes discomfort and pain for infected individuals. The objective of this study was to document oral conditions of HIV positive patients and the pattern and frequency of oral and dental lesions. METHODS: All patients with confirmed HIV infection who were treated at the Department of Dentistry, University Hospital in Hradec Králové, were examined. RESULTS: During the study period, 29 HIV positive patients were examined and treated--19 men, 10 women, with mean age of 32.9 years (range 22-58 years). 72.41% patients received ART. In total, all patients underwent 186 visits. The most frequent treatments were associated with teeth and periodontal lesions (71.80%), oral mucosal lesions were diagnosed and treated only in 3.96% cases. BACKGROUND: The goal of this prospective study was to determine the frequency of micrometastases in patients with squamous cell carcinoma (SCC) of the oral cavity, pharynx and larynx in whom elective neck dissection was indicated (cN0). PATIENTS AND METHODS: A total of 12 patients (10 males and 2 females) were enrolled in the study. The age ranged 42-73 years (median 62 years). Elective neck dissection was performed in all patients (8 ipsilateral, 4 bilateral) and a total of 256 lymph nodes were removed and sent for microscopic examination. RESULTS: The presence of tumor cells in cervical lymph nodes was found in 5/12 (42%) patients. Micrometastases of SCC were found in two patients and isolated tumor cells (ITC) in two other patients. In the remaining one patient with oropharyngeal SCC, a micrometastasis of papillary thyroid carcinoma (PTC) was detected. Positive lymph nodes were localized in level II in three patients with SCC of larynx, hypopharynx and tongue base, respectively, in level I in one patient with SCC of oral tongue and in level III in one patient with PTC. CONCLUSION: Our results indicate that SCC of head and neck has a high potential for creating micrometastases which frequency is higher compared to clinically detected macrometastases. AIM: The purpose of this study was to develop a revised version of the Brief Bedside Dysphagia Screening Test for determining penetration/aspiration risk in patients prone to dysphagia. The priority was to achieve high sensitivity and negative predictive value. METHODS: The study screeners conducted bedside assessment of the swallowing function in 157 patients with a neurological (mainly stroke) or an ear, nose, and throat diagnosis (mainly head and neck cancer). The results were compared with a gold standard, flexible endoscopic examination of swallowing. RESULTS: For the neurological subgroup (N = 106), eight statistically significant bedside assessment items were combined into the Brief Bedside Dysphagia Screening Test-Revised (BBDST-R). Cut-off score 1 produced the highest sensitivity (95.5%; 95% confidence interval CI [CI]: 84.9-98.7%) and negative predictive value (88.9%; 95% CI 67.2-96.9%). Úvod: Je uváděno, že prevalence neurodegenerativních onemocnění v malých evropských komunitách izolovaných od okolí je vyšší ve srovnání s běžnou populací. Tentýž jev jsme pozorovali v malém, relativně izolovaném regionu jihovýchodní Moravy. Cíl: Zjištění prevalence neurodegenerativního parkinsonizmu v izolovaném regionu jihovýchodní Moravy. Metodika: Byla použita metoda třífázového vyšetření. V první fázi byly do ordinací praktických lékařů distribuovány dotazníky, které byly vyplněny všemi pacienty, kteří se z jakéhokoliv důvodu dostavili v průběhu tří měsíců do ordinace praktického lékaře. Ve druhé fázi byly u pozitivních respondentů příznaky parkinsonizmu objektivizovány vyšetřením edukovaným ambulantním neurologem. Ve třetím stupni byli respondenti, u kterých byly při skríningovém vyšetření příznaky parkinsonizmu objektivizovány, přijati k detailnímu vyšetření za hospitalizace v terciárním centru pro diagnostiku a léčbu neurodegenerativních onemocnění. Výsledky: Celková prevalence parkinsonizmu v populaci starší 50 let byla 2,8 % (95% CI: 2,2-3,4); prevalence v populaci 50-64 let byla 1,9 % (95% CI: 1,2-2,5); a v populaci nad 65 let byla 4,06 % (95% CI: 2,9-5,1). Byly získány tři velké rodokmeny s patrnou autozomálně dominantní dědičností parkinsonizmu. Závěr: Zjištěná prevalence byla překvapivě vysoká a podstatně odlišná od hodnot prevalence publikovaných v jiných evropských zemích. Vzhledem k charakteristice regionu je předpokládán vliv genetických faktorů. Současný výzkum je zaměřen na genetické pozadí a DNA analýzu probandů z rodokmenů, ve kterých byla identifikována autozomálně dominantní dědičnost parkinsonizmu. We recently observed this phenomenon in a small specific region of south-eastern Moravia. Objective: To assess the prevalence of neurodegenerative parkinsonism in an isolated region with a rural population in south-eastern Moravia. Methods: A three-stage method of data collection was used. In the first phase, originally designed questionnaires were distributed to general practitioners and completed by all patients who visited them for any reason during a three-month period. In the second phase, positive responders were examined by trained primary care neurologists. Finally, the diagnosis was confirmed or excluded by a movement disorders specialist. Results: The overall prevalence in the population older than 50 years of age was 2.8% (95% CI: 2.2-3.4); the prevalence in the population from 50 to 64 years was 1.9% (95% CI: 1.2-2.5), and it was 4.06% (95% CI: 2.9-5.1) in the population over 65 years of age. Three large families with an autosomal-dominant inheritance patterns of parkinsonism were identified. Conclusions: The prevalence rates were surprisingly high; they substantially differed from the published prevalence rates in other European countries. Due to the characteristics of the region, we expected a particular impact of genetic factors, most probably the autosomal-dominant inheritance of parkinsonism. V databázi Globocan 2008 (IARC) bylo za rok 2008 z celosvětového počtu 12,663 milionů nových nádorů odhadováno 2,445 milionů v EU, ze 7,565 milionů zemřelých téměř 1,235 milionů v EU a z pětileté prevalence 28,8 milionů asi 6,617 milionů v EU. Početnější prevalence se týkala ve světě 18 % nádorů prsů, 11 % kolorekta, 11 % prostaty, 6 % plic, 5,5 % žaludku, 5,5 % cervixu, 4 % měchýře a 4 % dělohy, v EU 20 % ZN prsů, 14 % kolorekta, 18 % prostaty, 5 % plic, 4 % melanomu, 3 % ledvin, 3 % dělohy a 3 % NH lymfomu. V roce 2030 mohou počty nových nádorů dosáhnout ve světě 21 milionů případů, v EU 3 miliony, v ČR 93 tisíc (dg. C00-D09). Z předběžných celkových výdajů 288 572 mil. Kč na zdravotnictví za rok 2011 uhradily zdravotní pojišťovny 217 653 mil., z toho 19 217 mil. na onkologii. V letech 1990–2011 vzrostly celkové náklady na zdravotnictví o 257 534 mil. Kč s podílem 7,58 % na hrubém domácím produktu. Průměrné výdaje VZP na léčbu jednoho nádoru v roce 2010 dosáhly 19 030 Kč s nejvyššími náklady u novotvarů mízní a krvetvorné tkáně, mozku a CNS, trávicího ústrojí, úst a hltanu, dýchacích cest. Výkonnost globální ekonomiky nedosahuje potřebných prostředků na zdravotnictví. Český veřejný dluh překračuje 1 680 miliard Kč v době dokončení tohoto článku. I když celkové výdaje obyvatel na zdravotní péči rostou, zdá se, že jeho pokračující zadlužování zvýší také dluhy nemocnic a zdravotních pojišťoven. Here preliminary total health care expenditures in 2011 were 288,572 million CZK; of these, health insurance companies covered 217,653 million, with 19,217 million per one cancer treatment. Between 1990 and 2011, the total health expenditures increased by 257,534 million CZK, accounting for 7.58 % of the gross domestic product. In 2010 the General Health Insurance Company paid on average 19,030 CZK for the treatment of one cancer case, with the highest expenditures for neoplasms of the lymphoid and haemotopoietic tissues, brain and central nervous system, digestive organs, oral cavity and pharynx, and respiratory organs. The global economic performance cannot provide enough resources for public health care. The Czech public debt exceeded 1,680 billion CZK at the time this article was written. Although out-of-pocket money paid by cancer patients for treatment is growing, the continuing public indebtedness is likely to increase debts of hospitals and health insurance companies. Z 99 378 nových karcinomů kolorekta (KRK) u mužů a 78 977 u žen, evidovaných v registru nádorů ČR v letech 1976–2005, bylo 14 860 (15 %) KRK u mužů a 11 142 (14,1%) u žen spojeno s výskytem jiných zhoubných novotvarů (ZN). Po 6 504 primárních KRK u mužů následovalo 7 406 ZN, po 4 342 KRK u žen 4 798 následných ZN. Naopak 8 356 následným KRK u mužů předcházelo 10 152 primárních ZN, 6 800 KRK u žen předcházelo 7 829 ZN. Počet vícečetných KRK se zvýšil ze 141 v roce 1976 na 1 534 v roce 2005. V letech 1996–2005 dosáhly primární KRK 57 % časných, 30 % pokročilých a 13 % neznámých stadií, následné KRK 43,5 %, 34 % a 22,5 %. Vícečetné KRK byly u mužů nejpočetnější ve věku 50–69 let, u žen 70–79 let. Průměrná doba vzniku následného novotvaru po primárním KRK u mužů byla 5,1 roku, u žen 5,4 roku. Po primárních KRK byly nejpočetnější ZN GIT, kůže, pohlavních orgánů a močového systému. Ze synchronních ZN byla téměř polovina nádorů trávicích cest. Z 12 204 následných ZN bylo 19 % zjištěno v pokročilých stadiích z toho asi polovina ZN trávicích cest. Víc než třetinu primárních nádorů před 17 981 následnými KRK tvořily karcinomy kůže. U 1 715 (26,4 %) mužů a 1 206 (27,8 %) žen, přežívajících v roce 2007 s primárním KRK, byl vyšší podíl časných stadií než s následným KRK; naopak u 6 646 (79,5 %) mužů a 5 291 (77,8 %) žen, zemřelých v roce 2007 s následným KRK, byl vyšší podíl pokročilých stadií než s primárním KRK. Z uvedených dat se odhaduje, že z 8 265 KRK hlášených v roce 2010 bylo téměř 1 700 (21 %) vícečetných KRK, z toho 260 primárních a 1440 následných případů. Každé páté onemocnění KRK bylo spojeno s výskytem dalšího ZN. I když je obtížné odhadnout počet vícečetných případů mezi 53 340 přežívajícími s KRK v roce 2010, jejich pokračující trend zvyšuje zátěž diagnostických a léčebných kapacit., Among the total of 99,378 new colorectal cancers (CRCs) in males and 78,977 in females, based on the Czech Cancer Registry in 1976–2005, there were 14,860 (15 %) CRCs in males and 11,142 (14.1 %) in females associated with the other malignant neoplasms (MNs). The primary 6,504 CRCs in males were followed by 7,406 subsequent MNs, the primary 4,342 CRCs in females by 4,798 subsequent MNs. On the contrary before the subsequent 8,356 CRCs in males were registered 10,152 primary MNs, 6,800 CRCs in females 7,829 MNs. The number of multiple CRCs increased from 141 in 1976 to 1,534 in 2005. In 1996–2005 the primary CRCs reached 57 % of early, 30 % advanced and 13 % unknown clinical stages, respectively the subsequent CRCs of 43.5 %, 34 % and 22.5 %. The occurrence of all multiple CRCs were highest in the age group of 50–69 years in males, and of 70–79 years in females. The average interval between primary CRCs and subsequent MNs was 5.1 years in males and 5.4 years in females. The most frequent MNs following primary CRCs were cancers of the gastrointestinal tract, skin, genital organs and urinary tract. Nearly half of synchronous MNs after primary CRCs were cancers of the gastrointestinal tract. There were 19 % cases diagnosed at advanced stages from the total 12,204 subsequent MNs, one half of those in patients with gastrointestinal cancers. More than one third of primary MNs preceded by 17,981 subsequent CRCs were cancers of the skin. Among 1,715 (26.4 %) males and 1,206 (27.8 %) females survived up to 2007 with primary CRCs, there were higher early stages than with subsequent CRSc; on the contrary among 6,646 (79.5 %) males and 5,291 (77.8 %) females, died up to 2007 with subsequent CRCs, were higher advanced stages than with primary CRCs. By the above mentioned data is estimated that among a total of 8,265 CRCs registered in 2010 were nearly 1,700 (21 %) multiple CRCs, i.e. 260 primary and 1,440 subsequent cases. Every fifth CRC was associated with the occurrence of the other MNs. BACKGROUND: Though mid-term survival rates of over 95% in several series have been published, there is still a paucity of related literature regarding the role of vertical stem instability in the osteointegration of fluted tapered stems. This paper presents a comprehensive and prospective assessment on short-term experiences with uncemented modular femoral stem in the treatment of defective femur during revision surgery of total hip replacement. MATERIALS AND METHODS: Clinical and radiological monitoring of 20 consecutive patients with implanted tapered fluted revision stem (Lima Corporate, Udine, Italy) was of 27 months in average (20-35 months). The average pre-operative Merle d'Aubigné and Postel method score was 6.3 points (3-10 points). The frequency of femur defects, classified according to Paprosky, was IIIA = 9 and IIIB = 11. RESULTS: During last follow-up, the Merle d'Aubigné and Postel hip score was on average 11.7 (6-16 points). Compared to post-operation radiograph, stem migration of 1.9 mm (0-11 mm) on average was found. This vertical stem migration was observed only when comparing hip radiographs immediately after surgery, and at 6 weeks post-surgery. The Paprosky IIIA defects group, presented a subsided stem by an average of 1.5 mm. In the group of Paprosky IIIB defects, the stem subsidence was on average 2.3 mm. All 20 patients in the study showed excellent osteointegration of the uncemented revision modular stem. CONCLUSIONS: This study found and excellent osteointegration of the Lima uncemented tapered fluted revision modular stem in defective femur with a cortical bone segment present in the diaphyseal isthmus area. Obezita je známým rizikovým faktorem pro řadu zdravotních problémů v dospělosti. Cílem výzkumu bylo zjistit prevalenci nadměrné hmotnosti a obezity u 6–7letých dětí na základních školách v Olomouckém kraji v roce 2014 a vyhodnotit přístup rodičů k programu, který je zaměřen na primární prevenci obezity u dětí. Výzkumný soubor zahrnuje 337 dětí (chlapci n=172, dívky n=165), u kterých byl hodnocen nutriční stav pomocí metod standardizované antropometrie a metodou bioelektrické impedance (BIA). Pro každé dítě byl vypočítán BMI a následně bylo zařazeno do příslušného percentilového pásma BMI. Rodiče obdrželi písemné vyrozumění o hodnocení nutričního stavu svého dítěte a měli možnost informovat se o jeho léčbě v případě zjištění nadměrné hmotnosti nebo obezity. K vyhodnocení dat byl použit neparametrický Mann a Whitney U-test, nepárový t-test a test rozdílů mezi dvěma poměry. Nadměrná hmotnost byla zjištěna u 7,56 % chlapců a 5,45 % dívek, obezita u 8,14 % chlapců a 5,45 % dívek. Na základě analýzy tělesného složení pomocí BIA bylo podle procentuálního množství tukové tkáně zařazeno do kategorie obezita 10,47 % chlapců a 7,88 % dívek. Monitorování prevalence nadměrné hmotnosti a obezity u dětí v Olomouckém kraji ukázalo na růst obezity u 6–7letých chlapců a dívek a na nedostatečný zájem rodičů o aktivní zapojení do preventivních programů v boji proti obezitě., Obesity is an established risk factor for numerous health problems in adulthood. The aim of the research was to determine the prevalence of overweight and obesity in children aged 6 to 7 in elementary schools in the Olomouc region in 2014, and to assess the attitude of the parents to the programme which focuses on the primary prevention of obesity in children. The research group includes 337 children (boys n=172, girls n=165) whose nutritional status was evaluated using the methods of standardized anthropometry and bioelectrical impedance analysis (BIA). The BMI of each child was calculated and subsequently categorized within the respective BMI percentile range. The parents received a written notification of the nutritional status of their child, and those whose children were diagnosed with overweight or obesity could request information about treatment. The data were evaluated using the non-parametric Mann-Whitney U test, unpaired t-test, and the test of differences between two quantities. Overweight was found in 7.56% of boys and 5.45% of girls, while obesity in 8.14% of boys and 5.45% of girls. Based on the BIA analysis of body composition, depending on the percentage amount of adipose tissue, 10.47% of boys and 7.88% of girls were classified as obese. Cíl: Popsat profil nemocných s akutním srdečním selháním (ASS) z pohledu přednemocniční péče v České republice. Metodika: Data pochází z registru Zdravotnické záchranné služby (ZZS) Jihomoravského kraje z období leden 2010–prosinec 2011. Byl proveden konsekutivní sběr nemocných s podezřením na ASS. Vyloučeni byli nemocní po kardiopulmonální resuscitaci nebo zemřelí před příjezdem ZZS nebo pacienti s dominantně extrakardiální příčinou dušnosti. Srovnáni jsou nemocní transportovaní do nemocnice oproti pacientům léčeným ambulantně. Cílovým parametrem je 30denní celková mortalita. Výsledky: Z celkového počtu 131 798 nemocných bylo podezření na ASS vysloveno ve 1 964 (1,5 %) případech. Konečná diagnóza ASS byla nakonec stanovena u 1 310 pacientů, kteří byli transportováni do nemocnice s předpokladem hospitalizace. Ambulantně bylo ošetřeno 35 nemocných. Průměrný věk celé populace byl 78 let, nejčastější komorbiditou byla hypertenze (67,0 %), u téměř poloviny jsou do doby předání do nemocnice přítomny známky plicního edému (46,4 %). Ambulantně léčení nemocní jsou častěji muži (71,4 % vs 49,1 %), mají menší výskyt komorbidit a nevykazují známky těžší respirační insuficience (kyslíková saturace 96 % oproti 87 %, žádné známky plicního edému). Nejčastěji aplikovaným lékem v přednemocniční péči u pacientů s podezřením na ASS je furosemid (74,0 %). Jednoměsíční mortalita souboru dosahovala 20,7 %. Závěr: ASS představuje 1,5 % všech volání ZZS pro akutní potíže, většina nemocných je transportována do nemocnice s předpokladem hospitalizace. Časná mortalita nemocných je vysoká. Methods: Registry data of the Emergency Medical System (EMS) of the South Moravian region of Czech Republic were used. Patients with suspected AHF were enrolled consecutively during the period of January 2010–December 2011. Excluded were patients who died or had cardiac arrest before the arrival of EMS and those who presented with dyspnea of dominantly extra-cardiac etiology. We compared patients who were transported to the hospital with those treated in an outpatient setting. Overall mortality within the first 30 days after the initial EMS contact was assessed as a main endpoint. Results: From the 131,798 of patients who called EMS, 1,964 (1.5%) of cases were suspected of having AHF. The diagnosis of AHF was determined in 1,310 patients who were transported to the hospital; 35 were treated in Outpatients. The mean age of the study population was 78 years, the most common comorbidity was hypertension (67.0%) and almost one half of the study population had signs of pulmonary edema before being admitted to hospital (46.4%). Patients not transported to hospital were more frequently of male gender (71.4% vs. 49.1%), with lower incidence of comorbid conditions and did not have signs of respiratory distress (oxygen saturation 96% vs. 87% in those transported to the hospital; no signs of pulmonary edema). The most common drug used in pre‑hospital care in patients with suspected AHF was furosemide (74.0% patients). The 30-day mortality rate of the whole study population reached 20.7%. Conclusion: AHF represents 1.5% of all emergency calls due to acute symptoms. The vast majority of the patients are transported to the hospital with the assumption of hospitalization. Early mortality rates of these patients are high. INTRODUCTION: Annually more than 27,000 persons die of cancer in the Czech Republic and the overall incidence of malignancies is still increasing. These data shows the need for affordable and good follow-up care especially for patients without any cancer treatment due to irreversible progression of tumor. Currently the outpatient palliative cancer care gets more into the forefront. Prerequisite for a well working outpatient palliative care is cooperation with general practitioners and home health care agencies. The purpose of the so called program of palliative cancer care is to guide a patient in palliative cancer care and to improve the cooperation among health care providers. METHODS: During the period from January 2008 to October 2010 we evaluated in patient without any oncology treatment due to irreversible progression of tumor. RESULTS: In palliative outpatient clinic we treated 446 patients, 119 of them received home care services with average length of 27.8 days. 77 patients died at home, 51 in health facilities and 41 in inpatient hospice care. CONCLUSION: We present pilot study focusing on outpatient palliative cancer care which shows the real benefit from early indication of palliative cancer care.
