Cíl: Cílem této studie je poskytnout orientační normy k Addenbrookskému kognitivnímu testu (ACE‑<span class=""></span>R) a jeho jednotlivým kognitivním doménám pro českou populaci. Soubor a metodika: Do studie bylo zařazeno 143 osob (89 žen a 54 mužů) ve věku 55–<span class=""></span>89 let. Subjekty byly zástupci zdravé populace, bez onemocnění mozku, smyslové poruchy a závažného psychiatrického onemocnění, s MMSE nad 27 bodů. Osoby byly podrobeny detailnímu anamnestickému rozhovoru pro zjištění případných onemocnění mozku v minulosti a pro zjištění míry soběstačnosti v běžných denních činnostech. Osoby s poškozením mozku v anamnéze či porušenou soběstačností v běžných denních činnostech nebyly do studie zařazeny, stejně jako osoby s MMSE nižším než 27 bodů. Srovnání hodnot ACE‑<span class=""></span>R a jeho složek u mužů a žen bylo provedeno pomocí Mannova‑<span class=""></span>Whitneyova testu a srovnání hodnot u čtyř skupin daných věkem a vzděláním pomocí Kruskalova‑<span class=""></span>Wallisova testu. Výsledné p‑<span class=""></span>hodnoty byly korigovány na mnohonásobné testování užitím Bonferroniho korekce. Hraniční skóry byly vytvořeny na úrovni 2. a 7. percentilu. Výsledky: Byla prokázána negativní korelace s věkem (p < 0,001; r = –<span class=""></span>0,43) a pozitivní korelace s proměnnou vzdělání na celkový výsledek v testu (p < 0,001; r = 0,41) i v dosažených výsledcích některých jednotlivých kognitivních domén. Vliv pohlaví nebyl statistický prokázán. Hraniční skór pro všechny subjekty ve věku 55–<span class=""></span>89 na úrovni 2. percentilu je 74 bodů pro celkový skór v Addenbrookském kognitivním testu a 79 bodů na úrovni 7. percentilu. Závěr: Studie poskytuje orientační normy pro českou populaci pro Addenbrookský kognitivní test a nabízí bázi pro vytvoření kvalitních norem za předpokladu navýšení počtu subjektů., Aim: The aim of this study was to provide benchmark normal values for Addenbrooke’s cognitive examination (ACE-R) and its domains for the Czech population. Methods: The study included 143 healthy subjects (89 women and 54 men) aged 55–89 years, without brain injury, neurodegenerative disease, severe hearing or visual impairment and without a psychiatric disease, with MMSE above 27 points. Participants were interviewed in detail to ascertain any previous brain injury history and to determine the level of self-sufficiency in daily living activities. Individuals with a history of brain injury or impaired self-sufficiency were excluded from the study. ACE-R values and values for its domains in men and women were compared with Mann-Whitney test and values for the four age and education groups were compared using the Kruskal-Wallis test. P-values were corrected for multiple testing using Bonferroni correction. Results: Cut-off scores were set at 2nd and 7th percentile. Negative correlation with age (p < 0.001, r = –0.43) and positive correlation with education (p < 0.001, r = 0.41) were statistically significant for the overall test performance and also for the performance in individual cognitive domains except for the Attention and orientation domain. The effect of sex was not statistically significant. The cut-off score for the total score in Addenbrooke’s cognitive examination for all subjects aged 55–89 years is 74 points at the 2nd percentile and 79 points at the 7th percentile. Conclusion: The study suggests cut-off scores for the Czech population of ACE-R and provides the basis for the development of Czech norms for this test. Úvod: Cílem práce je zmapování aktuální situace v chirurgické léčbě karcinomu pankreatu v České republice získáním dat od poskytovatelů chirurgické léčby pomocí jednoduchého dotazníku a identifikace tzv. high volume center. Informace jsou získávány v zájmu organizace a plánování výzkumných záměrů na poli léčby karcinomu pankreatu. Metoda: Byla oslovena pracoviště, o nichž je známo, že se zabývají chirurgickou léčbou karcinomu pankreatu. Jednoduchý dotazník formuloval otázku na celkové počty resekcí pankreatu a samostatně pro dg. PDAC – C25 za poslední 2 roky (2014 a 2015), a otázky na využití diagnostických metod, aplikace neoadjuvance, hodnocení předoperačních rizik, možností rychlého peroperačního vyšetření histopatologem, aplikace Leeds protokolu, sledování morbidity a mortality včetně dlouhodobých výsledků a způsobu pooperačního sledování a léčby. Byl osloven ÚZIS se žádostí o analýzu četnosti vykázaných kódů pro výkony DPE, LPE, TPE a totéž ve vazbě na dg. C 25 za poslední 2 dostupné roky v celé ČR (2013, 2014). Výsledky: Celkem bylo osloveno 19 pracovišť identifikovaných předchozím auditem, která uvedla více než 10 resekčních výkonů na pankreatu za rok. Tři pracoviště na oslovení nereagovala a zbývajících 16 svá data ochotně poskytla, 13 z nich pak zcela kompletní. Závěr: Nadpoloviční většina potenciálně radikálních výkonů pro PDAC v České republice je provedena na šesti pracovištích. Všechna pracoviště, která se do dotazníkového průzkumu zapojila, mají k dispozici a sledují své výsledky., Introduction: The aim is to map the current situation in the surgical treatment of pancreatic cancer in the Czech Republic. This information has been obtained from surgical treatment providers using a simple questionnaire and by identifying the so called high volume centres. The information has been collected in the interest of organizing and planning research projects in the field of pancreatic cancer treatment. Method: We addressed centres known to provide surgical treatment of pancreatic cancer. A simple questionnaire formulated one question about the total number of pancreatic resections, also separately for the diagnoses PDAC – C25, in the last two years (2014 and 2015). Other questions focused on the use of diagnostic methods, neoadjuvant therapy, preoperative assessment of risks, the possibility of rapid intraoperative histopathology examination, Leeds protocol, monitoring of morbidity and mortality including long-term results, and the method of postoperative follow-up and treatment. ÚZIS (Institute of Health Information and Statistics of the Czech Republic) was addressed with a request to analyze the frequency of reported total numbers for DPE, LPE, TPE and to do the same with respect to diagnosis C 25 for the last two years, available for the entire Czech Republic (2013, 2014). Results: Altogether 19 institutions were identified by the preceding audit, which reported more than 10 pancreatic resections annually; these institutions were addressed with the questionnaire. Sixteen institutions responded to the questions, 13 of them completely. Conclusion: The majority of potentially radical surgeries for PDAC in the Czech Republic are carried out at 6 institutions. V důsledku globálního oteplování se stává šíření invazivních druhů komárů vážným zdravotnickým problémem. Mnohé z nich sají krev i na lidech a často se stávají nepříjemnými trapiči, četné druhy jsou však i významnými vektory lidských onemocnění. Z druhů sajících na člověku byly v Evropě zjištěny Aedes aegypti, Ae. albopictus, Ochlerotatus japonicus a Oc. koreicus, které byly do Evropy zavlečeny z východní a jihovýchodní Asie, a severoamerické druhy Oc. atropalpus a Oc. triseriatus. Aedes albopictus se již vyskytuje na rozsáhlém území především jižní a jihovýchodní Evropy a jeho výskyt je na řadě míst masivní, rovněž druhy Oc. japonicus a Oc. koreicus jsou na některých lokalitách již usazeny. Aedes aegypti a Ae. albopictus jsou nebezpečnými vektory původců onemocnění přenosných i na člověka. Za potenciální vektory je však třeba považovat i zbývající druhy., In connections with global warming become spreading of invasive mosquitoes species serious health problem. Aedes albopictus occur on the large territory mainly in the South and East South Europe and his presence is on some localities massive. Úvod: V České republice (ČR) jsou pacienti s diagnózou diabetes mellitus (DM) sledováni a léčeni převážně u specialistů (přibližně 80 % v odborné diabetologické ambulanci), menší část praktickými lékaři (do 20 %). Doposud nebyl zhodnocen dlouhodobý vývoj změn preskripce metforminu a sulfonylurey v České republice a jeho konkordance s doporučenými postupy. Cíl: Porovnání vývoje preskripce metforminu (MET) a sulfonylurey (SU) v letech 2002–2006 s vývojem v letech 2010–2014 v reprezentativním vzorku populace pacientů s DM v ČR vedených v databázi Všeobecné zdravotní pojišťovny České republiky (VZP), u které bylo v roce 2014 pojištěno 63 % populace ČR. Metodika: V databázi VZP jsme identifikovali všechny osoby, které měly v letech 2002–2006 a 2010–2014 záznam o diagnóze DM (E10–E16 podle MKN 10) nebo předpis jakékoliv antidiabetické terapie (ATC skupina A10). Pro analýzu byl extrahován soubor osob, které měly v daném roce předepsanou nejméně jednou látku ze skupiny A10 (308 962 evidovaných v roce 2002 a 426 695 evidovaných v roce 2014). Pro každý rok byl vyhodnocen počet osob, které měly předepsán nejméně jednou MET nebo SU. Počet osob léčených MET nebo SU byl pak vyjádřen v procentech ze všech, kteří měli v daném roce předepsanou jakoukoliv terapii ze skupiny A10. Výsledky: Preskripce metforminu stoupá lineárně ze 43 % na 77 %, preskripce sulfonylurey se lineárně snižuje z 65 % na 37 %. Závěr: Naše analýza je prvním zhodnocením vývoje preskripce metforminu v ČR a zhodnocením její konkordance s doporučenými postupy pro léčbu DM. Preskripce metforminu v ČR se zvýšila mezi roky 2002–2014 ze 43 % na 77 % a preskripce SU se snížila z 65 % na 37 %. Tento vývoj a aktuální poměr preskripce MET a SU je potvrzením implementace doporučených postupů do praxe a důkazem vysoké kvality péče o pacienty s DM2T v České republice. Klíčová slova: diabetes mellitus 2. typu – metformin – sulfonylurea, Introduction: In the Czech Republic, patients with diabetes mellitus (DM) are followed and treated predominantly by specialists (approx. 80% at a specialist diabetology clinic), a minor part by general practitioners (up to 20%). Long-term development of the changes in prescribing metformin and sulfonylurea in the Czech Republic and its concordance with recommended procedures has not been evaluated until now. Goal: Comparison of the development of metformin (MET) and sulfonylurea (SU) prescriptions in the period of 2002–2006 with that of 2010–2014 in a representative sample of the patient population with DM kept in the database of the General Health Insurance Company of the Czech Republic (VZP) which provided health care coverage for 63% of Czech Republic population in 2014. Methodology: We identified all individuals in the VZP database who had a record of DM diagnosis (E10 – E16 based on ICD 10) or who had any antidiabetic therapy prescribed (ATC group A10) in the periods of 2002–2006 and 2010–2014. A cohort of patients was extracted for analysis, who had an agent from A10 group prescribed at least once in a relevant year (n = 308 962 in 2002; n = 426 695 in 2014). A number of patients was evaluated for each year, who had at least once MET or SU prescribed. The number of patients treated with MET or SU was then expressed as a percentage of all who had any therapy from A10 group prescribed in the year in question. Results: Metformin prescriptions have linearly risen from 43% to 77%, while sulfonylurea prescriptions have linearly decreased from 65% to 37%. Conclusion: The analysis presents the first evaluation of the development of metformin prescriptions conducted in the Czech Republic and evaluation of its concordance with the recommended procedures for the treatment of DM. The amount of metformin prescribed in the Czech Republic increased from 43% to 77% while the amount of SU prescribed decreased from 65% to 37% between 2002 and 2014. This development and the current ratio between the prescribed amounts of MET and SU demonstrate the implementation of the recommended procedures into practice and prove the high quality of care for patients with DM2T in specialists – diabetologists‘ surgeries. INTRODUCTION: Cellulitis remains a very serious disease even today. Mortality, which varied between 10-40%, has been reduced owing to the standard securing of airway patency and use of an appropriate surgical treatment approach. MATERIALS AND METHODS: A total of 195 patients were hospitalised for cellulitis at the University Hospital in Hradec Králové during 2007-2011. The following parameters were evaluated: age, gender, dependence of incidence of the disease on the season of the year, frequency of attacks of the particular areas and their clinical characteristics, aetiology of the inflammation, types of patient complaints, prevalence of current systemic diseases, results of microbiological and selected laboratory analyses, socio-economic status of the patients, and duration of patient stay at the hospital. Statistical analysis was performed by using Pearson's correlation coefficient, the statistical significance level was p < 0.05. RESULTS: The mean age of the patients was 39.8 years. The group of 195 patients included 108 (55%) males and 87 (45%) females. The mean time between the first symptoms of the disease and admission to the Department was 5 days. From among the 195 patients, 116 (59.5%) were working persons, 79 (40.5%) were non-working (children, students, unemployed persons, women on maternity leave, retired people). The odontogenic origin of the disease was verified in 173 (88.7%) patients. In total, 65 (33.3%) patients had no coinciding complicating systemic disease, 22 (11.3%) patients had diabetes mellitus. The most frequent symptom of cellulitis was painful swelling, found in 194 (99.5%) patients, followed by jaw contracture, found in 153 (78.5%) patients. CONCLUSION: The results are largely very similar to those of previous studies performed in other countries, except that we found no correlation between the prevalence of cellulitis and the socio-economic status, nor have we confirmed Klebsiella pneumoniae sp. as the cause of cellulitis in patients with diabetes mellitus. and H. BACKGROUND: Human immunodeficiency virus (HIV) continues to be a serious health issue and one of the world most devastating epidemics. An estimated 1.5 million people died from AIDS-related illnesses in 2013, and an estimated 37 million people with AIDS have died worldwide since the epidemic has begun. HIV infection is known for its oral manifestations which causes discomfort and pain for infected individuals. The objective of this study was to document oral conditions of HIV positive patients and the pattern and frequency of oral and dental lesions. METHODS: All patients with confirmed HIV infection who were treated at the Department of Dentistry, University Hospital in Hradec Králové, were examined. RESULTS: During the study period, 29 HIV positive patients were examined and treated--19 men, 10 women, with mean age of 32.9 years (range 22-58 years). 72.41% patients received ART. In total, all patients underwent 186 visits. The most frequent treatments were associated with teeth and periodontal lesions (71.80%), oral mucosal lesions were diagnosed and treated only in 3.96% cases. CONCLUSION: Since the introduction of ART, the frequency of oral mucosal lesions is minimal in patients with HIV infection. and M. Šembera, V. Radochová, R. Slezák
BACKGROUND: The goal of this prospective study was to determine the frequency of micrometastases in patients with squamous cell carcinoma (SCC) of the oral cavity, pharynx and larynx in whom elective neck dissection was indicated (cN0). PATIENTS AND METHODS: A total of 12 patients (10 males and 2 females) were enrolled in the study. The age ranged 42-73 years (median 62 years). Elective neck dissection was performed in all patients (8 ipsilateral, 4 bilateral) and a total of 256 lymph nodes were removed and sent for microscopic examination. RESULTS: The presence of tumor cells in cervical lymph nodes was found in 5/12 (42%) patients. Micrometastases of SCC were found in two patients and isolated tumor cells (ITC) in two other patients. In the remaining one patient with oropharyngeal SCC, a micrometastasis of papillary thyroid carcinoma (PTC) was detected. Positive lymph nodes were localized in level II in three patients with SCC of larynx, hypopharynx and tongue base, respectively, in level I in one patient with SCC of oral tongue and in level III in one patient with PTC. CONCLUSION: Our results indicate that SCC of head and neck has a high potential for creating micrometastases which frequency is higher compared to clinically detected macrometastases. AIM: The purpose of this study was to develop a revised version of the Brief Bedside Dysphagia Screening Test for determining penetration/aspiration risk in patients prone to dysphagia. The priority was to achieve high sensitivity and negative predictive value. METHODS: The study screeners conducted bedside assessment of the swallowing function in 157 patients with a neurological (mainly stroke) or an ear, nose, and throat diagnosis (mainly head and neck cancer). The results were compared with a gold standard, flexible endoscopic examination of swallowing. RESULTS: For the neurological subgroup (N = 106), eight statistically significant bedside assessment items were combined into the Brief Bedside Dysphagia Screening Test-Revised (BBDST-R). Cut-off score 1 produced the highest sensitivity (95.5%; 95% confidence interval CI [CI]: 84.9-98.7%) and negative predictive value (88.9%; 95% CI 67.2-96.9%). Úvod: Je uváděno, že prevalence neurodegenerativních onemocnění v malých evropských komunitách izolovaných od okolí je vyšší ve srovnání s běžnou populací. Tentýž jev jsme pozorovali v malém, relativně izolovaném regionu jihovýchodní Moravy. Cíl: Zjištění prevalence neurodegenerativního parkinsonizmu v izolovaném regionu jihovýchodní Moravy. Metodika: Byla použita metoda třífázového vyšetření. V první fázi byly do ordinací praktických lékařů distribuovány dotazníky, které byly vyplněny všemi pacienty, kteří se z jakéhokoliv důvodu dostavili v průběhu tří měsíců do ordinace praktického lékaře. Ve druhé fázi byly u pozitivních respondentů příznaky parkinsonizmu objektivizovány vyšetřením edukovaným ambulantním neurologem. Ve třetím stupni byli respondenti, u kterých byly při skríningovém vyšetření příznaky parkinsonizmu objektivizovány, přijati k detailnímu vyšetření za hospitalizace v terciárním centru pro diagnostiku a léčbu neurodegenerativních onemocnění. Výsledky: Celková prevalence parkinsonizmu v populaci starší 50 let byla 2,8 % (95% CI: 2,2-3,4); prevalence v populaci 50-64 let byla 1,9 % (95% CI: 1,2-2,5); a v populaci nad 65 let byla 4,06 % (95% CI: 2,9-5,1). Byly získány tři velké rodokmeny s patrnou autozomálně dominantní dědičností parkinsonizmu. Závěr: Zjištěná prevalence byla překvapivě vysoká a podstatně odlišná od hodnot prevalence publikovaných v jiných evropských zemích. Vzhledem k charakteristice regionu je předpokládán vliv genetických faktorů. Současný výzkum je zaměřen na genetické pozadí a DNA analýzu probandů z rodokmenů, ve kterých byla identifikována autozomálně dominantní dědičnost parkinsonizmu. Redakční rada potvrzuje, že rukopis práce splnil ICMJE kritéria pro publikace zasílané do biomedicínských časopisů., Introduction: It has been suggested that the prevalence of neurodegenerative diseases in small, isolated European communities might be higher than in the general population. We recently observed this phenomenon in a small specific region of south-eastern Moravia. Objective: To assess the prevalence of neurodegenerative parkinsonism in an isolated region with a rural population in south-eastern Moravia. Methods: A three-stage method of data collection was used. In the first phase, originally designed questionnaires were distributed to general practitioners and completed by all patients who visited them for any reason during a three-month period. In the second phase, positive responders were examined by trained primary care neurologists. Finally, the diagnosis was confirmed or excluded by a movement disorders specialist. Results: The overall prevalence in the population older than 50 years of age was 2.8% (95% CI: 2.2-3.4); the prevalence in the population from 50 to 64 years was 1.9% (95% CI: 1.2-2.5), and it was 4.06% (95% CI: 2.9-5.1) in the population over 65 years of age. Three large families with an autosomal-dominant inheritance patterns of parkinsonism were identified. Conclusions: The prevalence rates were surprisingly high; they substantially differed from the published prevalence rates in other European countries. Due to the characteristics of the region, we expected a particular impact of genetic factors, most probably the autosomal-dominant inheritance of parkinsonism.
