Cíl: Cílem této studie je poskytnout orientační normy k Addenbrookskému kognitivnímu testu (ACE‑<span class=""></span>R) a jeho jednotlivým kognitivním doménám pro českou populaci. Soubor a metodika: Do studie bylo zařazeno 143 osob (89 žen a 54 mužů) ve věku 55–<span class=""></span>89 let. Subjekty byly zástupci zdravé populace, bez onemocnění mozku, smyslové poruchy a závažného psychiatrického onemocnění, s MMSE nad 27 bodů. Osoby byly podrobeny detailnímu anamnestickému rozhovoru pro zjištění případných onemocnění mozku v minulosti a pro zjištění míry soběstačnosti v běžných denních činnostech. Osoby s poškozením mozku v anamnéze či porušenou soběstačností v běžných denních činnostech nebyly do studie zařazeny, stejně jako osoby s MMSE nižším než 27 bodů. Srovnání hodnot ACE‑<span class=""></span>R a jeho složek u mužů a žen bylo provedeno pomocí Mannova‑<span class=""></span>Whitneyova testu a srovnání hodnot u čtyř skupin daných věkem a vzděláním pomocí Kruskalova‑<span class=""></span>Wallisova testu. Výsledné p‑<span class=""></span>hodnoty byly korigovány na mnohonásobné testování užitím Bonferroniho korekce. Hraniční skóry byly vytvořeny na úrovni 2. a 7. percentilu. Výsledky: Byla prokázána negativní korelace s věkem (p < 0,001; r = –<span class=""></span>0,43) a pozitivní korelace s proměnnou vzdělání na celkový výsledek v testu (p < 0,001; r = 0,41) i v dosažených výsledcích některých jednotlivých kognitivních domén. Vliv pohlaví nebyl statistický prokázán. Hraniční skór pro všechny subjekty ve věku 55–<span class=""></span>89 na úrovni 2. percentilu je 74 bodů pro celkový skór v Addenbrookském kognitivním testu a 79 bodů na úrovni 7. percentilu. Závěr: Studie poskytuje orientační normy pro českou populaci pro Addenbrookský kognitivní test a nabízí bázi pro vytvoření kvalitních norem za předpokladu navýšení počtu subjektů., Aim: The aim of this study was to provide benchmark normal values for Addenbrooke’s cognitive examination (ACE-R) and its domains for the Czech population. Methods: The study included 143 healthy subjects (89 women and 54 men) aged 55–89 years, without brain injury, neurodegenerative disease, severe hearing or visual impairment and without a psychiatric disease, with MMSE above 27 points. Participants were interviewed in detail to ascertain any previous brain injury history and to determine the level of self-sufficiency in daily living activities. Individuals with a history of brain injury or impaired self-sufficiency were excluded from the study. ACE-R values and values for its domains in men and women were compared with Mann-Whitney test and values for the four age and education groups were compared using the Kruskal-Wallis test. P-values were corrected for multiple testing using Bonferroni correction. Results: Cut-off scores were set at 2nd and 7th percentile. Negative correlation with age (p < 0.001, r = –0.43) and positive correlation with education (p < 0.001, r = 0.41) were statistically significant for the overall test performance and also for the performance in individual cognitive domains except for the Attention and orientation domain. The effect of sex was not statistically significant. The cut-off score for the total score in Addenbrooke’s cognitive examination for all subjects aged 55–89 years is 74 points at the 2nd percentile and 79 points at the 7th percentile. Conclusion: The study suggests cut-off scores for the Czech population of ACE-R and provides the basis for the development of Czech norms for this test. BACKGROUND: Human immunodeficiency virus (HIV) continues to be a serious health issue and one of the world most devastating epidemics. An estimated 1.5 million people died from AIDS-related illnesses in 2013, and an estimated 37 million people with AIDS have died worldwide since the epidemic has begun. HIV infection is known for its oral manifestations which causes discomfort and pain for infected individuals. The objective of this study was to document oral conditions of HIV positive patients and the pattern and frequency of oral and dental lesions. METHODS: All patients with confirmed HIV infection who were treated at the Department of Dentistry, University Hospital in Hradec Králové, were examined. RESULTS: During the study period, 29 HIV positive patients were examined and treated--19 men, 10 women, with mean age of 32.9 years (range 22-58 years). 72.41% patients received ART. In total, all patients underwent 186 visits. The most frequent treatments were associated with teeth and periodontal lesions (71.80%), oral mucosal lesions were diagnosed and treated only in 3.96% cases. BACKGROUND: The goal of this prospective study was to determine the frequency of micrometastases in patients with squamous cell carcinoma (SCC) of the oral cavity, pharynx and larynx in whom elective neck dissection was indicated (cN0). PATIENTS AND METHODS: A total of 12 patients (10 males and 2 females) were enrolled in the study. The age ranged 42-73 years (median 62 years). Elective neck dissection was performed in all patients (8 ipsilateral, 4 bilateral) and a total of 256 lymph nodes were removed and sent for microscopic examination. RESULTS: The presence of tumor cells in cervical lymph nodes was found in 5/12 (42%) patients. Micrometastases of SCC were found in two patients and isolated tumor cells (ITC) in two other patients. In the remaining one patient with oropharyngeal SCC, a micrometastasis of papillary thyroid carcinoma (PTC) was detected. Positive lymph nodes were localized in level II in three patients with SCC of larynx, hypopharynx and tongue base, respectively, in level I in one patient with SCC of oral tongue and in level III in one patient with PTC. CONCLUSION: Our results indicate that SCC of head and neck has a high potential for creating micrometastases which frequency is higher compared to clinically detected macrometastases. AIM: The purpose of this study was to develop a revised version of the Brief Bedside Dysphagia Screening Test for determining penetration/aspiration risk in patients prone to dysphagia. The priority was to achieve high sensitivity and negative predictive value. METHODS: The study screeners conducted bedside assessment of the swallowing function in 157 patients with a neurological (mainly stroke) or an ear, nose, and throat diagnosis (mainly head and neck cancer). The results were compared with a gold standard, flexible endoscopic examination of swallowing. RESULTS: For the neurological subgroup (N = 106), eight statistically significant bedside assessment items were combined into the Brief Bedside Dysphagia Screening Test-Revised (BBDST-R). Cut-off score 1 produced the highest sensitivity (95.5%; 95% confidence interval CI [CI]: 84.9-98.7%) and negative predictive value (88.9%; 95% CI 67.2-96.9%). Úvod: Je uváděno, že prevalence neurodegenerativních onemocnění v malých evropských komunitách izolovaných od okolí je vyšší ve srovnání s běžnou populací. Tentýž jev jsme pozorovali v malém, relativně izolovaném regionu jihovýchodní Moravy. Cíl: Zjištění prevalence neurodegenerativního parkinsonizmu v izolovaném regionu jihovýchodní Moravy. Metodika: Byla použita metoda třífázového vyšetření. V první fázi byly do ordinací praktických lékařů distribuovány dotazníky, které byly vyplněny všemi pacienty, kteří se z jakéhokoliv důvodu dostavili v průběhu tří měsíců do ordinace praktického lékaře. Ve druhé fázi byly u pozitivních respondentů příznaky parkinsonizmu objektivizovány vyšetřením edukovaným ambulantním neurologem. Ve třetím stupni byli respondenti, u kterých byly při skríningovém vyšetření příznaky parkinsonizmu objektivizovány, přijati k detailnímu vyšetření za hospitalizace v terciárním centru pro diagnostiku a léčbu neurodegenerativních onemocnění. Výsledky: Celková prevalence parkinsonizmu v populaci starší 50 let byla 2,8 % (95% CI: 2,2-3,4); prevalence v populaci 50-64 let byla 1,9 % (95% CI: 1,2-2,5); a v populaci nad 65 let byla 4,06 % (95% CI: 2,9-5,1). Byly získány tři velké rodokmeny s patrnou autozomálně dominantní dědičností parkinsonizmu. Závěr: Zjištěná prevalence byla překvapivě vysoká a podstatně odlišná od hodnot prevalence publikovaných v jiných evropských zemích. Vzhledem k charakteristice regionu je předpokládán vliv genetických faktorů. Současný výzkum je zaměřen na genetické pozadí a DNA analýzu probandů z rodokmenů, ve kterých byla identifikována autozomálně dominantní dědičnost parkinsonizmu. We recently observed this phenomenon in a small specific region of south-eastern Moravia. Objective: To assess the prevalence of neurodegenerative parkinsonism in an isolated region with a rural population in south-eastern Moravia. Methods: A three-stage method of data collection was used. In the first phase, originally designed questionnaires were distributed to general practitioners and completed by all patients who visited them for any reason during a three-month period. In the second phase, positive responders were examined by trained primary care neurologists. Finally, the diagnosis was confirmed or excluded by a movement disorders specialist. Results: The overall prevalence in the population older than 50 years of age was 2.8% (95% CI: 2.2-3.4); the prevalence in the population from 50 to 64 years was 1.9% (95% CI: 1.2-2.5), and it was 4.06% (95% CI: 2.9-5.1) in the population over 65 years of age. Three large families with an autosomal-dominant inheritance patterns of parkinsonism were identified. Conclusions: The prevalence rates were surprisingly high; they substantially differed from the published prevalence rates in other European countries. Due to the characteristics of the region, we expected a particular impact of genetic factors, most probably the autosomal-dominant inheritance of parkinsonism. První epidemiologickou studii nevyšetřené dyspepsie v České republice provedla naše skupina v roce 2001. V roce 2011 jsme provedli novou prospektivní multicentrickou studii stejnou metodikou. V 22 centrech bylo zařazeno 1 836 osob (ve věku 5–98 let). Celková prevalence dlouhodobou dyspepsie byla 12 %, a to 3,5 % u lidí ve věku 5–24 let, 18 % mezi 25–64letými a 15 % u lidí ≥ 65 let. Přestože v posledních 10 letech došlo k významnému poklesu prevalence infekce Helicobacter pylori, prevalence a sociodemografické determinanty dyspepsie se signifikantně nezměnily., Epidemiology of uninvestigated dyspepsia was studied in the Czech Republic for the first time in 2001. The aim of our current multi-centre prospective study was to evaluate dyspepsia using the same methods in a representative sample of general unselected population from the same geographical areas 10 years later. A total of 22 centres entered the study. A total of 1,836 subjects (aged 5–98 years) were enrolled. The overall prevalence of dyspepsia was 12 %; namely 3.5 % in subjects aged 5–24 years, 18 % among 25–64-year-old persons and 15 % in subjects ≥ 65 years. Úvod: Vysoká incidence kolorektálního karcinomu představuje v České republice aktuální a populačně závažný zdravotnický problém. Polovina všech nemocných je věku nad 70 let. Možnosti chirurgické i nechirurgické léčby v této skupině závisejí na faktorech, jen minimálně vyjádřitelných zavedenou onkologickou a anesteziologickou klasifikací (cTcNcM, ASA). Cílem práce je ukázat vliv věku na využití léčebných modalit do protokolu léčby a na její výsledky a na jiné možnosti hodnocení únosnosti k léčbě. Metody: Analýza údajů z databáze NOR z pětiletého období 2008–2012 zpracované Institutem biostatistiky a analýz MU. Výsledky: Ve všech sledovaných parametrech byl prokázán rozdíl mezi nemocnými pod a nad 70 let v neprospěch starších. Ve věku pod 70 let nebylo léčeno 11,2 % nemocných, ve věku na 70 let 25,2 %. Závěr: K rozhodování o způsobu vedení léčby včetně volby operace by mohla přispět klasifikace pomocí komplexního geriatrického vyšetření., Introduction: High incidence of colorectal cancer in the Czech Republic is an actual and demographically significant health issue. Half of all of the patients is older than 70 years. Both surgical and non-surgical treatment options in this group of patients depend on factors that are difficult to measure only by current oncological and anesthesiological classifications (cTcNcM, ASA). The objective of this paper is to measure the impact of age on the use of various treatment modalities within the protocol and their results, and also to suggest alternative options for therapy tolerance assessment. Methods: Analysis of data over a five-year period from the NOR database prepared by the Institute of Biostatistics and Analyses, Masaryk University. Results: In all parameters a difference was demonstrated between patients below the age of 70 and those above the age of 70 years. Older patients were disadvantaged. Only 11.2% of patients younger than 70 years were not treated, whereas 25.2% over the age of 70 years were not treated. Vysoce rizikové typy lidských papilomavirů (HR-HPV) jsou kauzálně spojeny s karcinomem děložního hrdla (KDH) a částí dalších malignit. Současně dostupné HPV-vakcíny obsahují antigeny dvou HR HPV – HPV16 a HPV18 zodpovědných v ČR za více než 70% KDH. Cílem studie bylo sledování výskytu znaků HPV-infekce ve vakcinované populaci, detekce předchozí, současné a přetrvávající HPV-infekce a sledování vlivu vakcinace na následnou HPV-infekci a vznik atypických nálezů na děložním hrdle. Zde prezentujeme výsledky prevalence znaků HPV-infekce při vstupu do studie. Celkem jsme nalezli 37,4 % HPV-DNA-pozitivních jedinců, polovinu séropozitivních žen proti některému z testovaných antigenů a téměř čtvrtinu s protilátkami proti typům HPV16/18. Současnou/přetrvávající/předchozí infekci typy HPV16/18 jsme nalezli u 4,7 %/6,7 %/15,4 % žen. Snížením účinnosti vakcín proti HPV16/18 je v naší studii ohroženo 11,4 % sledovaných, s nejvyšším rizikem ve věkové skupině 24–26 let (22,6 %)., High-risk human papillomaviruses (HR HPV) are causally linked to cervical cancer (CC) and some other malignancies. The currently available HPV vaccines contain antigens of two HR HPV types, HPV16 and HPV18 that account for more than 70% of CC in the Czech Republic. The study objective was to monitor the prevalence of HPV infection markers in the vaccinated population, to detect past, present, and persistent HPV infection, and to assess the effect of vaccination on subsequent HPV infection and development of atypical cervical lesions. Here we present the baseline prevalence data on the of HPV infection markers at enrolment. Overall, 37.4% of the study cohort were HPV DNA positive, half of the females were seropositive to at least one of the antigens tested, and nearly one quarter had antibodies against HPV16/18 types. Present/persistent/past infection with HPV16/18 types was detected in 4.7%/6.7%/15.4% of the females, respectively. BACKGROUND: Though mid-term survival rates of over 95% in several series have been published, there is still a paucity of related literature regarding the role of vertical stem instability in the osteointegration of fluted tapered stems. This paper presents a comprehensive and prospective assessment on short-term experiences with uncemented modular femoral stem in the treatment of defective femur during revision surgery of total hip replacement. MATERIALS AND METHODS: Clinical and radiological monitoring of 20 consecutive patients with implanted tapered fluted revision stem (Lima Corporate, Udine, Italy) was of 27 months in average (20-35 months). The average pre-operative Merle d'Aubigné and Postel method score was 6.3 points (3-10 points). The frequency of femur defects, classified according to Paprosky, was IIIA = 9 and IIIB = 11. RESULTS: During last follow-up, the Merle d'Aubigné and Postel hip score was on average 11.7 (6-16 points). Compared to post-operation radiograph, stem migration of 1.9 mm (0-11 mm) on average was found. This vertical stem migration was observed only when comparing hip radiographs immediately after surgery, and at 6 weeks post-surgery. The Paprosky IIIA defects group, presented a subsided stem by an average of 1.5 mm. In the group of Paprosky IIIB defects, the stem subsidence was on average 2.3 mm. All 20 patients in the study showed excellent osteointegration of the uncemented revision modular stem. CONCLUSIONS: This study found and excellent osteointegration of the Lima uncemented tapered fluted revision modular stem in defective femur with a cortical bone segment present in the diaphyseal isthmus area. Obezita je známým rizikovým faktorem pro řadu zdravotních problémů v dospělosti. Cílem výzkumu bylo zjistit prevalenci nadměrné hmotnosti a obezity u 6–7letých dětí na základních školách v Olomouckém kraji v roce 2014 a vyhodnotit přístup rodičů k programu, který je zaměřen na primární prevenci obezity u dětí. Výzkumný soubor zahrnuje 337 dětí (chlapci n=172, dívky n=165), u kterých byl hodnocen nutriční stav pomocí metod standardizované antropometrie a metodou bioelektrické impedance (BIA). Pro každé dítě byl vypočítán BMI a následně bylo zařazeno do příslušného percentilového pásma BMI. Rodiče obdrželi písemné vyrozumění o hodnocení nutričního stavu svého dítěte a měli možnost informovat se o jeho léčbě v případě zjištění nadměrné hmotnosti nebo obezity. K vyhodnocení dat byl použit neparametrický Mann a Whitney U-test, nepárový t-test a test rozdílů mezi dvěma poměry. Nadměrná hmotnost byla zjištěna u 7,56 % chlapců a 5,45 % dívek, obezita u 8,14 % chlapců a 5,45 % dívek. Na základě analýzy tělesného složení pomocí BIA bylo podle procentuálního množství tukové tkáně zařazeno do kategorie obezita 10,47 % chlapců a 7,88 % dívek. Monitorování prevalence nadměrné hmotnosti a obezity u dětí v Olomouckém kraji ukázalo na růst obezity u 6–7letých chlapců a dívek a na nedostatečný zájem rodičů o aktivní zapojení do preventivních programů v boji proti obezitě., Obesity is an established risk factor for numerous health problems in adulthood. The aim of the research was to determine the prevalence of overweight and obesity in children aged 6 to 7 in elementary schools in the Olomouc region in 2014, and to assess the attitude of the parents to the programme which focuses on the primary prevention of obesity in children. The research group includes 337 children (boys n=172, girls n=165) whose nutritional status was evaluated using the methods of standardized anthropometry and bioelectrical impedance analysis (BIA). The BMI of each child was calculated and subsequently categorized within the respective BMI percentile range. The parents received a written notification of the nutritional status of their child, and those whose children were diagnosed with overweight or obesity could request information about treatment. The data were evaluated using the non-parametric Mann-Whitney U test, unpaired t-test, and the test of differences between two quantities. Overweight was found in 7.56% of boys and 5.45% of girls, while obesity in 8.14% of boys and 5.45% of girls. Based on the BIA analysis of body composition, depending on the percentage amount of adipose tissue, 10.47% of boys and 7.88% of girls were classified as obese.
