- Retrospektivní studie analyzovala novotvary následující po primárních bronchogenních karcinomech (BCA) a primární novotvary před BCA u přežívajících, hlášených v Národním onkologickém registru ČR v letech 1976–2010. Z 166 239 hlášených BCA u mužů se vyskytlo s jiným novotvarem 16 622 BCA, tj. 10 %, z toho 4 395 (2,6 %) primárních a 12 227 (7,4 %) následných BCA; podobně z 37 619 hlášených BCA u žen bylo s jiným novotvarem 5 322 BCA tj. 14,1%, z toho 1 022 (2,7 %) primárních a 4 300 (11,4 %) následných BCA. Průměrná doba vzniku následného novotvaru byla 4,1 roku u mužů a 3,9 roku u žen. Z následných novotvarů (po BCA) bylo asi 23 % trávicích a 14 % močových cest, dále u mužů 14 % dýchacích cest, 14 % kůže a 11 % prostaty, u žen 11 % rodidel, 11 % kůže a 11 % prsů. Z primárních novotvarů (před BCA) bylo u mužů asi 38 % kůže, 13 % močových, 13 % trávicích a 10 % dýchacích cest, u žen 27 % kůže, 21 % rodidel, 17 % prsů a 9 % trávicích a 5 % močových cest. Mezi roky 1976 a 2010 roční počet primárních BCA vzrostl z 65 na 119, podíl jejich stádií (ST) I, II klesl v pětiletých intervalech z 53 % na 22 %, podíl ST III, IV vzrostl z 20 % na 64 %; počet následných BCA vzrostl z 23 na 1 089, jejich ST I, II klesla z 33 % na 20 % a ST III, IV vzrostla z 29 % na 63 %. Podrobnější rozbor omezilo vyšší zastoupení neznámých stádií. Z údajů lze odhadovat asi 1 155 novotvarů ve ST III, IV (19 % u mužů, 22 % u žen ze všech následných ZN) zjištěných po primárních BCA, s měsíčním průměrem tří nemocných v průběhu 34 let. Četnější ST III, IV z 4 966 vybraných následných nádorů byla u ZN vaječníků (47 %), dalších ZN plic (muži 45 %, ženy 52 %), hrtanu (muži 45 %, ženy 52 %), dutiny ústní (muži 41%, ženy 39 %), žaludku (muži 28 %, ženy 42 %), kolorekta (muži 33 %, ženy 37 %), děložního hrdla (28 %), prsů (25 %) a pankreatu (muži 23 %, ženy 26 %). Zejména následným pokročilým stádiím by měla předcházet dispenzární doporučení a časná intervence. Omezené zdroje a přetrvávající rizika životního stylu neumožňují zpomalit v blízké budoucnosti nárůst primárních a následných novotvarů u onkologicky přežívajících nejen v pneumologii., The neoplasms following primary lung cancer (LC) and primary neoplasms before LC analyzed a retrospective study in survivors, notified in the National Czech Cancer Registry between 1976 and 2010. A total 16 622 LC in males associated with other neoplasms, presented 10 % of 166 239 newly registered LC in males, of which were 4,395 (2.6 %) primary and 12,227 (7.4 %) subsequent LC; a total 5,322 LC in females, presented 14.1 % of 37,619 newly registered LC in females, of which were 1,022 (2.7 %) primary and 4,300 (11.4 %) subsequent LC. The average interval of occurrence of subsequent neoplasms was 4.1 years in males and 3.9 years in females. Among subsequent neoplasms (after LC) were about 23 % cancers of digestive and 14% urinary tract, in males 14% cancers of respiratory tract, 14 % skin and 11 % prostate, in females 11% cancers of female genital organs, 11 % skin and 11% breast. Among primary neoplasms (before LC) were about 38 % cancers of skin, 13 % urinary, 13 % digestive and 10 % respiratory tract in males; 27 % cancers of skin, 21 % female genital organs, 17% breast, 9 % digestive and 5 % urinary tract in females. The yearly number of primary LC has increased since 1976 to 2010 from 65 to 119 cases, their representation at the early clinical stages decreased in 5-year periods from 53% to 22 %, at the advanced stages increased from 20 % to 64 %; the number of subsequent LC increased from 23 to 1089 cases, their early clinical stages decreased from 33 % to 20 %, the advanced stages increased from 29 % to 63 %. The higher proportion of LC at the unknown stages limited detailed analysis. In our study were estimated about 1,155 neoplasms at the advanced stages (19 % in males and 22 % in females of all subsequent neoplasms) diagnosed after primary LC, which presented a monthly average of three cancers during 34 years. The most frequented advanced stages of 4,966 selected subsequent cancers were cancers of ovary (47 %), other lung (males 45 %, females 52 %), larynx (males 45 %, females 52 %), oral cavity (males 41 %, females 39 %), stomach (males 28 %, females 42%), colorectum (males 33%, females 37 %), cervix uteri (28 %), breast (25 %) and pancreas (males 23 %, females 26 %). Mainly subsequent advanced stages must be prevented by the follow-up guidelines and early interventions. - V letech 1959–2006 bylo u českých mužů evidováno 81 884 nových karcinomů prostaty (KP) s nárůstem z 12,3 na 96,1 na 100 tis. mužů. Úmrtnost se zvýšila z 21,3 na 27,2 na 100 tis. mužů. Podíl KP ze všech nádorů se zvýšil u incidence ze 7,1 % na 13,2 %, u mortality z 6,7 % na 8,9 %. V letech 1989–2005 vzrostla prevalence KP z 98 na 408,5 na 100 tisíc mužů a dosáhla téměř 20 500 žijících. Z 11 183 nemocných s vícečetnými KP bylo evidováno 4 608 primárních a 6 575 následných případů, představujících 17,4 % ze všech evidovaných KP v letech 1976–2005. Z primárních KP bylo léčeno 89,7 % duplicit a 10,3 % multiplicit. Počet primárních KP vzrostl na 282 v roce 1996 a pak klesal, následných KP trvale rostl na 705 v roce 2005. Téměř polovina KP se vyskytla ve věku 70–79 let a třetina ve věku 50–69 let. Poměr 1 115 (21,3 %) synchronních a 4 121 (78,7 %) metachronních následných novotvarů byl 1:3,7 s průměrnou dobou vzniku následného novotvaru 4,1 roku. Z 5 236 následných novotvarů po primárních KP byly nepočetnější nádory kůže 28,6 %, trávicí 26,7 %, močové 17,8 % a respirační 10,9 % soustavy. Z 5 613 novotvarů před KP jako druhou diagnózou, bylo 35,3 % nádorů kůže, 21 % trávicí, 19,6 % močové a 8,7 % respirační soustavy. Z nemocných s primárním KP k 17. 10. 2007 žilo 22,4 % a zemřelo 77,6 %, s následným KP žilo 31,6 % a zemřelo 68,4 % mužů. Vzhledem k výskytu KP ve spojení s dalšími novotvary není u české populace zatím dostatečná intervence ke zdravému životnímu stylu také vzhledem k očekávanému počtu 42 tisíc mužů, žijících s KP v roce 2015., In Czech men there were registered 81,884 new prostate cancers (PC) during 1959–2006, increased from 12.3 to 96.1 per 100,000 men. - INTRODUCTION: Cellulitis remains a very serious disease even today. Mortality, which varied between 10-40%, has been reduced owing to the standard securing of airway patency and use of an appropriate surgical treatment approach. MATERIALS AND METHODS: A total of 195 patients were hospitalised for cellulitis at the University Hospital in Hradec Králové during 2007-2011. The following parameters were evaluated: age, gender, dependence of incidence of the disease on the season of the year, frequency of attacks of the particular areas and their clinical characteristics, aetiology of the inflammation, types of patient complaints, prevalence of current systemic diseases, results of microbiological and selected laboratory analyses, socio-economic status of the patients, and duration of patient stay at the hospital. Statistical analysis was performed by using Pearson's correlation coefficient, the statistical significance level was p < 0.05. RESULTS: The mean age of the patients was 39.8 years. The group of 195 patients included 108 (55%) males and 87 (45%) females. The mean time between the first symptoms of the disease and admission to the Department was 5 days. From among the 195 patients, 116 (59.5%) were working persons, 79 (40.5%) were non-working (children, students, unemployed persons, women on maternity leave, retired people). The odontogenic origin of the disease was verified in 173 (88.7%) patients. In total, 65 (33.3%) patients had no coinciding complicating systemic disease, 22 (11.3%) patients had diabetes mellitus. The most frequent symptom of cellulitis was painful swelling, found in 194 (99.5%) patients, followed by jaw contracture, found in 153 (78.5%) patients. CONCLUSION: The results are largely very similar to those of previous studies performed in other countries, except that we found no correlation between the prevalence of cellulitis and the socio-economic status, nor have we confirmed Klebsiella pneumoniae sp. as the cause of cellulitis in patients with diabetes mellitus. and H. - BACKGROUND: Human immunodeficiency virus (HIV) continues to be a serious health issue and one of the world most devastating epidemics. An estimated 1.5 million people died from AIDS-related illnesses in 2013, and an estimated 37 million people with AIDS have died worldwide since the epidemic has begun. HIV infection is known for its oral manifestations which causes discomfort and pain for infected individuals. The objective of this study was to document oral conditions of HIV positive patients and the pattern and frequency of oral and dental lesions. METHODS: All patients with confirmed HIV infection who were treated at the Department of Dentistry, University Hospital in Hradec Králové, were examined. RESULTS: During the study period, 29 HIV positive patients were examined and treated--19 men, 10 women, with mean age of 32.9 years (range 22-58 years). 72.41% patients received ART. In total, all patients underwent 186 visits. The most frequent treatments were associated with teeth and periodontal lesions (71.80%), oral mucosal lesions were diagnosed and treated only in 3.96% cases. - BACKGROUND: The goal of this prospective study was to determine the frequency of micrometastases in patients with squamous cell carcinoma (SCC) of the oral cavity, pharynx and larynx in whom elective neck dissection was indicated (cN0). PATIENTS AND METHODS: A total of 12 patients (10 males and 2 females) were enrolled in the study. The age ranged 42-73 years (median 62 years). Elective neck dissection was performed in all patients (8 ipsilateral, 4 bilateral) and a total of 256 lymph nodes were removed and sent for microscopic examination. RESULTS: The presence of tumor cells in cervical lymph nodes was found in 5/12 (42%) patients. Micrometastases of SCC were found in two patients and isolated tumor cells (ITC) in two other patients. In the remaining one patient with oropharyngeal SCC, a micrometastasis of papillary thyroid carcinoma (PTC) was detected. Positive lymph nodes were localized in level II in three patients with SCC of larynx, hypopharynx and tongue base, respectively, in level I in one patient with SCC of oral tongue and in level III in one patient with PTC. CONCLUSION: Our results indicate that SCC of head and neck has a high potential for creating micrometastases which frequency is higher compared to clinically detected macrometastases. - AIM: The purpose of this study was to develop a revised version of the Brief Bedside Dysphagia Screening Test for determining penetration/aspiration risk in patients prone to dysphagia. The priority was to achieve high sensitivity and negative predictive value. METHODS: The study screeners conducted bedside assessment of the swallowing function in 157 patients with a neurological (mainly stroke) or an ear, nose, and throat diagnosis (mainly head and neck cancer). The results were compared with a gold standard, flexible endoscopic examination of swallowing. RESULTS: For the neurological subgroup (N = 106), eight statistically significant bedside assessment items were combined into the Brief Bedside Dysphagia Screening Test-Revised (BBDST-R). Cut-off score 1 produced the highest sensitivity (95.5%; 95% confidence interval CI [CI]: 84.9-98.7%) and negative predictive value (88.9%; 95% CI 67.2-96.9%).
