Cíl: Cílem této studie je poskytnout orientační normy k Addenbrookskému kognitivnímu testu (ACE‑<span class=""></span>R) a jeho jednotlivým kognitivním doménám pro českou populaci. Soubor a metodika: Do studie bylo zařazeno 143 osob (89 žen a 54 mužů) ve věku 55–<span class=""></span>89 let. Subjekty byly zástupci zdravé populace, bez onemocnění mozku, smyslové poruchy a závažného psychiatrického onemocnění, s MMSE nad 27 bodů. Osoby byly podrobeny detailnímu anamnestickému rozhovoru pro zjištění případných onemocnění mozku v minulosti a pro zjištění míry soběstačnosti v běžných denních činnostech. Osoby s poškozením mozku v anamnéze či porušenou soběstačností v běžných denních činnostech nebyly do studie zařazeny, stejně jako osoby s MMSE nižším než 27 bodů. Srovnání hodnot ACE‑<span class=""></span>R a jeho složek u mužů a žen bylo provedeno pomocí Mannova‑<span class=""></span>Whitneyova testu a srovnání hodnot u čtyř skupin daných věkem a vzděláním pomocí Kruskalova‑<span class=""></span>Wallisova testu. Výsledné p‑<span class=""></span>hodnoty byly korigovány na mnohonásobné testování užitím Bonferroniho korekce. Hraniční skóry byly vytvořeny na úrovni 2. a 7. percentilu. Výsledky: Byla prokázána negativní korelace s věkem (p &lt; 0,001; r = –<span class=""></span>0,43) a pozitivní korelace s proměnnou vzdělání na celkový výsledek v testu (p &lt; 0,001; r = 0,41) i v dosažených výsledcích ně­kte­rých jednotlivých kognitivních domén. Vliv pohlaví nebyl statistický prokázán. Hraniční skór pro všechny subjekty ve věku 55–<span class=""></span>89 na úrovni 2. percentilu je 74 bodů pro celkový skór v Addenbrookském kognitivním testu a 79 bodů na úrovni 7. percentilu. Závěr: Studie poskytuje orientační normy pro českou populaci pro Addenbrookský kognitivní test a nabízí bázi pro vytvoření kvalitních norem za předpokladu navýšení počtu subjektů., Aim: The aim of this study was to provide benchmark normal values for Addenbrooke’s cognitive examination (ACE-R) and its domains for the Czech population. Methods: The study included 143 healthy subjects (89 women and 54 men) aged 55–89 years, without brain injury, neurodegenerative disease, severe hearing or visual impairment and without a psychiatric disease, with MMSE above 27 points. Participants were interviewed in detail to ascertain any previous brain injury history and to determine the level of self-sufficiency in daily living activities. Individuals with a history of brain injury or impaired self-sufficiency were excluded from the study. ACE-R values and values for its domains in men and women were compared with Mann-Whitney test and values for the four age and education groups were compared using the Kruskal-Wallis test. P-values were corrected for multiple testing using Bonferroni correction. Results: Cut-off scores were set at 2nd and 7th percentile. Negative correlation with age (p &lt; 0.001, r = –0.43) and positive correlation with education (p &lt; 0.001, r = 0.41) were statistically significant for the overall test performance and also for the performance in individual cognitive domains except for the Attention and orientation domain. The effect of sex was not statistically significant. The cut-off score for the total score in Addenbrooke’s cognitive examination for all subjects aged 55–89 years is 74 points at the 2nd percentile and 79 points at the 7th percentile. Conclusion: The study suggests cut-off scores for the Czech population of ACE-R and provides the basis for the development of Czech norms for this test. 4. Analýza vývoje preskripce metforminu a sulfonylurey v České republice

Úvod: V České republice (ČR) jsou pacienti s diagnózou diabetes mellitus (DM) sledováni a léčeni převážně u specialistů (přibližně 80 % v odborné diabetologické ambulanci), menší část praktickými lékaři (do 20 %). Doposud nebyl zhodnocen dlouhodobý vývoj změn preskripce metforminu a sulfonylurey v České republice a jeho konkordance s doporučenými postupy. Cíl: Porovnání vývoje preskripce metforminu (MET) a sulfonylurey (SU) v letech 2002–2006 s vývojem v letech 2010–2014 v reprezentativním vzorku populace pacientů s DM v ČR vedených v databázi Všeobecné zdravotní pojišťovny České republiky (VZP), u které bylo v roce 2014 pojištěno 63 % populace ČR. Metodika: V databázi VZP jsme identifikovali všechny osoby, které měly v letech 2002–2006 a 2010–2014 záznam o diagnóze DM (E10–E16 podle MKN 10) nebo předpis jakékoliv antidiabetické terapie (ATC skupina A10). Pro analýzu byl extrahován soubor osob, které měly v daném roce předepsanou nejméně jednou látku ze skupiny A10 (308 962 evidovaných v roce 2002 a 426 695 evidovaných v roce 2014). Pro každý rok byl vyhodnocen počet osob, které měly předepsán nejméně jednou MET nebo SU. Počet osob léčených MET nebo SU byl pak vyjádřen v procentech ze všech, kteří měli v daném roce předepsanou jakoukoliv terapii ze skupiny A10. Výsledky: Preskripce metforminu stoupá lineárně ze 43 % na 77 %, preskripce sulfonylurey se lineárně snižuje z 65 % na 37 %. Závěr: Naše analýza je prvním zhodnocením vývoje preskripce metforminu v ČR a zhodnocením její konkordance s doporučenými postupy pro léčbu DM. Preskripce metforminu v ČR se zvýšila mezi roky 2002–2014 ze 43 % na 77 % a preskripce SU se snížila z 65 % na 37 %. Tento vývoj a aktuální poměr preskripce MET a SU je potvrzením implementace doporučených postupů do praxe a důkazem vysoké kvality péče o pacienty s DM2T v České republice. Klíčová slova: diabetes mellitus 2. typu – metformin – sulfonylurea, Introduction: In the Czech Republic, patients with diabetes mellitus (DM) are followed and treated predominantly by specialists (approx. 80% at a specialist diabetology clinic), a minor part by general practitioners (up to 20%). Long-term development of the changes in prescribing metformin and sulfonylurea in the Czech Republic and its concordance with recommended procedures has not been evaluated until now. Goal: Comparison of the development of metformin (MET) and sulfonylurea (SU) prescriptions in the period of 2002–2006 with that of 2010–2014 in a representative sample of the patient population with DM kept in the database of the General Health Insurance Company of the Czech Republic (VZP) which provided health care coverage for 63% of Czech Republic population in 2014. Methodology: We identified all individuals in the VZP database who had a record of DM diagnosis (E10 – E16 based on ICD 10) or who had any antidiabetic therapy prescribed (ATC group A10) in the periods of 2002–2006 and 2010–2014. A cohort of patients was extracted for analysis, who had an agent from A10 group prescribed at least once in a rele­vant year (n = 308 962 in 2002; n = 426 695 in 2014). A number of patients was evaluated for each year, who had at least once MET or SU prescribed. The number of patients treated with MET or SU was then expressed as a percentage of all who had any therapy from A10 group prescribed in the year in question. Results: Metformin prescriptions have linearly risen from 43% to 77%, while sulfonylurea prescriptions have linearly decreased from 65% to 37%. Conclusion: The analysis presents the first evaluation of the development of metformin prescriptions conducted in the Czech Republic and evaluation of its concordance with the recommended procedures for the treatment of DM. The amount of metformin prescribed in the Czech Republic increased from 43% to 77% while the amount of SU prescribed decreased from 65% to 37% between 2002 and 2014. This development and the current ratio between the prescribed amounts of MET and SU demonstrate the implementation of the recommended procedures into practice and prove the high quality of care for patients with DM2T in specialists – diabetologists‘ surgeries. Retrospektivní studie analyzovala novotvary následující po primárních bronchogenních karcinomech (BCA) a primární novotvary před BCA u přežívajících, hlášených v Národním onkologickém registru ČR v letech 1976–2010. Z 166 239 hlášených BCA u mužů se vyskytlo s jiným novotvarem 16 622 BCA, tj. 10 %, z toho 4 395 (2,6 %) primárních a 12 227 (7,4 %) následných BCA; podobně z 37 619 hlášených BCA u žen bylo s jiným novotvarem 5 322 BCA tj. 14,1%, z toho 1 022 (2,7 %) primárních a 4 300 (11,4 %) následných BCA. Průměrná doba vzniku následného novotvaru byla 4,1 roku u mužů a 3,9 roku u žen. Z následných novotvarů (po BCA) bylo asi 23 % trávicích a 14 % močových cest, dále u mužů 14 % dýchacích cest, 14 % kůže a 11 % prostaty, u žen 11 % rodidel, 11 % kůže a 11 % prsů. Z primárních novotvarů (před BCA) bylo u mužů asi 38 % kůže, 13 % močových, 13 % trávicích a 10 % dýchacích cest, u žen 27 % kůže, 21 % rodidel, 17 % prsů a 9 % trávicích a 5 % močových cest. Mezi roky 1976 a 2010 roční počet primárních BCA vzrostl z 65 na 119, podíl jejich stádií (ST) I, II klesl v pětiletých intervalech z 53 % na 22 %, podíl ST III, IV vzrostl z 20 % na 64 %; počet následných BCA vzrostl z 23 na 1 089, jejich ST I, II klesla z 33 % na 20 % a ST III, IV vzrostla z 29 % na 63 %. Podrobnější rozbor omezilo vyšší zastoupení neznámých stádií. Z údajů lze odhadovat asi 1 155 novotvarů ve ST III, IV (19 % u mužů, 22 % u žen ze všech následných ZN) zjištěných po primárních BCA, s měsíčním průměrem tří nemocných v průběhu 34 let. Četnější ST III, IV z 4 966 vybraných následných nádorů byla u ZN vaječníků (47 %), dalších ZN plic (muži 45 %, ženy 52 %), hrtanu (muži 45 %, ženy 52 %), dutiny ústní (muži 41%, ženy 39 %), žaludku (muži 28 %, ženy 42 %), kolorekta (muži 33 %, ženy 37 %), děložního hrdla (28 %), prsů (25 %) a pankreatu (muži 23 %, ženy 26 %). Zejména následným pokročilým stádiím by měla předcházet dispenzární doporučení a časná intervence. Omezené zdroje a přetrvávající rizika životního stylu neumožňují zpomalit v blízké budoucnosti nárůst primárních a následných novotvarů u onkologicky přežívajících nejen v pneumologii., The neoplasms following primary lung cancer (LC) and primary neoplasms before LC analyzed a retrospective study in survivors, notified in the National Czech Cancer Registry between 1976 and 2010. A total 16 622 LC in males associated with other neoplasms, presented 10 % of 166 239 newly registered LC in males, of which were 4,395 (2.6 %) primary and 12,227 (7.4 %) subsequent LC; a total 5,322 LC in females, presented 14.1 % of 37,619 newly registered LC in females, of which were 1,022 (2.7 %) primary and 4,300 (11.4 %) subsequent LC. The average interval of occurrence of subsequent neoplasms was 4.1 years in males and 3.9 years in females. Among subsequent neoplasms (after LC) were about 23 % cancers of digestive and 14% urinary tract, in males 14% cancers of respiratory tract, 14 % skin and 11 % prostate, in females 11% cancers of female genital organs, 11 % skin and 11% breast. Among primary neoplasms (before LC) were about 38 % cancers of skin, 13 % urinary, 13 % digestive and 10 % respiratory tract in males; 27 % cancers of skin, 21 % female genital organs, 17% breast, 9 % digestive and 5 % urinary tract in females. The yearly number of primary LC has increased since 1976 to 2010 from 65 to 119 cases, their representation at the early clinical stages decreased in 5-year periods from 53% to 22 %, at the advanced stages increased from 20 % to 64 %; the number of subsequent LC increased from 23 to 1089 cases, their early clinical stages decreased from 33 % to 20 %, the advanced stages increased from 29 % to 63 %. The higher proportion of LC at the unknown stages limited detailed analysis. In our study were estimated about 1,155 neoplasms at the advanced stages (19 % in males and 22 % in females of all subsequent neoplasms) diagnosed after primary LC, which presented a monthly average of three cancers during 34 years. The most frequented advanced stages of 4,966 selected subsequent cancers were cancers of ovary (47 %), other lung (males 45 %, females 52 %), larynx (males 45 %, females 52 %), oral cavity (males 41 %, females 39 %), stomach (males 28 %, females 42%), colorectum (males 33%, females 37 %), cervix uteri (28 %), breast (25 %) and pancreas (males 23 %, females 26 %). Mainly subsequent advanced stages must be prevented by the follow-up guidelines and early interventions. INTRODUCTION: Cellulitis remains a very serious disease even today. Mortality, which varied between 10-40%, has been reduced owing to the standard securing of airway patency and use of an appropriate surgical treatment approach. MATERIALS AND METHODS: A total of 195 patients were hospitalised for cellulitis at the University Hospital in Hradec Králové during 2007-2011. The following parameters were evaluated: age, gender, dependence of incidence of the disease on the season of the year, frequency of attacks of the particular areas and their clinical characteristics, aetiology of the inflammation, types of patient complaints, prevalence of current systemic diseases, results of microbiological and selected laboratory analyses, socio-economic status of the patients, and duration of patient stay at the hospital. Statistical analysis was performed by using Pearson's correlation coefficient, the statistical significance level was p < 0.05. RESULTS: The mean age of the patients was 39.8 years. The group of 195 patients included 108 (55%) males and 87 (45%) females. The mean time between the first symptoms of the disease and admission to the Department was 5 days. From among the 195 patients, 116 (59.5%) were working persons, 79 (40.5%) were non-working (children, students, unemployed persons, women on maternity leave, retired people). The odontogenic origin of the disease was verified in 173 (88.7%) patients. In total, 65 (33.3%) patients had no coinciding complicating systemic disease, 22 (11.3%) patients had diabetes mellitus. The most frequent symptom of cellulitis was painful swelling, found in 194 (99.5%) patients, followed by jaw contracture, found in 153 (78.5%) patients. CONCLUSION: The results are largely very similar to those of previous studies performed in other countries, except that we found no correlation between the prevalence of cellulitis and the socio-economic status, nor have we confirmed Klebsiella pneumoniae sp. as the cause of cellulitis in patients with diabetes mellitus. and H. BACKGROUND: Human immunodeficiency virus (HIV) continues to be a serious health issue and one of the world most devastating epidemics. An estimated 1.5 million people died from AIDS-related illnesses in 2013, and an estimated 37 million people with AIDS have died worldwide since the epidemic has begun. HIV infection is known for its oral manifestations which causes discomfort and pain for infected individuals. The objective of this study was to document oral conditions of HIV positive patients and the pattern and frequency of oral and dental lesions. METHODS: All patients with confirmed HIV infection who were treated at the Department of Dentistry, University Hospital in Hradec Králové, were examined. RESULTS: During the study period, 29 HIV positive patients were examined and treated--19 men, 10 women, with mean age of 32.9 years (range 22-58 years). 72.41% patients received ART. In total, all patients underwent 186 visits. The most frequent treatments were associated with teeth and periodontal lesions (71.80%), oral mucosal lesions were diagnosed and treated only in 3.96% cases. BACKGROUND: The goal of this prospective study was to determine the frequency of micrometastases in patients with squamous cell carcinoma (SCC) of the oral cavity, pharynx and larynx in whom elective neck dissection was indicated (cN0). PATIENTS AND METHODS: A total of 12 patients (10 males and 2 females) were enrolled in the study. The age ranged 42-73 years (median 62 years). Elective neck dissection was performed in all patients (8 ipsilateral, 4 bilateral) and a total of 256 lymph nodes were removed and sent for microscopic examination. RESULTS: The presence of tumor cells in cervical lymph nodes was found in 5/12 (42%) patients. Micrometastases of SCC were found in two patients and isolated tumor cells (ITC) in two other patients. In the remaining one patient with oropharyngeal SCC, a micrometastasis of papillary thyroid carcinoma (PTC) was detected. Positive lymph nodes were localized in level II in three patients with SCC of larynx, hypopharynx and tongue base, respectively, in level I in one patient with SCC of oral tongue and in level III in one patient with PTC. CONCLUSION: Our results indicate that SCC of head and neck has a high potential for creating micrometastases which frequency is higher compared to clinically detected macrometastases.
