V současné době jsou jak celosvětově (alirokumab byl prvním lékem této skupiny na světě registrovaným americkou lékovou agenturou FDA), tak v Evropě registrována zcela nová hypolipidemika, která se v mnoha směrech odlišují od léků do současnosti podávaných. Přinášejí přitom další významný posun v léčbě poruch tukového metabolizmu i v preventivní kardiologii. Alirokumab je plně humánní monoklonální protilátka proti enzymu PCSK9 (proprotein konvertáza subtilizin/kexin typu 9). Enzym PCSK9 hraje významnou roli v metabolizmu LDL-cholesterolu tím, že ovlivňuje odbourávání, a posléze pak i počet a aktivitu LDL receptorů. Z klinického hlediska je podstatné, že se léky této skupiny podávají parenterálně, subkutánní injekcí. V případě Praluentu? je interval podání 1krát za 2 týdny. Dávka je potom 75 mg nebo 150 mg v injekci s obsahem 1 ml. Z klinického hlediska je významné především to, že alirokumab snižuje koncentraci LDL-cholesterolu o 50–60 %, snižuje hladinu lipoproteinu a o 25–30 %, pozitivně ovlivňuje i další složky lipidového metabolizmu a především má velmi pravděpodobně potenciál ke snížení kardiovaskulárního rizika. I když výsledky morbiditně-mortalitních studií jsou očekávány až v příštích letech, první analýzy vyznívají výrazně ve prospěch klinicky významného snižování KV-příhod. Alirokumab (Praluent?) je možno podávat v monoterapii (zejména u nemocných se statinovou intolerancí), zejména však bude podáván v kombinaci s dalšími hypolipidemiky (zejména statiny) u nemocných, u kterých se nedaří dosáhnout cílových hodnot. They are bringing another advancement in the treatment of disorders of lipid metabolism and in preventive cardiology. Alirocumab is a fully human monoclonal antibody to PCSK-9 enzyme (proprotein convertase subtilisin kexin-9). PCSK-9 enzyme plays an important role in the metabolism of LDL-cholesterol through affecting the breakdown and eventually the amount and activity of LDL-receptors. From the clinical point of view it is essential that drugs from this group are administered parenterally, as a subcutaneous injection. In the case of Praluent? the interval between administration is two weeks. The dose is then 75 or 150mg in a 1ml injection. From the clinical point of view it is particularly important that alirocumab decreases LDL-C concentrations by 50–60%, it decreases Lp/a/ levels by 25–30%, and it also positively influences other components of lipid metabolism and, above all, is very likely to have a potential to decrease a cardiovascular risk. Although the resuIts of morbidity and mortality studies are expected in the coming years, initial analyses strongly indicate a clinically significant reduction of CV events. Alirocumab, Praluent can be administered as monotherapy (mainly to statin-intolerant patients), however it will be primarily administered in combination with the other hypolipidemic drugs (in particular statins) where the effort to reach target values has not succeeded. Možnosti ovlivnit metabolizmus plazmatických lipoproteinů a snížit tak riziko aterotrombotických komplikací se v posledních letech rozšiřují. Kromě vývoje zcela nových přístupů modifikujících hladiny aterogenních lipoproteinových částic získáváme nové farmakologické formy běžných hypolipidemik – nanopartikule obsahující statiny, fibráty v nanočásticích s vysokou biologickou dostupností. Zapomenuty nejsou ani pryskyřice působící příznivě na glukózovou homeostázu a doplňující vhodně terapii statinem. Pokračují testy účinnosti terapií cílících na metabolizmus HDL částic. Ani pro inhibitory CETP nebo mimetika apoliproteinu A1 zatím nemáme přesvědčivé důkazy. Zcela nové postupy zahrnující blokádu MTTP, inhibici PSCK9 nebo terapie využívající anti-sense technologie sbírají důkazy z klinických studií velmi rychle. Zatím víme o jejich příznivých účincích na hladiny aterogenních lipidů u nemocných s familiární hypercholesterolemií, ale přibývají data i z dalších pacientských populací., Options for modification of lipoprotein metabolism and, thus, for reduction of atherothrombotic complication have widened over recent years. Apart from the development of novel approaches new pharmacological formulations of common lipid lowering drugs have been prepared- e.g. statin-containing nanoparticles, fibrate nanoparticles with a much higher bioavailability etc. Even the oldest lipid lowering agents – resins – have not been forgotten due to its once again discovered positive impact of these agents on glucose homeostasis while optimally complementing the action of statins. Clinical trials of therapies targeting HDL particle metabolism are being in progress despite we have not gathered any unambiguous evidence of positive effect of the CETP inhibitors or apoA1 mimetics on the progression of atherosclerosis. Brand new approaches in the treatment of dyslipidemia including MTTP and PCSK9 inhibition or therapies utilizing anti-sense technologies rapidly accumulate evidence from clinical studies. Statiny výrazně snižují kardiovaskulární riziko a snížení cholesterolu pomocí těchto léků se stalo milníkem v prevenci kardiovaskulárních chorob u širokého okruhu pacientů. Přesto je adekvátní použití statinů u mnoha pacientů limitováno nežádoucími symptomy, které u některých pacientů vedou k přerušení léčby a nízké adherenci k terapii u jiných. Téma statinové intolerance má proto velký klinický význam. Statinová intolerance může být definována jako výskyt myalgie nebo jiných nežádoucích symptomů, které souvisejí se statinovou terapií a vedou k jejímu přerušení. Nicméně, i když dojde v průběhu léčby statiny k těmto příznakům, jsou tyto nežádoucí symptomy často nesouvisející s léčbou a většina pacientů s anamnézou epizod těchto symptomů je schopná tolerovat adekvátní statinovou terapii. Identifikování skutečných případů statinové intolerance má proto velký klinický význam k vyhnutí se zbytečnému přerušení léčby statiny u pacientů, kteří by z ní měli benefit. U pacientů se skutečnou statinovou intolerancí by měly být podávány statiny v pozměněném dávkovacím schématu, a jestliže jsou tolerovány, měla by být dávka postupně zvyšována až k nejvyšší tolerované. S tímto opatrným přístupem je většina pacientů schopná tolerovat alespoň některý stupeň statinové terapie. Kromě toho mohou být k dosažení požadovaných cílů zapotřebí i jiné léky snižující hladinu lipidů. Jestliže je obtížné dosažení cílových hodnot lipidů, může ve snížení kardiovaskulárního rizika pomoci přísná kontrola rizikových faktorů. Klíčová slova: myalgie – nízkodávkovaná statinová terapie – statinová intolerance – statiny – svalové vedlejší účinky, Statins significantly decrease the cardiovascular risk and the reduction of cholesterol by statins has become a milestone in the prevention of cardiovascular diseases for a large group of patients. In spite of that an adequate use of statins in many patients is limited by adverse symptoms which lead to interruption of the therapy in some patients and low adherence to the therapy in others. Therefore the subject of statin intolerance is of great clinical importance. Statin intolerance can be defined as an incidence of myalgia or other adverse symptoms associated with a statin therapy, which lead to its interruption. Nonetheless, even if such symptoms develop during the statin therapy, these adverse effects are frequently not associated with the treatment and most patients with an anamnesis suggesting episodes of such symptoms are able to tolerate an adequate statin therapy. It is therefore of great clinical importance to identify the actual cases of statin intolerance in order to avoid the unnecessary interruption of the statin therapy in the patients who would benefit from it. Regarding patients with a proven statin intolerance, statins should be administered according to an altered scheme and if tolerated, the dose should be gradually increased until the highest tolerated level is reached. When this careful approach is followed, most patients are able to tolerate some level of statin therapy at least. Besides that, there may be also other lipid-reducing drugs needed to reach the required goals. If the achievement of the lipid target values is difficult, a rigorous control of the risk factors may help in reducing the cardiovascular risk. Diagnostika a léčba rizikových faktorů kardiovaskulárních onemocnění je efektivním způsobem jak snižovat riziko kardiovaskulárních příhod. I přesto, že jsou aktualizované doporučené postupy prevence kardiovaskulárních onemocnění všeobecně známé a dostupné a přesto, že máme k dispozici nepřeberné množství dostupných léčebných prostředků k ovlivnění rizikových faktorů, je třeba zdůraznit, že stále nejsou dostatečně využívány. Tomuto procesu brání řada faktorů, počínaje omezenou platností výsledků klinických studií pro klinickou praxi, osobními preferencemi a zkušenostmi lékařů, konče nedostatkem času, ekonomickými faktory a postoji pacientů. K neuspokojivé prevenci rizikových faktorů kardiovaskulárních onemocnění přispívá mj. nedostatečný screening rizikových faktorů, nepřesné stanovení rizika a váhavost v dodržování cílových hodnot daných doporučenými postupy. Námi provedené studie prokázaly, že důraz na systematickou aplikaci principů prevence kardiovaskulárních chorob a snaha o co nejpřísnější dodržování cílových hodnot vede ke zlepšení kontroly kardiovaskulárních rizikových faktorů, z čehož jasně vyplývá, že převedení znalostí daných doporučenými postupy v proveditelný návyk je důležité pro úspěšnou prevenci kardiovaskulárních chorob v klinické praxi a může vést k podstatnému snížení kardiovaskulárního rizika v běžné populaci. Praxe bohužel stále ukazuje, že přestože se zlepšení výše uvedených kroků jeví velmi jednoduše, převedení teorie do praxe stále pokulhává., Prevention through evaluation and treatment of cardiovascular risk factors is an efficient approach to reduce the risk of cardiovascular events, however, the problem remains that the available treatment options are underused. Implementation of cardiovascular disease prevention guidelines into clinical practice is therefore important for decreasing the burden of cardiovascular disease in general population. However, there are many barriers to this process, including questionable relevance of scientific results for clinical practice, personal preferences and expertise of the doctors, patient attitudes, lack of time, and economical factors. All these factors need to be taken into account for any change in the clinical practice to be successful. With respect to cardiovascular disease prevention, insufficient screening for risk factors, inappropriate risk estimation and hesitation to keep to the guidelines-based treatment targets contribute most to inadequate control of risk factors, and this has been repeatedly demonstrated to be difficult to improve. In this context, our studies demonstrate that the emphasis on systematic application of the principles of cardiovascular prevention results in improved control of cardiovascular risk factors. Adequate support for transforming the guidelines-based knowledge into practicable habit appears therefore important for successful prevention of cardiovascular disease in clinical practice and may translate into substantial reduction of cardiovascular risk in general population., and Tomáš Štulc, Michaela Šnejdrlová, Richard Češka
