Angiomyolipom ledviny (AML) je benigní mezenchymový nádor, který se vyskytuje sporadicky nebo v rámci komplexu tuberózní sklerózy (TSC). Celková prevalence AML je 0,44 % a vyskytuje se čtyřikrát častěji u žen. Prevalence v ženské populaci je tedy 0,6 %, zatímco u mužů 0,28 %. Jeho diagnostika je založena na přítomnosti tukové složky v nádoru a lze tedy identifikovat předoperačně pomocí zobrazovacích metod. Morfologie AML je velmi variabilní a někdy tedy tuková složka chybí, což může způsobit diagnostické rozpaky. Zcela zásadní roli má aktivní sledování pacientů s jasnou diagnózou AML, jehož růst je pomalý s minimální morbiditu. Pacient však může přijít ve stadiu komplikací, které zahrnují hematurii, bolest či krvácení do retroperitonea způsobené rupturou nádoru. Mezi hlavní rizikové faktory krvácení jsou velikost nádoru, procento zastoupení cévní složky, kterou tvoří aneuryzmatické cévy a přítomnost TSC. Selektivní arteriální embolizace úspěšně řeší hemoragické komplikace a redukuje objem AML. V rámci dlouhodobých výsledků však selhává díky revaskularizaci a dalšímu růstu AML. Zde pak mají lepší výsledky chirurgické operace, kde se snažíme v maximální míře šetřit funkci ledviny. Slibné dlouhodobé výsledky má též radiofrekvenční ablace, která je ale silně limitována velikostí AML. U objemnějších AML se zdá být dobrou alternativou resekčního výkonu kryoablace, která však vyžaduje hůře dostupné speciální přístrojové vybavení. Novinkou je pak systémová léčba mTOR inhibitory (everolimus, sirolimus), které mají nezastupitelné místo u nemocných s TSC, kde jsou AML mnohočetné a recidivující. The main risk factors for bleeding are tumors volume, percentage of the vascular component in the tumor and the presence of TSC. Selective arterial embolization resolves hemorrhagic complications and reduces the volume of the tumor. The long-term outcomes of embolisation are poor due to tumor revascularization and growth. Nephron sparring surgery is associated with better long term outcomes. Radiofrequency ablation is another option, but strongly restricted by the volume tumor. An additional option is cryoablation, which has better results in cases where tumor is large, but this requires special equipment. Systematic treatment using mTOR inhibitors (everolimus, sirolimus, has recently been introduced, which are essential in patients with TSC, where the disease is often complicated by multiple and recurrent lesion. Uroteliální karcinomy vyžadují komplexní přístup ke stagingu, jelikož jejich typickou vlastností je multifokalita - možnost lokálně agresivního chování i vzdálené diseminace. V komplexním zobrazení je možné využít MDCT, MRI i 18F-FDG-PET/CT. Při hodnocení nádorů je zásadní odlišit invazi nádoru do okolní tukové tkáně nebo do parenchymu ledviny, eventuálně do okolních orgánů. Lokální šíření je možné posoudit poměrně spolehlivě pomocí MDCT i MRI. Pro posouzení vzdálené diseminace onemocnění je optimální metodou 18F-FDG-PET/CT, která nabízí odhalení metastáz v uzlinách ve skeletu, játrech i plicním parenchymu. Indikace zobrazovacích metod prodělaly v posledních letech mimořádný vývoj. Spojené aplikace magnetické rezonance, jako jsou spektroskopie, difuzní zobrazení a farmakokinetická analýza pomohly zařadit do diagnostického algoritmu karcinomu prostaty efektivní komplexní vyšetřovací protokol pro nemocné se zvýšenými hladinami PSA. Pro případ nutností přesného posouzení lokálního šíření tumoru zůstávají nejefektivnější T2 vážené obrazy s vysokým prostorovým rozlišením. Zavedení cholinu značného fluorem 18 do klinické praxe umožnilo rutinní použití PET/CT nejen pro vlastní detekci nádoru, ale i pro hodnocení uzlinových a kostních metastáz. Při restagingu je odhalení příčiny biochemického relapsu onemocnění také možnou indikací 18F-FCH-PET/CT, které vykazuje vyšší úspěšnost než magnetická rezonance. The united applications of the magnetic resonance imaging including the spectroscopy, diffusion imaging and pharmacokinetic evaluation helped to introduce the complex diagnostic algorithm to patients with pathologically elevated PSA levels. In cases of needed precise local staging, the most effective modality remains general T2 weighted imaging using high resolution data acquisition. The introduction of the choline labeled with fluorine 18 enables the routine clinical use of PET/CT not only in detection of the tumor, but also in detection of lymph node and skeleton dissemination. In cases of restaging of the disease including biochemically relapsing tumor, the 18F-FCH-PET/ CT reached the important improvement in comparison to magnetic resonance. Renální karcinom jako nejvýznamnější solidní nádor ledvin dosahuje v České republice nejvyšší incidence v celosvětovém měřítku. Odlišit jej mezi ostatními solidními a cystoidními lézemi je nejdůležitější úlohou zobrazovacích metod. Základní metodou je ultrasonografie, kterou je možné rozšířit pomocí aplikace kontrastní látky na metodu vysoce spolehlivou v diferenciální diagnostice solidních útvarů. K předoperačnímu zobrazení nádorů ledvin je nezbytné zobrazení cévního zásobení ledviny i vlastního tumoru pomocí CT angiografie nebo MR angiografie. Magnetickou rezonancí lze blíže specifikovat především hypovaskularizované nádory, u kterých se difuzní vážení ukazuje jako velmi důležité při odlišení papilárních renálních karcinomů.
