Výzkum se dnes široce zabývá tématem společného výskytu kardiovaskulárních onemocnění a deprese. Zvažuje různé aspekty, mezi nimiž je i pátrání po tom, zda existuje příčinná souvislost mezi depresí a kardiovaskulárními onemocněními nebo zda má deprese vliv na horší rekonvalescenci pacientů po infarktu myokardu a na vyšší výskyt komplikací. Deprese je spojována s výskytem komorbidit, jako je metabolický syndrom nebo diabetes mellitus. Určitou roli může hrát také horší funkce autonomního nervového systému nebo nezdravý životní styl. Deprese se vyskytuje u pacientů po infarktu myokardu ve větší míře než u normální populace a je považována za rizikový faktor, který zhoršuje prognózu pacienta. Proto je důležité se zaměřit na skríning deprese a případně i anxiety u těchto pacientů, tyto u nich včas odhalit a zajistit optimální léčbu., Researchers currently focus on a wide range of aspects associated with coincidence of cardiovascular diseases and depression. These include a search for a possible causal link between depression and coexisting cardiovascular disorder as well as the association between depression and poorer recovery after myocardial infarction or higher complication rate. Major depression is connected with comorbidities such as the metabolic syndrome and diabetes mellitus. Impaired autonomous nerve system function and unhealthy lifestyle can also play their role. Depression is more frequent in patients with a history of myocardial infarction and is considered a major risk factor of poorer prognosis. It is undoubtedly important to screen this patient group for both depression and anxiety so that appropriate treatment can be initiated if needed. Vyhodnocení léčebných postupů a jejich výsledků je stále podstatnější součástí aktuální medicíny, ve které je kladen důraz na efektivní využívání zdravotnických technologií (HTA). Na rozdíl od randomizovaných studií zahrnujících přísně selektované skupiny pacientů splňující vstupní a vylučovací kritéria, poskytují reálnou představu o bezpečnosti a efektivitě léčby multicentrické klinické registry. Dobře připravené registry jsou výzkumnou databází a uživatelským prostředím umožňujícím sběr parametrických a snadno analyzovatelných dat. Pro zajištění maximální kvality zadaných údajů existují některé technologické prostředky, nicméně zásadním je spolupráce uživatele, který do registru data zadává. Smysluplným je pouze takový registr, který poskytuje odpovědi na odborné hypotézy, pro které byl vytvořen. Klíčová role je přisuzována zpětné vazbě, tzn. průběžnému informování uživatelů formou výstupů z registru, tzv. benchmarking nebo reporting. Příkladem registru, který splňuje výše definovaná kritéria, je anglicky vedený registr s garancí České kardiologické společnosti (ČKS), který obsahuje údaje o asi 95 % katetrových implantací aortální chlopně v České republice – Czech TAVI Registry (CTR). CTR patří i díky plnění výše popsaných zásad k nejkvalitněji vedeným registrům, které jsou spravovány Institutem biostatistiky a analýz Masarykovy univerzity (IBA MU). V práci jsou uvedeny některé výstupy z CTR, registru, který je založen na obecném systému pro design klinických registrů pojmenovaném TrialDB2. Ten představuje databázový systém klinických registrů rozvíjený IBA MU. Autoři v této práci popisují v současnosti používané databázové systémy pro vedení registrů a vlastní analytické výstupy, které jsou všem centrům a uživatelům pravidelně poskytovány formou reportingu. The emphasis is put on an effective use of the health technologies (HTA). Unlike randomised studies, which involve strictly selected groups of patients who meet inclusion and exclusion criterias, the multicentre clinical registries provide a real-life picture of the treatment safety and effectiveness. Well prepared registries involve both research database and a friendly user interface enabling collection of parametric and easily analyzable data. Although there are some technological aspects aiming to ensure a maximum quality of entered data, cooperation with the users and data managers is essential. Such a registry, otherwise meaningful, must provide answers to previously defined medical hypotheses. Regular feedback to users (so called benchmarking or reporting) is considered to be of key importance. The Czech TAVI Registry (CTR) is a good example of reaching all of the above defined criterias. This registry contains data of approximately 95 % of all transcatheter aortic valve implantations (TAVI) performed in the Czech Republic. It is based on a general system aimed at the design of clinical trials, namely the TrialDB2 (a database system for clinical registries developed by the Institute of Biostatistics and Analyses at the Masaryk University (IBA MU). CTR has been run as an English-language version under the auspices of the Czech Society of Cardiology and represents one of the top-quality registries maintained by IBA MU. This paper presents the currently available database systems and some reports from this particular registry.
