Bronchogenní karcinom je jedním z nejčastěji se vyskytujících zhoubných nádorů. Incidence v ČR u mužů sice mírně klesá, v roce 2009 to bylo 87,45 případů na 100 000 obyvatel, u žen dochází k neustálému nárůstu incidence, která v roce 2009 dosáhla 35,75 případů na 100 000 obyvatel. Téměř u 75 % pacientů je diagnóza stanovena ve stadiu lokálně pokročilého nebo generalizovaného onemocnění (st. IIIB, IV). V minulosti byli tito pacienti léčeni systémovou chemoterapií, v posledních letech byla do klinické praxe zavedena biologická léčba, která je zaměřena na buněčné signální cesty a jejich ovlivnění. Tyto léčebné přípravky jsou také uváděny pod názvem „cílená léčba“, protože reagují s přesně specifikovaným buněčným molekulárním cílem. U buněk řady solidních nádorů epiteliálního původu byla popsána zvýšená aktivace receptoru pro epidermální růstový faktor (EGFR1), proti jehož neřízené aktivaci je zaměřená cílená terapie nízkomolekulárními inhibitory tyrozinkináz (gefitinib, erlotinib), které, po navázání na protein, inhibují fosforylaci tyrozinkinázové části receptoru a vedou k následnému blokování signálních drah. V důsledku tohoto procesu dochází ke snížení mitotické aktivity buňky, zvýšení senzitivity k apoptóze a zpomalení, eventuálně zastavení, nádorového růstu. Iressa (gefitinib) byla autorizována pro terapii NSCLC 1.1.2009 na podkladě studií INTEREST a IPASS. Je indikována v 1. linii léčby lokálně pokročilého nebo generalizovaného nemalobuněčného bronchogenního karcinomu (NSCLC) u pacientů s pozitivní EGFR mutací (mutace zvyšující citlivost k léčbě v exonech 18 a 19, bodové mutace na exonu 21). Jde o perorální formu léčby s podáváním 1x denně 250 mg. Jedná se o první cílenou monoterapii, která prokázala lepší účinnost v porovnání s kombinovanou chemoterapií v 1. linii léčby pokročilého NSCLC s aktivní mutací EGFR-TK., Lung cancer is one of the most frequent malignant tumors. These therapeutic products are marked under the name of „targeted therapy“, because they react with precisely specified molecular target. The number of cells of solid tumors of epithelial origin has been reported to increase activation of the receptor for epidermal growth factor (EGFR), at which a targeted therapy of low molecular weight inhibitors of tyrosine kinases (gefitinib, erlotinib) is aimed. After binding to the protein they inhibit phosphorylation of the tyrosine kinase part of the receptor and lead to blocking subsequent signaling pathways. As a result of this process, mitotic activity of cells is reduced, sensitivity to apoptosis increases and the growth of the tumor slows down or, eventually, stops. Iressa (gefinitib) was authorized for the treatment of NSCLC on 1st January 2009, on the basis of INTEREST and IPASS trials. It is indicated in the first line treatment of locally advanced or metastatic non-small cell lung cancer (NSCLC) patients with positive EGFR mutation (mutations in exon 18 and 19, point mutation in exon 21). It is a form of treatment with oral administration of 250 mg once a day. Karcinoid patří mezi neuroendokrinní tumory, které mohou produkovat endokrinně aktivní látky. Představuje 1–2 % plicních tumorů. Dle svých histopatologických charakteristik se dělí na typický a atypický karcinoid, od čehož odvisí i prognóza (13, 24). Typický karcinoid (TC) má příznivou prognózu, je často zjištěn náhodně, pětileté přežití je přes 90 %. Atypický karcinoid (AC) se vyznačuje agresivnějším chováním, časnějším metastazováním i nižším dlouhodobým pětiletým přežitím (40–60 %) (5, 13). Sledovali jsme soubor 52 pacientů s histologicky potvrzenou diagnózou karcinoidu, kteří se na naší klinice léčili v období 2002–2013. Typický karcinoid byl diagnostikován u 35 pacientů (67 %), u 17 pacientů (33 %) se jednalo o karcinoid atypický. Sledování potvrdilo v literatuře uváděné četnosti výskytu a prognózu pacientů, analyzuje též možnosti a efektivitu endobronchiální léčby a výsledky léčby obou typů nádoru. Zajímavým zjištěním je vysoká incidence duplicitních (i triplicitních) malignit u pacientů s atypickým karcinoidem (25 % nemocných)., Carcinoid is a neuroendocrine tumour that can produce endocrine active substances. It represents 1-2 % of pulmonary tumours. Based on histopathological characteristics, it is divided into typical and atypical carcinoid, which also affects the prognosis. {11,18} Typical carcinoid (TC) has a favourable prognosis, is often found incidentally, and the five-year survival rate is over 90 %. Atypical carcinoid (AC) exhibits a more aggressive behaviour, more frequent metastasizing, and a lower five-year survival rate (40-60 %). {5,11} We followed up a group of 52 patients with a histologically confirmed diagnosis of carcinoid who were treated at our clinic in the years 2002 to 2013. Typical carcinoid was diagnosed in 35 patients (67 %) and 17 patients (33 %) had atypical carcinoid. The follow-up confirmed the incidence rates and prognosis of patients reported in the literature, while analyzing the options and efficacy of endobronchial treatment and the results of treatments for both types of tumours. Úvod: Nízkomolekulární inhibitory tyrozinkináz receptoru pro epidermální růstový faktor (EGFR) představují moderní, účinné preparáty užívané k léčbě pacientů s pokročilým nemalobuněčným karcinomem plic (NSCLC). Aktivační mutace genu EGFR predikují dobrý efekt léčby EGFR tyrozinkinázovými inhibitory. Cílem této studie bylo zmapování výskytu mutací genu EGFR u pacientů s NSCLC v České republice. Metodika: Celkem 486 pacientů s pokročilým stadiem NSCLC (stadium IIIB a IV) neskvamózního histologického typu bylo vyšetřeno na přítomnost mutace genu EGFR na třech pneumoonkologických pracovištích. Porovnání zastoupení EGFR mutace dle pohlaví, věku a kouření bylo provedeno pomocí Fisherova-exaktního testu. Výsledky: Mutace genu EGFR byla prokázána u 74 (15,2 %) pacientů. Mutace byly prokázány u 25 (8,8 %) mužů vs. u 49 (24,3 %) žen (p < 0,001), u 12 (6,7 %) kuřáků vs. u 20 (11,2 %) exkuřáků vs. u 38 (37,3 %) nekuřáků (p < 0,001) a u 32 (15,7 %) pacientů do 65 let věku vs. u 42 (14,9 %) pacientů nad 65 let věku (p = 0,898). Závěr: Mutace genu EGFR byly prokázány u 15,2 % vyšetřených pacientů, nejčastěji u žen a nekuřáků. Dosažené výsledky jsou dobře srovnatelné s doposud publikovanými daty., Introduction: Low-molecular-weight tyrosine kinase inhibitors for epidermal growth factor receptor (EGFR) are modern, effective agents used to treat patients with advanced non-small-cell lung carcinoma (NSCLC). Activating EGFR gene mutations predict a good effect of treatment with EGFR tyrosine kinase inhibitors. The goal of this study was to map the occurrence of EGFR gene mutations in patients with NSCLC in the Czech Republic. Methods: A total of 486 patients with advanced stage of NSCLC (stage IIIB and IV) of nonsquamous histological type were investigated for the presence of EGFR gene mutations at three pneumo-oncology centres. A comparison of distribution of the EGFR mutation according to sex, age, and smoking status was performed by means of Fisher's exact test. Results: EGFR gene mutation was demonstrated in 74 (15.2%) patients. Mutations were shown in 25 (8.8%) men vs. 49 (24.3%) women (p < 0.001), in 12 (6.7%) smokers vs. 20 (11.2%) ex-smokers vs. 38 (37.3%) non-smokers (p < 0.001), and in 32 (15.7%) patients under 65 years of age vs. 42 (14.9%) patients over 65 years of age (p = 0.898). Conclusion: EGFR gene mutations were shown in 15.2% of the patients examined, most commonly in female patients and non-smokers.
