Kvalita výživy ve stáří ovlivňuje zásadním způsobem celkový stav organizmu a významné deficity mohou přispívat k multimorbiditě starších nemocných. Včasnou detekcí malnutrice ve stáří se můžeme vyhnout zhoršení funkční výkonnosti, závažným zdravotním komplikacím, zvýšené nemocnosti, úmrtnosti a zhoršení kvality života. Vyšší věk je spojen s častějším výskytem poruch výživy, především s malnutricí. Prevalence malnutrice u hospitalizovaných geriatrických pacientů se nachází v rozmezí mezi 30–60 % a u dlouhodobě institucionalizovaných dosahuje 52–85 %. Nepřesné posuzování a sledování nutričního stavu seniorů ve zdravotnických zařízeních zdravotnickým personálem může mít zásadní vliv na vznik malnutrice během hospitalizace. Cílem příspěvku je zhodnotit stav výživy seniorů na gerontopsychiatrickém oddělení a posoudit vliv faktorů jako deprese, úroveň kognitivního stavu a soběstačnosti na vznik malnutrice., The quality of nutrition in old age affects significantly the overall condition of the body, and significant deficits may contribute to multimorbidity in elderly patients. Early detection of malnutrition in old age can avoid worsening functional performance, serious medical complications, increased morbidity, mortality and impaired quality of life. Increasing age is associated with a greater incidence of nutritional disorders, especially with malnutrition. The prevalence of undernutrition in hospitalized geriatric patients is in the range between 30 to 60% and in the long-term institutionalized it reaches 52–85%. Inaccurate assessment and monitoring of nutritional status of older people in healthcare facilities healthcare professionals can have a significant influence on the occurrence of malnutrition during hospitalization. Cíl: Cílem tohoto příspěvku je prezentovat výsledky pilotního šetření testování dotazníku PNAP „Hodnocení potřeb pacientů v paliativní péči“ provedeného na onkologické klinice Fakultní nemocnice v Ostravě. Dále pak prezentovat zjištěnou důležitost bio-psycho-sociálně-spirituálních potřeb pacientů s ukončenou onkologickou léčbou. Metodika: Výzkumný soubor tvořilo 52 pacientů onkologické kliniky FN Ostrava, u nichž byla ukončena onkologická léčba v důsledku léčebně neovlivnitelné progrese onkologického onemocnění. Pro hodnocení potřeb byl použit dotazník - Hodnocení potřeb pacientů v paliativní péči (PNAP), který se skládá ze 42 položek sdružených do 5 domén. Výsledky: Za nejdůležitější považovali onkologičtí pacienti potřeby v doméně autonomie a fyzické potřeby. Nejméně byli respondenti spokojeni se saturací psychických a spirituálních potřeb. Pacienti mladší 60 let považovali sociální potřeby za signifikantně důležitější a současně byli méně spokojeni s jejich saturací (p<0,05). Ženy považovaly za důležitější psychické a spirituální potřeby. Se saturací těchto potřeb byly však více spokojeny (p<0,05) než muži. Z jednotlivých položek považovali respondenti nejčastěji za důležité: řešení bolesti, únavy, dušnosti, vyrovnat se s nemocí, komunikaci se zdravotníky a jejich podporu, rozhodování, respekt a úctu při ošetřování, vnitřní klid, naději a smysl života. Za nedostatečně saturované potřeby uvedli únavu, bolest, vyrovnanost s nemocí, strach ze závislosti, pokračování v obvyklých aktivitách, pozitivní náhled na život a naději. Závěr: Dotazník PNAP vychází z holistického pojetí péče. Zjištěné nedostatečně saturované potřeby by se měly stát předmětem ošetřovatelských intervencí., Aim: The aim of this contribution is to present the results of pilot research into a testing of the PNAP questionnaire “Patient needs assessment in palliative care“ implemented at the oncological clinic of the University Hospital Ostrava, as well as to present an established significance of bio-psycho-social-spiritual needs of patients with terminated oncological treatment. Methods: The sample was made up of 52 patients of the oncological clinic of the University Hospital Ostrava, whose oncological treatment was terminated as a result of an incurable progression of oncological disease. For needs assessment the questionnaire Patient needs assessment in palliative care (PNAP) was used, consisting of 42 items clustered into five domains. Results: Oncological patients considered the needs in the domains of autonomy and physical needs as the most important. They were the least satisfied with the saturation of psychological and spiritual needs. Patients under 60 years considered social needs as significantly more important and at the same time they were less satisfied with their saturation (p<0.05). Women considered psychological and spiritual needs as more important. However, they were more satisfied with the saturation of these needs than men (p<0.05). The following individual items were considered the most often as important: treating the pain, fatigue, dyspnoea, coming to terms with an illness, communication with health workers and their support, decision making, respect and dignity while being treated, inner peace, hope and the meaning of life. Insufficiently saturated needs were reported: fatigue, pain, reconciliation with an illness, fear from dependence, continuation in common activities, positive opinion of life and hope. Conclusion: The questionnaire PNAP is based on a holistic conception of care. Cíl: Cílem šetření bylo zjistit vliv kognitivní výkonnosti seniorů s demencí na jejich soběstačnost. Metodika: Výběrový soubor byl sestaven ze 106 seniorů (30 mužů a 76 žen, průměrný věk 83,25), u kterých byla potvrzena diagnóza demence. Statistická analýza byla provedena softwarem SPSS verze 10.0. Testování měřících nástrojů Mini Mental State Examination (MMSE) a Dotazníku soběstačnosti (DAD-CZ) probíhalo u seniorů žijících ve třech sociálních zařízeních. Pro testování vztahů a závislosti byl použit ANOVA test a Spearmanův korelační koeficient. Statistické testy byly hodnoceny na hladině významnosti α = 0,05. Výsledky: Naše studie potvrdila signifikantní vztahy mezi dotazníky MMSE a DAD (rs = 0,7619, p = 0,001) a současně byly zjištěny statisticky významné rozdíly v hodnocení soběstačnosti v závislosti na věku seniorů. Věk, vzdělání a pohlaví neměly vliv na výsledné hodnoty testu MMSE. Závěr: Výskyt kognitivních poruch je u starších osob poměrně častý. Problematika hodnocení soběstačnosti a kognitivních funkcí proto nesmí být opomíjenou oblastí v péči o seniory ani v zařízeních dlouhodobé péče. Pozdní detekce kognitivních poruch zhoršuje soběstačnost seniorů, která velmi často vede k rostoucí izolaci a větší závislosti a v konečném důsledku může vést k snížení kvality života., Aim: The aim of the investigation was to determine the influence of cognitive efficiency of the elderly with dementia on their self-sufficiency. Methods: The sample consisted of 106 elderly (30 men and 76 women, average age 83,25), with confirmed dementia. Statistical analysis was performed by software SPSS version 10.0. Testing of measuring tools Mini Mental State Examination (MMSE) and Self-sufficiency questionnaire (DAD-CZ) took place in the elderly living in three social settings. To test the relationship and dependencies the ANOVA test and Spearman correlation coefficient were used. Statistical tests were evaluated at a significance level α = 0,05. Results: Our study confirmed the significant relationship between MMSE and DAD (rs = 0,7619, p = 0,001) and also were found statistically significant differences in self-evaluation depending on the age of elderly. Age, education and gender had no impact on the resulting value of the MMSE test. Conclusion: The incidence of cognitive impairment in the elderly is relatively common. The problematics with self-sufficiency and cognitive functions should not therefore be neglected area in caring for the elderly even in the long-term-care facilities. Late detection of cognitive impairment worsens self-sufficiency of elderly, which very often leads to increased isolation and increased dependency and ultimately can lead to reduced quality of life., Radka Kozáková, Lucie Bártová, and Literatura 17
