Cíl: cílem našeho šetření bylo vyhodnotit moţnou přítomnost bolesti u klientů s demencí, kteří byli přeloţeni na gerontopsychiatrické lůţko psychiatrické léčebny v Kroměříţi z jiného zdravotnického zařízení. Metodika: studium překladové ošetřovatelské dokumentace z odesílajících zdravotnických zařízení, škála pro hodnocení bolesti u pacientů s demencí (PAINAD). Soubor: klienti přeloţení do psychiatrické léčebny v Kroměříţi, obojího pohlaví, starší 65 let, pro demenci jakéhokoliv typu. Tíţe demence byla středně těţká a těţká (bylo hodnoceno při přijetí na gerontopsychiatrické lůţko celkovým skóre dosaţeným v Mini Mental State Examination - MMSE - v rozmezí 0 - 17). Výsledky: pomocí škály PAINAD byla zachycena bolest u 26,7 % přeloţených klientů se středně těţkou nebo těţkou demencí. Bolest však byla řádně zadokumentována v překladových ošetřovatelských zprávách jen u 8,96 % klientů. Téměř třetina ošetřovatelské překladové dokumentace (29,9 %) měla zcela nevyplněný, nebo nedostatečně vyplněný formulář pro hodnocení bolesti. Téměř pětina (19,4 %) ošetřovatelských překladových zpráv fyzicky vůbec neobsahovala ţádný takový formulář. Závěr: přítomnost nepodchycené nebo nedostatečně vyhodnocené bolesti u klientů s kognitivními poruchami je závaţným problémem moderního ošetřovatelství. Nutná je potřeba neustálé edukace zdravotnického personálu, kvalitního vedení ošetřovatelské dokumentace, ale také potřeba důslednějšího systému kontrol z řad vedoucích pracovníků., Aim: to evaluate presence of pain in clients of both sexes, older than 65, who have been admitted to Mental hospital Kroměříţ due to dementia of any type. Methods: study of nursing documentation from dispatching health care facilities, PAINAD scale (Pain assessment in advanced dementia). Severity of dementia was moderate of severe (this was evaluated by Mini-Mental State Examination total score during admission process to our hospital, scores ranged from 0 to 17). Results: pain was detected in 26.7% of clients with moderate or severe dementia by means of PAINAD scale, compared to 8.96% of clients with pain which has been reported in nursing documentation. Almost in one third of nursing documentation (29.9%) no data or incomplete data about pain were found. Almost one fifth (19.4%) of nursing documentation have not contained any tool for pain screening. Concusion: undiagnosed pain in cognitively impaired clients is a severe problem of nursing care. Background: Non-adherence to treatment in seniors with dementia is a frequent and potentially dangerous phenomenon in routine clinical practice which might lead to the inappropriate treatment of a patient, including the risk of intoxication. There might be different causes of non-adherence in patients with dementia: memory impairment, sensory disturbances, limitations in mobility, economical reasons limiting access to health care and medication. Non-adherence leads to serious clinical consequences as well as being a challenge for public health. Aim: to estimate prevalence of non-adherence in seniors with dementia and to study correlation between cognitive decline and non-adherence. Subjects and Methods: Prospective study, analyzing medical records of seniors with dementia admitted to the inpatient psychogeriatric ward in the Kromeriz mental hospital from January 2010 to January 2011. Cognitive decline measured by MMSE, prevalence of Non-adherence to treatment and reasons for patient Non-adherence were studied. Results: Non-adherence to any treatment was detected in 31.3% of seniors; memory impairment was the most common cause of non-adherence to treatment. Conclusion: In conclusion, non-adherence to treatment in the studied group of seniors with dementia correlates with the severity of cognitive impairment – a higher cognitive decline correlates with a higher risk of non-adherence to treatment. and J. Úvod: Demence patří mezi onemocnění poškozující nejen paměť, ostatní kognitivní funkce, ale i soběstačnost a funkční stav jedince. Kauzální léčba demence dosud neexistuje, onemocnění časem dospívá do terminální fáze projevující se imobilitou, inkontinencí, poruchami nutrice a plnou závislostí na okolí. Primární, sekundární i terciární prevence je u tohoto onemocnění klíčová. Metodika: Byla uspořádána observační studie hodnotící tíži demence u pacientek odeslaných k první hospitalizaci na ženský gerontopsychiatrický primariát psychiatrické nemocnice v Kroměříži. Studována byla příjmová lékařská zpráva, zkoumána byla tíže kognitivního deficitu, délka průběhu nemoci od prvních příznaků po odeslání k hospitalizaci, předchozí ambulantní léčba demence. Výsledky: Většina pacientek odeslaných na gerontopsychiatrické lůžko k léčbě demence přichází až ve stadiu středně těžké nebo těžké demence. Dalším závažným zjištěním je, že mezi prvotními příznaky demence a odesláním k nastavení léčby na gerontopsychiatrické oddělení uplyne relativně dlouhá doba, během které mohla být nasazena adekvátní a efektivní léčba. Dalším zjištěním je fakt, že u většiny pacientek odeslaných k prvotní hospitalizaci na gerontopsychiatrii nebyla vedena žádná ambulantní léčba demence ve snaze ovlivnit její časnější stadia. Závěr: Existují významné rezervy v sekundární prevenci demencí. Včasná a dostupná moderní diagnostika demence, včasná léčba demence, ale i vysoká erudice ambulantních lékařů může přispět ke stabilizaci onemocnění, oddálení těžkých stadií demence a zlepšení kvality života u pacientů s demencí., Introduction: Dementia is a disease leading to memory impairment, cognitive decline, loss of self-sufficency and deterioration of functional status. Causal therapy of dementia does not exist, disease is progredient in time up to terminal phase with imobility, incontinentia, problems with nutrition and functional dependence on caregivers aid. Primary, secondary and tertiary prevention of dementia has crucial position. Methods: Observational study evaluating stage of dementia among patients admitted to psychogeriatric ward, Mental hospital in Kroměříž, has been designed. Admission diagnosis, severity of cognitive decline, time past between first symtomps of dementia and admission to hospital, and anamnesis of prior outpatient treatment of dementia have been monitored. Results: Most of patients were admitted to psychogeriatric ward in moderate or severe stage of dementia, with long time between first symtomps of dementia and admission to hospital with loosing ideal time for effective therapy. Most of patients admitted to psychogeriatric ward had no prior outpatient therapy of dementia in anamnesis. Conclusion: There are serious shortcomings in secondary prevention of dementia. Early and modern diagnostic management of dementia, as well as early and effective treatment management, with high level of erudition in field of dementia among primary care physicians may stabilize natural course of dementias, postpone serious stages of dementia and patients quality of life improvement., Jan Lužný, Blanka Skopalová, and Literatura
