Cíl: Hlavním cílem výzkumného šetření bylo zjistit, které faktory ovlivňují pouţívání ošetřovatelských diagnóz v klinické praxi. Dalším cílem bylo zjistit rozdíly v hodnocení faktorů ovlivňujících pouţívání ošetřovatelských diagnóz v praxi podle věku, vzdělání a typu pracoviště. Metodika: Pro získání dat byl sestaven nestandardizovaný dotazník zaměřený na zjištění faktorů ovlivňujících pouţívání ošetřovatelských diagnóz. Výzkumný soubor tvořilo 253 registrovaných všeobecných sester z 9 zdravotnických zařízení v České republice. Sestry hodnotili na Likertově škále od 1 do 5, jak jednotlivé faktory ovlivňují pouţívání ošetřovatelské diagnostiky v praxi. Výzkumné šetření probíhalo od června 2011 do listopadu 2011. Výsledky: Podle našeho výzkumného souboru nejvíce ovlivňují pouţívání ošetřovatelských diagnóz v praxi následující faktory: počet pacientů na 1 sestru, administrativní zátěţ, syndrom vyhoření, nedostatek času, rozdíly v teorii a praxi a nedostatek personálu. Statisticky významné rozdíly v hodnocení faktorů podle věku byly zjištěny v hodnocení 2 faktorů: rozdíly v teorii a praxi, nedostatek motivace. Podle dosaţeného vzdělání byly statisticky významné rozdíly zjištěny v hodnocení 4 faktorů: nejednotnost ošetřovatelských diagnóz, terminologie diagnóz, rozdíly v teorii a praxi, rutinní práce. V závislosti na typu pracoviště byly statisticky významné rozdíly nalezeny v hodnocení 5 faktorů: terminologie diagnóz, rozdíly v teorii a praxi, nedostatek praktických zkušeností s ošetřovatelskou diagnostikou, rutinní práce, syndrom vyhoření. Závěr: Rozeznání faktorů ovlivňujících pouţívání ošetřovatelských diagnóz v praxi a zlepšení institucionální podpory můţe ulehčit implementaci ošetřovatelské diagnostiky v klinické praxi, Aim: The main aim of the study was to identify which factors influenced the use of nursing diagnoses in clinical practice. Another aim was to ascertain whether age, education and type of workplace affected the relative importance accorded to these factors. Methods: A non-standardized questionnaire on factors influencing the use of nursing diagnoses was administered to a sample consisting of 243 registered general nurses from 9 healthcare facilities in the Czech Republic. Using Likert´s 5-point scale, the nurses rated the importance of several factors in influencing their use of nursing diagnoses in practice. The research was conducted between June 2011 and November 2011. Results: Based on our sample, there are several factors which influence the use of nursing diagnoses in practice. Some of them are: number of patients per nurse, administrative strain, burnout syndrome, lack of time, discrepancies between theory and practice, lack of staff. Statistically significant age-related differences in rating factors were found for the following: discrepancies between theory and practice, lack of motivation. Statistically significant differences in ratings relative to education were found for four factors: disunity of nursing diagnoses, diagnostic terminology, discrepancies between theory and practice, workplace routine. Based on type of workplace, statistically significant differences in ratings were found for five factors: diagnostic terminology, discrepancies between theory and practice, lack of practical experience in nursing diagnostics, workplace routine, burnout syndrome. Conclusion: Recognizing the factors which influence the use of nursing diagnoses and enhancing institutional support can facilitiate the implementation of nursing diagnostics in practice., Helena Macková, Renáta Zeleníková, and Literatura 14
Cieľ: Cieľom práce bolo validizovať defi nujúce charakteristiky ošetrovateľskej diagnózy Defi cit vedomostí vybraným súborom slovenských sestier – expertov, teda zistiť, ktoré defi nujúce charakteristiky sú sestrami považované za hlavné a ktoré za vedľajšie. Metódy: Na hodnotenie defi nujúcich charakteristík ošetrovateľskej diagnózy Defi cit vedomostí bol zvolený Fehringov model validity diagnostického obsahu. Výsledky: Za hlavné defi nujúce charakteristiky boli označené: „slovné vyjadrenie problému“ a „chybné, nepresné dodržiavanie inštrukcií“. Za vedľajšie defi nujúce charakteristiky boli označené: „chybné, nepresné výsledky testu“ a „neprimerané správanie“. Charakteristika „prehnané správanie“ dosiahla vážené skóre menej ako 0,5. Záver: Nie všetky defi nujúce charakteristiky ošetrovateľskej diagnózy Defi cit vedomostí uvedené v klasifi kačnom systéme NANDA-International sú pre vybraný súbor sestier rovnako významné pre jej stanovenie., Aim: The aim of the study was to validate the nursing diagnosis Deficient Knowledge by a chosen sample of Slovak nurses – experts, i. e. to find out which defining characteristics are considered major and minor. Methods: The Fehring’s Diagnostic Content Validity Model was used to evaluate the defi ning characteristics of nursing diagnosis Deficient Knowledge. Results: Th e following defi ning characteristics were marked as major: “verbalization of the problem”, “inaccurate compliance with instructions”. The following characteristics were marked as minor: “poor and inaccurate performance in a test” and “inappropriate behaviour”. The defining characteristic “exaggerated behaviour” had the weighted ratio less than 0.5. Conclusion: Not all defining characteristics of nursing diagnosis Deficient Knowledge found in NANDA-International classification system are equally significant for chosen sample of Slovak nurses to determine this nursing diagnosis., Renáta Zeleníková, Katarína Žiaková, and Literatura 16
Cieľ: Cieľom práce bolo validovať definujúce charakteristiky ošetrovateľskej diagnózy Deficit objemu telových tekutín vybraným súborom slovenských sestier – expertov, teda zistiť, ktoré definujúce charakteristiky sú sestrami povaţované za hlavné a ktoré za vedľajšie. Metodika: Na hodnotenie definujúcich charakteristík ošetrovateľskej diagnózy Deficit objemu telových tekutín bol zvolený Fehringov model validity diagnostického obsahu (DCV model). Definujúce charakteristiky hodnotili sestry, ktoré získali minimálne 4 body podľa modifikovaných Fehringových kritérií. Súbor tvorilo 116 slovenských sestier – expertov. Definujúce charakteristiky boli sestrami hodnotené na Likertovej škále od 1 do 5 (1 – ţiadna významnosť, 2 – malá významnosť, 3 – stredná významnosť, 4 – veľká významnosť, 5 – najväčšia významnosť). Zber údajov prebiehal v období máj aţ október 2010. Výsledky: Za hlavné definujúce charakteristiky bolo súborom slovenských sestier – expertov zo 17 charakteristík označených 6 charakteristík: zníţený turgor koţe, zníţená diuréza, suché sliznice, zníţené napätie jazyka, zníţená venózna náplň, zníţený krvný tlak. Za vedľajšie definujúce charakteristiky boli označené nasledujúce charakteristiky: zvýšená merná hmotnosť moču, suchá pokoţka, zvýšený hematokrit, slabosť, smäd, oslabený pulz, zníţený objem pulzu (nitkovitý pulz), zvýšená pulzová frekvencia, zmeny v psychickom stave, zvýšená telesná teplota. Charakteristika „náhla strata hmotnosti (s výnimkou tretieho priestoru)“ dosiahla váţené skóre menej ako 0,5. Záver: Súbor slovenských sestier za významné znaky pre určenie ošetrovateľskej diagnózy Deficit objemu telových tekutín povaţuje 6 znakov., Aim: The aim of this study is to validate the defining characteristics of a nursing diagnosis of Deficient Fluid Volume by considering a selected sample of Slovak nurse – experts. In other words, to ascertain which defining characteristics are rated by these nurses as major or minor. Methods: Fehring’s Diagnostic Content Validation model (DCV model) was used for validation of this nursing diagnosis. Slovak nurses who met the modified criteria (4 points) recommended by Fehring comprised the sample of experts - a total of 116 Slovak nurses – experts. Defining characteristics were rated by the nurses using Likert’s 5-point scale (1 – not at all characteristic, 2 – very little characteristic, 3 – somewhat characteristic, 4 – considerably characteristic, 5 – very characteristic). Data collection occurred between May and October 2010. Results: Out of 17 defining characteristics of a nursing diagnosis of Deficient Fluid Volume, the Slovak nurses’ sample classified six cues as major: decreased skin turgor, decreased urine output, dry mucous membranes, decreased tongue turgor, decreased venous filling, and decreased blood pressure. The following cues were classified as minor defining characteristics: increased urine concentration, dry skin, elevated hematocrit, weakness, thirst, decreased pulse volume and pressure, increased pulse rate, changes in mental status, increased body temperature. The weighted score of the defining characteristic „sudden weight loss (except in the third space) was less than 0.5. Conclusion: The Slovak nurses’ sample studied considered six characteristics as important in justifying the nursing diagnosis Deficient Fluid Volume., Renáta Zeleníková, Katarína Žiaková, and Literatura 29
