The aim of our study was to determine the effect of selected cytostatics on a human ovarian cancer cell line A2780 as a model system for ovarian cancer treatment. This cell line is considered cisplatin-sensitive. Panel of tested cytostatics included cisplatin, paclitaxel, carboplatin, gemcitabine, topotecan and etoposide. These cytostatics have a different mechanism of action. To evaluate cytotoxic potential of the tested compounds, the methods measuring various toxicological endpoints were employed including morphological studies, MTT assay, dynamic monitoring of cell proliferation with xCELLigence, cell cycle analysis, caspase 3 activity and expression of proteins involved in cell cycle regulation and cell death. Bisphosphonates (BP) are potent inhibitors of bone resorption used mainly in the treatment of metastatic bone disease and osteoporosis. By inhibiting bone resorption, they prevent complications as pathological fracture, pain, tumor-induced hypercalcemia. Even though patient’s benefit of BP therapy is huge, various side effects may develop. Bisphosphonate-related osteonecrosis of the jaws (BRONJ) is among the most serious ones. Oncologic patients receiving high doses of BP intravenously are at high risk of BRONJ development. BPs impair bone turnover leading to compromised bone healing which may result in the exposure of necrotic bone in the oral cavity frequently following tooth extraction or trauma of the oral mucosa. Frank bone exposure may be complicated by secondary infection leading to osteomyelitis development with various symptoms and radiological findings. In the management of BRONJ, conservative therapy aiming to reduce the symptoms plays the main role. In patients with extensive bone involvement resective surgery may lead to complete recovery, provided that the procedure is correctly indicated. Since the treatment of BRONJ is difficult, prevention is the main goal. Therefore in high risk patients dental preventive measures should be taken prior to bisphosphonate administration. Nádory měkkých tkání jsou rozsáhlou a značně heterogenní skupinou onemocnění vyžadující multimodalitní diagnostický přístup. Magnetická rezonance se stala díky vysoké senzitivitě a kontrastnímu rozlišení nejefektivnější metodou při jejich detekci a lokálním stagingu. Její možnosti při stanovení histologického typu nádoru a často i při rozhodnutí, zda je leze benigní či maligní jsou značně omezené. Z tohoto důvodu musí být až na výjimky kombinována s histologickým vyšetřením, které je rovněž nezbytné pro kompletní staging. Článek se zabývá technikou vyšetřování nádorů měkkých tkání pomocí magnetické rezonance a využitím metody při stagingu a diferenciální diagnóze. Krátce jsou zmíněny i možnosti dalších zobrazovacích modalit., Soft tissue tumors are large and heterogenous group of diseases requiring multimodality diagnostic approach. Magnetic resonance is, thanks to its high sensitivity and contrast resolution, the most effective method for their detection and local staging. Its possibilities in determining the histological type of tumor and in the decision whether the lesion is benign or malignant are quite limited. For this reason it MRI must be, with few exceptions, combined with histological examination, which is also necessary for complete staging. INTRODUCTION: Leptin is an adipokine which has a direct relationship to obesity. Our aim was to measure this hormone in pregnant women at three months intervals throughout their pregnancies to determine the serum value of those who developed preeclampsia. MATERIAL AND METHODS: We followed 19 women (median age 24.8 +/- 5.7 years) with pre-gestational Body Mass Index (BMI) less than 25 kg/m2, 21 (median age 26.1 +/- 4.6 years) with BMI higher than 25 kg/m2 and 16 (median age 30.9 +/- 5.8 years) with Gestational Diabetes Mellitus (GDM) (median age 30.9 +/- 5.8 years), recruited in the 1st trimester of pregnancy. Serum levels of leptin were measured with radioimmunoassay (RIA) technique. RESULTS: In the first trimester of pregnancy leptin levels showed statistically significant differences between normal weight and overweight-obese women (p < 0.001), diabetic women (p < 0.05) and the subgroup of preeclamptic women (p < 0.001). For those women with PGBMI > or = 40 kg/m2 and leptin > or = 40 ng/ml in the second trimester, the Odds Ratio (OR) to develop preeclampsia was of 47.95% CI (4.1-527.2). Analyzing leptin values with ROC curves, the greatest area under the curve (AUC) was for leptin in the second trimester (0.773, CI: 0.634-0.911). CONCLUSION: Women with morbid obesity (BMI > or = 40 kg/m2) had significantly higher levels of serum leptin (p < 0.01) and a value of 40 ng/ml of this hormone seems to be predictive of developing preeclampsia in this group of patients. and H. Cíl studie: Sledování a vyhodnocení přínosu výpočtu ROMA indexů u ovarialního karcinomu a porovnání s hodnotami sérových nádorových markerů HE4 a CA125. Typ studie: Prospektivní studie Materiál a metody: V období od června 2010 do prosince 2011 bylo v Imunoanalytické laboratoři FN Plzeň vyšetřeno sérum 553 pacientek gynekologicko-porodnické kliniky. Pacientky byly rozděleny do skupin podle diagnóz (benigní, maligní) a dále pak do dalších dvou skupin podle menopauzálního statusu. Hodnoceny byly tedy celkem čtyři skupiny. U všech pacientek bylo stanoveno HE4 a CA125 chemiluminiscenční metodou na přístroji Architect i1000 (Abbott, USA). Dále byly vypočítány ROMA indexy a jejich hodnoty porovnány se sérovými hodnotami HE4 a CA125. Pro veškeré další statistické výpočty byl použit statistický software SAS 9.2. Výsledky: ROMA indexy, HE4 a CA125 byly významně zvýšeny ve skupině premenopauzálních i postmenopauzálních pacientek s histologicky potvrzeným karcinomem ovarií oproti skupinám s benigním tumorem ovarií. Pro diferenciální diagnostiku benigních a maligních nádorů ovarií je možné použít ROMA indexy, HE4 a CA125 (shodně p-Value <0,0001). Podle hodnot AUC jsme sestavili pořadí užitečnosti jednotlivých parametrů v premenopauzálním a postmenopauzálním období. Nejvyšší přínos měl v našem souboru žen výpočet ROMA2 indexu, který se kalkuluje v postmenopuzálním období (AUC=0,9777), následuje pak CA125 postmenopauzálně (AUC=0.9715) a CA125 premenopauzálně (AUC=0,9534). Nejnižší výtěžnost měl HE4 premenopauzálně (AUC=0,8702). Závěr: Hodnoty ROMA1 indexu, ROMA2 indexu, HE4 a CA125 byly významně zvýšené ve skupinách ovariálních karcinomů oproti skupinám benigních nádorů ovarií. Výše uvedené parametry lze tudíž použít pro diferenciální diagnostiku ovariálního karcinomu. Pokud bychom měli hovořit o možném využití při prvozáchytu karcinomu ovarií, v našem souboru se jevil nejlépe ROMA2 index. ROMA1 index nedosáhl takové efektivity. Z hodnot AUC je možné dále vysledovat, že HE4 nedosáhl tak dobrých výsledků jako CA125. Jejich vzájemná kombinace a vypočet zejména ROMA2 indexu však ukazuje na oprávněnost zařazení HE4 do rutinního klinického využití., Objective: Monitoring and evaluation of the benefit of ROMA indexes calculation in ovarian cancer and comparison with values of serum tumor markers HE4 and CA125 Design: Prospective study Material and Methods: In the period from June 2010 to December 2011 was in the Immunoanalytical laboratory, University Hospital Pilsen serum of 553 patients from department of Gynecology and Obstetrics examined. Patients were divided into groups by diagnosis (benign, malignant) and then into the other two groups according to menopausal status. Total of four groups were evaluated. For all patients HE4 and CA125 were determined using the chemiluminescent Architect i1000 instrument (Abbott, USA). In addition, ROMA indexes were calculated. In the next step ROMA values were compared with the serum HE4 and CA125 values. Statistical software SAS 9.2 was used for all other statistical calculations. Results: ROMA indexes, HE4 and CA125 were significantly increased in the group of premenopausal and postmenopausal patients with histologically confirmed ovarian cancer compared with groups of benign ovarian tumors. For differential diagnosis of benign and malignant ovarian tumors can be used ROMA indexes, HE4 and CA125 (both p-value <0.0001). According to the AUC, we compiled a ranking usefulness of the various parameters in pre-menopausal and postmenopausal period. The highest contribution had in our group of women the calculation of ROMA2 index that is calculated in postmenopusal period (AUC = 0.9777), followed by the CA125 postmenopausal (AUC = 0.9715) and CA125 premenopausal (AUC = 0.9534). The lowest yield was HE4 premenopausal (AUC = 0.8702). Conclusion: Values of ROMA1 index, ROMA2 index, HE4 and CA125 were significantly increased in groups of ovarian cancer compared with benign ovarian tumors groups. All the parameters can be used for differential diagnosis of ovarian cancer. If we were talking about a possible use in primary detection of ovarian cancer ROMA2 index was the best one. ROMA1 index did not reach such efficiency. According AUC can be observed that HE4 didn‘t achieve such good results as CA125. Článek uvádí přehled současných poznatků o žaludečních neuroendokrinních nádorech. Jde o heterogenní skupinu zahrnující 3 typy, které se odlišují svým biologickým chováním a léčebným přístupem. Typ 1 vzniká na podkladě chronické korporální atrofické gastritidy, je relativně málo maligní, léčba spočívá v endoskopickém odstranění drobných nádorů, u větších lézí či infiltrativních forem je nutná resekce žaludku, výjimečně gastrektomie. Typ 2 vzniká v rámci Zollinger-Ellisonova syndromu. Oba uvedené typy jsou gastrin-dependentní. Naproti tomu typ 3 je solitární nádor, vznikající bez závislosti na gastrinu, jeho maligní potenciál je značný. Nádor nutno léčit chirurgicky, u pokročilých forem je nezbytná paliativní terapie., The paper presents recent opinions on gastric neuroendocrine neoplasms. This is a heterogeneus group including three types that distinguish in their biological behaviour, and require different therapeutical approach. Type 1 arises in setting of chronic corporal atrophic gastritis, malignant potential of this type is lower, this type 1 tumour may be treated with endoscopic removal of multiple small tumour lesions. Gastric resection is indicated in larger lesions with infiltrative pattern of growth. Gastrectomy is rarely indicated. Type 2 arises as consequence of Zollinger-Ellison syndrome. Both above-mentioned type are gastrin-dependent. Type 3 is a solitary, gastrin-independent tumour with higher malignant potential. Cíl. Zjistit, zda je možné pomocí ultrazvukového vyšetření s kontrastní látkou, odlišení zánětlivého postižení charakterizovaného angiogenezí (revmatoidní artritidy) od změn degenerativních. Zjistit, zda aplikace kontrastní látky pomůže odlišit aktivní fázi od změn chronických u již stanovené revmatoidní artritidy (RA). Metoda. Vyšetření probíhala na přístroji Philips iu22 s povrchovou lineární sondou 12-17 MHz, kontrastní látka (k.l.) použitá v této práci byl sulfurhexafluorid (SonoVue®, Bracco) v množství 5 ml. První skupina (odlišení revmatoidní artritidy od degenerativních změn) zahrnovala 63 pacientů, druhá skupina pacientů (stanovení aktivní fáze revmatoidní artritidy) obsahovala vyšetření 55 pacientů. Všichni pacienti byli sledováni revmatologem, byla provedena zvyklá laboratorní vyšetření, u pacientů s revmatoidní artritidou bylo stanoveno DAS28 skóre. Výsledky. Při statistickém zpracování byla senzitivita odlišení degenerativních změn od revmatoidní artritidy 75%, specificita 83,7%, u druhého cíle byla senzitivita aktivní fáze RA 80,6 %, specifkita 66,7% při DAS skóre vyšším jak 5. Na hladině významnosti a = 0,05 byl shledán statisticky významný rozdíl v DAS skóre mezi pacienty s negativním a pozitivním výsledkem ultrazvuku (p < 0,001). Závěr. Senzitivita a specificita vyšetření ultrazvukového vyšetření s kontrastní látkou při odlišení zánětu od změn degenerativních může přispět v diferenciální diagnostice u zánětů, které jsou charakterizovány angiogenezí. U stanovení aktivní fáze RA byla shledána vysoká senzitivita a nižší specificita při ultrazvukovém vyšetření. V obou případech by se však tato metoda mohla stát součástí algoritmu vyšetření., Aim. The aim of this work was to determine whether the contrast enhanced ultrasound examination would be useful in differentiation between inflammatory joint disease (characterized by angiogenesis) and degenerative disease and to determine whether this method could help to distinguish the active phase of chronic rheumatoid arthritis (RA). Method. All patients were examined on the ultrasound device Philips iu22 using linear probe 12-17 MHz, the applied contrast agent was sulfurhexafluorid (SonoVue®, Bracco) in quantities of 5 ml. The first group (differentiation between RA and degenerative changes) includes 63 patients, a second group (determination of the active phase of RA) includes 55 patients. All the patients underwent also clinical examination performed by rheumatologist and laboratory examination, in patients with RA the DAS28 score was established. Results. The sensitivity for the differential diagnosis between the degenerative disease and RA was 75%, specificity 83.7%. For the determination of RA active phase the sensitivity was 80.6%, specificity 66.7%. For the correlations we used the cut-off DAS score 5 to identify the active RA patients. We found statistically significant difference in DAS score between RA patients with negative and positive findings on ultrasound (p < 0,001). Conclusion. We found that contrast enhanced ultrasound provides clinically acceptable sensitivity and specificity for differentiation between RA and degenerative disease, high sensitivity and comparatively lower specificity for the RA active phase identiffication.
