Cíl: Cílem studie bylo zjistit, jak rodinní pečovatelé pečující o blízkou osobu v České republice hodnotí definující charakteristiky ošetřovatelské diagnózy Přetížení pečovatele. Metodika: Na klinickou validizaci ošetřovatelské diagnózy Přetížení pečovatele byl použit „Na pacienta zaměřený Fehringův model klinické diagnostické validity“. Výzkum u rodinných pečovatelů probíhal v měsících srpen a září 2010 v Agentuře domácí péče Victoria na území města Šumperk. Do výzkumného souboru bylo vybráno 50 rodinných pečovatelů, kteří pečovali o své blízké v domácím prostředí déle než 2 roky. Souhlas se zařazením do výzkumu vyjádřilo 23 rodinných pečovatelů, co je 46 %. Výsledky: Rodinní příslušníci za hlavní definující charakteristiky Přetížení pečovatele označili celkem 8 charakteristik: obavy z možné institucionalizace péče příjemce; obavy, jak vyřešit péči o člena rodiny, když nebudu schopen tuto péči vykonávat; obavy o zdraví příjemce péče v budoucnosti; obavy související se schopností pečovatele poskytovat péči v budoucnosti; konflikt v rodině; nejistota ze změny vztahu s příjemcem péče; smutek ze změny vztahu s příjemcem péče; starost o členy rodiny. Za vedlejší bylo označeno 10 definujících charakteristik: obtíže v dokončování požadovaných úkolů, obtíže se sledováním příjemce péče v průběhu nemoci, obtíže ve vykonávání požadovaných úkolů, vysoký krevní tlak, kardiovaskulární onemocnění, somatizace, frustrace, depresivní nálada, porušený spánek, stres. Závěr: Pro skupinu vybraných pečovatelů nebyly všechny definující charakteristiky uvedeny v klasifikačním systému NANDA International významné., Aim: The aim of the study was to find out how the family caregivers caring for their close relatives in the Czech Republic evaluate defining characteristics of the nursing diagnosis Caregiver Role Strain. Methods: Fehring´s patient – focused clinical diagnostic validity model was used to clinical validation of the nursing diagnosis Caregiver Role Strain. The research was ongoing during the months August and September 2010 at Home Care Agency Victoria in region of city Šumperk. Fifty family caregivers, who take care for their family members more than 2 years, were chosen to the research sample. Twenty three caregivers (46 %) agreed with their including to the research. Results: Caregivers classified in total the following eight defining characteristics as major ones: apprehension about possible institutionalization of care receiver, apprehension about care receiver´s care if caregiver unable to provide care, apprehension about the future regarding care receiver´s health, apprehension about the future regarding caregiver´s ability to provide care, family conflict, uncertainty regarding changes relationship with care receiver, grief regarding changed relationship with care receiver, concerns about family members. Caregivers classified ten defining characteristics as minor ones: difficulty completing required tasks, difficulty watching care receiver go through the illness, difficulty performing required tasks, hypertension, cardiovascular disease, somatization, frustration, feeling depressed, disturbed sleep, stress. Cieľ: Cieľom práce bolo validizovať defi nujúce charakteristiky ošetrovateľskej diagnózy Defi cit vedomostí vybraným súborom slovenských sestier – expertov, teda zistiť, ktoré defi nujúce charakteristiky sú sestrami považované za hlavné a ktoré za vedľajšie. Metódy: Na hodnotenie defi nujúcich charakteristík ošetrovateľskej diagnózy Defi cit vedomostí bol zvolený Fehringov model validity diagnostického obsahu. Výsledky: Za hlavné defi nujúce charakteristiky boli označené: „slovné vyjadrenie problému“ a „chybné, nepresné dodržiavanie inštrukcií“. Za vedľajšie defi nujúce charakteristiky boli označené: „chybné, nepresné výsledky testu“ a „neprimerané správanie“. Charakteristika „prehnané správanie“ dosiahla vážené skóre menej ako 0,5. Záver: Nie všetky defi nujúce charakteristiky ošetrovateľskej diagnózy Defi cit vedomostí uvedené v klasifi kačnom systéme NANDA-International sú pre vybraný súbor sestier rovnako významné pre jej stanovenie., Aim: The aim of the study was to validate the nursing diagnosis Deficient Knowledge by a chosen sample of Slovak nurses – experts, i. e. to find out which defining characteristics are considered major and minor. Methods: The Fehring’s Diagnostic Content Validity Model was used to evaluate the defi ning characteristics of nursing diagnosis Deficient Knowledge. Results: Th e following defi ning characteristics were marked as major: “verbalization of the problem”, “inaccurate compliance with instructions”. The following characteristics were marked as minor: “poor and inaccurate performance in a test” and “inappropriate behaviour”. The defining characteristic “exaggerated behaviour” had the weighted ratio less than 0.5. Cieľ: Príspevok má design prierezovej štúdie, s parciálnym cieľom testovať psychometrické vlastnosti nástroja kvality ţivota (Caregiver Quality of Life Index Cancer) opatrovateľov v starostlivosti o zomierajúcich z hľadiska formy poskytovanej starostlivosti. Metodika: Výskumný súbor tvorilo 263 neprofesionálnych opatrovateľov, poskytujúcich starostlivosť zomierajúcemu v podmienkach domácej, kombinovanej a paliatívnej/hospicovej starostlivosti v troch krajoch Slovenskej republiky (Prešovský, Ţilinský, Banskobystrický). Psychometrické vlastnosti nástroja pouţitého v našej štúdii boli testované na základe analýzy spoľahlivosti, korelačnej a faktorovej analýzy. Výsledky: Faktorovou analýzou sa v našom výskume podarilo potvrdiť 4-faktorovú štruktúru nástroja CQOLC (záťaţ/narušenie ţivotného štýlu; sociálna opora; pozitívna adaptácia; finančná situácia). Extrahované faktory na základe odpovedí respondentov v našom výskumnom súbore nie sú úplne identické s originálnou verziou škály CQOLC. Najlepšiu konštruktovú validitu a vnútornú konzistenciu škály CQOLC sme potvrdili u respondentov, ktorí poskytujú domácu starostlivosť zomierajúcemu príbuznému. Záver: Psychometrickou analýzou dotazníkov sme zistili, ţe reliabilita a konštruktová validita nami pouţitého nástroja je porovnateľná s originálnou verziou, napriek odchýlkam v lokalizácii niektorých poloţiek. Nástroj CQOLC je pouţiteľný na posúdenie kvality ţivota neprofesionálnych opatrovateľov v kontexte starostlivosti o zomierajúcich v domácom prostredí., Aim: The paper has the design of the cross-sectional study. The aim of the study was to test the psychometric properties of the quality of life (Caregiver Quality of Life Index of Cancer) the caregivers who provide care of the dying patients in terms of forms of care. Methods: The sample of our research consisted of 263 non-professional caregivers, providing home care of the dying, combined care and palliative care in three districts of the Slovak Republic (Prešovský, Ţilinský, Banskobystrický). Psychometric properties of the instrument used in our study were tested on the basis of the analysis of the reliability, of the correlation analysis and factor analysis. Results: Factor analysis of questionnaire CQOLC in our research confirmed the fourfactor structure of the instrument CQOLC (burden/lifestyle disruption; social support; a positive adaptation; the financial concerns). Extracted the factors on the basis of the answers of the respondents in our research are not completely identical with the original version CQOLC. The best construct validity and reliability of the CQOLC we have confirmed with the caregivers, who provide a home care of the dying patients. Conclusion: Psychometric analysis of the questionnaires we found that, the reliability and construct validity is comparable to the original version of CQOLC, despite variations in the location of some items. The CQOLC could thus form the basis for the implementation of the assessment of the quality of life of nonprofessional caregivers in the context of care for the dying at home., Radka Šerfelová, and Literatura 25
Cieľ: Obsahová validácia ošetrovateľskej diagnózy Chronická zmätenosť a identifi kácia defi nujúcich charakteristík v podmienkach slovenskej a českej ošetrovateľskej praxe. Metódy: Na validáciu bol použitý retrospektívny Diagnostic Content Validity Model (DCV Model) podľa Fehringa. Za expertov boli považované sestry, ktoré získali minimálne 4 body podľa modifi kovaných Fehringových kritérií. Súbor expertov tvorilo 144 slovenských a 107 českých sestier. Výsledky: Za hlavné defi nujúce charakteristiky (vážené skóre VS viac ako 0,75) považovali zhodne slovenskí aj českí experti: narušená interpretácia podnetov a reakcia na ne, progresívna kognitívna porucha, klinicky podložená organická porucha mozgu, zmeny v časopriestorovej orientácii a dlhotrvajúca kognitívna porucha, ktoré sú totožné s charakteristikou syndrómu demencie. Závery: Experti nepovažovali všetky defi nujúce charakteristiky za rovnako významné. Určitý podiel zohralo zloženie samotného súboru expertov, ktorý naplnil iba spodnú hranicu kritérií na experta (6 bodov), nekompatibilita NANDA-I Taxonómie II so Zoznamom sesterských diagnóz platným v SR a absencia štandardizovanej ošetrovateľskej terminológie v Českej republike., Aim: Content validation of the nursing diagnosis Chronic confusion and identification of the defining characteristics in Slovak and Czech nursing practices. Method: Fehring’s retrospective Diagnostic Content Validity Model (DCV Model) was used. Experts were nurses who obtained minimum four points according to modified Fehring’s criteria. The group of respondents comprised of 144 Slovak and 107 Czech nurses. Results: Slovak and Czech experts alike considered the following characteristics to be defining (weighted score higher than 0.75): altered interpretation and altered response to stimuli, progressive cognitive impairment, clinical evidence of organic impairment, changes in spatiotemporal orientation and long-standing cognitive impairment, which are congruent with the characteristic features of dementia syndrome. Conclusion: Experts did not regard all the defining characteristics to be of the same importance. This was due to the composite of respondents, who only reached bottom level of criteria for an expert (6 points), incompatibility of NANDA-I Taxonomy II with the List of nursing diagnosis used in the Slovak Republic and lastly the absence of standardised nursing terminology in the Czech Republic., Martina Tomagová, Ivana Bóriková, and Literatura 28
