- Background: There are several environmental factors which influence the preschoolers psychomotor development and the most considerable ones are socioeconomic status and day cares learning&care environment. Aim of this study is to assess how the change of socioeconomic gradient and the quality of care environment influence the average and the differences in psychometric domains of ASQ3 in preschoolers. Method: We assessed the psychometric parameters of all preschoolers enrolled in some of day cares in Vlora city, Albania using the ASQ3. We also assessed the quality of care environment according to ECERSR and the socioeconomic status upon the fathers occupation. Results: In the end of this research we found that not only a considerable percentage of preschoolers included in this study have psychomotor delays, but also that these delays and the average of psychomotor development have a significant relationship with the environment factors, as socioeconomic status and care environment. Conclusions: Children development depends more on instructing capacities of kindergartens staff than mothers. By the level of provision with physicians, Kazakhstan ranks as one of the leaders in the world, at the same time there have been a deficit of physicians in the Republic over the last 20 years, particularly in rural areas. - Lichen sclerosus (LS) je onemocnění neznámé etiologie, postihující především postmenopauzální ženy. LS patří mezi nejčastější non-neoplastické epiteliální změny na vulvě, má typický makroskopický vzhled a klinicky je charakterizován intenzivním pruritem, případně dyspareunií. Mezi rizika LS patří tendence k jizvení a vznik dlaždicového karcinomu v terénu LS. Léčba LS je obyčejně dlouhodobá, opakovaná a jejím základem je lokální aplikace silného kortikosteroidu. Při adekvátní konzervativní léčbě je chirurgické řešení indikováno zřídka. LS belongs to the most frequent non-neoplastic epithelial disorders of the vulva, it has a typical macroscopic pattern, and it is characterized with an intensive pruritus or dyspareunia. Patients with LS have a risk of scarring of external genitalia and risk of developing squamous cell carcinoma of the vulva. Treatment of LS is usually long-term, repeated, and it is based on local potent corticosteroids. Surgical treatment is rarely indicated in the management of LS. - Judikatura ESLP v oblasti problematiky translidí významným způsobem ovlivňuje právní diskurs v jednotlivých státech, přičemž v oblasti práv translidí se zdá, že se vyvíjí směrem k inovativnímu, otevřenějšímu přístupu k translidem. Tento přístup lze vypozorovat na základě rozšiřující interpretace zejména článku 8 a článku 12 Evropské úmluvy o lidských právech. V oblasti výkladu článku 8 ESLP se posunul od vysoce restriktivního přístupu, kdy ponechával národní zákonnou úpravu v dikci jednotlivých států, přes relativně neutrální přístup, který interpretoval negativní ochranu soukromého života, až přes extenzivní přístup, jenž naopak otevíral přístup k aktivnímu chápání soukromého života jako vyjádření autonomie a umožnění seberealizace každého člověka. Podobně v oblasti výkladu článku 12 týkajícího se rodinného života se judikatura ESLP výrazně posunula, když od zastávaného názoru, že práva uzavřít manželství a založit rodinu mohou mít pouze osoby opačného pohlaví určeného na základě biologických kritérií, se posunula k otevřenějšímu, pojetí ochrany tohoto práva, podle něhož se práva uzavřít manželství mohou stěžovatelé domáhat i v případech, kdy nehodlají či nemohou založit rodinu. Došlo tak k oddělení jednoty práva na uzavření manželství a práva na rodinu., ECHR case law on the issue of transgender significantly affects legal discourse in each country, and the rights of transsexuals seems to be moving towards innovative, open attitude towards transsexuals. This approach can be demonstrated in the expanding interpretation, in particular, of Article 8 and Article 12 of the European Convention on Human Rights. In the interpretation of Article 8 of the ECHR the attitude has shifted from highly restrictive approach which kept the primacy of national legislation, to the relatively neutral approach, which interpreted the negative protection of private life and to the extensive approach which on the other hand opened access to an active understanding of private life as expression of autonomy and self-realization. Similarly, in the interpretation of Article 12 relating to family life, the ECHR case law has significantly changed when it shifted from the initial position in which the Court held that the right to marry and found a family extends only to a person of the opposite sex determined on the basis of biological criteria to a more open approach according to which the right to marry extends also to those who can not or do not intend to start a family. These factors are mostly put into five categories: patient characteristics, disease characteristics, treatment factors, factors of the treatment setting, and factors of patient-provider relationship. Just this patient-provider relationship is a key figure in the study of compliance. However, „relationship“ may be a term difficult to be understood (and influenced, as well). Considering the patients´ satisfaction with treatment, the power between providers and patients, the open communication between them, sharing information, balanced activity of both parties, providers´ interest in patients´ beliefs and experiences may make the term „relationship“ clearer. These concepts refer to the extent to which patient´s behavior (in terms of taking medication, following diets, or achieving lifestyle changes) coincides with medical or health advice. The extent of non compliance is high. Therefore new interventions increasing cooperation between patients and health-care providers are being looked for. Among the most important strategies there is the motivation method helping the patient himself to find the reasons for compliance. Educative technique is outstanding as well: it enables patient to understand the subjects of the treatment, which he consider to be most important. Behavioral approach helps patients to gain skills necessary for compliance.
