In view of the high anti-oxidative potential oftocotrienol, the role of the tocotrienol-rich fraction (TRF) of palm oil in preventing pregnancy induced hypertension (PIH) was explored in a randomized double-blind placebo-controlled clinical trial in an urban teaching hospital. Healthy primigravidae were randomized to receive either oral TRF 100 mg daily or placebo, from early second trimester until delivery. Out of 299 women, 151 were randomized into the TRF arm and 148 into the placebo arm. A total of 15 (5.0%) developed PIH. Although there was no statistically significant difference in the incidence of PIH (4/151 or 2.6% in the TRF arm vs. 11/148 or 7.4% in the placebo arm, p = 0.058) between the two arms, there was a tendency towards a lower incidence of PIH in the TRF arm compared to the placebo arm. With TRF supplementation, the relative risk (RR) of PIH was 0.36 (95% CI 0.12-1.09). In conclusion, although TRF from palm oil does not statistically significantly reduce the risk of development of PIH in the population studied, the 64% reduction in incidence of PIH is substantial. The investigation is highly distressing for the patient and his parents. During VCUG sensitive tissues are exposed to ionizing radiation. Correct examination technique lowers both the distress as well as the final dosis of radiation. Dopplerovské vyšetření ledvin u novorozenců nespadá do skupiny základních a rutinních ultrazvukových vyšetření. Vykazuje určitá specifika, ať už v přístupu k těmto malým pacientům, tak i v odlišnosti hemodynamiky v oblasti renovaskulárního systému v tomto věku. Taktéž skupina nejčastějších patologií cévního systému ledvin u novorozenců je poměrně odlišná od patologií starších dětí a dospělých. V článku bych chtěla upozornit na specifika přístupu k novorozeneckým pacientům a na základní odlišnosti hemodynamiky ledvin u novorozenců., Doppler examination of the kidneys in neonates does not fall within the group of basic and routine ultrasound examinations. Ve vybraných městských lokalitách v ČR byl dlouhodobě monitorován vývoj hluku. Časové řady byly zkoumány metodou lineárního regresního modelu. V období 1994–2006 došlo ke statisticky významnému vývoji hluku (vyjádřeného pomocí hlukového ukazatele pro den-večer-noc Ldvn) v necelé polovině ze 40 monitorovaných lokalit. V 10 z nich se jednalo o pokles a v 7 z nich o nárůst Ldvn. Změny v jednotlivých lokalitách se pohybovaly v rozpětí od mínus 4 dB/10 let (pokles) do plus 5,5 dB/10 let (růst). V období 2009–2011 bylo hodnoceno 15 lokalit, ke změně dříve zjištěných trendů vývoje došlo ve dvou z nich. Za celé časové období 1994–2011 došlo k meziroční změně hluku o více než 4 dB ve třech případech. Při celkovém hodnocení všech monitorovaných lokalit za celé časové období nebyl zjištěn významný rostoucí ani klesající trend vývoje hlučnosti. Zjišťované změny jsou charakteristické pro městské dlouhodobě obydlené lokality ležící uvnitř zástavby, nelze je celostátně zobecňovat na lokality jiného charakteru. Zhodnocení dlouhodobého monitorování hluku ukazuje, že toto monitorování umožňuje nejen zachycení lokálních změn hlučnosti, ale též celkové hodnocení trendu vývoje., Noise values in selected urban localities in the Czech Republic were subjected to long-term monitoring. Time sequences were analysed by linear regression. A statistically significant development in noise values (expressed by noise indicator Lden) was detected in almost one half of 40 monitored sites during 1994–2006. Lden was decreased in 10 instances and elevated in 7 instances. Changes in the various localities ranged from minus 4 dB/10 years (decrease) to plus 5.5 dB/10 years (increase). A total of 15 localities were evaluated in the 2009–2011 period; changes in trends previously detected were found in two cases. Over the whole monitoring period 1994–2011 an inter-annual change of over 4 dB was registered in three cases. An overall evaluation of all monitored localities for the whole monitoring period did not reveal any significantly increasing or decreasing noise trends. The detected changes are characteristic of urban long-term residential and built-up areas and cannot be generalized to other types of locality. Activated hepatic stellate cells (HSC) are a major source offibrous proteins in cirrhotic liver. Inducing or accelerating their apoptosis is a potential way of liver fibrosis treatment. Extracellular matrix (ECM) surrounding cells in tissue affects their differentiation, migration, proliferation and function. Type I collagen is the main ECM component in fibrotic liver. We have examined how this protein modifies apoptosis of normal rat HSC induced by gliotoxin, cycloheximide and cytochalasin D in vitro and spontaneous apoptosis of HSC isolated from CCl4-damaged liver. The present article describes two unrelated cases of progressive myoclonic epilepsy (PME) of the Lafora's disease and Unverricht-Lundborg types who were treated with topiramate (TPM) as add-on therapy for their myoclonus. After the initiation of topiramate therapy both cases responded with marked decrease in myoclonic seizure frequency and improvement of quality of life.
