Gliomy limbického a paralimbického systému představují anatomicky a tím pádem i operačními přístupy unikátní skupinu mozkových nádorů. Mikrochirurgie v kombinaci s moderními perioperačními postupy zvyšuje šance na radikální odstranění těchto gliomů. Z hlediska lokalizace je dělíme do tří základních skupin: 1. gliomy inzuly, 2. amygdalohippokampálního komplexu a 3. cingula. V minimonografii se věnujeme anatomii i funkci limbického a paralimbického systému a v návaznosti na ni představujeme ucelený pohled na jednotlivé neurochirurgické přístupy k těmto oblastem. Maximální resekce za zachování funkční integrity mozku je předpokladem následné smysluplné onkologické léčby. Taktéž prezentujeme výsledky operací našeho pracoviště ve smyslu morbidity a radikality za posledních sedm let., Limbic and paralimbic gliomas represent a unique group of brain tumors both from an anatomic and surgical point of view. Microsurgery, together with modern perioperative procedures, enables a wider extent of resection of these gliomas. Based on localization, we divide gliomas into three basic groups: 1. insular, 2. amygdalohippocampal complex and 3. cingulum gliomas. In our review, we describe the anatomy and function of the limbic and paralimbic system and in relation to that, we present a description of the specific neurosurgical approaches to these regions. Maximal resection with brain function preservation represents the basis for subsequent oncologic therapy. We also present the surgical results of our department in the last seven years with regard to morbidity and radicality. Základní ideou dekompresivní kraniektomie (DK) je chirurgické řešení konfliktu vzrůstajícího nitrolebečního obsahu s pevným objemem kostní schránky. Zvětšený nitrolebeční obsah je většinou představován edémem mozku při nejrůznějších patologických stavech. Přestože DK zažívá v posledních letech renesanci a objevily se důkazy o její účinnosti, její použití není dosud paušálně akceptováno. Nejčastější indikací DK je kraniocerebrální poranění. Důkazy o její efektivitě jsou však limitované. Jsou očekávána data o účinnosti sekundární DK u nemocných s refrakterní nitrolebeční hypertenzí po selhání konzervativní léčby. Provedení bifrontální sekundární dekomprese ještě před vyčerpáním konzervativní léčby nepřináší nemocným s difuzním poraněním benefit. Postavení primární DK u akutního subdurálního hematomu je testováno v randomizované studii. Nejsilnější důkazy o efektivitě DK jsou u nemocných s maligním hemisferálním infarktem, přesto frekvence operace stále neodpovídá incidenci maligního hemisferálního infarktu. DK by měla být zvážena u nemocných s trombózou mozkových splavů a refrakterní nitrolebeční hypertenzí, kteří mohou z operace profitovat i při oboustranné mydriáze. DK s evakuací nebo bez evakuace hematomu může snížit mortalitu nemocných s expanzivním supratentoriálním intracerebrálním krvácením, kteří jsou v bezvědomí nebo mají refrakterní nitrolebeční hypertenzi. Dekomprese může přinést benefit selektované podskupině nemocných se subarachnoidálním krvácením, její použití však není v současných doporučeních specifikováno., The fundamental idea behind decompressive craniectomy (DC) is a surgical solution to the conflict of an increasing intracranial content within the fixed-volume intracranial cavity. The expanding intracranial content is caused by brain edema caused by a variety of pathologies. In spite of DC renaissance over the recent years and some evidence of its effectiveness, its use is not yet generally accepted. Traumatic brain injury is by far the most frequent indication for DC. However, the evidence of its effectiveness is limited. Data on the role of secondary decompression in patients with refractory intracranial hypertension after conservative treatment failure are expected to be released soon. Bifrontal early DC is not superior to medical management in patients with diffuse traumatic injury. Primary decompression in patients with acute subdural haematoma has been investigated in a randomized trial. The most conclusive evidence is in patients with malignant middle cerebral artery infarction. In spite of this, the surgery is still underutilized and the frequency of its use does not correspond with the incidence of malignant infarction. When decompression is performed within 48 hours of stroke onset in patients younger than 60 years, it reduces mortality and improves functional outcome. DC is also a lifesaving procedure in patients over 60 years of age for whom it improves chances of survival without total dependency. Decompression should be considered in patients with cerebral venous thrombosis that causes intractable intracranial hypertension. Furthermore, patients with bilateral mydriasis can also profit from the procedure. DC with or without hematoma evacuation might reduce mortality in patients with large supratentorial intracerebral hemorrhage who are in coma or have refractory intracranial hypertension. Even though the DC can be effective in selected subgroups of patients with subarachnoid hemorrhage, current guidelines, do not specify the role of decompression in these patients. Mezi nejnáročnější součásti klinického neurologického vyšetření patří vyšetření citlivosti vyžadující detailní edukaci pacienta a jeho následnou spolupráci. Při správné metodice vyšetření a jeho exaktním hodnocení lze často odhalit i diskrétní poruchy citlivosti, kterých si pacient nemusí být vědom nebo není schopen je validně popsat. Našim cílem je podat přehled nejčastěji používaných metod vyšetření běžně hodnocených senzitivních a algických modalit (taktilní, vibrační a termické čití, termoalgické čití, hodnocení hluboké tlakové bolesti a dále mechanická algická citlivost a hodnocení dynamické mechanické alodynie), včetně interpretace jejich nálezů a využitelných normativních dat (pokud jsou dostupná), a to od nejjednodušších kvalitativních a semikvantitativních metod po složitější metody kvantitativní. Kvalitativní či semikvantitativní metody jsou v běžné klinické praxi využívány podstatně častěji. Jejich výhodou je rychlost a metodická nenáročnost testování a široká dostupnost používaných pomůcek. Nevýhodou je naopak skutečnost, že poskytují pouze orientační informaci o citlivosti pro danou modalitu a spolehlivě detekují většinou až pokročilé poruchy senzitivity. Absence kvantifikace limituje možnosti těchto metod v longitudinálním sledování pacientů i při srovnání nálezů mezi pacienty či jejich skupinami. Kvantitativní metody souhrnně označované termínem kvantitativní testování senzitivity (Quantitative Sensory Testing, QST) jsou časově a metodicky náročnější a vyžadují použití specializovaných pomůcek. Jejich předností jsou kvantifikované výstupy s lepší diagnostickou validitou, které potenciálně diagnostikují i klinicky méně vyjádřené abnormity a jsou lépe využitelné např. pro longitudinální sledování. Tyto metody se častěji uplatňují v klinických studiích a jsou nezastupitelné v diagnostice neuropatie tenkých vláken., Sensory examination represents one of the most complicated parts of clinical neurological examination that requires detailed instructions to patients and their attentive cooperation. Using correct methodology of testing and precise evaluation of the results, sensory tests frequently disclose even very fine sensory abnormities which patients may not be able to describe or even may not be aware of. The aim of this paper is to provide an overview of methods most frequently used to evaluate common sensory and pain modalities (touch, vibration and thermal sensation, thermoalgic sensation, deep pain pressure, mechanical detection threshold and sensitivity, evaluation of dynamic mechanical allodynia), including interpretation of the findings and recommended normal values (if applicable). In each of the tested modalities, most commonly used methods are described from the easiest qualitative and semiquantitative methods (usually more frequently used in clinical practice) up to more complicated quantitative tests. Main advantages of qualitative and semiquantitative methods include their low methodological and time burden and high availability of the equipment used. Quantitative sensory testing (QST) is more demanding and requires the use of specialized, not widely available, equipment. These methods, however, provide quantified results with better diagnostic validity, may disclose even less pronounced sensory abnormities and are more useful in long-term follow-up as well as in inter-individual comparisons, both at individual and group level.
