Úvod: Takotsubo kardiomyopatie (takotsubo cardiomyopathy – TCM) je vzácné srdeční onemocnění charakterizované v akutní fázi přechodnou dysfunkcí levé komory, které může být komplikováno fibrilací komor nebo srdečním selháním. V diferenciální diagnostice musí být vždy vyloučen akutní infarkt myokardu. Cílem práce je prezentovat charakteristiku pacientů s TCM, jejich jednoletou prognózu a přínos magnetické rezonance k diagnostice těchto pacientů v klinické praxi. Metodika: Vyhodnoceno bylo celkem 47 pacientů s prokázanou TCM. U všech pacientů byla vyloučena významná ateroskleróza koronárních tepen pomocí selektivní koronární angiografie. Celkem u 10 pacientů (21,3 %) bylo provedeno vyšetření magnetickou rezonancí (MRI) srdce. Pacienti byli prospektivně sledováni minimálně 12 měsíců. Výsledky: Soubor pacientů s TCM tvořily z 89,4 % ženy, průměrný věk pacientů byl 62,3 ? 11,1 let. Třicet devět pacientů (83,0 %) mělo apikální a 8 midventrikulární formu TCM. Zjevný provokující stresový faktor jsme identifikovali u 18 pacientů (38,3 %). Časté byly další komorbidity, zejména onemocnění štítnice, psychiatrická, plicní nebo alergická onemocnění. U 42 pacientů (89,4 %) byla přítomna dysfunkce levé komory, 4 pacienti (8,5 %) byli v kardiogenním šoku. Čtyři pacienti (8,5 %) prodělali během hospitalizace fibrilaci komor. Roční mortalita v našem souboru činila 14,9 %, jedna pacientka (2,1 %) prodělala v průběhu sledování recidivu ataky TCM. Závěr: Přestože u pacientů s TCM dochází k úpravě funkce levé komory, akutní fáze může být komplikována rozvojem kardiogenního šoku nebo fibrilací komor. U části pacientů je indikováno vyšetření MRI k vyloučení ischemické nebo zánětlivé etiologie., Introduction: Takotsubo cardiomyopathy (TCM) is a rare cardiac disease. In the acute phase it may imitate acute myocardial infarction of the anterior wall with ST elevations. This can be complicated by ventricular fibrilation or heart failure. The aim of this study is to show the characteristics of patients with takotsubo cardiomyopathy, the one year outcome and the application of magnetic resonance imaging of the heart in the diagnosis of this disease. Methods: Forty seven patients with takotsubo cardiomyopathy were enrolled in this study. Severe stenosis of coronary arteries was excluded by selective coronarography. In twelve patients the magnetic resonance imaging of the myocardium was performed. The patients were observed for at least 12 months. Results: 89.4 % of takotsubo patients were women. Average of age was 62.3 ? 11.1 years. We found obvious stress factors in 18 patients (38.3 %). Many patients suffered from thyroid disorders, psychiatric, lung or allergic diseases. Thirty nine patients (83.0 %) had the apical and eight the midventricular type of takotsubo cardiomyopathy. In 42 patients (89.4 %) developed systolic left ventricular dysfunction and four patients (8.5 %) were in cardiogenic shock. Four patients (8.5 %) manifested ventricle fibrilation. One patient (2.1 %) had recurring takotsubo cardiomyopathy. Conclusion: Left ventricle function is mostly recovered in patients with takotsubo cardiomyopathy, but the acute phase can be complicated by cardiogenic shock or ventricular fibrilation. Autoři ve svém sdělení prezentují novou, minimálně invazivní metodu léčby benigní hyperplazie prostaty. Jde o technologii „The RezūmTM System“ založenou na použití sterilní vodní páry, která je pod tlakem aplikována do prostatických laloků, kde působí ohraničené termické léze a výsledně potom zmenšení objemu prostatické tkáně s následným poklesem symptom skóre, objemu postmikčního rezidua a zvýšení maximálního průtoku Qmax. Součástí práce je i zhodnocení vlastních výsledků použití této léčby. Sharply confined thermal lesions consequently lead to the reduction of prostatic tissue volume, which results in symptom score reduction, postvoiding residual volume reduction and increase of maximal flow Qmax. Objective: The study aimed to evaluate plasma adiponectin (ADP), adipocyte fatty acid binding protein (A-FABP) and fibroblast growth factor 21 (FGF 21) levels as potential predictors of severity of acute pancreatitis (AP) in day 4 after admission. Simultaneously, the classical proinflammatory makers were analysed as well. Settings: Department of Clinical Biochemistry and 2nd Department of Internal Medicine, Faculty Hospital Olomouc, I. P. Pavlova 6, 775 20 Olomouc, Czech Republic; Hospital Písek, Karla Čapka 589, 397 23 Písek, Czech Republic. Study design: The study was conducted in subjects with acute pancreatitis (n = 84, 37 females, 47 males). The analyses were performed in the groups according to the mild/severe classification of AP, and partly in the computed-tomography severity index (CTSI) score subgroups. Results: From adipokines, only FGF 21 tended to be higher in the severe AP subgroup in day 4. The multiple regression analysis revealed a positive association of A-FABP with procalcitonin (PCT), whilst FGF 21 was associated positively with ADP and negatively with C-reactive protein (CRP) in patients with the severe AP. The receiver-operator characteristics (ROC) analysis confirmed that not adipokines, but only CRP and interleukin 6 (IL-6) were suitable as potential predictors of the disease severity. The cut-off values were established for both parameters: 100 mg/L for CRP and 37 ng/L for IL-6, with negative predictive values (NPV) 96 % and 92 %, and positive predictive values (PPV) 39 % and 29 %, respectively. Conclusion: The role of ADP, A-FABP and FGF 21 has been limited in a prediction of the disease severity in day 4 after admission, while CRP and IL-6 might be useful to exclude a severe AP., Cíle studie: Studie byla zaměřena na vyhodnocení sérových hladin adiponektinu (ADP), adipocytárního proteinu vázajícího mastné kyseliny (A-FABP) a fibroblastového růstového faktoru 21 jako potenciálních prediktorů závažnosti akutní pankreatitidy (AP) 4. den po přijetí. Výsledky: Ze sledovaných adipokinů měl u pacientů s těžkou AP pouze FGF 21 tendenci k vyšším hladinám v den 4 po přijetí. Za pomoci mnohočetné regresní analýzy byla zjištěna pozitivní asociace A-FABP s prokalcitoninem (PCT), zatímco FGF 21 asocioval u těžké AP pozitivně s ADP a negativně s C-reaktivním proteinem (CRP). ROC (receiver-operator characteristics) analýza potvrdila, že pouze CRP a interleukin 6 (IL-6) jsou využitelné jako potenciální prediktory závažnosti onemocnění. Úvod: Posouzení významu PET/CT pro iniciální staging karcinomu jícnu s důrazem na metastatické postižení uzlin a průkaz vzdálených metastáz. Dále bylo cílem práce ověření významu PET/CT vyšetření při hodnocení efektu neoadjuvantní léčby. Metody: V prospektivní studii byl sledován soubor 354 nemocných s karcinomem jícnu, kteří byli vyšetřeni na I. chirurgické klinice LF UP a FN Olomouc v letech 2006–2012. Vstupní PET/CT vyšetření bylo provedeno u 349 nemocných. Analyzovali jsme přínos vyšetření ke stanovení stagingu onemocnění a v závislosti na něm jsme stanovovali strategii léčby. Na základě vstupního PET/CT byl u 102 nemocných zjištěn různý stupeň generalizace a byli indikovaní k paliativní či symptomatické terapii. U 247 pacientů bylo zjištěno omezení nádoru na jícen ev. regionální uzliny. Po posouzení celkového stavu a dle přání pacienta byla dále u 188 nemocných indikovaná neoadjuvantní radiochemoterapie (RCHT), léčbu nedokončilo 32 nemocných. U 156 pak bylo v průměrném odstupu 8,4 týdnů od ukončené neoadjuvantní terapie provedeno kontrolní vyšetření PET/CT. Na základě tohoto vyšetření bylo kompletní vymizení známek nádoru (complete response- CR) zjištěno u 38 vyšetřených (24,4 %), regrese u 89 (57,0 %), stacionární nález u 10 (6,4 %) a progrese u 19 (12,2 %). K chirurgické léčbě bylo indikováno 97 pacientů, z toho resekci jícnu bylo možné provést u 85 a u 12 byl výkon pouze paliativní resp. šlo o exploraci. Výsledky: Vstupní PET/CT vyšetření u 349 pacientů u naprosté většiny správně posoudilo rozsah onemocnění v souhlase s histologicky ověřenou diagnózou. Falešně negativní výsledek byl jen u 5 nemocných (1,43 %). Při hodnocení efektu neoadjuvantní léčby byli pacienti rozděleni do skupin dle nálezu při kontrolním PET/CT po neoadjuvanci a bylo vyhodnocováno jejich přežívání. Byl zjištěn signifikantní rozdíl (p=0,0004) v přežívání mezi skupinami s rozdílnou reakcí na neadjuvantní léčbu (CR (n=38), regrese (n=89), stacionární nález (n=10), progrese (n=19)) bez ohledu na další léčbu po neoadjuvanci, ve prospěch pacientů s lepší reakcí na neadjuvantní léčbu. Signifikantní rozdíl v přežívání byl zaznamenán i mezi skupinou pacientů po neoadjuvanci radikálně operovaných (n=85) a neoperovaných (n=59) ve prospěch operovaných (p=0,003). Nejdelšího průměrného přežívání 38,6 měsíců (medián 29,0 měsíců) bylo dosaženo ve skupině operovaných po neoadjuvanci, kde kontrolní PET/CT ukázalo CR. Nebyl však prokázán signifikantní rozdíl (p=0,587) ve výsledcích mezi skupinami operovaných s rozdílnou reakcí na neadjuvantní léčbu (regrese, stacionární nález). Počet případů v jednotlivých hodnocených skupinách není zatím natolik velký, abychom získané výsledky mohli považovat za jednoznačně průkazné a ve sledování a zařazování dalších nemocných do studie budeme dále pokračovat. Závěr: V práci je dokumentován význam a přínos PET/CT v iniciálním stagingu karcinomu jícnu, zejména v průkazu metastatického onemocnění, a to jak v postižení uzlin, tak v odhalení vzdálených metastáz. PET/CT má velký význam pro stanovení léčebné strategie. Dále byl ověřen význam PET/CT vyšetření při hodnocení efektu neadjuvantní léčby., Introduction: To evaluate the significance of PET/CT for the initial staging of esophageal cancer with emphasis on metastatic lymph node affection and detection of distant metastases. Furthermore, the aim of the work was to analyze the significance of PET/CT examination when evaluating the effect of neoadjuvant therapy. Methods: A set of 354 patients with esophageal cancer treated at the 1st Department of Surgery, University Hospital Olomouc and Medical Faculty at Palacky University in Olomouc between the years 2006−2012 were analyzed in a prospective study. The initial PET/CT examination was performed in 349 patients. We analyzed the benefit of this examination in regard to disease staging and based on the result, therapeutic strategy was determined. The initial PET/CT showed varying degrees of disease generalization in 102 patients, these patients were indicated for palliative or symptomatic therapy. In 247 patients, the disease was limited only to the esophagus and /or regional lymph nodes. After considering the patient’s overall condition and taking into account the wishes of the patient, 188 patients were indicated for neoadjuvant chemoradiotherapy (CRT); 32 patients did not complete this treatment. In 156 patients a follow-up PET/CT scan was performed after an average of 8.4 weeks following completion of neoadjuvant therapy. Based on this examination, a complete response- CR, was observed in 38 patients (24.4%), regression of the tumor in 89 (57.0%), stationary findings were seen in 10 (6.4%), and progression in 19 (12.2%). Ninety-seven patients were indicated for surgical resection; however, esophagectomy was only possible in 85 patients, in the remaining 12 patients only an explorative laparotomy was performed due to disease progression. Results: The initial PET/CT examination performed in 349 patients correctly described the extent of the disease in accordance with the histologically confirmed diagnosis in virtually all patients. A false positive result was seen in only 5 patients (1.43%). When evaluating the effect of neoadjuvant therapy, the patients were divided into groups based on the findings of the follow-up PET/CT after neoadjuvant therapy and their overall survival was evaluated. A significant difference (p=0.0004) in survival was observed between the groups based on the different reactions to neoadjuvant therapy (CR (n=38), regression (n=89), stationary findings (n=10), progression (n=19)) without taking into account the following treatment the patient received after neoadjuvant therapy. Patients who had a better response to neoadjuvant therapy had better survival results. There was also a significant difference in survival between the group of patients who completed neoadjuvant therapy and underwent radical surgical resection (n=85) versus those patients who completed neoadjuvant therapy but did not undergo subsequent surgery (n=59). The operated group had a significantly higher overall survival (p=0.003). The longest mean survival, 38.6 months (median 29.0 months), was achieved by the group of patients who completed neoadjuvant therapy, showed a complete response on the follow-up PET/CT, and underwent surgical resection. However, a significant difference was not observed (p=0.587) between the groups who underwent surgical resection and whose follow-up PET/CT results differed (regression or stationary findings). To date, the number of cases in the individual groups is not great enough to consider the obtained results conclusive, and we will continue to include more patients into the study and continue with the analysis. Conclusion: The work documents the significance and benefit of PET/CT in the initial staging of esophageal cancer, especially in detecting metastatic disease- positive lymph nodes as well as distant metastases. AIM: The ability of two newly developed oximes (K727, K733) to reduce tabun-induced acute neurotoxic signs and symptoms was evaluated and compared with currently available trimedoxime in rats. METHODS: The neuroprotective effects of the oximes studied combined with atropine on Wistar rats poisoned with tabun at a lethal dose (380 µg/kg i.m.; 90% of LD50 value) were evaluated. Tabun-induced neurotoxicity was monitored by the functional observational battery consisting of 38 measurements of sensory, motor and autonomic nervous functions at 2 hours following tabun challenge. RESULTS: All tested oximes combined with atropine enable tabun-poisoned rats to survive till the end of experiment. Both newly developed oximes (K727, K733) combined with atropine were able to decrease tabun-induced neurotoxicity in the case of lethal poisoning although they did not eliminate all tabun-induced acute neurotoxic signs and symptoms. CONCLUSION: The ability of both novel bispyridinium oximes to decrease tabun-induced acute neurotoxicity was slightly lower than that of trimedoxime. Úvod: Tato práce referuje první zkušenosti s laparoskopicky asistovanou transanální totální excizí mezorekta u pacientů s karcinomem středního a dolního rekta. Metody: V období od října 2013 do května 2014 bylo na dvou pracovištích (CHK FN Brno, CHO KOC Nový Jičín) operováno 17 pacientů technikou transanální totální mezorektální excize. V souboru bylo 11 mužů a 6 žen ve věku 49–81 let. Vzdálenost dolního okraje tumoru od análního okraje byla 30–80 mm, medián 60 mm. U všech pacientů byla konstruována ručně šitá kolo-anální anastomóza. Výsledek: U všech operovaných byl dosažen negativní distální resekční okraj. Cirkumferentní resekční okraj byl pozitivní ve dvou případech. Kvalita totální mezorektální excize byla hodnocena v 8 případech jako kompletní a v 9 případech jako téměř kompletní. Dehiscence kolo-anální anastomózy byla zaznamenána ve dvou případech (12 %). Operační čas se pohyboval v rozsahu 212–375 minut (medián 280 minut). Medián pooperační hospitalizace byl 9 dnů v rozsahu 6 až 30 dnů, v případě řešení anastomotické a rané komplikace. Závěr: Na základě prvních získaných zkušeností hodnotíme výkon jako realizovatelný a bezpečný v podmínkách našich chirurgických pracovišť s uspokojivými patologickými výsledky. Přesné indikace zatím nelze jednoznačně doporučit. Předpoklad je, že k transanální totální mezorektální excizi by měli být indikováni pacienti s nízce uloženým karcinomem rekta, event. s úzkou pánví, objemným mezorektem a objemnou prostatou., Introduction: First experiences with laparoscopic low anterior resection with transanal mesorectal excision in patients with low and mid rectal cancer are referred. Methods: During a 9 month period (between October 2013 and May 2014), 17 laparoscopically assisted rectal resections with transanal total mesorectal excision, hand-sewn colo-anal anastomosis and protective ileostomy were performed at two institutions (CHK FN Brno, CHO KOC Nový Jičín). There were 11 males and 6 females, with a mean age 68 years (range 49–81 years) in our cohort. The tumor was located 30–80mm from the anal verge (median 60 mm). Results: A negative distal resection margin was achieved in all patients. The circumferential resection margin was positive in two cases. The TME quality was described as complete in 8 cases and nearly complete in 9 cases. Mean operative time was 280 min (range 212–375 min) with no intra-operative complications. Anastomotic leakage was observed in two patients (12%). Median length of postoperative hospital stay was 9 days with a range of 6–30 days, in case of anastomotic and wound complications. Conclusion: According to our first experience we evaluated this method as feasible and safe with satisfactory pathological outcomes. We cannot recommend exact indications yet. BACKGROUND: Intussusception of the large bowel in adults is a very rare pathological condition. However, it has its clinical importance because intussusception is very often associated with an intraluminal lesion. CASE REPORT: We report two cases of the large bowel intussusception, ileocolic and colorectal. Both intussusceptions were associated with a malignant tumor. However, the clinical presentation was different. One of the intussusceptions was of non-transient character, while the second one resolved spontaneously before operation. Both patients underwent surgery and malignant tumors were found and removed. RESULTS: The purpose of the article is to draw attention to intussusception and emphasize that intussusception, either transient or non-transient, should be further examined. Cíl práce: V kazuistice prezentujeme raritní nález primárního plazmablastického lymfomu penisu, který klinicky imitoval karcinom glans penis. Jedná se o druhý publikovaný případ tohoto typu lymfomu penisu ve světě. Kazuistika: Pacient, narozen 1929, byl odeslán na naše pracoviště ošetřujícím urologem pro nově zjištěný exofytický tumor glans penis, indikovali jsme parciální amputaci penisu. Histologické vyšetření amputátu prokázalo primární plazmablastický lymfom. Následná restagingová vyšetření neprokázala systémové onemocnění, pacient je nyní 22 měsíců po výkonu bez známek onemocnění a zůstává v hematologické dispenzární péči. Závěr: Primární plazmablastický lymfom penisu představuje extrémně raritní diagnózu, ve světové literatuře je popsán pouze jediný případ. Jedná se o agresivní typ lymfomu, který obvykle vede v krátké době ke smrti pacienta. Ačkoli léčba těchto nádorů je převážně založena na chemoterapii, u našeho pacienta vedl radikální operační výkon k úplnému vyléčení., We report a case of an elderly man who has presented with tumorous affection involving the uncircumcised penile glans and prepuce. Primary clinical examination gave strong suspicion for primary carcinoma of penis, therefore a suprapubic cystostomy and partial amputation of penis were performed. Histopatological examination revealed uncommon diagnosis of plasmablastic lymphoma involving glans penis and prepuce. The vast majority of penile tumours are squamous cell carcinomas, primary malignant lymphoma of penis is extremely rare. As staging results were negative, no adjuvant systemic chemotherapy was neccesary. Úraz podtlakem v bazénu je skupina specifických raritních poranĕní s velkým rizikem fatálních důsledků. Častĕjší obĕtí s častĕji vážnějšími důsledky jsou zejména malé dĕti. Při tomto druhu poranĕní dochází ve vĕtší či menší míře k tonutí postiženého. Případ 19-letého pacienta zachyceného v oblasti hýždí nezajištěným otvorem sací drenáže zajímavĕ ilustruje nĕkteré zvláštnosti této problematiky. Preventivní opatření jsou v současnosti již známá, problém bývá vĕtšinou s jejich důslednou aplikací. K fatálním příčinám patří kromĕ utonutí, hypovolemický šok z náhlé redistribuce tekutin třetího prostoru a eviscerace s devastací vnitřních orgánů. Lokalizované často bizardní a velké otoky a sufúze lze v naprosté vĕtšinĕ případů řešit konzervativnĕ. Pro prevenci je klíčové rozšíření informace o existenci těchto poranĕní do obecného povědomí. Pro jejich správný management je pak důležité prohlubování povědomí a sdílení zkušeností s řešením těchto raritních poranĕní v odborné veřejnosti., Swimming pool suction injuries are unique and rare with a substantive risk of fatal consequences. Little children under the age of 8 are the most frequent victims with serious injuries. Drownings of different seriousness are also a usual part of accidents. The case of a 19 year old man trapped in the gluteal area by a unsecured suction drainage hole illustrates the uniqueness of this problem in an interesting way. Prophylactic arrangements are well known but the problem is with their strict application. Fatal causes excluding drowning include hypovolemic shock from the sudden redistribution of intersticial fluid and blood and also the evisceration of the bowel and other abdominal organs. Localised often bizarre and large swellings and sufusions can be treated nonoperatively in the vast majority of cases. For the prevention of these injuries it is important to inform the public and increase their awareness to these injuries. „Upside-down stomach syndrome“ je sporadicky se vyskytující typ velké paraezofageální hiátové hernie vyžadující při známkách uskřinutí neodkladné chirurgické řešení. Autoři článku prezentují kazuistiku 53letého nemocného s volvulem žaludku při „upside-down stomach syndromu“. Chirurgický výkon byl komplikován poraněním abdominální části jícnu, které bylo úspěšně léčeno mimo jiné použitím samoexpandibilního metalického stentu. The authors present a case report of a 53-year-old male patient with gastric volvulus related to the upside-down stomach syndrome. Surgical treatment was complicated by an injury to distal oesophagus, which was successfully treated using a self-expandable metallic stent among other methods. Despite the complicated postoperative course with a necessity of reoperation, insertion of an oesophageal stent, thoracotomy for a mediastinal abscess and secondary healing of the laparotomy, the patient was discharged in a good condition with healed oesophageal perforation and laparotomy after 52 days., and L.
