Kolorektální karcinom patří ve vyspělých zemích mezi nejčastější nádorová onemocnění a Česká republika je celosvětově na špici v jeho výskytu. Roční incidence u nás přesahuje 7 000 osob ročně. Primárně je 20 % pacientů diagnostikováno ve stadiu IV a téměř u poloviny pacientů se v průběhu onemocnění vyskytnou metastázy. V posledních 15 letech onkologická léčba metastického onemocnění prodělala významný vývoj. Standardem se stala terapie fluorouracilem, jehož modulace leucovorinem vedla k prodloužení mediánu přežití u infúzních režimů až k 15 měsícům (symptomatická terapie 6–8 měsíců). Kombinace fluorouracilu s oxaliplatinou nebo irinotecanem vede k dalšímu prodloužení k 20 měsícům. Následné přidání biologické léčby (bevacizumab, cetuximab, panitumumab) posouvá medián nad 20 měsíců. Cetuximab je monoklonální chimérická IgG1 protilátka, která cíleně blokuje extracelulární část EGFR. V České republice je zaregistrován pro léčbu pacientů s generalizovaným kolorektálním karcinomem s EGFR pozitivitou a K-ras wild type v monoterapii nebo v kombinaci s chemoterapií po selhání terapie irinotecanem a oxaliplatinou. Ve 2 registračních studiích (EMR 62 202–007-BOND 1, IMCL CP02–9 923) vedl ke klinickému benefitu u více jak poloviny nemocných. Ve studii publikované Cunninghamem a spolupracovníky byli pacienti zařazeni do ramene s cetuximabem a irinotecanem anebo do ramene s cetuximabem samotným (PR byla dokumentována u 23 % pacientů, SD u 33 % léčených pacientů). Medián přežití byl prodloužen z 6,9 měsíců na 8,6 měsíců. Evropská léková agentura (EMEA) doporučila cetuximab k léčbě metastického kolorektálního karcinomu (s expresí EGFR, K-ras wild type) v kombinaci s chemoterapií do I. linie léčby na základě studie CRYSTAL. Letos v dubnu byly publikovány v JCO výsledky této studie, která potvrdila efekt cetuximabu v I. linii léčby K-ras wild type karcinomů kolorekta s efektem prodloužení přežití (medián OS = 23,5 versus 20,0 měsíců; hazard ratio (HR) = 0,796; P = 0,0093), PFS (medián PFS = 9,9 versus 8,4 měsíců; HR = 0,696; P = 0,0012) a četnost léčebných odpovědí (RR) (RR = 57,3 % versus 39,7 %; odds ratio = 2,069; P < 0,001) ve srovnání se samotným FOLFIRI., Colorectal cancer in third-line treatment Colorectal cancer is among the most common malignancies in the developed countries and the Czech Republic is the leading country in terms of its incidence worldwide. The annual incidence in the Czech Republic exceeds 7 000 persons. Primarily, 20 % of patients are diagnosed with stage IV and nearly half of the patients develop metastases during the course of the disease. In the past 15 years, oncological treatment for metastatic disease has made significant progress. Treatment with fluorouracil has become a standard; its modulation with leucovorin has resulted in a prolonged median survival in infusion regimens of up to 15 months (6–8 months in symptomatic treatment). A combination of fluorouracil with oxaliplatin or irinotecan leads to a further extension up to 20 months. A subsequent addition of biological therapy (bevacizumab, cetuximab, panitumumab) pushes the median beyond 20 months. Cetuximab is a chimeric IgG1 monoclonal antibody which targets the extracellular domain of the EGFR. In the Czech Republic, it is registered for the treatment of patients with generalized colorectal cancer with EGFR positivity and K-ras wild type after a failed treatment with irinotecan. In two registration studies, it resulted in a clinical benefit in more than half of the patients (PR was documented in 23 % patients; SD in 33 % of the patients treated). The median survival increased from 6.9 months to 8.6 months. Autoři prezentují sestavu 76 nemocných radikálně operovaných pro karcinom hlavy pankreatu a analyzují pooperační komplikace a přežívání. Materiál a metodika: V období 1/2006–9/2010 podstoupilo 76 nemocných potenciálně kurativní hemipankreatoduodenektomii pro adenokarcinom hlavy pankreatu. Pacienti jsou prospektivně sledováni, diskutovány jsou komplikace a přežití. Výsledky: 30denní pooperační mortalita je 2,6 %. Na základě TNM klasifikace 11 % mělo stadium I, 78 % stadium II a 3 % stadium III. Nejzávažnější komplikací byla insuficience pankreatojejunální anastomózy (6,5 %) a opožděná evakuace žaludku (11,8 %). Ve stadiu I přežívá 1., 2., 3. a 4. rok 82 %, 52 %, 35 % a 35 % operovaných. Závěr: Resekce hlavy pankreatu je bezpečná operace s nízkou mortalitou, ale neuspokojivým dlouhodobým přežíváním. Toto je zřejmě způsobeno agresivní biologickou aktivitou nádoru a pozdní diagnostikou., The authors refer seventy six patients radically operated for pancreatic head carcinoma and analyzed postoperative complications and survival. Material and Methods: During 1/2006–9/2010 76 patients underwent potentially curative hemipancreato-duodenectomy for adenocarcinoma of the head of pancreas. The patients are prospectively followed, complications and survival are discussed. Results: The 30-days postoperative mortality was 2,6 %. According to TNM classification 11 % of patients had stage I, 78 % stage II and 3 % stage III. Most severe complications were leak from pancreato-jejunal anastomosis (6,5 %) and delayed gastric emptying (11.8 %). The survival analyses showed 1,2,3 and 4 years survival as follows: 82 %, 52 %, 35 %, 35 % respectively. Conclusion: Resection of the pancreatic head is a safe procedure with low mortality but disappointing long-term survival. Comparative assessment of clinical efficiency of omeprazole and rabeprazole in treating gastroesophageal reflux disease (GERD). 65 patients with a verified diagnosis GERD were examined. Comparative clinical investigations of using of omeprazole and rabeprazole have revealed effectiveness of both drugs in the therapy of GERD. The target of research was the implementation of the comparative analysis of conservative therapy of preparation of basic therapy and also new methods of treatment (anticytokine therapy). Appointment of salofalk in twos tablets (0, 5) 34 times a day per os, and also 2,0 gr in the form of a rectal enema unitary for a period of seven days, then had transferd the patients to reception 2,0 gr salofalk per os, allowed to achieve substantial improvement patients cliniclaboratory and endoscopic symptoms with a heavy intensification of ulcer colitis variation, providing full cliniclabaratory remissions achievements of disease at 8 (34 %) patients. In the Minister of the Uzbek Public Health of the Republican Science Coloproctological Center at 10 patients with the IDI have spent the therapy remicade, 7 of them had NUC and 3 patients had the Crohn's disease. Therefore, all patients no longer than on the first week after introduction of remicade had improvement of a course disease which was expressed in defering stool, extinction or reduction of pathological impurity in excrement and subsidence bellyaches. During colonoscopical inspection, in 1218 weeks after the first infusion at 80% of the patients had observed disappearance of ulcers and erosion. In terms of 6 months of supervision of relapse of disease was not noted. All new directions in treatment of the IDI, apparently, are perspective, but while are proved only theoretically and had no practical development. The authors demonstrate the possibility of improving surgical results by the reduction of perioperative bleeding in thoracic surgery associated with extended resection procedures. We focused on patients in whom the expected perioperative blood loss was greater than 500 ml. The first group consisted of patients with lung cancer stage III A after neoadjuvant chemotherapy had been indicated to extend the resection procedure. The second group consisted of patients with chest wall and mediastinum tumors of various etiologies. The third group consisted of patients with post-inflammatory thoracic complications in whom combined decortication and pleurectomy was necessary. Colonic polyps and polyposis are studied in 183 patients. It has been found, that sensitivity of rectoscopy is 66.7%, irrigoscopy 61.1%, virtual colonoscopy computeraided tomography of colon (VCCATC) 79.1%. Endoscopic polypectomy was the main treatment method used in patents with colonic polyps and polyposis (18.6%). In order to study cardiac pathology in different subtypes of ischemic stroke we examined 165 stroke patients. The 1st group included 90 (54.5%) patients with hemodynamic stroke, the 2nd one - 75 (45.5%) patients with cardioembolic stroke. Control group consisted of 45 individuals without cerebrovascular diseases. Cardiac pathologies with the prevalence of various types of ischemic heart disease were observed in all groups. The parameters of left ventricular stroke volume and ejection fraction were lower in patients with hemodynamic stroke than in other groups, but were within normal limits. Potential sources of cardiogenic embolism were found in all groups: in the 1st group at 74.4%, in the 2nd one at 100%. Endometriální stromální nádory představují skupinu benigních, low grade maligních i high grade maligních nádorů. Benigní variantou je endometriální stromální uzel, pro který je typická absence angioinvaze a ohraničený expanzivní růst bez invaze do myometria, či její pouze ojedinělý výskyt nepřesahující 3 mm. Low grade maligní nádory představuje endometriální stromální sarkom (dříve označovaný jako low grade endometriální stromální sarkom), který je tvořen buňkami stejného charakteru jako endometriální stromální uzel, typickým a diagnostickým rysem je však infiltrativní růst a/nebo přítomnost angioinvaze. High grade sarkomy jsou podle současné WHO klasifikace nádorů ženského genitálu z roku 2003 označovány jako nediferencované endometriální sarkomy, jedná se však o heterogenní skupinu zahrnující monomorfní variantu (dříve označovanou jako high grade endometriální stromální sarkom) a pleomorfní variantu. V následujícím textu je probrána klasifikace endometriálních stromálních nádorů a jejich základní morfologické, imunohistochemické a genetické znaky., Endometrial stromal tumors represent a group of benign, low grade malignant and high grade malignant neoplasms. Benign tumors of this group are called endometrial stromal nodules. These tumors are characterized by expansile, noninfiltrative margins and an absence of angioinvasion. In rare occurrences, protrusions of the tumor up to 3 mm can be present. Low grade malignant tumors, called endometrial stromal sarcomas, consist of tumor cells similar to those of endometrial stromal nodules. However, diagnostically typical feature is the more extensive myoinvasion than is allowable in an endomterial stromal nodule and/or the presence of angioinvasion. High grade sarcomas are, according to the current WHO classification of tumors of female genital tract (2003), called undifferentiated endometrial sarcomas. This category includes the monomorphic type (formerly high grade endometrial stromal sarcoma) and the pleomorphic type. Cíl studie: Metabolismus železa musí být velmi pečlivě regulován. Klíčovým systémovým regulátorem je peptidový hormon hepcidin, který způsobuje “uzamčení” železa uvnitř buňky; to pak nemůže být utilizováno. Dialyzovaní pacienti často trpí anémií, která je způsobena mnoha faktory, mj. stavem mikrozánětu a také zřejmě zvýšenou retencí hepcidinu v důsledku snížené glomerulární filtrace. Naším cílem bylo popsat vztahy mezi hepcidinem a dalšími parametry metabolismu železa, zánětu a erytropoezy u těchto pacientů. Typ studie: Observační Název a sídlo pracoviště: Ústav klinické biochemie a hematologie LF UK a FN v Plzni, Alej Svobody 80, 304 60 Plzeň Materiál a metody: Do studie bylo zařazeno 164 chronicky dialyzovaných pacientů (věk 66 ± 13, 25 - 92 let; 101 mužů a 63 žen) a 37 zdravých kontrol (věk 55 ± 20, 21 - 92 let; 16 mužů a 21 žen), u kterých byl stanoven jejich kompletní krevní obraz, parametry metabolismu železa, zánětu a výživy. Výsledky: Koncentrace železa, transferrinu a hemoglobinu byla významně nižší u dialyzovaných pacientů (p<0,0001), naopak ferritin (p<0,0001), solubilní transferrinové receptory(p<0,05), hepcidin (p=0,0003), CRP a IL-6 (p<0,0001) byly u těchto pacientů významně vyšší než u zdravých kontrol. Hepcidin slabě koreloval pozitivně s CRP a ferritinem a negativně s transferrinem u dialyzovaných pacientů, u zdravých kontrol nebyla nalezena žádná korelace hepcidinu s ostatními parametry. Závěr: Zdá se, že vliv zánětu na hladiny hepcidinu není u hemodialyzovaných pacientů zcela zásadní a na jeho koncentraci se podílí i jiné faktory, např. jeho retence., Objective: Peptide hormone hepcidin is a key systemic regulator of the iron metabolism. Hepcidin binds to the iron cell exporter ferroportin so iron is kept in the cells unavailable for erythropoiesis. The synthesis of hepcidin is up-regulated by high iron stores and inflammation. Dialyzed patients have very often impaired iron management – they suffer from anemia, which is caused by many factors including the state of micro inflammation and hepcidin retention due to decreased glomerular filtration rate. Our aim was to describe the relationship of hepcidin and other parameters of iron metabolism, erythropoiesis and inflammation. Design: Observation Settings: Institute of Clinical Biochemistry and Hematology, Charles University Hospital in Pilsen, Alej Svobody 80, 304 60 Plzeň Material and Methods: Complete blood cell count, iron, ferritin, transferrin, CRP, albumin, creatinine, hepcidin, soluble transferrin receptors (sTfR) and IL-6 were measured in samples from 164 patients included in chronic hemodialysis program (age 66 ± 13, 25 - 92 years), 63 women and 101 men and 37 control healthy volunteers (age 55 ± 20, 21 - 92 years), 21 women and 16 men. Results: Iron, transferrin and hemoglobin were significantly lower in the patients group (p<0.0001) while ferritin (p<0.0001), sTfR (p<0.05), hepcidin (p=0.0003), CRP and IL-6 (p<0.0001) were significantly higher in the patients group. Hepcidin weakly positively correlated with CRP and ferritin and weakly negatively correlated with transferrin in hemodialyzed patients. No correlation of hepcidin with other biochemical parameters in controls was found. Conclusion: It seems that the influence of inflammation on hepcidin levels in hemodialyzed patients is not crucial and other factors including its retention in end-stage renal disease participate., T. Cíl: Cílem studie bylo zjistit, jak rodinní pečovatelé pečující o blízkou osobu v České republice hodnotí definující charakteristiky ošetřovatelské diagnózy Přetížení pečovatele. Metodika: Na klinickou validizaci ošetřovatelské diagnózy Přetížení pečovatele byl použit „Na pacienta zaměřený Fehringův model klinické diagnostické validity“. Výzkum u rodinných pečovatelů probíhal v měsících srpen a září 2010 v Agentuře domácí péče Victoria na území města Šumperk. Do výzkumného souboru bylo vybráno 50 rodinných pečovatelů, kteří pečovali o své blízké v domácím prostředí déle než 2 roky. Souhlas se zařazením do výzkumu vyjádřilo 23 rodinných pečovatelů, co je 46 %. Výsledky: Rodinní příslušníci za hlavní definující charakteristiky Přetížení pečovatele označili celkem 8 charakteristik: obavy z možné institucionalizace péče příjemce; obavy, jak vyřešit péči o člena rodiny, když nebudu schopen tuto péči vykonávat; obavy o zdraví příjemce péče v budoucnosti; obavy související se schopností pečovatele poskytovat péči v budoucnosti; konflikt v rodině; nejistota ze změny vztahu s příjemcem péče; smutek ze změny vztahu s příjemcem péče; starost o členy rodiny. Za vedlejší bylo označeno 10 definujících charakteristik: obtíže v dokončování požadovaných úkolů, obtíže se sledováním příjemce péče v průběhu nemoci, obtíže ve vykonávání požadovaných úkolů, vysoký krevní tlak, kardiovaskulární onemocnění, somatizace, frustrace, depresivní nálada, porušený spánek, stres. Závěr: Pro skupinu vybraných pečovatelů nebyly všechny definující charakteristiky uvedeny v klasifikačním systému NANDA International významné., Aim: The aim of the study was to find out how the family caregivers caring for their close relatives in the Czech Republic evaluate defining characteristics of the nursing diagnosis Caregiver Role Strain. Methods: Fehring´s patient – focused clinical diagnostic validity model was used to clinical validation of the nursing diagnosis Caregiver Role Strain. The research was ongoing during the months August and September 2010 at Home Care Agency Victoria in region of city Šumperk. Fifty family caregivers, who take care for their family members more than 2 years, were chosen to the research sample. Twenty three caregivers (46 %) agreed with their including to the research. Results: Caregivers classified in total the following eight defining characteristics as major ones: apprehension about possible institutionalization of care receiver, apprehension about care receiver´s care if caregiver unable to provide care, apprehension about the future regarding care receiver´s health, apprehension about the future regarding caregiver´s ability to provide care, family conflict, uncertainty regarding changes relationship with care receiver, grief regarding changed relationship with care receiver, concerns about family members. Caregivers classified ten defining characteristics as minor ones: difficulty completing required tasks, difficulty watching care receiver go through the illness, difficulty performing required tasks, hypertension, cardiovascular disease, somatization, frustration, feeling depressed, disturbed sleep, stress.
