Úvod: Bolesti v pravém podbřišku jsou nejčastějším důvodem hospitalizace na chirurgických pracovištích obecně. Zároveň akutní apendicitida je nejčastěji se vyskytující náhlá příhoda břišní. Včasná indikace k apendektomii je jedinou metodou léčby, obvykle s nekomplikovaným průběhem a krátkou délkou hospitalizace. V rámci diferenciálně-diagnostické rozvahy je nutné pátrat i po dalších symptomech, zejména dlouhodobých, při akutní exacerbaci často ustupujících do pozadí. Kazuistika: Prezentujeme případ 38leté pacientky, která byla přijata pro akutní bolesti břicha v pravém podbřišku. Klinický i laboratorní nález nasvědčoval pro typicky probíhající akutní apendicitidu, byla proto indikována k apendektomii. Při operaci byl nalezen hemorhagický výpotek v pánvi, na apendixu makroskopicky netypický nález pro akutní zánět. Překvapivý byl nález ztluštění stěny terminálního ilea a zvětšení pravých adnex. Nález si vyžádal rozšíření operačního výkonu na ileocekální resekci a pravostrannou adnexektomii. Definitivní diagnózu stanovilo histologické vyšetření potvrzující endometriózu apendixu, terminálního ilea a pravého ovaria. Dlouhodobou dispenzarizací nemocné byla následně zjištěna prekanceróza, resp. mikroadenokarcinom hrdla děložního. Závěr: Stanovení diagnózy endometriózy apendixu předoperačně je prakticky nemožné. V rámci diferenciální diagnostiky lze na ni pomýšlet u fertilních žen s chronickými gynekologickými obtížemi neobjasněné etiologie. Diagnostickou, resp. léčebnou metodou volby je laparoskopie, která dovolí přehlédnutí celé peritoneální dutiny, včetně malé pánve, provedení apendektomie a eventuálně excizi podezřelých ložisek z endometriózy. Definitivní stanovení diagnózy je doménou histopatologického vyšetření. Při průkazu endometriózy apendixu nebo jakékoli jiné lokalizace je nezbytné předání pacientky do péče gynekologa., Introduction: In general, pain in the lower right abdomen is the most frequent reason for hospital surgical admissions, acute appendicitis representing the most common cause of operation for acute abdomen. Timely appendectomy remains the only treatment in the early stages of inflammation and is usually uncomplicated, requiring only a short hospital stay. A differential diagnostic analysis necessitates a search for other, particularly long-term symptoms that might be driven to the background in cases of acute exacerbation. Case report: The case report presents a 38-year-old female patient who was admitted for lower right abdominal pain. Clinical examination and a blood test both suggested typical acute uncomplicated appendicitis, and therefore the patient underwent appendectomy. Haemorrhagic peritoneal fluid and nodularity of the appendix not typical for appendicitis was found. Oedema of the terminal ileum and a right adnexal tumour were a surprising finding. A more extensive surgical procedure involving ileocaecal resection and right-side adnexectomy was finally performed with regard to the intraoperative finding. The definitive diagnosis of appendiceal endometriosis, endometrial mass in the terminal intestine and ovarian endometriosis was established by histological evaluation. Long-term follow-up revealed microadenocarcinoma of cervix uteri. Conclusion: It is generally very difficult to confirm appendiceal endometriosis before operation, and revealing primary appendiceal endometriosis is virtually impossible. It is advisable to consider endometriosis in fertile women with chronic abdominal pain of unclear aetiology and gynaecological symptoms in their personal history. The best diagnostic and therapeutic method, respectively, is laparoscopy enabling exploration of the entire peritoneal cavity including the minor pelvis, and performing appendectomy as well as excision of suspicious endometrial lesions. Úvod: Klivus je centrální struktura baze lební v těsné blízkosti mozkového kmene a životně důležitých cév. K frakturám klivu dochází většinou při vysokoenergetických úrazech, jednou z nejčastějších komplikací bývá nazální likvorea. Kazuistika: Představujeme případ muže ve středním věku, u kterého se po běžném úrazu hlavy rozvinula nazální likvorea. CT vyšetření ukázalo masivní pneumocefalus a frakturu klivu v oblasti sinus sphenoidalis. Provedli jsme endoskopický endonazální výkon a defekt úspěšně uzavřeli. Diskuze: Pneumatizace sfenoidální dutiny vykazuje velkou interindividuální variabilitu. V našem případě se jedná o extrémně vzdušný sinus v kombinaci s gracilním klivem, což bylo predisponujícím faktorem pro zlomeninu. Traumatická likvorea představuje riziko likvorové hypotenze a vzniku meningitidy. Klasický chirurgický přístup je mikroskopický transnazální. Endoskopie je moderní miniinvazivní variantou. Závěr: Endoskopické endonazální ošetření traumatické likvorey u zlomenin baze lební v oblasti klivu je metodou volby. Clival fractures are usually caused by a high-energy trauma. Cerebrospinal fluid leak is one of the most common complications. Case report: A middle-aged male sustained a mild head trauma, followed by a nasal cerebrospinal fluid leak. CT scan revealed the massive pneumocephalus and the fracture of the clivus in the posterior wall of the sphenoidal sinus. We performed an endoscopic endonasal surgery to seal the defect. Discussion: Pneumatization of a sphenoidal sinus shows high variability. In the presented case, extreme pneumatization of the sinuses was combined with the gracile clivus, which was the predisposing factor for fracture. Traumatic cerebrospinal fluid leak carries the risk of intracranial hypotension and meningitis. Microscopic transseptal management is the classical surgical approach, while endoscopy provides the modern miniinvasive option. Conclusion: Endoscopic endonasal treatment of traumatic cerebrospinal fluid leak is the treatment of choice for the clival fractures. Key words: skullbase fracture – neurosurgery – endoscopy, and M. Autoři prezentují formou kazuistického sdělení případ 57leté ženy s velmi vzácným extragenitálně lokalizovaným maligním Mülleriánským karcinosarkomem retroperitonea infiltrujícím dolní dutou žílu. Je popsána resekce tumoru s částí stěny duté žíly i resekce metastázy v oblasti pánve. Dále autoři diskutují možnosti diagnostiky rozsevu tohoto typu nádoru a doporučenou adjuvantní onkologickou terapii., The authors present the case of a 57-year-old woman with a very rare extragenital malignant retroperitoneal Müllerian carcinosarcoma invading the inferior vena cava. Extramedulárne (EM) postihnutie pri akútnej myeloblastovej leukémii (AML) je relatívne vzácne a vyskytuje sa najčastejšie v 2 formách – ako myeloidný sarkóm (MS) alebo leukemia cutis (LC). Doposiaľ nebol presvedčivo objasnený molekulový mechanizmus EM postihnutia, je však pravdepodobné, že podiel chemokínových receptorov, adhezívnych molekúl a ich vzájomných interakcií, ktoré kontrolujú migráciu leukemických buniek do periférnych tkanív, je kľúčový. Vznik izolovaného EM relapsu (IEMR) po alogénnej transplantácii krvotvorných kmeňových buniek (ASCT) predstavuje menej častý klinický prejav s nepriaznivou prognózou, najmä v kombinácii s relapsom systémovým. Je pravdepodobne dôsledkom nepomeru medzi GvL efektom v kostnej dreni (KD) a periférnych tkanivách. Jeho liečba nie je jednoznačne definovaná. Predložená kazuistika ukazuje rýchly EM relaps v oboch prsných žľazách u pacientky s akútnou leukémiou so zmiešaným fenotypom (MPAL) po ASCT. Bioptická verifikácia pod ultrazvukovou kontrolou potvrdila prítomnosť leukemickej infiltrácie. Cieľom práce je ukázať menej obvyklý prípad IEMR s dôrazom na komplexnú diagnostiku a veľmi dobrú liečebnú odpoveď po ukončení imunosupresie., Extramedullary (EM) manifestation of acute myeloid leukemia (AML) is relatively rare, it occurs frequently in 2 forms: myeloid sarcoma (MS) or leukemia cutis (LC). The molecular mechanism of EM involvement has not been clearly explained yet, but it is obvious that homing of leukemic cells to peripheral tissues is controlled by chemokine receptors, adhesion molecules and their interactions. The isolated EM relapse (IEMR) after allogeneic haematopoietic stem cell transplantation (ASCT) is an uncommon clinical manifestation with an unfavourable prognosis, especially in case of systemic relapse. It is probably the result of GvL imbalance in the bone marrow and peripheral tissues. The treatment of IEMR is not clearly defined. We present a case report of the rapid EM involvement in both breasts in patient with acute leukemia (mixed phenotype) (MPAL) after ASCT. Verification biopsy under ultrasound-guidance confirmed the presence of leukemic infiltration. Endometrióza znamená přítomnost tkáně endometria mimo dutinu děložní. Hlubokou infiltrující endometriózu (deep infiltrating endometriosis – DIE) představují rektovaginální léze, infiltrující onemocnění střev, močového měchýře, močovodu a DIE se vzácně objevuje i v dalších lokalizacích. Negynekologické komplikace DIE rozdělujeme do dvou hlavních kategorií: samotné onemocnění zahrnuje pánevní a břišní bolest, střevní striktury, opakující se krvácení z konečníku, dyschezii, chronickou anémii, dysurii, hematurii, stenózu ureterů, ztrátu ledvinných funkcí a komplikace léčby, ty jsou charakterizovány vznikem fistul, dysfunkcí močového měchýře a opakujícími se záněty urinárního traktu. DIE přináší riziko pro vznik malignit zasažených orgánů. DIE může být léčena konzervativně, nebo chirurgicky. Předkládaná práce je přehledem gastrointestinálních a urologických komplikací DIE a předkládá také kazuistiku pacientky centra pro léčbu endometriózy v nemocnici ve Znojmě (ČR)., Endometriosis is the presence of endometrial tissue outside the endometrial cavity. Deep infiltrating endometriosis (DIE) includes rectovaginal lesions as well as infiltrative disease to the bowel, urinary bladder, ureter and other rare locations. Non gynecological complications of DIE can be divided into two major categories: complications of the disease itself which include pelvic and abdominal pain, bowel stricture, cyclic rectal bleeding, dyschezia, chronic anemia, dysuria, hematuria, ureteral stenosis as well as loss of kidney function and complications of the treatment which include fistula formation, bladder void dysfunction and recurrent urinary tract infections. DIE has potential for malignant transformation in the affected organ. DIE may be managed conservatively or surgically. However definitive treatment is surgical. Hypotalamický hamartom může způsobovat rozvoj katastrofické a extrémně farmakorezistentní epilepsie s epileptickou encefalopatií. První epileptický záchvat je typicky gelastický, posléze následují další typy záchvatů. Diagnostika malého hypotalamického hamartomu pomocí magnetické rezonance mozku může být obtížná. Neurochirurgická léčba včetně radiochirurgie je vzhledem k uložení komplikovaná s rizikem vedlejších účinků. Odstranění hypotalamického hamartomu však velmi účinně ovlivňuje epileptické záchvaty a brání rozvoji encefalopatie. V léčbě uveálních melanomů se v některých případech používá radiochirurgie CyberKnifem. Histologickými a imunohistologickými změnami v nádorové tkáni po terapii se zabývá poměrně málo autorů. V tomto sdělení uvádíme výsledky vyšetření dvou takových případů. U obou pacientů byla provedena enukleace bulbu pro bolestivý glaukom. U prvého pacienta šlo o recidivující melanom, který byl ošetřen gama nožem před deseti lety. Pro lehkou progresi ložiska melanomu bylo provedeno ošetření CyberKnifem celkovými dávkami 45Gy ve 3 frakcích. Po necelých pěti měsících po ozáření bulbu došlo k regresi recidivujícího nádoru z původní prominence 1,4 mm v ploše 6 mm × 0,6 mm na prominenci 0,3 mm v ploše 3 mm × 0,3 mm. Histologicky byla zaznamenána rezidua řídce celulárního ložiska regresivně změněných nádorových buněk a přítomnost reaktivních změn optického nervu. Druhý pacient byl měsíc po radioterapii CyberKnifem s následnou komplikovanou operací katarakty a s enukleací bulbu bez regrese – tedy zániku nádorových buněk. Velikost tumoru 11,1 mm × 15mm s prominencí 9,3mm přetrvávala i po ozáření dávkou 40Gy/1Fr. V histologickém vyšetření byly nádorové buňky bez patologických změn. V imunohistochemickém vyšetření jsme pozorovali u prvého pacienta vysokou difuzní expresi proteinu bcl-2 až do výše 75 % a méně než 1% pozitivitu ve vyšetření s proliferačním markerem Ki-67. U druhého případu byla situace odlišná, s výraznou až 75% pozitivní expresí bcl-2 proteinu v nádorových buňkách v povrchových vrstvách tumoru a s klesajícím počtem pozitivní exprese v hlubokých vrstvách nádoru, kde dosahovala maxima do 25 %. Proliferační aktivita s pozitivní jadernou expresí v Ki-67 vzrůstala od povrchu, kde se pohybovala v rozmezí 1–3 % do hloubky nádoru a k jeho bázi, kde dosahovala až 5 %. Naše pozorování ukazuje na výraznou redistribuci genetické aktivity uvnitř nádoru. Ta se projevuje „overexpresí“ bcl-2 proteinu a u druhého případu je patrná rozdílná pozitivita bcl-2 proteinu v různých vrstvách nádoru. Vztah tohoto fenomenu k radioterapii není dosud jasný., Radiosurgery with CyberKnife is used in the treatment of uveal melanomas in some cases. Histological and immunohistological changes in tumour tissue following treatment have been addressed by relatively few authors. This paper presents the results of investigations of two such cases. Enucleation of the eye was performed for painful glaucoma in both patients. Patient 1 had a recurrent melanoma treated with a Gamma Knife ten years ago. Due to slight progression in the melanoma site, treatment with CyberKnife was used with total doses of 45 Gy in 3 fractions. Some five months following eye irradiation, there was regression of the recurrent tumour from the original height of 1.4 mm in an area of 6 mm x 0.6 mm to that of 0.3 mm in an area of 3 mm x 0.3 mm. Histologically, residua of a sparsely cellular locus of regressively altered tumour cells and the presence of reactive changes in the optic nerve were noted. Patient 2 underwent radiotherapy with CyberKnife one month prior, with subsequent complicated cataract surgery and enucleation of the eye without regression, i.e. destruction of tumour cells. The size of the tumour of 11.1 mm x 15 mm with a height of 9.3 mm remained unchanged even after irradiation with a dose of 40 Gy/1 fr. No pathological changes in the tumour cells were seen on histological examination. In patient 1, immunohistochemistry showed high diffusion expression of the bcl-2 protein of up to 75 % and less than 1 % positivity on examination with the proliferation marker Ki-67. In patient 2, the situation was different, with significant, as high as 75 % positive expression of the bcl-2 protein in tumour cells in the superficial tumour layers and a decreasing rate of positive expression in the deep tumour layers where it reached no more than 25 %. The proliferation activity with positive nuclear expression of Ki-67 increased from the surface, where it ranged from 1 % to 3 %, to the depth of the tumour and toward its base, where it reached up to 5 %. Our observation suggests significant redistribution of genetic activity within the tumour. It is manifested by overexpression of the bcl-2 protein and, in patient 2, different bcl-2 protein positivity in the various tumour layers is apparent. We report the case of a 52year old man with severe acute pancreatitis. In this case report we discuss the undesirable effects of parenteral nutrition and the importance of nutritional support at patients with severe necrotizing pancreatitis. Severe acute pancreatitis is usually accompanied by systemic inflammatory response syndrome, which results in hypermetabolism with prominent protein catabolism. Providing nutrition to these patients is of paramount importance. An adequate nutritional support is crucial in patients with severe and complicated pancreatitis.
