To study defects and chromosomal abnormalities of the fetus, we retrospectively analyzed results of comprehensive dynamic survey of 26,404 pregnant women aged 1850 years old at 6 to 40 weeks of pregnancy. Of them, 25,956 (98.3%) women had physiological course of pregnancy, 448 (1.7%) women had abnormal pregnancy. For the diagnosis of fetal defects, we carried out ultrasound, biochemical, invasive and cytogenetic studies. The results of study showed that the majority of fetal defects and pathological course of pregnancy was noted in women at the age of 2125 years old, since at this age period women have the highest number of pregnancies. At the older age, we noted a gradual decrease in the number of pregnant women, as well as the number of abnormalities of the fetus. Based on the analysis of the results obtained, we have developed an algorithm for early fetal ultrasound examination. Introduction Cutaneous malignant melanoma prevalence, incidence and mortality rates are increasing in white populations worldwide more rapidly than any other cancer site (American Cancer Society, 2006). Despite the potential importance of regular skin selfexamination and promotion of selfprotection practices, little is known about the prevalence of these practices in medical students in Albanian population. Methods This is a descriptive, quantitative crosssectional study. In this study were included a sample of 150 individuals chosen among the students of Faculty of Medicine based on their family history for skin cancer. This study was started on October the 3rd and finished on November the 12th. Subjects had to fill in a structured, selfadministered questionnaire. All participants lived within Republic of Albania but at the time of the study were students in University of Tirana, Faculty of Medicine. Results In this study that we conducted we included 150 individuals supposed to have a risk for skin cancer based on their family history. 200 individuals were approached for participation. Of these, 150 individuals returned questionnaire data, yielding a response rate of 75% among eligible, successfully contacted participants. The mean age of the sample was 20.05 years (Std. Dev. = 0.925), with males (22%) and females (78%) represented unequally, because the gender configuration of the faculty itself has gender disparities with more than 80% females and only 20% male students. The majority of the sample was born in city or town (85.3%), 14.7% was born in a village. In terms of income level, 79.3% of participants had a medium income family background, 15.3% high income and 5.3% had a low income level family background. Conclusion It is important for those individuals with family history for skin cancer (which inherently indicates risk for skin cancer) to develop selfexamination and SSE behaviours and practices in order to have a protection and at least an early detection (if onset) of the different forms of Melanoma. Cíl: Charakterizovat změny struktur kolenního kloubu, které se podílejí na poruše funkce kloubu po plastice předního zkříženého vazu. Metodika: V souboru 44 pacientů po operaci předního zkříženého vazu odeslaných k vyšetření magnetickou rezonancí pro potíže v operovaném kolenním kloubu jsme hodnotili celistvost náhrady LCA a výskyt dalších strukturálních změn kloubu. V souboru bylo 36 mužů (81,8 %) a osm žen (18,2 %). Medián věku pacientů byl 31, 0 let, minimální věk 18 let a maximální věk 52 let. Pacienty jsme vyšetřili na přístroji Signa HDxT 1,5T (GE Healthcare, Milwaukee, USA) s použitím cívky HD 1,5T knee coil. Výsledky: V souboru 44 vyšetřených pacientů po plastice LCA jsme u tří pacientů (6,8 %) diagnostikovali selhání graftplastiky LCA. U dalších pacientů s příznivým průběhem graftu LCA, ale s klinickými obtížemi v kolenním kloubu jsme prokázali následné leze, které se podílely na pooperačním dyskomfortu pacientů. Šlo o leze menisků v 88,6 %, akcentovanou nitrokloubní tekutinu v 75 %, kostní edém v 45,5 %, prosáknutí Hoffova tělesa v 36,4%, náplň Bakerovy pseudocysty v 15,9%, lézi laterálního kolaterálního vazu v 11,4 %, lézi zadního zkříženého vazu v 6,8 %. U dvou pacientů (4,5 %) jsme vyšetřili z důvodu předchozího selhání reoperovaný, nyní pevný graft LCA. Závěr: Mezi nejčastější příčiny dysfunkce kolenního kloubu po plastice LCA mimo selhání graftu LCA patří leze menisků, zmnožení nitrokloubní tekutiny, kostní edém, prosáknutí Hoffova tělesa, náplň Bakerovy pseudocysty, leze laterálního kolaterálního vazu, leze zadního zkříženého vazu. Diagnostika a přesné určení typu leze kolenního kloubu je pro ošetřujícího lékaře důležitá z důvodu stanovení dalšího konzervativního nebo chirurgického terapeutického postupu. Klíčová slova: graftplastika předního zkříženého vazu, kolenní kloub, magnetická rezonance, ruptura náhrady předního kříženého vazu, Aim: To characterize changes in the structures of the knee joint which contribute to the failure of the joint to function after the reconstruction of the anterior cruciate ligament. Methods: In a group of 44 patients after surgery of the anterior cruciate ligament sent for MRI examination for problems in the knee joint that had been operated on, we evaluated the integrity of the ACL replacement and the occurrence of other structural changes in the joint. The group comprised 36 men (81.8%) and 8 women (18.2%). The median age of the patients was 31.0 years, the minimum age was 18 years and the maximum age was 52 years. The patients were examined with the HDxT Signa 1.5 T (GE Healthcare, Milwaukee, USA) using the HD 1.5T knee coil. Results: In the group of 44 patients examined after ACL reconstruction, we diagnosed a failure of the ACL graft in 3 patients (6.8%). For other patients with a favorable course of the ACL graft, but with clinical problems in the knee joint, we demonstrated subsequent lesions that caused postoperative discomfort in the patients. These lesions include a me-niscal lesion in 88.6%, increased synovial fluid in 75%, a bone edema in 45.5%, Hoffas fat pad syndrome in 36.4%, filling of the Baker pseudocyst in 15.9%, a lesion of the lateral collateral ligament in 11.4% and a lesion of the posterior cruciate ligament in 6.8%. In 2 patients (4.5%) we examined a currently stable ACL graft that had been reoperated because of the failure of a previous graft. Conclusion: The most common causes of dysfunction of the knee joint after ACL reconstruction, besides the failure of the ACL graft, include a meniscal lesion, an increase in the amount of synovial fluid, a bone edema, Hoffas fat pad syndrome, filling of the Baker pseudocyst, a lesion of the lateral collateral ligament and a lesion of the posterior cruciate ligament. Diagnosis and accurate determination of the type of lesion of the knee joint are important for an orthopedic surgeon because of the determination of further conservative or surgical therapeutic procedures. Cieľ: Cieľom prieskumu bolo zistiť prevalenciu a rizikové faktory hypertenzie bieleho plášťa (WCH) u detí. Metodika: Formou retrospektívnej štúdie sme analyzovali zdravotné záznamy 140 detí (36 dievčat, 104 chlapcov) s vysokým krvným tlakom nameraným v kardiologickej ambulancii, u ktorých bolo vykonané 24-hodinové ambulantné monitorovanie krvného tlaku (ABPM) a určený typ hypertenzie. Hypertenzia bola diagnostikovaná, keď bol systolický a/alebo diastolický krvný tlak meraný auskultačnou technikou ≥ 95. percentil pre pohlavie, vek a výšku. WCH bola definovaná ako priemerná hodnota krvného tlaku v ambulancii nachádzajúca sa v percentilovom pásme hypertenzie a hodnota tlaku krvi počas 24-hodinového monitorovania ambulantného tlaku nachádzajúca sa v pásme normálnych hodnôt krvného tlaku. Výsledky: V skupine 140 detí bola potvrdená prítomnosť WCH u 51 detí (36,4 %) – 13 dievčat a 38 chlapcov. Z celkového súboru sa WCH vyskytovala u 36,1 % zaradených dievčat a 36,5 % zaradených chlapcov. Systolický TK sa v kardiologickej ambulancii nachádzal nad 95. percentilom, diastolický TK pod 90. percentilom. Väčšina pacientov s WCH bola vo veku 15 – 19 rokov (80,4 %) a v pásme od 3. do 90. percentilu BMI (72,5 %). U pacientov prevládala negatívna rodinná anamnéza hypertenzie (54,9 %) a zriedkavé návštevy kardiologickej ambulancie (2-4x). Krvný tlak v ambulancii merala deťom výlučne sestra. Záver: Zistenie prevalencie WCH a jej rizikových faktorov je u detí významné, pretože prevažuje názor, že v detskom veku je WCH predispozíciou k trvalej hypertenzii. Na základe zistení rizikových faktorov, môžeme vykonávať prevenciu WCH., Aim: The aim of this study was to find out the prevalence and risk factors of white coat hypertension (WCH) in children. Methods: We realized the retrospective study and analyzed records of 140 children (36 girls, 104 boys) with high blood pressure measured in cardiology outpatients' clinic, to whom a 24-hour ambulatory blood pressure monitoring (ABPM) was made and the type of hypertension was specified. Hypertension was diagnosed when systolic and/or diastolic blood pressure measurements with auscultatory technique were ≥ 95th percentile for sex, age and height. White coat hypertension (WCH) has been defined as mean blood pressure (BP) readings in outpatients' clinic being located in the hypertensive range while 24-hour ambulatory blood pressure monitoring is in the normal range. Results: In the group of 140 children the presence of WCH was confirmed in 51 children (36,4 %) - 13 girls and 38 boys. WCH occurred in 36,1 % of girls and 36,5 % of boys in the total sample of respondents. Systolic BP in cardiology outpatients' clinic reached the value above the 95th percentile, diastolic BP below the 90th percentile. The most patients with WCH were 15 – 19 years old (80,4 %) and in 3rd - 90th percentile of BMI (72,5 %). Patients prevailed negative family history of hypertension (54,9 %) and fewer visit (2-4 times) of cardiology outpatients' clinic. Blood pressure of children was measured exclusively by a nurse in this outpatients' clinic. Conclusion: Determination of the prevalence of WCH and its risk factors in children is important because of prevailing opinion that WCH in children is predisposition in permanent hypertension. V České republice dochází setrvale k mírnému vzestupu užívání nelegálních návykových látek zejména mezi mladou populací, ale užívání přechází i z bývalých teenagerů do věku dnešních rodičů. Tento jev s sebou přináší i stoupající počet žen, které užívají návykové látky i v těhotenství. Pomoc při řešení zdravotních a sociálních problémů spojených s užíváním návykových látek těhotným ženám a matkám uživatelkám nabízí specializované zařízení Centrum pro rodinu – Drop In, o. p. s., které dlouhodobě spolupracuje s hygienickou službou i akademickou obcí. V letech 2004–2014 bylo v péči Centra pro rodinu celkem 1126 žen ve fertilním věku s průměrným věkem 26,7 let, celkem 76 těhotných žen a 90 dětí., In the Czech Republic there is a consistent slight increase of using illegal habit-forming substances especially among the young population, but abuse passes from former teenagers to the age of current parents. This phenomenon brings with it an increasing number of women who use addictive substances in pregnancy. Assistance in solving health and social problems associated with substance abuse for pregnant women and using mothers offers the specialized institution, Centre for the Family – Drop In, o. p. s., which cooperates with the Public Health (Hygienic) Service and academic sphere in the long term. INTRODUCTION: Annually more than 27,000 persons die of cancer in the Czech Republic and the overall incidence of malignancies is still increasing. These data shows the need for affordable and good follow-up care especially for patients without any cancer treatment due to irreversible progression of tumor. Currently the outpatient palliative cancer care gets more into the forefront. Prerequisite for a well working outpatient palliative care is cooperation with general practitioners and home health care agencies. The purpose of the so called program of palliative cancer care is to guide a patient in palliative cancer care and to improve the cooperation among health care providers. METHODS: During the period from January 2008 to October 2010 we evaluated in patient without any oncology treatment due to irreversible progression of tumor. RESULTS: In palliative outpatient clinic we treated 446 patients, 119 of them received home care services with average length of 27.8 days. 77 patients died at home, 51 in health facilities and 41 in inpatient hospice care. CONCLUSION: We present pilot study focusing on outpatient palliative cancer care which shows the real benefit from early indication of palliative cancer care. Cíl. Cílem práce bylo zjistit, zda je možné při použití úpravy CT obrazů hybridní iterativní technikou, označovanou iDose, snížit radiační zátěž při CT vyšetřování mozku u dětí a adolescentů o 30 %. Metoda. CT vyšetření mozku bylo provedeno na přístroji CT 64 (Philips Haelthcare) u 40 pacientů různých věkových kategorií. Získaná data byla rekonstruována s různými kernely a následně iterativní technikou iDose (Philips Healthcare, dále ID) různé intenzity (ID30%, ID50%, ID70%) a s přidanou volbou MR (multi-resolution). Takto rekonstruované skeny byly porovnány s obrazy z rutinního vyšetření s konvenční rekonstrukcí FBP (filtered back projection). Hodnocení výsledných obrazů bylo provedeno jednak subjektivně, jednak objektivně. Výsledky. Použití filtrů ID30%, ID50% ID70% sníží šum a potlačí artefakty v obrazech, avšak při intenzitě 70% již snižuje ostrost a zhoršuje podání detailů. Kompenzaci tohoto nežádoucího jevu je možné dosáhnout zapnutím volby MR při rekonstrukci. Kombinace "ostřejšího" kernelu UB a ID rekonstrukce s přidanou volbou MR byla nejlepší pro zlepšení detailu obrazu a celkového užitku pro diagnostiku. Lepších výsledků bylo dosaženo u dospělých, u dětských pacientů byl přínos menší. Po redukci dávky záření při CT vyšetření o 30 % došlo ke zhoršení kvality obrazů, které bylo kompenzováno užitím kombinované rekonstrukce ID70UA + MR nebo ID70UB + MR. Při subjektivním hodnocení výsledných obrazů byla variabilita mezi hodnotícími 1,1-1,3 bodů. Závěr. iDose umožňuje provádět CT vyšetření s redukcí radiační dávky, aniž by došlo ke snížení diagnostické hodnoty obrazů., Aim. To evaluate the possibility of 30% radiation dose reduction using an innovative hybrid iterative reconstruction technique called iDose on image quality of CT brain imagesin children, adolescents and adults. Methods. Brain CT was done in forty patients of various age, scanned on CT64 (Philips Healthcare). Obtained data were reconstructed using various kernels and consequently by the new iDose (Philips Healthcare, NL) hybrid iterative reconstruction prototype on 3 different iD levels (ID30, ID 50, ID70 and multi-resolution /MR/ option). The images were compared to conventional FBP (filtered back projection). The assessment was subjective and objective. Results. Analysis of the variance showed a decrease of noise corresponding to ID levels (Fig. 1) and no differences to MR option. The combination of the filter UA in combination with ID70 led to smoother images but with fewer details, as shown in Fig. 1. An improvement of image quality has been seen when using the MR option. This combination using the higher kernel filters UB showed an improvement of smaller details and higher impact in the adult group while was less pronounced for the children group (Fig. 1). The reduction of dose decreased the image quality in both FBP and ID images, however the ID70UAMR and ID70UBMR reconstructions have compensated for this deterioration. The inter-observer variability was found at 1.1 and 1.3 points respectively. Cíl: Cílem předložené práce je detailní popis klinického nálezu u čtyř dětí s náhle vzniklým poklesem zrakových funkcí psychogenní příčiny. Metodika: Retrospektivně byla vyhodnocena zdravotnická dokumentace čtyř dívek s průměrným věkem 13 let (medián 14 let, interval 9–17 let), které byly sledovány na Oční klinice LF OU a FN Ostrava v době od 1. 1. 2014 do 31. 12. 2014. U všech dívek bylo provedeno komplexní oční a neurologické vyšetření včetně magnetické rezonance (MR) mozku, vizuálních evokovaných potenciálů (VEP), elektroretinografie (ERG) a v jednom případě lumbální punkce. Všechny dívky absolvovaly vyšetření dětským psychologem. Výsledky: U žádné z dívek v našem souboru případů jsme neprokázali organickou příčinu poklesu zrakové ostrosti. Výsledky oftalmologického, neurologického i doprovodných vyšetření (VEP, ERG, MR, lumbální punkce) byly ve všech případech negativní. Psychologické vyšetření prokázalo u všech dívek převahu psychosomatického původu obtíží. Po absolvování třech psychoterapeutických sezení u dětského psychologa došlo k plné úpravě zrakové ostrosti u všech dívek v našem souboru případů. Závěr: Neorganická ztráta zraku je klinická diagnóza vyhrazená pro stavy jedno- nebo oboustranného nevysvětlitelného poklesu zrakové ostrosti při absenci organického očního nebo celkového onemocnění. Diagnostický proces tohoto onemocnění je zdlouhavý, vyžadující multioborovou spolupráci. Základním léčebným přístupem je psychoterapie, která by měla probíhat pod dohledem zkušeného dětského psychologa., Purpose: The objective of the present paper is a description of clinical findings in four children with non-organic visual loss (NOVL). Methods: A retrospective case series analysis was performed on four girls who were followed up at the Ophthalmology Department of the University Hospital Ostrava from 1st January 2014 to 31st December 2014. The mean age of these patients was 13 years (median 14 years, range 9–17 years). Ophthalmological and neurological examinations were performed, including brain MRI, visual evocated potentials, electroretinography as well as lumbar puncture in one case. All girls were examined by a children psychologist. Results: An organic cause of reduced visual acuity was not identified in any of the four cases. The results of ophthalmological, neurological and other examinations (VEP, ERG, MRI, lumbar puncture) were negative. Psychological examination confirmed predominant psychosomatic background of the disorder in all cases. Visual acuity resolved completely in all patients after three psychotherapy sessions led by a child psychologist. Conclusion: Non-organic visual loss is a clinical diagnosis of inexplicable monocular or binocular visual acuity impairment without any organic eye disorder or general disease. Diagnostic process is lengthy and requires cooperation of various medical specialities. Psychotherapy is the primary therapeutic approach and should be supervised by an experienced child psychologist. Mentální anorexie je psychické onemocnění projevující se jako multifaktoriální porucha příjmu potravy, která postihuje především adolescentní dívky a mladé ženy, vedoucí k hladovění a nadměrné ztrátě hmotnosti. Může mít za následek více komplikací, které ovlivňují různé orgánové systémy. Ty vedou k závažným poruchám s nejvyšší úmrtností mezi všemi mentálními nemocemi. Pokud jde o prevalenci jiných poruch příjmu potravy, jako je bulimie a záchvatovité přejídání, tyto poruchy příjmu potravy jsou méně nebezpečné pro většinu klinických případů, nicméně také obtížně léčitelné. V našem sdělení se zabýváme také nedávnými změnami klasifikace poruch příjmu potravy a nabízíme stručný přehled o klinickém obrazu a epidemiologii nejčastějších poruch příjmu potravy se zaměřením na možnosti a úskalí prevence v uvedeném kontextu., Anorexia nervosa is a mental dysfunction manifested as multifactor eating disorder. It mainly affects adolescent and young women and leads to starvation and excessive weight loss, with associated complications that affect a number of organ systems. Other eating disorders such as bulimia nervosa and binge eating are clinically less dangerous but equally difficult to manage. Úvod: Plicní embolie (PE) je spolu s ischemickou chorobou srdeční a arteriální hypertenzí nejčastějším onemocněním kardiovaskulárního systému. Pro svou vysokou mortalitu stojí za pozornost. Cíl práce: Popsat charakteristiku pacientů s PE, poskytnout údaje o léčbě a nemocniční mortalitě. Dále stanovit výskyt trombů v pravostranných srdečních oddílech u pacientů s PE a efektivitu/bezpečnost trombolytické terapie u této subpopulace. Zhodnotit efekt/význam základního onkologického screeningu u pacientů s PE (tj. efektivitu prováděných vyšetření na odhalení skryté malignity). Metodika: Vycházíme ze sledování konsekutivních pacientů s PE v naší nemocnici (spádová oblast okres Znojmo, 130 000 obyvatel) od července roku 2011 do dubna roku 2014. Shromáždili jsme data o 188 pacientech s akutní či subakutní PE, u kterých byla diagnóza potvrzena perfuzním plicním scanem, CT angiografií nebo echokardiografickým nálezem se současnou typickou klinickou symptomatologií. Výsledky: V našem souboru bylo 71 mužů (37,8 %) a 117 žen (62,2 %) průměrného věku 66 let (16–94), 72,9 % pacientů bylo starších 60 let. Anamnéza tromboembolické nemoci (TEN) byla přítomna u 37 pacientů (19,7 %), malignita u 36 (19,1 %), známky flebotrombózy u 36 (19,1 %), vrozená trombofilie u 11 (5,9 %), nedávný úraz s imobilizací u 10 (5,3 %), nedávná operace u 14 pacientů (7,4 %) a fibrilace síní u 22 (11,7 %) pacientů. Tromby v pravostranných srdečních oddílech byly detekovány u 3 pacientů (1,7 %) ze 176 vyšetřených. Nemocniční mortalita dosáhla 5,6 %, 3měsíční mortalita 9,4 % (údaje zjištěny u 85,1 % pacientů) a roční mortalita 19,1 % (údaje zjištěny u 61,2 % pacientů). Skrytou malignitu jsme za hospitalizace zjistili jen u 3 pacientů (1,6 %), dalších 6 nádorových onemocnění se manifestovalo až s delším časovým odstupem. Medián délky hospitalizace byl 7 dnů. Trombolýza byla podána u 14 pacientů (7,4 %). Krvácivé komplikace antikoagulační nebo trombolytické léčby se během hospitalizace vyskytly u 4 pacientů (2,1 %) – epistaxe, tvorba hematomů v oblasti končetin s nutností chirurgického ošetření a u 2 pacientů hemateméza. Mozkové krvácení v našem souboru nebylo zaznamenáno. Závěr: PE není vzácné onemocnění, setkáváme se s ní napříč všemi odbornostmi, přesto je vzhledem ke své heterogenitě symptomů a nejisté prognóze nadále klinickým problémem. Nemocniční mortalita pacientů s PE je vyšší než u pacientů s akutním koronárním syndromem, což je ve shodě s publikovanými daty. Záchyt trombů v pravostranných srdečních oddílech je oproti údajům v literatuře o polovinu méně častý. Dušnost a bolesti na hrudi ve výskytu korelují s uváděnými daty, ale četnost synkopy a hemoptýzy je v našem souboru mnohem menší. Screening skryté malignity by mohl být efektivnější. Terapie se jeví jako bezpečná, život ohrožující krvácení se nevyskytlo ani při podání trombolýzy., Introduction: Pulmonary embolism (PE) together with coronary heart disease and arterial hypertension are most common diseases of cardiovascular system. Due to its high mortality rate it is worth of attention. Aim: to describe characteristics of patients with PE, provide data about treatment and inpatient mortality rate. Also to identify an occurence of right-sided heart thrombi in patients with PE and efficiency/safety of thrombolytic therapy in this subpopulation. To evaluate effectiveness/importance of basic oncology screening in patients with PE (meaning efficiency of provided examinations to uncover hidden malignancy). Methods: Our registry is based on observation of consecutive patients with PE hospitalized in our hospital (catchment area of Znojmo region, 130 000 inhabitants) since July 2011 until April 2014. We collected data about 188 patients diagnosed with acute or subacute PE by perfusion lung scan, CT angiography or typical symptoms with echocardiography findings. Results: In the cohort there were 71 men (37.8 %) and 117 women (62.2 %), average age 66 years (16–94), 72.9 % of patients were older than 60 years of age. History of thromboembolic disease was present in 37 patients (19.7 %), malignancy in 36 of them (19.1 %), signs of deep vein thrombosis in 36 patients (19.1 %), hereditary thrombophilia in 11 (5.9 %), recent injury with immobilisation in 10 (5.3 %), recent surgery in 14 patients (7.4 %) and atrial fibrillation in 22 patients (11.7 %). Right heart thrombi werefound in 3 patients (1.6 %) out of 176 who were examined. Hospital mortality rate reached 5.6 %, 3 months mortality rate was 9.4 % (data collected from 85.1 % of all patients) and 1 year mortality rate was 19.1 % (data from 61.2 % of all patients). An occult cancer was diagnosed during hospital stay only in 3 patients (1.6 %), another 6 malignancies manifested themselves after longer period of time. Median length of hospital stay was 7 days. Thrombolysis was used in 14 patients (7.4 %). Bleeding complications of anticoagulant or thrombolytic therapy occured in 4 patients (2.1 %) during hospital care – epistaxis, severe haematoma of extremities with necessity of surgical treatment and haematemesis in 2 patients. Cerebral hemorrhage was not present in our cohort of patients. Conclusion: PE isn’t rare condition, we can encounter it in various medical fields, but due to its diversity of symptoms and unclear prognosis, it continues to be serious clinical problem. Hospital mortality rate is higher in patients with PE than in those with acute coronary syndrome, which is in accordance with published data. Detection of right-sided heart thrombi is about half of that described in literature. Prevalence of dyspnoe and chest pain are consistent with reported data, but occurence of syncope and hemoptysis in our registry is far less common. Screening of occult cancer could be more effective.
