The aim of this study was to compare the isolation systems OptraDam® Plus and OptiDam™ with the conventional rubber dam in terms of objective and subjective parameters. The isolation systems were applied during the dental treatment of the patients. The time of preparation, placement, presence and removal were measured and the quality of isolation was evaluated. The median time of rubber dam placement was 76 s (Q1=62 s; Q3=111.25 s). The application time of OptraDam® Plus was significantly longer compared to the other systems (P ® plus. In the last decade the health of young people in Ukraine as well as in many other countries of the world significantly deteriorated according to the results of screening tests. Because of this the main task of prophylactic medicine is development and introduction of new methods of early diagnostics of diseases. The aim of the work to assess the peculiarities of adaptation of students with different types of vegetative regulation. Material and methods: 415 second year students of Lugansk State Medical University were tested. The mechanisms of physiological functions of the body regulation were assessed by registration of cardiac rhythm variability (CRV). Analysis of CRV was performed by three methods: statistical, geometrical, and spectral analysis. The express method proposed by Shlyk, N.I. (2009) was later used to determine the prevailing type of vegetative regulation. To assess adaptability the method of R.M. Baevskij (1979) was used. Results: In the result of investigation 4 types of functional states of regulatory systems were determined: I type 295 (71 %) of tested, II in 14 (3 %), III in 96 (23 %) and IV only in 10 (2%). I type of functional state of regulatory systems in the wakeful state at rest was characterized by moderate prevalence of central regulation of heart rhythm (MPCR), decreased activity of autonomic regulation. It was noticed that some of the parameters of CRV (like RR, SDNN, RMSSD and pNN50) which characterize autonomic regulation, were statistically lowere (р≤0.01) compared to type III, and some (like AMo and SI) higher, in both female and male students. Summarized spectrum capacity and components of its wave structure (HF, LF, VLF) which characterize central regulation were statistically significantly lower (р≤0.01) for the I type of regulation compared to the III one which is characterized by moderate prevalence of autonomic regulation of the heart rhythm. In female students with type I autonomic regulation compared to male students such CRV parameters as RR and AМo were statistically significantly lower (р≤0.05), while the parameters of total potency (TP) and high frequency waves (HF) were higher (р≤0.05), which could point to more centralized heart rhythm regulation in male compared to female. The evaluation of regulatory systems overstress allowed to determine the state of adaptation and risk of overstress or breakdown in students with different types of autonomic regulation. Východiska: Vzhledem ke geriatrizaci ošetřovatelské péče se ageismus stává často diskutovaným tématem. Z tohoto důvodu je velmi důležité, aby nelékařští zdravotničtí pracovníci byli v tomto ohledu dostatečně vzděláváni, protože právě dostatečná informovanost a praktická zkušenost může ovlivnit míru inklinace k ageistickým postojům. Cíl: Cílem výzkumného šetření bylo zhodnotit ageistické postoje u studentů středních zdravotnických škol v závislosti na škole a ročníku, kterou respondenti navštěvovali a zjistit rozdíly ve vnímání seniorské populace. Metody: V souladu s cíli práce byl proveden kvantitativní výzkum formou dotazníkového šetření. Na základě bodového ohodnocení odpovědí v dotazníku byla zjištěna míra ageistických postojů u respondentů. Výsledky: Práce prokázala, že je statisticky významný vztah mezi ageistickými postoji u studentů a jejich příslušností ke střední škole. Stejně tak byl prokázán rozdíl mezi ageistickými postoji u studentů druhých a čtvrtých ročníků středních zdravotnických škol. Závěry: Z práce vyplývá, že ve sledovaném souboru se u většiny studentů objevuje střední inklinace k ageistickým postojům s ohledem na navštěvovanou školu. Dále pak, že k ageistickým postojům inklinují více studenti čtvrtých ročníků, kteří již absolvovali odbornou praxi u lůžka nemocného, než studenti druhých ročníků, kteří tuto praxi ještě neabsolvovali., Background: Due to geriatrization of nursing care becomes oen discussed theme. Because of this it’s very important that non-medical health staff will be enough educated. It is that sufficient awareness and practical experience that can affect the rate of inclination to ageist attitudes. Objective: e objective of this research was to evaluate ageist attitudes of nursing school students according to school which the respondents visited and find out the differences in perception of senior population among the students of second and fourth grade. Methods: Due to the aims of this work the quantitative research has been done through a questionnaire survey. e rate of ageist attitudes was observed based on scoring responses. Results: A statistically significant relationship between symptoms of ageist attitudes this work proved as well as the differences between symptoms of ageist attitudes among second and fourth grade students. Conclusions: e work shows that in the sample survey, the majority of students appearing middle inclination to the myths of old age. Antrochoanal polyps are benign lesions originating from the mucosa of the maxillary sinus. Nasal obstruction and rhinorrhea are their main symptoms. Their endoscopical and radiological appearance makes them relatively easy to diagnose. These polyps are usually presented unilaterally, although bilateral presentation is also possible. We described two cases of atypically giant antrochoanal polyps: in a 15-year-old child and in a 38-year-old man. In both cases, the diagnosis was done by nasal endoscopy and computed tomography (CT) of the paranasal sinuses and supported by histopathological analysis. In the first patient, the excised polyp had the histological characteristic of an angiomatous antrochoanal polyp. Because of their unusual dimension, the combined transoral and endonasal endoscopic approach was performed for complete polyp excision. We discussed the clinical, histopathological and immunohistochemical characteristics of choanal polyps in comparison to inflammatory nasal polyps, and the applicable surgical techniques for treatment of these polyps. and C. INTRODUCTION: Cellulitis remains a very serious disease even today. Mortality, which varied between 10-40%, has been reduced owing to the standard securing of airway patency and use of an appropriate surgical treatment approach. MATERIALS AND METHODS: A total of 195 patients were hospitalised for cellulitis at the University Hospital in Hradec Králové during 2007-2011. The following parameters were evaluated: age, gender, dependence of incidence of the disease on the season of the year, frequency of attacks of the particular areas and their clinical characteristics, aetiology of the inflammation, types of patient complaints, prevalence of current systemic diseases, results of microbiological and selected laboratory analyses, socio-economic status of the patients, and duration of patient stay at the hospital. Statistical analysis was performed by using Pearson's correlation coefficient, the statistical significance level was p < 0.05. RESULTS: The mean age of the patients was 39.8 years. The group of 195 patients included 108 (55%) males and 87 (45%) females. The mean time between the first symptoms of the disease and admission to the Department was 5 days. From among the 195 patients, 116 (59.5%) were working persons, 79 (40.5%) were non-working (children, students, unemployed persons, women on maternity leave, retired people). The odontogenic origin of the disease was verified in 173 (88.7%) patients. In total, 65 (33.3%) patients had no coinciding complicating systemic disease, 22 (11.3%) patients had diabetes mellitus. The most frequent symptom of cellulitis was painful swelling, found in 194 (99.5%) patients, followed by jaw contracture, found in 153 (78.5%) patients. CONCLUSION: The results are largely very similar to those of previous studies performed in other countries, except that we found no correlation between the prevalence of cellulitis and the socio-economic status, nor have we confirmed Klebsiella pneumoniae sp. as the cause of cellulitis in patients with diabetes mellitus. and H. Úvod: Naše výsledky byly zhodnoceny za účelem posouzení bezpečnosti a efektivity endoskopické endonazální techniky v léčbě kraniofaryngeomů. <p align="">Materiál a metody: Od roku 2008 bylo endoskopicky endonazálně operováno14 pacientů s typickým supraselárním extraventrikulárním kraniofaryngeomem. Soubor se skládá z 11 mužů a 3 žen (věk 17–60 roků, průměrný věk 38 let). Jedenáct pacientů mělo předoperační deficit zorného pole v rozsahu od malého deficitu ve vnějším kvadrantu až k slepotě jednoho oka a těžkému deficitu druhého oka. Byla použita endoskopická endonazální technika čtyř rukou s peroperační 3,0 T MR. <p align="">Výsledky: V sedmi případech bylo dosaženo radikální resekce. Ve čtyřech případech byla provedena resekce subtotální, ve dvou případech resekce parciální. V jednom případě se jednalo o vypuštění kraniofaryngeomové cysty. V resekci po iMR bylo pokračováno ve třech případech. K úpravě zorného pole došlo v pěti případech. Pooperační diabetes insipidus se nově objevil ve třech případech. Reoperace pro únik mozkomíšního moku byla nezbytná ve třech případech. <p align="">Závěr: Endoskopické techniky v léčbě kraniofaryngeomu jsou bezpečné. Nicméně jsou spojeny s vyšším výskytem pooperačního úniku mozkomíšního moku ve srovnání s transkraniálními přístupy., Introduction: We reviewed our results in order to evaluate safety of endoscopic endonasal technique in the treatment of craniopharyngiomas. <p align="">Material and methods: Since 2008, endoscopic endonasal approach was used in 14 patients with typical suprasellar extraventricular craniopharyngioma. This cohort consisted of 11 males and three females (age 17 to 60 years, average age 38 years). Eleven patients had preoperative visual field deficit ranging from small deficit in the outer quadrant to blindness on one eye and a severe deficit of the second eye. Endoscopic endonasal four hands technique was used with intraoperative 3.0 T MRI. <p align="">Results: In seven cases, the radical resection was achieved. Subtotal resection was performed in four cases. The resection was partial in two cases. In one case, a cyst was drained. Resection after iMRI was performed in three cases. Visual field deficit improved in five cases. Postoperative diabetes insipidus developed in three cases. Reoperation for CSF leakage was necessary in three cases. <p align="">Conclusion: Endoscopic technique in the treatment of craniopharyngioma is safe. Cieľ: Včasná diagnostika a liečba infekcie horných močových ciest je v detskom veku náročná a veľmi dôležitá vzhľadom na prognózu ochorenia. Cieľom našej práce bolo overiť možnosti využitia merania indexu rezistencie počas akútnej pyelonefritídy pri diagnostike ochorenia a pri určovaní rozsahu postihnutia obličiek. Metódy: Do súboru bolo zaradených 53 detí (43 dievčat, 10 chlapcov) vo veku 0-19 rokov s prvým atakom akútnej pyelonefritídy. Pacienti boli rozdelení do troch vekových skupín - deti do 4 rokov, od 4 do 7 rokov a deti nad 7 rokov a porovnávali sa so skupinou 29 detí bez príznakov akútnej infekcie a s normálnou funkciou obličiek. U 37 detí sme urobili analýzu rozsahu postihnutia zápalovým procesom. U všetkých pacientov bolo realizované ultrasonografické vyšetrenie uropoetického traktu do 72 hodín od začiatku hospitalizácie zamerané na morfológiu obličiek a meranie indexu rezistencie (RI) prostredníctvom dopplerovskej sonografie. Vo všetkých troch vekových kategóriách boli zistené signifikantne vyššie hodnoty RI pri zápalom postihnutých obličkách oproti obličkám kontrolnej skupiny. Výsledky: U 24 detí (64,8 %) išlo o unilaterálny zápalový proces a u 13 detí (35,2 %) išlo o proces bilaterálny. Záver: Na základe výsledkov možno skonštatovať, že meranie RI má výrazný prínos v diagnostike nejednoznačných prípadov akútnej pyelonefritídy, ako aj v možnosti určenia rozsahu postihnutia obličiek a možno ho odporučiť ako diagnostický postup doplňujúci štandardné diagnostické metodiky., Aim: Early diagnosis and treatment of infections of the upper urinary tract in children is difficult and very important with regard to the prognosis of the disease. The aim of our study was to verify the possibility of using measurements of resistivity index during acute pyelonephritis in the diagnosis of disease and to determine the extent of renal involvement. Methods: 53 children were eligible in the group (43 girls, 10 boys) at the age of 0-19 years at the first attack of acute pyelonephritis. The patients were divided into three age groups - children up to 4 years, 4 to 7 years and children older than 7 years and they were compared with a group of 29 children without symptoms of acute infection and normal kidney function. Ultrasonography examination of uropoietic tract oriented to kidney morphology was performed in all patients up to 72 hours from the beginning of hospitaliza-tion and RI was measured by beans of Doppler sonography In all the three age categories the authors detected significantly higher values of RI in kidneys affected by inflammation as compared with kidneys of the control group. Results: In 24 children (64.8%) was a unilateral inflammatory process and in 13 children (35.2%) was a bilateral process. BACKGROUND: In the cholesteatoma surgery ossicles can be replaced to reconstruct middle ear function. It is important that these ossicles are free of squamous epithelium, to prevent residual disease. This study focuses on the histological findings of the malleus and incus harvested during cholesteatoma surgery. MATERIALS AND METHODS: Eighty middle ears ossicles were examined in vivo and histologically to consider the relationship of cholesteatoma to ossicles, grade of bone destruction and invasion of cholesteatoma to deeper layers of bone. RESULTS: Serious ossicular destruction was observed more frequently in incus compared to malleus (p=0.0065). Difference of ossicles destruction between children and adults was not significant (p=0.3032). Deep invasion of cholesteatoma into the vascular spaces or inner core of the bone was not observed. CONCLUSIONS: Autograft ossicles from cholesteatomatous ears should not necessarily be rejected for reconstruction of the ossicular chain. Autorky příspěvku seznamují s dílčími výsledky vlastní sociometricky zaměřené metody, kterou aplikovaly ve třídách druhého stupně jedné ZŠ v České republice. V příspěvku je věnována pozornost hlavně těm výstupům, které se vztahují ke skupince dětí s nadváhou a obezitou. Pro adolescentní děti je prostředí školy a školní třídy důležitou součástí sociální reality, která značně ovlivňuje utváření jejich sebedůvěry, sebevědomí, identity a z těchto hledisek ovlivňuje po řadu kritických let kvalitu jejich života. Některé klinicky zaměřené zahraniční publikace tuto realitu reflektují a okolnostem života dětí s nadváhou a obezitou ve školním prostředí věnují pozornost (Kiess et al., 2009; Pallan et al., 2012; Taylor et al., 2005; Skidmore, Yarnell, 2004). Vzhledem k malému počtu dětí s nadváhou a obezitou byly výsledky v diskusi a v závěrečné části příspěvku shrnuty jen popisným způsobem., The authors of this article acquaint with the partial results of their own sociometric focused method, that they applied on the second stage of one elementary school in the Czech Republic. Attention is paid mainly to those outcomes, that relate to a group of children with overweight and obesity. For adolescent children are school environment and school classes important part of social reality, that greatly affects the formation of self-confidence, self-esteem, identity and affects many aspects of their quality of life during these crucial years. Even some clinically oriented foreign publications reflect this reality and pay attention to these circumstances of these children´s life (Kiess et al., 2009; Pallan et al., 2012; Taylor et al., 2005; Reilly et al., 2012; Skidmore, Yarnell, 2004). Kolorektální karcinom u dětí je velmi vzácné onemocnění. Léčebné výsledky jsou neuspokojivé a celkové přežití dětských pacientů s kolorektálním karcinomem je nízké. Pacienti mají často histologicky nepříznivý typ nádoru a často jsou diagnostikování až v pokročilém stadiu onemocnění. Na diagnózu kolorektálního karcinomu by mělo být pomýšleno u všech dětí s nejasnými bolestmi břicha provázených intermitentní poruchou střevní pasáže., Carcinoma of the colon and rectum is a rare malignancy in children. The overall survival rate in children with colon carcinoma is poor due to the advanced stage of disease at the time of diagnosis and due to unfavorable histological features.
