The target of research was the implementation of the comparative analysis of conservative therapy of preparation of basic therapy and also new methods of treatment (anticytokine therapy). Appointment of salofalk in twos tablets (0, 5) 34 times a day per os, and also 2,0 gr in the form of a rectal enema unitary for a period of seven days, then had transferd the patients to reception 2,0 gr salofalk per os, allowed to achieve substantial improvement patients cliniclaboratory and endoscopic symptoms with a heavy intensification of ulcer colitis variation, providing full cliniclabaratory remissions achievements of disease at 8 (34 %) patients. In the Minister of the Uzbek Public Health of the Republican Science Coloproctological Center at 10 patients with the IDI have spent the therapy remicade, 7 of them had NUC and 3 patients had the Crohn's disease. Therefore, all patients no longer than on the first week after introduction of remicade had improvement of a course disease which was expressed in defering stool, extinction or reduction of pathological impurity in excrement and subsidence bellyaches. During colonoscopical inspection, in 1218 weeks after the first infusion at 80% of the patients had observed disappearance of ulcers and erosion. In terms of 6 months of supervision of relapse of disease was not noted. Colonic polyps and polyposis are studied in 183 patients. It has been found, that sensitivity of rectoscopy is 66.7%, irrigoscopy 61.1%, virtual colonoscopy computeraided tomography of colon (VCCATC) 79.1%. Endoscopic polypectomy was the main treatment method used in patents with colonic polyps and polyposis (18.6%). Cíl. Retrospektivně zhodnotit efekt a dlouhodobou průchodnost transjugulární intrahepatální portosystémové spojky (TIPS) u nemocných s trombózou jaterních žil. Metoda. V letech 1992-2011 jsme vytvořili TIPS celkem u 38 pacientů s trombózou jaterních žil nekontrolovatelnou medikamentózní léčbou. Léčeno bylo devět mužů a 29 žen ve věku 13-76 let (medián 33 let), v šesti případech se jednalo o děti. Predisponující trombofilní stav byl prokázán u 84,2 % osob. Klinická forma Buddova-Chiariho syndromu byla v šesti případech akutní, v 18 případech subakutní a ve 14 případech chronická. Indikací k výkonu byl u 31 nemocných ascites, u pěti pacientů jaterní selhání a u dvou krvácení do zažívacího traktu. Zákroky byly prováděny standardní technikou v analgosedaci nebo v celkové anestezii. U 17 pacientů jsme do kanálu TIPS implantovali nepotažený stent nebo kombinaci potaženého a nepotaženého stentu, u 21 nemocných jsme zkrat vyztužili pouze pro TIPS dedikovaným potaženým stentem (stentgraftem). Sledování průchodnosti zkratu po výkonu bylo založeno na pravidelných ultrasonografických, klinických a laboratorních kontrolách, při prokázané poruše průchodnosti byla provedena reintervence. Výsledky. Celková doba sledování souboru byla 8 dní - 207 měsíců (medián 52 měsíců). Technického úspěchu jsme dosáhli u 100% nemocných, hemodynamická úspěšnost výkonu byla 86,8 %, s poklesem mediánu portosystémového gradientu z 23 mm Hg na 8 mm Hg, klinicky bylo dosaženo ústupu obtíží u 89,5% nemocných. Významné komplikace jsme zaznamenali celkem u sedmi osob, žádný pacient nezemřel v přímé souvislosti se zákrokem. V průběhu sledování zemřelo celkem jedenáct nemocných (29 %), přežití souboru za 1 rok a za 5 let od TIPS bylo 85,3 % a 65,4 %. Během doby sledování jsme provedli celkem 50 reintervencí pro dysfunkci spojky, průměrný počet reintervencí přepočtený na jednoho nemocného za dobu 5 let byl 1,65 výkonu u pacientů se zkratem vytvořeným stentem a 0,67 výkonu u nemocných, u kterých byl primárně použit stentgraft. Ve skupině se zavedeným nekrytým stentem byla primární průchodnost 52,9% za 1 rok a 20 % za 5 let od výkonu, u skupiny s implantovaným potaženým stentem byla jednoletá primární průchodnost 80 % a pětiletá primární průchodnost 33,3%. V průběhu sledování jsme zároveň u osmi pacientů provedli angioplastiku dolní duté žíly pro její symptomatickou stenózu, u šesti z těchto nemocných byl do dolní duté žíly zaveden stent. Závěr. Vytvoření TIPS je velmi efektivní metodou léčby pacientů s trombózou jaterních žil. Použití pro TIPS dedikovaných potažených stentů vede knižšímu počtu dysfunkcí zkratu s nižším počtem reintervencí nezbytných k obnově průchodnosti, nutností je však současná důsledná antikoagulační léčba a léčba základního hematologického onemocnění., Aim. To retrospectively evaluate the efficacy and long-term patency of transjugular intrahepatic portosystemic shunt (TIPS) in patients with hepatic vein thrombosis. Method. In years 1992-2011, totally 38 patients with hepatic vein thrombosis resistant to medical therapy underwent TIPS creation in our department. We treated 9 males and 29 females, age 13-76 years (median 33 years), 6 patients were children. The underlying thrombophilic state was confirmed in 84.2% of treated patients. The Budd-Chiari syndrome was acute in 6, subacute in 18 and chronic in 14 cases. Ascites was indication for TIPS in 31 patients, liver failure in 5 and gastrointestinal bleeding in 2 persons. All the procedures were performed in anal-gosedation or under general anestesia using standard technique. In 17 patients, we implanted non-covered stent or combination of non-covered and covered stents during TIPS creation, in the other 21 patients only the TIPS dedicated covered stents had been used. TIPS patency during follow-up after the procedure was based on regular ultra-sonographic, clinical and laboratory examinations. In case of shunt dysfunction, a rein-tervention was performed. Results. The total follow-up period was 8 days - 207 months (median 52 months). The technical success rate was 100%, hemo-dynamical success rate 86.8% with median portosystemic gradient decrease from 23 mm Hg to 8 mm Hg, and clinically we were succesfull in 89.5% of patients. In 7 patients, severe complications occurred during shunt creation, but none of the patients died in direct connection with the procedure. Totally 11 patients (29%) died during follow-up period, the 1-year and 5-years survival rates were 85.3% and 65.4%, respectively. Due to TIPS dysfunction, totally 50 reinterventions were needed, the average 5-year reintervention rate per patient was 1.65 procedures in the bare stent group and 0.67 porcedures in the covered stent group. In the non-covered stent group, we achieved primary patency rates 52.9% 1 year and 20% 5 years after TIPS creation, in the covered stent group the 1-year primary patency rate was 80% and 5-year primary patency rate 33.3%. Angioplasty of symptomatic stenosis of the inferior vena cava was performed in 8 patients during follow-up, in 6 of these patients also stent implantation was necessary. Conclusion. TIPS creation is a very effective treatment of patients with hepatic vein thrombosis. Few studies concerning the occurrence of cow’s milk allergy with the use of double-blind, placebo controlled food challenge test in adolescents and adult patients suffering from atopic dermatitis exist. Aim: To evaluate the occurrence of cow’s milk allergy in adolescents and adults suffering from atopic dermatitis. Method: Altogether 179 persons suffering from atopic dermatitis were included in the study: 51 men and 128 women entered the study with the average age of 26.2 (s.d. 9.5 years). Complete dermatological and allergological examinations were performed. Results: The positive results in specificIgE and in skin prick tests were recorded in 12% of patients. According to the open exposure tests and double-blind, placebo controlled food challenge tests these patients are only sensitized to cow’s milk without clinical symptoms of allergy. Double-blind, placebo controlled food challenge test confirmed food allergy to cow milk only in one patient (worsening of atopic dermatitis), the oral allergy syndrome was observed in another one patient, occurrence of this allergy was altogether 1.1%. LDL-aferéza je extrakorporální eliminační metodika, která usiluje o pokud možno specifické odstranění LDL-cholesterolu z cirkulující krve. V současnosti je používáno 6 účinných metod k selektivnímu odstranění LDL-cholesterolu. Mezi hlavní indikace k léčbě LDL-aferézou patří diagnózy homozygotní familiární hypercholesterolemie, heterozygotní familiární hypercholesterolemie refrakterní nebo při intoleranci k standardní léčbě, a dále pacienti se zvýšením lipoproteinu(a) refrakterní nebo intolerantní k farmakoterapii. Dosud však není jednoty o tom, jaká hladina LDL-cholesterolu je rozhodující pro zahájení LDL-aferézy, názory se liší i v jednotlivých státech. I když neexistují klasické velké randomizované studie, existuje dostatečný počet dobře řízených studií k tomu, aby v celém světě bylo uznáváno, že je efekt LDL-aferézy na kardiovaskulární prognózu u závažné hypercholesterolemie významný., LDL-apheresis is an extracorporeal elimination technique, which specifically removes LDL-cholesterol from the circulation. There are six methods for the selective LDL-cholesterol removal these days. The main indications for LDL-apheresis are the diagnosis of homozygous familial hypercholesterolemia, heterozygous familial hypercholesterolemia which is refractory the standard care and intolerance of routine care, and also patients with lipoprotein(a) increase resistant to the farmacotherapy. There is still debate which LDL-cholesterolemia is indication for LDL-apheresis therapy, and the recommendation differs among various countries. Lokální chemoprofylaxe neinvazivních nádorů močového měchýře je používána více než 40 let. Využíváno je několik agens, mezi nejčastější patří mitomycin C (MMC) a epirubicin. Názor na léčebná schémata není jednotný. Avšak na podkladě EBM je zřejmé, že adjuvantní instilace cytotoxických látek do močového měchýře prodlužuje dobu beznádorového intervalu. Cíl. Ischemické cévní mozkové příhody (iCMP) jsou stále jednou z nejčastějších příčin mortality a morbidity postihující i mladší populaci v produktivním věku. Klíčovým faktorem ovlivňujícím prognózu po iCMP je včasná rekanalizace uzavřené mozkové tepny s obnovením perfuze v postižené mozkové tkáni. Cílem práce bylo zhodnocení bezpečnosti a efektivity kombinované revaskularizační terapie sestávající z iniciální intravenózní trombolýzy a přímo navazující mechanické trombektomie u pacientů s akutní iCMP a uzávěrem mozkové tepny. Metoda. Do souboru pacientů byli zařazeni všichni po sobě jdoucí pacienti s akutní iCMP a prokázaným uzávěrem střední mozkové tepny v úseku M1-2 nebo distální části arteria carotis interna nebo uzávěrem arteria basilaris, a to v období od června 2010 do dubna 2012. Všichni nemocní byli léčeni kombinovanou revaskularizační terapií, tzn. iniciální intravenózní trombolýzou a přímo navazující mechanickou trombektomií systémem Solitaire. Iniciální stupeň neurologického deficitu byl stanoven pomocí NIHSS skóre. Výsledný klinický stav byl zhodnocen po 90 dnech pomocí modifikované Rankinovy škály (mRS). Výsledky. Do souboru bylo podle kritérií zařazeno 50 pacientů (průměrný věk 66, 8 ? 14,6 roku) se vstupním mediánem NIHSS skóre 18,0 (8-32). Rekanalizace (TICI 2A, B a 3) bylo dosaženo u 94 % pacientů, kompletní rekanalizace (TICI 3) u 72% pacientů. Průměrný rekanalizační čas (čas od začátku symptomů do dosažení maximální rekanalizace) byl v našem souboru 244,2 ? 87,9 minut. Symptomatické intracerebrální krvácení se vyskytlo u 6 % pacientů. Medián mRS po 90 dnech byl v našem souboru 1, 60 % pacientů mělo devadesátidenní klinický výsledek dle mRS hodnocen jako dobrý (mRS 0-2). Tříměsíční mortalita byla ve sledovaném souboru 14 %. Závěr. Kombinovaná revaskularizační terapie akutní iCMP sestávající z iniciální intravenózní trombolýzy a přímo navazující mechanické trombektomie je bezpečná a efektivní. Okamžité provedení mechanické tromektomie po iniciální intravenózní trombolýze vede k výraznému zkrácení rekanalizačního času, který je rozhodující pro dobrý klinický výsledek u pacientů intervenovaných pro akutní ischemickou cévní mozkovou příhodu., Aim. Ischemic stroke is today one of the leading causes of mortality and morbidity and affects even a younger population of working age. A key factor influencing prognosis after ischemic stroke is early recanalization of close cerebral artery and reperfusion in the affected brain tissue. The aim of our study is to evaluate the safety and effectiveness of re-vascularization combined therapy consisting of initial intravenous thrombolysis and immediately after that performed mechanical thrombectomy in the treatment of patients with acute ischemic stroke. Methods. Consecutive non-selected acute ischemic stroke patients with baseline CTA or MRA documented occlusion of middle cerebral artery (MCA) (segment M1-2) or distal part of internal carotid artery (ICA) or basilar artery (BA) and treated between June 2010 and April 2012 at our stroke center were included in the study. All patiens were treated by combined therapy consisting of initial intravenous thrombolysis and immediately after that performed mechanical thrombectomy by Solitaire stent. Stroke severity was assessed using National Institutes of Health Stroke Scale (NIHSS), 90-day clinical outcome using modified Rankin Scale (mRS). Results. Fifty enrolled patients (mean age 66.8 ? 14.6 years) had baseline median NIHSS 18.0 (8-32). Overall recanalization (TICI 2A, B and 3) was achieved in 94% of patients, complete (TICI 3) in 72% of patients. The mean time from stroke onset-to-maximal recanalization was 244.2 ? 87.9 min. Symptomatic intracerebral hemorrhage occurred in 6% of patients. The median mRS score at 90 days was 1 and 60% patients had good outcome (mRS 0-2). Overall 3-month mortality was 14%. Conclusion. Combined revascularization therapy of acute ischemic stroke consisting of initial intravenous thrombolysis and immediately after that performed mechanical thrombectomy is safe and effective. Úvod: Recidíva pectus excavatum je najzávažnejšou neskorou komplikáciou primárnej korekcie. V indikovaných prípadoch je možné pacientovi odporučiť rekorekciu modifikovanou Ravitchovou technikou (HMRR), alebo minimálne invazívnu korekciu (MIRPE). Práca sa zameriava na vyhodnotenie dostupných operačných techník u dospelých pacientov a adolescentov s recidívou pectus excavatum. Metódy: Autori retrospektívne vyhodnotili výsledky svojej práce. V období od júna 2006 do októbra 2014 odoperovali na Klinike hrudníkovej chirurgie 126 výkonov pre pectus excavatum. HMRR bolo metódou voľby v 51 prípadoch a MIRPE v 75. Korekcia pre recidívu pectus excavatum bola indikáciou pre 12 (9,5 %) výkonov u 11 pacientov. Tí absolvovali šesť korekcií technikou HMRR a 6 technikou MIRPE. HMRR ako primárny výkon podstúpilo 10 pacientov a MIRPE jeden. Medián veku súboru bol 23,5 (17−44) roku, medián odstupu od prvej operácie bol 9,5 (2−31) roku. Muži tvorili 75 % operantov. Výsledky: Napriek potrebe torakoskopickej adheziolýzy a implantácie dvoch dláh u všetkých pacientov sa MIRPE vyznačovala kratším operačným časom (120 vs. 172 min) a menším počtom komplikácií (16,7 % vs. 50 %). V skupine s HMRR sme zaznamenali tri komplikácie (ranový seróm, migráciu dlahy do perikardu s masívnym hemoperikardom, oddelenie priamych brušných svalov od sterna) a v skupine s MIRPE jednu (symptomatický fluidotorax). U pacienta s migráciou dlahy vznikla ďaľšia recidíva pectus excavatum. Nezaznamenali sme rozdiel v dľžke pooperačnej hospitalizácie. Záver: Napriek limitovaným skúsenostiam považujeme MIRPE v indikovaných prípadoch za bezpečnú a efektívnu techniku pri riešení recidívy u dospelých pacientov a adolescentov., Introduction: Recurrent pectus excavatum is the most serious late complication after primary repair. Redo open repair (Highly Modified Ravitch Repair, HMRR) or minimally invasive repair of pectus excavatum (MIRPE) are usually performed in indicated cases. This paper focuses on the evaluation of available redo surgical techniques in adult and adolescent patients with recurrent pectus excavatum. Methods: 126 operative corrections, predominantly in adult patients, were performed by the authors between June 2006 and October 2014. HMRR was the method of choice in 51 cases, and MIRPE in 75 cases. Recurrent pectus excavatum was the indication in 12 repair procedures (9.5%) in 11 patients. Prior repairs included HMRR in 10 patients and MIRPE in one case. Both HMRR and MIRPE were indicated as redo procedure in six cases. The median age in the redo group was 23.5 (17−44) years and the median interval between the primary correction and the redo procedure was 9.5 (2−31) years. The male to female ratio was 3:1. Results: The use of MIRPE resulted in shorter operation time (120 vs. 172 min). There was no difference in the length of postoperative hospitalisation. Three complications (50%) were recorded in the HMRR group (wound seroma, intrapericardial bar migration with hemopericardium, displaced rectus abdominis muscle) and one (16.7%) occurred in the MIRPE group (symptomatic fluidothorax). Conclusion: Despite our limited experience with both techniques in the treatment of recurrent pectus excavatum we believe that MIRPE should be regarded as a safe and effective technique for the redo repair in adolescent and adult patients., and M.
