The publication Ethical and Legal aspects of Stem Cell Research presents collection of essays, which took their part on the conference of the same name on 18 th of May 2013 in Prague. Colletion includes six articles from different authors. All of them are trying to reflect central theme of the collection, each of them from different positions and aspects of the problem. Jozef Fulka Jr. deals with cloning, Marek Vácha with basic biological facts about stem cells. Adam Doležal reflects ethical and legal issue s on so called “surplus embryos”. Josef Kuře considers the utility of research from the social aspect. Tento příspěvek je druhým příspěvkem v sérii článků zaměřených na vývoj etického pojetí eutanazie. Problém eutanazie řeší na pozadí slavné Rachelsovy publikace The End of Life. Rachels je autor zastávající novodobou koncepci utilitaristické morálky. Ve své publikaci The End of Life velmi podrobně probírá všechny důležité momenty filosofické a etické diskuse o eutanazii a vytyčuje tak kontury území, na němž se jak zastánci jeho závěrů, tak i jejich odpůrci, musí pohybovat. Hovoří o posvátnosti lidského života, rozebírá různé formy eutanazie, kriticky hodnotí některé důležité distinkce, široce a někdy nekriticky užívané v lékařské etice: rozdíl mezi usmrcením (killing) a ponecháním zemřít (letting die), mezi řádnými a mimořádnými léčebnými prostředky, mezi aktivní a pasivní eutanazií. Důležitým prvkem Rachelsovy reflexe je i diskuse o důležitosti úmyslu (intence) v etickém vyhodnocování lidských skutků., This contribution is the second of a series of articles devoted to the ethical evaluation of euthanasia. It discusses the problems of euthanasia on the background of Rachel´s famous book The End of Life. Rachels as an author is holding a modern concept of utilitarian morality. In his publication The End of Life he discusses in detail all the important moments of philosophical and ethical debate about euthanasia. In this way he also outlines the contours of the territory for his proponents as well as their opponents for recent discussion on euthanasia and end of life decisions. He talks about the problems of conception of the sanctity of human life, discusses the various forms of euthanasia, critically evaluates some important distinctions between terminology in medical ethics: the difference between killing (killing) and allowing to die (letting die), between ordinary and extraordinary means of treatment, the active and passive euthanasia. Motivation Until recently it was considered that 65 years is cutoff for defining patients as elderly, but newer reports indicate that this age limit shift to 70 years of age. Elderly patients with advanced non small cell lung cancer, associated comorbidities and poor performance status represent a specific population and a challenge for use of chemotherapy. Primary aim was to evaluate the impact of mono therapy with oral etoposide on overall survival in elderly patients (≥ 70 years of age) with advanced non small cell lung cancer and poor performance status (PS) ≥ 2 (clinical stage IIIb and IV ), and as well to evaluate tolerability of this therapy. Secondary aim was to evaluate response rate. Methods Retrospectively, medical records of 79 female and male patients with advanced non small cell lung cancer and poor performance status treated with oral etoposide (2x25 mg 20 days/10 days pause) in period from 2007 till 2010 were checked for relevant data. Data regarding demographics, performance status, overall survival, response rates and drug toxicity were collected. For statistical analysis we used Pearson chi-square test, T-test, Kaplan-Meier product limited method and Cox regression. Results Median overall survival (OS) was 31 weeks, in patients with PS 2 overall survival was 34 weeks, and in group with PS 3 was only 24 weeks. Partial response was registered in 20.2% of patients, stable disease in 41.85 % and disease progression in 38% of patients. Treatment was well tolerated, febrile neutropenia and toxic deaths were not registered. Toxic effects didn't have statistically significant influence on OS. Conclusion Oral etoposide used as mono therapy has been shown as moderate effective and very safe in treating elderly patients with advanced non small cell lung cancer and poor performance status so it represents a good therapy option for treating this specific population., Zoran Andrić, Vladimir Kovčin, Slobodanka Crevar, Zafir Murtezani, Sanja Kostić, and Literatura
The aim of this review is to summarize main aspects of the book Evropská úmluva o lidských právech written by Kmec, J., Kosař, D., Kratochvíl, J., Bobek, M., published in 2012 in Prague. As the title suggests the book itself is focused on Convention for the protection of human rights and fundamental freedom and relating questions, e.g. application to the European court of human rights, case proceeding and case - law. This review briefly informs about the authors, however it ́s mainly concentrated on th e publication ́s content. Cíl: Prezentovat scintigrafický nález u 15leté dívky s Ewingovým sarkomem. Metoda: Patnáctiletá pacientka s nevýznamnou osobní i rodinnou anamnézou byla přijata na dětské oddělení spádové nemocnice pro zhoršující se únavu, nechutenství, váhový úbytek (10 kg), bolesti kloubů a kostí, vysoké laboratorní zánětlivé parametry (CRP 160 mg/l, FW 120/128) a anémii. Byla provedena třífázová scintigrafie skeletu k vyloučení zánětlivého postižení sakroiliakálního skloubení (SIS) vpravo. Pacientce byla dle hmotnosti (59,5 kg) podána aktivita 750 MBq 99mTc-metylendifosfonátu (MDP). Za 10 minut po intravenózní aplikaci radiofarmaka byla provedena 2. fáze třífázové kostní scintigrafie zaměřená na oblast sakroiliakálního skloubení. S odstupem tří hodin byla vyšetřena 3. fáze (kostní). Vyšetření bylo provedeno na dvouhlavé gamakameře AXIS firmy PICKER se semikvantitativním vyhodnocením na počítači. Gamakamera byla osazena kolimátory pro nízké energie s vysokým rozlišením a paralelními otvory. Výsledky: Třífázová scintigrafie skeletu metodou celotělové scintigrafie zjistila ve 3. fázi výrazně skvrnitě zvýšené vychytávání v axiálním skeletu, v kalvě okcipitálně ve střední čáře, v obou kyčelních kloubech a v levé fibule. Diferenciálně diagnosticky nález naznačoval možnost hematologické malignity, proto byla pacientka přeložena na onkohematologické oddělení dětské kliniky FN Olomouc. Zde bylo doplněno vyšetření skeletu pomocí magnetické rezonance (MR), plnohodnotné vyšetření kostní dřeně a trepanobiopsie. Nález podporoval diagnózu Ewingův sarkom, respektive Ewingův sarkom - primitivní neuroektodermový nádor (ES/PNET) s nejasným primárním ložiskem. Pacientka byla předána k další péči na pracoviště Kliniky dětské onkologie do Brna. Zde byl nález uzavřen jako Ewingův sarkom/PNET s primárním ložiskem na levé fibule a s nálezem mnohočetných metastáz ve skeletu a kostní dřeni, T2N0M1, klinické stádium Enneking III. Závěr: Kazuistika poukazuje na význam vysoce senzitivních nukleárně medicínských vyšetření při složité diagnostice onemocnění nejen dětského věku a na jejich přínos z hlediska urychlení dalšího vyšetřovacího postupu a léčby., x, Jana Páterová, Jaromír Bernátek, Blanka Kadllčíková, and Literatura
Cílem mé práce je zamyslet se nad existencialismem a postmoderním přístupem z pohledu vzájemných podobností i rozdílů. Existenciální analýza stojí podle mého názoru mezi modernou a postmodernou, protože na jedné straně vychází z poznávání objektivního světa (moderna), ale na druhou stranu k ní přidává požadavek subjektivního prožívání (postmoderna). Došel jsem k následujícím podobným předpokladům existenciálního a postmoderního přístupu: obrat k subjektivnímu prožívání, nereduktivní přístup k životní zkušenosti, schopnost svobodné volby, princip osobní zodpovědnosti, zdůrazňování vnitřních zdrojů klienta a orientace na budoucnost, širší kontext mezilidských vztahů či dané kultury, neexistence jediné pravdy, důraz na plynutí času, obrat k jazyku a přizpůsobení se jazyku klienta. Naproti tomu se oba směry vyznačují rozdíly v teoriích poznání, v různém zaměření na smysl a cíl života, v jiném pojetí emocí a z toho vycházejícího uchopování nepříjemných prožitků. Rozdílný pohled na některá témata vnímám jako podněcující, protože člověku skýtá příležitost přemýšlet o věcech novým, obohacujícím způsobem., The purpose of my work is to think of the existentialism and the postmodern approach from the view of their mutual similarities and differences. In my opinion, the existential analysis stands between the modernism and the postmodernism, because on the one hand it is based on the cognition of the objective world (the modernism) and on the other hand, there is also the requirement of subjective experience there (the postmodernism). I came to the following similar premises of the existential and postmodern approach: the return to subjective experience, non-reductive attitude to the life experience, the ability of free choice, the principle of personal responsibility, the accent on inner sources of the client and the focus on the future, broader context of the interpersonal relationships or the given culture, the absence of the only truth, the accent on the passage of time, the return to the language and the accommodation to the language of the client. On the contrary, both of the directions differ in the theories of cognition, in different focus on the meaning and goal of life, in different conception of emotions and consequent grasping of unpleasant experiences. Cíl: Cílem studie bylo provést analýzu expozičních parametrů skiagrafického a skiaskopického módu, a porozumět tak fungování expoziční automatiky angiografického systému, který je využíván pro intervenční kardiologické výkony. Dále bylo cílem zhodnotit vliv přídavné filtrace na dávku pacientům. Metoda: Studie byla provedena pro systém Axiom Artis MP se zesilovačem obrazu. Do analýzy byly zahrnuty expoziční parametry plynoucí z funkce expoziční automatiky pro několik skiagrafických módů používaných pro dospělé pacienty, ale stejně tak pro módy používané v pediatrii. Jednalo se hlavně o analýzu následujících parametrů: napětí rentgenky, proud rentgenky, šířka pulzu a přídavná filtrace. Výsledky: Hlavní rozdíl mezi expozičními parametry pro SG a SS režim byl zjištěn pro proud rentgenky a šířku pulzu, menší rozdíl pak pro přídavnou filtraci. Pro AEC platí, že s rostoucí tloušťkou fantomu se zvyšuje nejprve proud rentgenky, šířka pulzu a napětí zůstává konstantní. Při dalším zvětšení tloušťky fantomu narůstá proud a šířka pulzu, napětí je udržováno na konstantní hodnotě. Při ještě větší tloušťce fantomu se zvyšuje i použité napětí a současně se zmenšuje přídavná filtrace. Závěr: Expoziční parametry skiagrafického módu se odlišují od parametrů skiaskopického módu. Skiaskopický mód pracuje s nižším proudem a vyšší filtrací, což znamená snížení dávky na kůži pacientů, ale současně je to spojeno s méně kvalitním obrazem. Použití větší přídavné filtrace není možné pro obézní pacienty kvůli nadměrnému zatížení rentgenky., Aim: The aim of this work was to analyse exposure parameters of cine mode and fluoroscopy mode and therefore to understand the automatic exposure control of an angiography system used for interventional cardiology procedures. The influence of the additional filtration on dose to patients was assessed. Methods: The study was performed on the angiography system Axiom Artis MP with an image intensifier. Exposure parameters selected by the automatic exposure control of many cine modes for adult patients as well as for pediatrie patients were analysed. Mainly it included tube voltage, tube current, pulse width and additional filtration. Results: The main difference between cine mode and fluoroscopy mode was in used tube current and pulse width, smaller difference was between additional filtration. With increasing thickness of the phantom, the AEC makes the tube current increase, pulse width and voltage is kept constant. With still increasing phantom width, current and pulse width increase, voltage is kept constant. Then tube current, pulse width and also voltage increase. With voltage increased, the additional filtration is lowered. Conclusion: The exposure parameters of cine mode were a little bit different from parameters of fluoroscopy mode. Thicker additional filtration significantly lowers the skin dose to patients but cannot be used for obese patients due to excessive X-ray tube loading., Lucie Súkupová, and Literatura
The important biological role of saliva in maintaining of the homeostasis of the oral cavity environment, preventing infection and tooth decay is widely accepted. Salivary glands insufficiency may violate the balance between health and disease. Clinical data indicated that the destruction, agenesis and aplasia of salivary gland commonly followed with hypo salivation, low salivary flow, which resulted in severe caries and periodontal disease. Salivary gland dysfunction also frequently found in preterm and low birth weight newborns. But the information about structural background of abnormal salivation in early childhood is still limited. A presence of any correlation between salivary glands structural development and intrauterine restrictions of fetal growth (IUGR) is unclear. The aim of present study was to determine morphological and morphometric peculiarities of human parotid gland in case of IUGR at late gestation. Material and methods: Parotid glands of twenty human fetuses 2022 weeks of gestation with diagnosed IUGR from late abortions material were compared with ten fetal glands in cases of induced abortions due to psychological reasons (control group). Tissue samples were immersionfixed in 10% buffered formalin solution, embedded in paraffin wax. Histological slides were stained routinely with hematoxylin & eosin, with Van Gieson's Stain. Microscopical examination was performed on magnification x 40 and x 100. Stereometric study by point count method at magnification x 40 allowed finding out volume fractions (VF) of glands parenchyma and stroma. VF of lobules components (glands wall, glands lumen, ducts wall, ducts lumen, vessels, intralobular connective tissue) were registered at magnification x100. Morphometry of the secretory portion of the parotid gland was conducted on the Zeiss microscope with the help of the AxioVision Rel.4.8 program. The mature (differentiated) end pieces were measured, including their area (in mkm2), width, height, perimeter (all in mkm). Similar measurements were done in the foci of immature secretory ends of a gland. Additionally the mean height of the epithelial cells layer within the mature secretory end pieces was measured. The differences were analyzed by methods of mathematical statistics using the software Microsoft Excel; data was compared with control measurements by Students ttest. Results: The results of present research have shown the delayed differentiation of fetal parotid glands parenchymal components in case of IUGR. The parenchymal VF did not reach control values. Furthermore, VF of lobules components was also decreased. Ducts lumens appeared to be significantly narrower than at physiological gestation. Interlobular and intralobular connective tissue stroma, in contrast to the controls, occupied vast areas, and their volume fraction was increased. Deficit of the parenchymal components of the gland was enhanced by slower maturation of glands. In cases with IUGR, differentiated glands occupied smaller area, with reduced width, height and perimeter. Epithelium lining the differentiated glands is characterized by significantly lower height compared to the control group. Delayed differentiation resulted in higher proportion of immature glands. Their area, width, height and perimeter increased. IUGR was also accompanied with a variety of pathological changes. Conclusion: Present evidences suggest that IUGR leads to impaired growth and maturation of the parotid gland. Structural immaturity and lack of differentiated parenchymal elements of the organ may form the basis of its secretory functions lesion. INTRODUCTION: Autologous stem cell transplantation (ASCT) became standard of care for patients with multiple myeloma (MM) under the age of 65 years. We routinely perform ASCT for newly diagnosed MM since 1996 in our department. PATIENTS AND METHODS: We retrospectively analyzed all 285 transplants in 185 patients done for MM from January 1996 till December 2010. We analyzed overall survival (OS) and progression-free survival (PFS) regarding conditioning, stage, complete or very good partial remission (CR, VGPR) achievement, renal impairment, single vs. double transplant. RESULTS: Estimated 10-years survival of the whole set of patients is 39% (median survival 95 months). Patients with renal impairment show same OS as those without (p = 0.22). Patients show similar overall survival and event free survival regardless of type of transplant. We observed better outcome in terms of overall survival in patients treated with new drugs (p = 0.0014). Reaching CR or VGPR was not translated into better OS (p = 0.30) and EFS (p = 0.10). Also stage of the disease and whether single or double transplant was used did not make any significant difference in the outcome. CONCLUSION: Stem cell transplantation greatly improved outcome of patients with MM. Poor outcome of allogeneic transplantation in our group of patients is related to high transplant related mortality (20% vs. 0%) and unexpected high relapse rate. There is a trend towards better survival, when new drugs are incorporated at any time in the course of the disease.
