V souvislosti s ochranou lidských práv zaručených Úmluvou o ochraně lidských práv a základních svobod se v poslední době čím dál častěji diskutuje otázka, zda přístup k asistované reprodukci také spadá pod rámec ochrany poskytovaný Úmluvou. Příspěvek přináší přehled rozhodnutí Evropského soudu pro lidská práva, která se problematikou asistované reprodukce zabývaly. Na pozadí těchto rozhodnutí se příspěvek snaží analyzovat hranice rozšiřujícího pojetí práva na respektování rodinného a soukromého života podle čl. 8 Úmluvy a poukázat na skutečnost, že v oblasti řešení složitých biomedicínckých a bioetických otázek nemá Soud dostatečnou autoritu na to, aby se stal finálním arbitrem při posuzování hodnotových měřítek společnosti v jednotlivých evropských státech., In the last few decades decided European Court of Human Rights very problematic cases involving “right to access to assisted reproduction”. These cases are concerning very controversial ethical aspects of biomedical developments in the field of medical techniques of artificial procreation and reshape the content (meaning) of Article 8 of European Convention of Human Rights. This paper provides short overview of cases of European Court of Human Rights involving “right to access to assisted reproduction” and focuses especially on the latest developments in this field (case S.H. and Others v. Austria - Application no 57813/00) . Although the technology of medically assisted procreation had been available in Europe for several decades, many of the issues to which it gave rise, remained the subject of debate. There is no consensus towards the use of IVF treatment in general and even less clear are trends in some particular questions (f.e. allowing gamete donation). Cílem našeho sdělení je demonstrovat možnosti základního vyšetření likvoru (CSF, z angl. cerebrospinal fluid = mozkomíšní mok) při sledování následného vývoje stavu centrálního nervového systému (CNS) u pacienta po subarachnoidálním krvácení. Klíčovou problematikou je především odlišení purulentních zánětlivých změn bakteriální etiologie a neinfekčního serózního zánětu ve smyslu úklidové reakce v CNS při opakovaně negativním mikrobiologickém vyšetření CSF. Možným řešením je společné hodnocení cytologického obrazu CSF a energetických poměrů v likvorovém kompartmentu vedoucí k určení charakteru zánětlivého procesu v CNS. Informační potenciál základní likvorologie však tímto není vyčerpán. Na případu dále předvádíme sledování poruchy cirkulace CSF vedoucí ke vzniku hydrocefalu, detekci probíhající destrukce tkáně při ischemizaci mozkového parenchymu a hodnocení přítomnosti krvácení do likvorových cest. Vývoj komplexního likvorového obrazu je pak průběžně konfrontován s výsledky zobrazovacích vyšetření a přispívá k optimalizaci terapeutických postupů až do ukončení intenzivní péče o pacienta., Our paper demonstrates the possibilities of basic examination of the cerebrospinal fluid (CSF) when monitoring the central nervous system (CNS) in a patient after subarachnoid hemorrhage. The key problem is to differentiate between purulent inflammation of bacterial etiology and non-infectious serous inflammation, the so called „clean-up reaction”, in the CNS during repetitively negative microbiological examinations of the CSF. Combined evaluation of the cytological image and energy turnover in the CSF compartment can help in specifying the character of inflammation of the CNS. The information potential of the CSF examination, however, is not depleted yet. In this case report we further demonstrate the failure of the CSF circulation leading to hydrocephalus, detection of tissue destruction during brain ischemia and detection of hemorrhage into the cerebrospinal fluid pathways. Histologická verifikace CT nebo MRI suspektního tumoru mozku je v současnosti nutnou podmínkou pro indikaci komplexní onkologické terapie. Strukturální patologie postihující zadní jámu lební jsou méně častým cílem stereotaktické biopsie pro koncentraci vitálních struktur v omezeném prostoru a komplikovanější přístup. Volba přístupu je dána především lokalizací léze ve vztahu k jednotlivým strukturám mozkového kmene. Je možné využít přístup transfrontální v dlouhé ose mozkového kmene (léze postihující mezencefalon, horní pons), transtentoriální přístup (léze mozečkové hemisféry a vermis mozečku) a přímý transcerebellární přístup (léze hemisféry mozečku a pontu, zvláště laterálního). Lesions affecting posterior fossa structures are less frequent target for stereotactic biopsy because of the concentration of vital structures in the limited space and more demanding approach. The choice of the approach is determined mainly by the lesion relationship to brainstem components. It is possible to use transfrontal approach along the long axis of the brainstem (lesions affecting mezencephalic and upper pontine region), transtentorial (lesions affecting cerebellar hemisphere and vermis) and direct transcerebellar (lesions of cerebellar hemisphere and pons, especially lateral). The paper analyses the use of the modified transcerebellar approach in 2 patients with lesions of unclear etiology affecting posterior fossa structures. We report the case of a 22-year old male, a self-confessed recreational drug user who developed cardiogenic shock because of severe destruction of the aortic valve by rapidly progressive aortic valve endocarditis. The disease progression was acute; in a matter of days, the clinical manifestations were life-threatening necessitating urgent aortic valve replacement surgery. Cultivation revealed Streptococcus viridans as the microbial agent. Subsequent recovery with antibiotic treatment was without complication. We present a 73 year old female with intractable pruritus and nonspecific cutaneous rash for a period of 9 months. She had recieved symptomatic therapy with no improvement. A complete examination revealed axillary and abdominal lymphadenopathy. A biopsy confirmed the diagnosis of Hodgkins lymphoma with Langerhans cell histiocytosis. She received 5 cycles of chemotherapy with resolution of pruritus and reduction in axillary and abdominal lymphadenopathy. The patient presented 6 months later with relapse and succumbed to the illness. Simultaneous occurrence of Langerhans cell histiocytosis and Hodgkins lymphoma may lead to misdiagnosis. Here we report the case of a 31-year-old man admitted to our hospital with echocardiografic and Cardiac Magnetic Resonance signs of myocarditis complicated by ventricular tachycardia, initially resolved with direct current shock. After the recurrence of ventricular tachycardia the patient was submitted to electrophysiological study revealing a re-entrant circuit at the level of the medium segment of interventricular septum, successfully treated with transcatheter ablation. Behçet's disease is a multisystem chronic inflammatory disease generally presenting as recurrent oral-genital ulcers and uveitis. Vascular involvement is a common presentation. However, cardiac involvement is rare in this disorder. In this case report, we present a large right ventricular thrombus detected both in computed tomography and echocardiography in a 32 year-old male patient with complaints of fever, haemoptysis and weight loss. V kazuistice je popsán průběh onemocnění 55letého nemocného s neuroendokrinním tumorem a postižením jater, pankreatu, mezenteria, tenkého střeva a mediastina. Acenocoumarol is a vitamin K antagonist that is used for the treatment of acquired and congenital, both arterial and venous, thrombotic diseases. Its use is complicated by the narrow therapeutic range. Bleeding following oral anticoagulation, despite rare, remains the major complication. Most cases of hemorrhagic episodes usually require short hospitalization and transfusion, while surgical drainage of the hematoma is not recommended. However, in cases that conservative treatment isn’t successful, surgical intervention remains an option. Kazuistika pojednává o třicetileté pacientce, která byla opakovaně hospitalizována pro levostrannou ileofemorální trombózu. Pacientka byla indikována k lokální farmakologické trombolýze. V průběhu trombolýzy bylo však vyjádřeno podezření na možnost komprese cév zvenčí. Následovala další vyšetření, která odhalila měkkotkáňovou nebolestivou formaci na mediální straně stehna jako příčinu na léčbu rezistentní trombózy. Rezistence se nacházela v těsném kontaktu s nervově-cévním svazkem na stehně. Dle histologie se jednalo o bifázický synoviální sarkom. V době diagnózy byly již přítomny vzdálené plicní metastázy I přes intenzivní neoadjuvantní léčbu dochází k progresi zejména plicního postižení. Chirurgická léčba není toho času indikována. Pacientka umírá 6 měsíců od stanovení diagnózy., Case report deals with a 30-year-old female patient, who was repeatedly admitted to hospital with left sided ileofemoral thrombosis. The patient was scheduled to local farmacological thrombolysis. During the thrombolysis a suspicion of compression of the vessels from outside was expressed. A further examination revealed an extensive soft-tissue non-painful mass on the medial side of the thigh as a cause of the thrombosis, which was not responding to the treatment. The lesion was in close contact with the femoral neurovascular bundle. The histological examination proved biphasic synovial sarcoma. At the time of diagnosis, distant lung metastases were already present. Despite intensive neoadjuvant therapy the disease, especially the lung affection progressed.
