Práce upozorňuje na význam pregraduálního vzdelávaní v geriatrii v přípravě budoucích lékařů. Požadavky Sekce geriatrické medicíny (UEMS), Evropské společnosti pro geriatrickou medicínu (EUGMS) a Evropské akademie pro medicínu stárnutí (EAMA) na vzdělávání studentů lékařských fakult a obsah kurikula v evropském regionu byly podrobně zpracovány mezinárodní pracovní skupinou odborníků geriatrů a publikovány po názvem Deklarace Evropské akademie nadace Yuste 2004. Deklarace obsahuje aktuální doporučení pro lékařské fakulty v evropském regionu. Navrhuje ustavení samostatných geriatrických klinik/geriatrických kateder na všech lékařských fakultách a přípravu vysokoškolských učitelů se speciaUzací v geriatrii a gerontológii. Výuka geriatrie na lékařských fakultách je součástí kurikula a zahrnuje jak prekhnické, tak klinické obory. Deklarace dále definuje minimální požadavky na výuku samostatného oboru geriatrie (rozsah a obsah), cíle výuky a kompetence, které by student po absolvování výuky měl zvládnout. Jako minimální je požadován rozsah nejméně 50 hodin., The article stresses the importance of the undergraduate education in geriatric medicine for the training of future physicians. Requirements of the Geriatric Medicine Section of UEMS, European Geriatric Medicine Society (EUGMS) and European Academy for Medicine of Aging (EAMA) for education of medical students and for the content of medical curriculum have been thoroughly reviewed by the international working party of geriatricians and pubhshed as „Declaration of the European Academy of Yuste Foundation 2004". Declaration contains recommendation for medical faculties in European reh suggests estabhshing Departments of geriatrics/ Chairs of Geriatrics at each medical faculty and training of university teachers in geriatric medicine and gerontology. Geriatric medicine is taught as part of the curriculum and stretches from preclinical discipUnes to clinical medicine. Declaration defines further the minimal requirements for geriatrics as an independent specialty, its goals and content and competencies which student will gain after the training is finished. Terapie statiny pozitivně působí na stabilizaci aterosklerotických plátů, restauruje funkce endotelu, má protizánětlivý efekt a signifikantně snižuje riziko akutních koronárních syndromů včetně gerontologické populace. Po disrupci plátu může snížení hladiny lipidů v séru pomocí statinů ovlivnit následující trombózu svým inhibičním divem na agregaci destiček, ovlivněním koagulačních faktorů a exprese tkáňového faktoru, vzniku trombinu a ovlivněním reologických vlastnosti a fibrinolytického systému. Vzhledem k endoteliální dysfunkci během stárnúti per se a k příznivému vlivu inhibitorů HMG-CoA reduktázy (statinů) na funkci buněk endotelu a stabilitu aterosklerotických plátů lze považovat využiti těchto farmak v léčbě endotelově dysfunkce gerontologické populace jak za klinicky relevantní, tak přínosné i nadějné., The treatment by statins positively effects stabilisation of atherosclerotic plaque, restores endothelial function, has anti-inflammatory effect and significantly improves the risk of acute coronary syndrome including the elderly. Lipid lowering by statins after plaque rupture may influence the following thrombosis via inhibitory effect on platelet aggregation, coagulation factors and tissue factor expression, thrombin formation, and may improve blood rheology and fibrinolysis.
