Based on survey of 257 women with nondeveloping pregnancies of various geneses, it was shown that the level of apoptosis marker P53 in the decidual tissue is increased, and antiapoptotic protein Bcl2 simultaneously is reduced, compared with similar data of women with physiological pregnancies. Brachial Plexus is formed by the union of the anterior rami of cervical 5, 6, 7, 8 and thoracic 1 nerves. These nerves unite and divide to form the key nerves innervating the upper limb. Variations in the course of these nerves are clinically important to anesthetists, neurologists and orthopedicians. We report bilateral variations in the arterial and neural structures in the upper limb of a 65 year old cadaver. The muscles of the arm on one side were innervated by the median nerve with absence ofmusculocutaneous. While on the other side the musculocutaneous nerve contributed to the formation of the median nerve. There was a presence of high bifurcation of brachial artery on both sides. Knowledge of such variations in the innervations of muscles and the arterial supply of the limbs are important to remember before performing any reconstructive procedures or interventions on the limb. and V. Cíl: Výměnný obrázkový komunikační systém (VOKS) je na podmínky České republiky (ČR) modifi kovaná metoda vycházející z komunikačního systému PECS (The picture exchange communication system), který byl vytvořen v Chicagu v USA. Výzkum měl za cíl zmapovat spokojenost uživatelů s touto metodou, zhodnotit obtížnost osvojení metody VOKS a zmapovat dodržování doporučených postupů. Uživatelem autoři rozumí, pracovníka, který využívá metodu VOKS při komunikaci s klienty s PAS. Ve výzkumné části jsou prezentovány výsledky šetření, které se uskutečnilo v pěti vybraných speciálních zařízení a čtyřech školách na území celé ČR. Metody: Výzkumné šetření proběhlo v období od září 2010 do prosince 2010. Byl vytvořen dotazník vlastní konstrukce, který obsahoval 24 otázek. Kritériem pro zařazení respondentů do výzkumu, byla práce s metodou VOKS v zaměstnání, které respondenti aktuálně vykonávali. Výzkumný soubor tvořil 54 respondentů. Výsledky: Analýzou výsledků bylo zjištěno, že většina respondentů je spokojena s metodou VOKS. Rozdíly jsou patrné v době osvojení metody VOKS v praxi. Na tomto konkrétním vzorku respondentů se nepodařilo určit obecně platný čas potřebný k osvojení metody VOKS. Jako problematické se jeví i přesné dodržování postupů. Hlavními důvody nedostatečného dodržování postupů v praxi je potřeba individuálního přizpůsobení klientovi, nedostatek personálu a nedostatek času na nácvik. Velkým problémem je aplikace metody v době dovolených proškoleného personálu. Závěr: Výzkum naznačil, že metoda VOKS se prozatím, z pohledu individuálního hodnocení respondentů, jeví jako vhodná pro komunikaci s klienty s různorodými poruchami řeči. Metoda VOKS může přispět pracovníkům ve speciálních zařízeních ke zkvalitňování ošetřovatelské péče a k lepšímu uspokojování potřeb klienta., Aims: Czech picture exchange communication system called VOKS is a modifi ed method of communication system PECS (Picture Exchange Communication System) which was created in Chicago in the USA. The research aim was to assess user satisfaction with this method, to evaluate diffi culties with acquiring the VOKS methods and to map compliance with recommended practice. A “user” is understood as somebody who uses VOKS method to communicate with clients suff ering from ASD. The research section presents results of the survey which took place in selected fi ve specialised facilities and four schools all over the Czech Republic. Methods: The survey covered the period from September 2010 to December 2010. We designed our own questionnaire which contained 24 questions. The criterion for inclusion of respondents in the research was current professional working experience with the VOKS method. The research sample consisted of 54 respondents. Results: The results of the analysis revealed that most respondents are satisfi ed with the VOKS methods. Differences are evident in the time when they adopted VOKS methods in practice. With this particular sample of respondents the time necessary to master the VOKS method could not be established. Accurate compliance with recommended procedures proved to be problematic. The main reason for poor compliance with the procedures in practice is the necessity to adapt it for an individual client, staff shortages and insuffi cient time for training. A major problem is the application of the method when the trained staff is on holiday. Conclusion: The research indicates that for the time being the respondents consider the VOKS method to be a suitable method for communicating with clients with a variety of speech disorders. Unilateral otosclerosis combined with avascular necrosis of stapes crura is a rare entity. It should be considered in a case of high grade otosclerosis. Symptoms are the same as in patients who suffer from common otosclerosis. Patients complain on progressive hearing loss and tinnitus. The diagnosis is made clinically by conventional audiologic evaluation and radiologically by x-ray mastoid Schuller's view and CT scan. HRCT scan makes visible all parts of ossicular chain and gives surgeon some information about ossicular chain damage. Významný úspech liečby s inhibítormi tyrozínkinázy (TKI) u chorých s Philadelphia chromozóm (Ph) pozitívnou chronickou myelocytovou leukémiou (CML) dramaticky zmenil prognózu tohto ochorenia. Aj napriek tomu, že prvolíniová liečba CML použitím imatinibu znamená významný benefit pre väčšinu chorých, ukazuje sa, že 25–35 % pacientov potrebuje v priebehu 5 rokov zmenu liečby z dôvodu rezistencie alebo zlyhania liečby. Najčastejším mechanizmom zodpovedným za rezistenciu proti imatinibu u pacientov s CML je rozvoj mutácií v BCR-ABL kinázovej doméne. Tieto mutácie vyvolávajú rôznu mieru rezistencie proti imatinibu a zatiaľ čo niektoré z nich môžu reagovať na zvýšené dávky imatinibu, iné sú rezistentné proti nilotinibu a ďalšie vykazujú zvýšenú mieru rezistencie proti dasatinibu. Práca je prehľad liečebných možností v primárnej liečbe chronickej fázy CML a pri jej zlyhaní. Ukazuje sa, že pokroky v molekulových metódach umožňujú lepšie porozumieť podstatu choroby, zvážiť benefit a riziko terapie, individualizovať terapeutický prístup a prispôsobiť liečbu CML včas tak, aby sa minimalizovalo riziko progresie do akcelerovaných fáz., The phenomenal success of therapy with tyrosine kinase inhibitors (TKI) in Philadelphia chromosome (Ph) – positive chronic myeloid leukemia (CML) has dramatically changed the prognosis of this disease. Despite the fact that first-line therapy of CML using imatinib represents revolutionized the treatment of this disease, it became clear that during 5 years 25–35 % of the patients require change in the therapy due to the development of imatinib resistance or failure. The most frequent mechanism responsible for imatinib resistance is development of mutation in BCR-ABL kinase domain. Mutations cause different level of imatinib resistance and while some of them can be overcome by increased dose of imatinib, others seem to be resistant to nilotinib and others are more resistant to dasatinib. This review is focused on primary treatment and treatment after imatinib failure. Klasifikácia SZO (2008) je jediná celosvetovo akceptovaná a v praxi používaná klasifikácia nádorových ochorení kostnej drene (KD), ktorá definuje aj postavenie a význam biopsie KD v diagnostickom algoritme. Myeloproliferatívne neoplázie (MPN) tvoria pestrú skupinu ochorení s variabilne dynamickým rozvojom z počiatočných štádií, cez štádiá s plno rozvinutou klinicko-laboratórnou manifestáciou až do pokročilých štádií – fibrotickej, resp. blastovej transformácie, sprevádzaných zmenami morfológie a laboratórneho obrazu. V počiatočných štádiách je určenie typu MPN len na základe klinicko-laboratórnych údajov pre podobnosť ich klinickej manifestácie nespoľahlivé. Dôkaz pre MPN typických genetických mutácií potvrdzuje klonalitu ochorenia, ale jeho typizačný význam je limitovaný. Naopak, bioptické vyšetrenie KD v kontexte dostatočných klinických informácií, výsledkov laboratórnych vyšetrení a s podporou genetických vyšetrení umožňuje okrem potvrdenia MPN aj bližšiu typizáciu, ktorá je prognosticky relevantná a ovplyvňuje aj voľbu terapie. Zároveň umožňuje odlíšenie reaktívnych zmien hemopoézy, zhodnotenie prognosticky významných morfologických znakov (stupeň myelofibrózy, množstvo blastov) a sledovanie progresie ochorenia do fibrotickej, resp. blastovej transformácie. V týchto pokročilých štádiách a po terapii nie je však typizácia MPN len na základe morfológie KD možná. Výpovedná hodnota biopsie závisí od kvality a kvantity vzorky drene, hodnotenie morfológie KD závisí aj od vedomostí a skúseností vyšetrujúceho patológa., WHO classification (2008) is the only world-wide accepted and in routine praxis used classification of bone marrow (BM) neoplasms defining also the importance of BM biopsy within the diagnostic algorithm. Myeloproliferative neoplasias (MPN) represent a heterogenous group of disorders with variable dynamic transformational potential from initial into advanced stages such as blast crisis or myelofibrotic transformation. Because of similar manifestations, the typing of MPN in their initial stages based on clinical and laboratory parameters is not optimal. The finding of MPN-typical genetic mutations proves the disease clonality, but not a diagnosis of individual MPN types. In contrast, the BM biopsy verifies both the diagnosis and MPN type. It helps also to discriminate MPN from reactive hematopoietic changes, to evaluate prognostically relevant BM features (e.g. grade of myelofibrosis, blasts number) and the follow-up to terminal stages. However, the MPN typing in terminal and posttherapeutical stages based on morphology alone is not sufficient. The diagnostic value of BM biopsy depends on specimen´s quality, the objectivity of BM morphology evaluation depends on pathologist´s training and experiences. Bisphosphonates (BP) are potent inhibitors of bone resorption used mainly in the treatment of metastatic bone disease and osteoporosis. By inhibiting bone resorption, they prevent complications as pathological fracture, pain, tumor-induced hypercalcemia. Even though patient’s benefit of BP therapy is huge, various side effects may develop. Bisphosphonate-related osteonecrosis of the jaws (BRONJ) is among the most serious ones. Oncologic patients receiving high doses of BP intravenously are at high risk of BRONJ development. BPs impair bone turnover leading to compromised bone healing which may result in the exposure of necrotic bone in the oral cavity frequently following tooth extraction or trauma of the oral mucosa. Frank bone exposure may be complicated by secondary infection leading to osteomyelitis development with various symptoms and radiological findings. In the management of BRONJ, conservative therapy aiming to reduce the symptoms plays the main role. In patients with extensive bone involvement resective surgery may lead to complete recovery, provided that the procedure is correctly indicated. Since the treatment of BRONJ is difficult, prevention is the main goal. Therefore in high risk patients dental preventive measures should be taken prior to bisphosphonate administration. Brachyterapie je metoda léčby zářením, při které se radioaktivní zdroje zavádějí přímo do oblasti nádoru. Umožňuje aplikaci vysoké dávky záření v krátkém čase a dosažení vysokého stupně lokální kontroly nádoru. Největší klinická zkušenost byla získána s brachyterapií s kontinuálním nízkým dávkovým příkonem (0,4–2,0 Gy/hod), kdy se zářiče zaváděly manuálně. V současně době byla tato metoda nahrazena na většině pracovišť automatickými afterloadingovými přístroji využívajícími vysoký dávkový příkon (> 12 Gy/hod) a frakcionaci. U časných ORL nádorů lze užít brachyterapii jako samostatnou léčebnou metodu. Její výhodou v porovnání s chirurgickou léčbou je většinou lepší kosmetický a funkční efekt, v porovnání se zevním ozařováním lepší kontrola nádoru a eliminace postradiační xerostomie. U rozsáhlejších nádorů je brachyterapie indikována k navýšení dávky po zevní radioterapii. Lze ji použít i pro léčbu recidiv v ozářeném terénu. Klasickými indikacemi jsou nádory dutiny ústní, orofaryngu a nasofaryngu. Přes současný technologický pokrok metod zevní radioterapie zůstává brachyterapie důležitou léčebnou modalitou u nádorů ORL., Brachytherapy is a method of radiation treatment, when the radioactive sources are directly applicated into the tumor region. Brachytherapy allows to deliver high doses to the tumor in a short time with excellent local tumor control. The largest experience was achieved with continual low dose rate brachytherapy (0.4–2Gy/hr) and manual insertion of sources. Recently this method was mostly replaced by automatic afterloading devices and high dose rate (> 12Gy/hr) fractionated treatment. Brachytherapy can be used in early stages of head and neck cancer as a sole treatment method. The advantage in comparison with surgery is a better functional and cosmetic outcome and in comparison with external beam radiotherapy a better tumor control and absence of xerostomia. In locally advanced tumors brachytherapy can be used for dose escalation of external beam radiotherapy. Brachytherapy is also useful for salvage treatment of recurrences after previous radiotherapy. Classical indications are oral cancer, oropharyngeal and nasopharyngeal tumors. Nejzhoubnější gynekologický nádor, karcinom ovaria, je příčinou více než 50 % úmrtí v této skupině nádorů. Více než 60 % případů je diagnostikováno až v pokročilých stadiích s výrazně sníženou pravděpodobností přežití pacientek. Diagnostické nástroje pro efektivní časnou detekci nebo screening zatím neexistují. Terapeutické možnosti (chirurgická operace, chemoterapie) jsou nedostatečně efektivní, neboť karcinom ovaria se vyznačuje silnou tendencí k recidivě spojenou s chemorezistencí. Jedním z nejdéle a nejrozsáhleji používaným markerem pro diagnostiku primárních nádorů i recidivy je CA125/MUC16. Tento glykoprotein je znám již od 80. let 20. století, ale teprve nejnovější studie odhalují jeho biologické funkce v karcinogenezi a interakcích s buňkami imunitního systému. V tomto článku se blíže podíváme na jeho biologický význam a především na současný stav použití CA125 jako významného diagnostického markeru u karcinomu ovaria. V diagnostice primárních nádorů se totiž začínají objevovat nové markery, navíc je význam monitoringu CA125 z hlediska detekce recidivy zpochybňován rozsáhlými klinickými studiemi, které nenalezly přínos pro dlouhodobé přežití pacientek. Také potenciální význam CA125 v imunoterapii karcinomu ovaria se nezdá být tak velký, jak se předpokládalo., The most fatal gynecologic malignancy, ovarian cancer, causes more than 50% deaths in this tumor group. Most of cases (>60%) are diagnosed in advanced stages with poor 5-year survival prognosis. Diagnostic tools for an effective early detection or screening have not been found yet. Treatment possibilities (surgery, chemotherapy) are insufficient while high tendency to recurrence and chemoresistance occurs. CA125/MUC16 has been one of the most extensively used markers for a detection of primary tumors, or recurrence for a long time since its discovery in 80´s. However, the structure and biological functions of CA125 have been discovered relatively recently. CA125 may play an important role in carcinogenesis and interactions with cells of immune system. We reviewed the known biological functions and particularly the current state of using this marker as the diagnostic tool in ovarian cancer. Recently, many new markers emerged ambitiously to replace CA125; moreover, the importance of monitoring CA125 for the recurrence detection has also been questioned in large clinical trials. These studies have not found an impact for overall survival/mortality of patients.
