Melanocytární léze vykazují značnou morfologickou heterogenitu. Jejich diagnostika je náročná i přes současné využití moderních histopatologických a imunohistochemických metod. Významným přínosem v diagnostice melanocytárních lézí je zavedení molekulárně genetické metody fluorescenční in situ hybridizace s použitím čtyřbarevné sondy značící geny CCND1, RREB1, MYB a centromeru chromozomu 6. V letech 2008–2012 bylo v naší laboratoři úspěšně metodou fluorescenční in situ hybridizace vyšetřeno 160 vzorků různých typů melanocytárních lézí, 44 névů a 116 melanomů. Změny asociované s diagnózou melanomu byly prokázány u 116 vzorků (72,5 %), normální nález u 41 vzorků (25,6 %), ve dvou případech bylo rozhodnutí na hranici hodnotitelnosti metody (1,2 %) a v jednom případě se jednalo o vyšetření recidivy melanomu v jizvě, který byl svým nálezem atypický (monozomie chromozomu 6) (0,6 %). Na základě dosažených výsledků byla stanovena senzitivita metody 96,6 % a specifita metody 91%. Tyto výsledky ukazují, že fluorescenční in situ hybridizace je vhodnou metodou pro zajištění přesné a rychlé diagnostiky nejednoznačných melanocytárních lézí. Právě rychlost a správnost diagnózy je u vysoce maligních melanomů zásadní a má neoddiskutovatelný vliv na prognózu a úspěšnost léčby pacienta., Melanocytic lesions exhibit significant morphological heterogenity, making their diagnosis difficult, although both sophisticated histopathological and immunohistochemical methods are used. The introduction of molecular genetic method flourescence in situ hybridization (FISH) has contributed significantly to the diagnosing of melanocytic lesions. Between 2008 and 2012, we studied a total of 160 samples of different types of melanocytic lesions, including 44 nevi and 116 melanomas, using a four-color probe mix for CCND1, RREB1 and MYB genes and for the centromere of chromosome 6. A positive result was found in 116 melenoma samples (72.5 %) and a negative result in 41 samples (25.6 %). A borderline value was found in two samples (1.2 %), and an atypical finding (monosomy of chromosome 6) was seen in a melanoma recurrence in one case (0.6 %). Sensitivity and specificity were 96.6 % and 91 %, respectively. These results show that FISH is a suitable and relatively low-cost method for accurate, rapid diagnosis of ambiguous melanocytic lesions. Cíl: Cílem je zjistit, který hodnotící nástroj je v odborné literatuře doporučován jako nejvhodnější pro posouzení komplikací v dutině ústní dětí, ve věku 2-18 let léčených chemoterapií a/nebo radioterapií. Metodika: Analýza příspěvků vyhledávaných v licencovaných a volně přístupných databázích (Medline, Scopus, Proquest Public Health, Ebsco, Cochrane Collaboration) za období 1985-2012. Výsledky: K posouzení komplikací v dutině ústní dětí léčených chamoterapií bylo identifikováno šest vhodných hodnotících nástrojů - Oral Mucositis Daily Questionnaire, Oral Mucositis Assessment Scale, Western Consortium Cancer Nursing Research Staging System for Stomatitis, Oral Assessment Guide, Children's International Mucositis Evaluation Scale a Oral Mucositis Rating Scale. Většina z nich, je modifikací hodnotících nástrojů určených dospělé populaci. K nejnovějším nástrojům náleží Children's International Mucositis Evaluation Scale (ChIMES), která je od počátku vyvíjena pro dětské pacienty a je v současnosti ověřována v klinické praxi. Škála zahrnuje subjektivní hodnocení bolesti v ústech, potíže s polykáním, omezení při jídle, omezení při pití a objektivní hodnocení léze. Zahrnuto je dále užívání léků proti bolesti v ústech nebo krku, v hodnoceném dni. Nejčastěji užívaná byla ve studiích hodnocena Oral Assessment Guide (OAG), která byla všemi autory doporučována k užití. Závěr: OAG je vhodný hodnotící nástroj pro dětskou populaci. Jejím pozitivem jsou kromě možnosti stanovení stadia komplikací v dutině ústní také doporučené intervence pro jednotlivá stadia poškození dutiny ústní. Posouzení je vhodné k použití kvalifikovanou sestrou., Aim: The aim is to find out which assessment tool is recommended in the scientific literature as the most suitable for an assessment of complications in the oral cavity in children, from the age of 2 to 18, who undergo chemotherapy and/or radiotherapy. Methods: Analysis of papers searched in licenced and freely available databases (Medline, Scopus, Proquest Public Health, Ebsco, Cochrane Collaboration) in the period of 1985-2012. Results: For an assessment of complications in the oral cavity in children undergoing chemotherapy, six suitable assessment tools were identified - Oral Mucositis Daily Questionnaire, Oral Mucositis Assessment Scale, Western Consortium Cancer Nursing Research Staging System for Stomatitis, Oral Assessment Guide, Children's International Mucositis Evaluation Scale a Oral Mucositis Rating Scale. Most of them are a modification of assessment tools for an adult population. One of the newest assessment tool is Children's International Mucositis Evaluation Scale (ChIMES), which has been developed for paediatric patients from the very beginning. At present the scale is being verified in clinical practice. The scale includes a subjective assessment of pain in the mouth, problems with swallowing, limitations at eating and drinking, and an objective lesion assessment. It also includes taking analgesics to releive the pain in the mouth and the throat on the day when an assessment is made. Oral Assessment Guide (OAG) is the most frequently used assessment tool in the studies and is recommended by all authors for the use. Conclusion: OAG is a suitable assessment tool for a children´s population. Its positive features are a possibility to determine a stage o f complications in the oral cavity as well as recommended interventions for individual stages of the damage to the oral cavity. Cíl: Součástí diagnostiky febrilní infekce močových cest (IMC) je ultrazvukové vyšetření a mikční cystouretrografie (MCU), které slouží k odhalení případných komplikací infekce a anomálií močových cest. Tak, jak se vyvíjí porozumění vzájemným vztahů mezi infekcí močových cest, jizvením a přítomností vezikoureterálního refluxu (VUR), mění se i doporučené vyšetřovací algoritmy. Materiál a metoda: Do studie bylo zahrnuto 184 pacientů, kteří podstoupili mikční cystouretrografii po první či opakované atace febrilní infekce močových cest. Byl sledován výskyt vezikoureterálního refluxu u obou skupin pacientů v korelaci s ultrazvukovým nálezem. Pacienti s vezikoureterálním refiuxem byli rozděleni do skupiny s nediktujícím (VUR I.-II. stupně) a dilatujícím (VUR III.-V. stupně) refiuxem. Výsledky: Celkově byl VUR zaznamenán u 39/184 (21 %) pacientů. Ve skupině pacientů po první atace febrilní IMC byl VUR zachycen u 23/156 (15 %) pacientů, ve srovnání s 16/28 (57 %) pacienty po opakovaných febrilních IMC (statisticky významný rozdíl, p < 0,0001). U 8/156 (5 %) pacientů po první atace febrilní IMC byl zjištěn dilatující VUR ve srovnání s 11/28 (39 %) pacienty s opakovanými febrilními infekcemi močových cest (statisticky významný rozdíl, p < 0, 0001). Závěr: V souladu s aktuálním doporučením American Academy of Paediatrics poukázaly výsledky naší studie na nízký výskyt VUR včetně dilatujícího VUR u pacientů po první atace febrilní IMC. Na základě našich zjištění nedoporučujeme provádět MCU plošně u všech pacientů ve věku 2-24 měsíců po první atace IMC., Aim: Ultrasound investigation and voiding cystouretrography (VCUG) are part of the diagnostic algorithm in children with febrile urinary tract infection (UTI) with the aim of depicting any complication of the infection and underlying anomalies of the urinary tract. As the understanding of the relationship between UTI, renal scarring and vesicoureteral reflux (VUR) evolves, new recommended imaging algorithms are being introduced. Methods: 184 patients that underwent VCUG after the first or recurrent febrile UTI were included in the study. The incidence of VUR in both groups have been evaluated and compared with ultrasound findings. Patients with VUR were divided into those with non-dilating (VUR grade I-II) and dilating (VUR grade III-V) reflux. Results: Overall VUR was diagnosed in 39/184 (21%) patients. VUR was seen in 23/156 (15%) patients in the group of patients after the first fibrile UTI, in comparison to 16/28 (57%) patients with recurrent fibrile UTI (statistically significant difference, p < 0,0001). 8/156 (5%) patients after the first febrile UTI had dilating VUR, compared to 11/28 (39%) patients after recurrent febrile UTI (statistically significant difference, p< 0,0001). Conclusion. In compliance with current recommendations of the American Academy of Paediatrics the results of this study have shown low incidence of VUR and dilating VUR in patients with a history of single febrile UTI.
