The target of research was the implementation of the comparative analysis of conservative therapy of preparation of basic therapy and also new methods of treatment (anticytokine therapy). Appointment of salofalk in twos tablets (0, 5) 34 times a day per os, and also 2,0 gr in the form of a rectal enema unitary for a period of seven days, then had transferd the patients to reception 2,0 gr salofalk per os, allowed to achieve substantial improvement patients cliniclaboratory and endoscopic symptoms with a heavy intensification of ulcer colitis variation, providing full cliniclabaratory remissions achievements of disease at 8 (34 %) patients. In the Minister of the Uzbek Public Health of the Republican Science Coloproctological Center at 10 patients with the IDI have spent the therapy remicade, 7 of them had NUC and 3 patients had the Crohn's disease. Therefore, all patients no longer than on the first week after introduction of remicade had improvement of a course disease which was expressed in defering stool, extinction or reduction of pathological impurity in excrement and subsidence bellyaches. During colonoscopical inspection, in 1218 weeks after the first infusion at 80% of the patients had observed disappearance of ulcers and erosion. In terms of 6 months of supervision of relapse of disease was not noted. All new directions in treatment of the IDI, apparently, are perspective, but while are proved only theoretically and had no practical development. Desmoplastický kulatobuněčný tumor (DSRCT) je raritní onemocnění patřící do skupiny sarkomů. V typickém případě se manifestuje rozsáhlou tumorózní infiltrací břišní dutiny bez evidentního primárního ložiska tumoru. Prognóza onemocnění je stále špatná, 5leté přežití je popisováno asi v 15 % případů i při užití kombinovaných protinádorových léčebných postupů. Uvádíme kazuistiku 34letého pacienta s DSRCT léčeného v našem centru. Zmiňujeme racionální postup v rámci diferenciální diagnostiky měkkotkáňové infiltrace v dutině břišní, podtrhujeme zásadní význam molekulárně genetické analýzy a zaměřujeme se na terapeutické možnosti tohoto zřídka se vyskytujícího onemocnění., Desmoplastic small round cell tumor (DSRCT) is a rare disease from sarcoma group family. Typically, there is an extensive infiltration of abdominal cavity, peritoneal effusion with no evident primary origin of tumor. Prognosis is unfavourable with 5-years overall survival described in 15 % of cases even though multimodal treatment strategy is performed. We introduce case report of 34-years old patient with DSRCT treated in our Oncology center. Reasonable schema of differential diagnosis of soft-tissue expansion in abdominal cavity is mentioned and we emphasize crucial role of molecular genetic analysis. Colonic polyps and polyposis are studied in 183 patients. It has been found, that sensitivity of rectoscopy is 66.7%, irrigoscopy 61.1%, virtual colonoscopy computeraided tomography of colon (VCCATC) 79.1%. Endoscopic polypectomy was the main treatment method used in patents with colonic polyps and polyposis (18.6%). Histiocytóza z Langerhansových buněk je vzácné onemocnění, vznikající proliferací Langerhansových dendritických buněk, jehož etiologie je nejasná. Vyskytuje se převážně v dětském věku. Vysoké procento pacientů má projevy nemoci v oblasti hlavy a krku. Nejčastěji postiženým orgánem je lebka, zejména temporální kost. V organizmu se může nacházet jedno nebo více oseálních i extraoseálních ložisek, nebo je postižen jeden nebo více orgánů. Klinické projevy imitují řadu jiných chorob, proto je diagnostika často obtížná a protrahovaná (1). Otorinolaryngolog se nejčastěji setkává se symptomy podobnými chronické externí otitidě, akutní mediotitidě, mastoiditidě, dermatitidě nebo onemocnění probíhá pod obrazem nádoru. V této práci přinášíme kazuistiku dospělého pacienta, který vyhledal ošetření pro otoreu nereagující na standardní léčbu., Langerhans´cells histiocytosis (LCH) is a rare disease, in which the proliferation of Langerhans´ cells is believed to be causative, affecting mainly children. The etiology still remains unclear. The head and neck region is frequently involved, the skull being the most often diseased site, especially the temporal bone. It may affect also another organs and systems with single or multiple osseal or extraosseal lesions. LCH can be challenging to diagnose with it´s varied presentations, which may resemble another common diseases. Výsledky na bázi dat Národního onkologického registru ČR z období 1976–2005 uvádí: a) Z 125 262 primárních novotvarů (52,1 % mužů, 47,9 % žen) bylo evidováno v časných klinických stadiích 37,1 % nádorů u mužů a 46,6 % u žen, v pokročilých stadiích 9,3 % u mužů a 9,5 % u žen. Neznámá stadia převažovala nad ostatními stadii až do roku 1994. Za celé období převažovali muži ve stadiu IV nad ženami. Nejvíce časných stadií se vyskytlo v Praze, pokročilých stadií v kraji Plzeňském. Po vyloučení neznámých stadií bylo z 54 187 duplicit u mužů 63,8 % v časných stadiích, u žen 71,3 %, v pokročilých stadiích u mužů 18,4 % a 15,8 % u žen. Z ostatních 10 292 multiplicit bylo 16,2 % časných stadií u mužů a 11,7 % u žen, v pokročilých stadiích 1,6 % u mužů a 1,2 % u žen. b) Z 165 050 následných novotvarů (54,4 % mužů, 45,6 % žen) – u případů po primárních stadiích I a II. bylo evidováno 21,7 % časných klinických stadií u mužů a 27 % u žen, 5 % pokročilých stadií u mužů a 6 % u žen – u případů po primárních stadiích III a IV bylo evidováno 2,5 % časných stadií u mužů a 3,2 % u žen, 1,8 % pokročilých stadií u mužů a 2,2 % u žen. Pozoruhodných bylo u mužů 12 418 následných nádorů v pokročilých stadiích (13,8 % ze všech 89 796 následných), u žen 10 189 (13,5 % ze všech 75 254 následných) po primárních nádorech stadia III a IV. Mezi následnými pokročilými stadii převažovalo – u mužů 36,9 % nádorů trávicích a 33,1 % dýchacích cest, 10,3 % pohlavních orgánů, 8,1 % močových cest – u žen 37 % nádorů trávicích cest, 21 % prsů, 17,2 % rodidel, 12,8 % dýchacích a 4,5 % močových cest. Je varující, že z vícečetných onemocnění byl každý sedmý nádor gastrointestina u mužů a pátý u žen po primárním novotvaru zjištěn v některém pokročilém klinickém stadiu. c) Ze 41 161 přežívajících k 17. 10. 2007 bylo 54,7 % časných stadií u mužů a 58 % u žen, 5 % pokročilých stadiích u mužů a 5,2 % u žen, neznámých stadií 40,3 % u mužů a 36,9 % u žen. Z 84 101 zemřelých bylo 30 % časných stadií u mužů a 39,8 % u žen, 11 % pokročilých stadií u mužů a 12,1 % u žen, neznámých stadií 59 % u mužů a 48,1 % u žen. Z výsledků lze doporučit, aby přehledy vícečetných diagnóz a jejich stadia se staly součástí ročních statistických přehledů novotvarů u české populace., The results based in the Czech Cancer Registry in 1976–2005: a) Of the 125,262 primary neoplasms (52.1 % males, 47.9 % females) there were registered 37.1 % of early clinical stages in males and 46.6 % in females, 9.3 % of advanced stages in males and 9.5 % in females. Unknown stages prevailed over the other stages until 1994. The cancers of stage IV prevailed in males during the total period. The most of early stages were in Prague, of advanced stages in Pilsen region. After exclusion of unknown stages, there were of 54,187 duplicities 63.8 % cases of early stages in males and 71.3 % in females; 18.4 % of advanced stages in males and 15.8 % in women. Of other 10,292 multiplicities there were 16.2 % of early stages in males and 11.7 % in females, 1.6 % of advanced stages in males and 1.2 % in females. b) Of the 165,050 subsequent neoplasms (54.4 % males, 45.6 % females) – in cases following the primary stages I and II there were registered 21.7 % of early stages in males and 27 % in females, 5 % of advanced stages in males and 6 % in females, – in cases following the primary stages III and IV there were registered 2.5 % of early stages in males and 3.2 % in females, 1.8 % of the advanced stages in males and 2.2 % in females. There were uncommon the number of 12,418 subsequent tumors of advanced stages in men (13.8 % of total 89,796 subsequent ones) and 10,189 in women (13.5 % of total 75,254 subsequent ones), following the primary cancers of stages III, IV. Between these subsequent and advanced stages predominated – in males 36.9 % cancers of gastrointestinal and 33.1 % respiratory tract, 10.3 % genital organs, 8.1 % urinary tract, – in females 37 % cancers of gastrointestinum, 21 % breast, 17.2 % genital organs, 12.8 % respiratory and 4.5 % urinary tract. It is worrying that of total multiple cases every the seventh gastrointestinal cancer in males and the fifth in females were detected at any advanced clinical stages after primary cancer. c) Of 41,161 survived up to October 2007 there were 54.7 % of early stages in males and 58 % in females, 5 % of advanced stages in males and 5.2 % in females, unknown stages 40.3 % in males and 36.9 % in females. Of 84,101 deaths there were 30 % of early stages in males and 39.8 % in females, 11 % of advanced stages in males and 12.1 % in females, unknown stages 59 % in males and 48.1 % in females. Článek seznamuje se současnými postupy pro ovlivnění bolestivých kostních metastáz u karcinomu prsu. Autoři se v rámci této problematiky zaměřují především na léčebné využití ionizujícího záření. To je aplikováno většinou technikou zevní radioterapie nebo systémovou intravenózní aplikací osteotropních terapeutických radiofarmak, která se váží do lemu zvýšené osteoblastické aktivity okolo metastáz. One possibility of reducing or relieving bone metastases pain is to administer of the radiotherapy at the site of metastases. The current medicine uses two ways of radiotherapy administration at the site of metastases: external-beam irradiation or intravenous administration of boneseeking therapeutic radiopharmaceuticals. Léčba chronické myeloidní leukemie (CML) se během posledního desetiletí zásadně změnila. Imatinib se stal standardním lékem první volby pro nemocné v chronické fázi CML. Dříve preferovaná alogenní transplantace krvetvorných buněk se odkládá do dalších linií léčby, přestože je to zatím jediná potenciálně kurativní metoda pro CML. Inhibitory tyrozinkináz 2. generace dasatinib a nilotinib, rozvoj molekulárního monitorování reziduální leukemie a identifikace řady mutací BCR-ABLl s různou senzitivitou k inhibitorům tyrozinkináz umožňují léčbu „šít na míru“. V článku je uveden přehled současných doporučení pro léčbu CML., In the past decade the management strategies of patients with chronic myeloid leukemia have been completely changed. Imatinib became the standard front-line therapy in patients with newly diagnosed chronic-phase CML. Allogeneic stem cell transplantation in CML patients is increasingly deferred despite its curative potential. Second generation tyrosine kinase inhibitors dasatinib and nilotinib together with the improved molecular monitoring of residual leukemia and the identification of BCR-ABL1 mutations with wide spectrum of sensitivity to tyrosine kinase inhibitors make possible the tailoring of CML treatment. Lymfomy jsou čtvrtým nejčastějším nádorovým onemocněním diagnostikovaným během těhotenství s incidencí 1:6 000 těhotenství. Počet případů přibývá s pokračujícím trendem plánování rodičovství v pozdějším věku matky. Chemoterapie a radioterapie podávaná během prvního trimestru těhotenství je spojena s vyšším rizikem vzniku kongenitálních malformací. Ve většině případů je proto doporučováno umělé přerušení těhotenství a léčbu zahájit podle standardních protokolů. U nízce rizikových pacientek je možné s léčbou vyčkat až do ukončení embryogeneze v prvním trimestru a pacientky po tuto dobu pouze observovat. Ve druhém a třetím trimestru je možné léčit pacientky s lymfomem kombinovanou chemoterapií bez zvýšeného rizika pro plod., Lymphoma is the fourth most frequent malignancy diagnosed during pregnancy, occuring in approximately 1:6 000 of deliveries. Its occurence may increase due to the current trend to postpone pregnancy until later in life. Chemotherapy and radiotherapy during the first trimester are associated with increased risk of congenital malformations. In the vast majority of cases, when lymphoma is diagnosed during the first trimester, treatment with a standard chemotherapy regimen, following pregnancy termination should be recommended. In the rare patients at low risk, therapy can be delayed until the end of the first trimester and of embryogenesis while keeping the patients under close observation. U stadia IB–IIA dochází k recidivě cervikálního karcinomu v 10 až 20 %, u stadia IIB–IVA již v 50 až 70 %. Po vyčerpání možností kurativní léčby chirurgické nebo radiační je prognóza špatná. Pouze 15 až 20 % pacientek s recidivou nemoci dosahuje přežití v délce jednoho roku. Hlavním cílem léčby je paliace. Recidivy nemoci, které nejsou vhodné k lokálnímu chirurgickému výkonu nebo ozáření, je možné léčit paliativní chemoterapií. Nízká léčebná odpověď a krátké celkové přežití pacientek jsou zklamáním. Stále zůstává potřeba nových léků a dalšího výzkumu zaměřeného na terapii cervikálního karcinomu. Do budoucích studií je nezbytné zapracovat biologickou léčbu v kombinaci s chemoterapii., The recurrence rate of cervical cancer is between 10–20 % for FIGO stages IB–IIA and 50–70 % for FIGO stages IIB–IVA. When cervical cancer is beyond curative treatment by surgery or radiotherapy, the prognosis is poor. The patiens with recurrent cervical cancer have 1 year – survival rate between 15–20 %. Palliation therapy is the primary objective. Recurrent lesions not amenable to radical local exscision or regional radiation are treated with paliative chemotherapy. Low response and short overall survival are disappointing. The need for novel agents and further research focused on the management of recurrent cervical cancer remains.
