- The target of research was the implementation of the comparative analysis of conservative therapy of preparation of basic therapy and also new methods of treatment (anticytokine therapy). Appointment of salofalk in twos tablets (0, 5) 34 times a day per os, and also 2,0 gr in the form of a rectal enema unitary for a period of seven days, then had transferd the patients to reception 2,0 gr salofalk per os, allowed to achieve substantial improvement patients cliniclaboratory and endoscopic symptoms with a heavy intensification of ulcer colitis variation, providing full cliniclabaratory remissions achievements of disease at 8 (34 %) patients. In the Minister of the Uzbek Public Health of the Republican Science Coloproctological Center at 10 patients with the IDI have spent the therapy remicade, 7 of them had NUC and 3 patients had the Crohn's disease. Therefore, all patients no longer than on the first week after introduction of remicade had improvement of a course disease which was expressed in defering stool, extinction or reduction of pathological impurity in excrement and subsidence bellyaches. During colonoscopical inspection, in 1218 weeks after the first infusion at 80% of the patients had observed disappearance of ulcers and erosion. In terms of 6 months of supervision of relapse of disease was not noted. All new directions in treatment of the IDI, apparently, are perspective, but while are proved only theoretically and had no practical development. - Východiska: Implementace mezinárodní klasifikace NANDA-I v podmínkách neonatologické péče v ČR. Cíl: Cílem práce bylo validovat diagnostické prvky ošetřovatelských diagnóz 00104 Neefektivní kojení a 0106 Snaha zlepšit kojení vybraným souborem neonatologických sester – expertek a zjistit, které diagnostické prvky považují za hlavní a které za vedlejší. Metody: Pro obsahovou validaci byl využit Fehringův model validity diagnostického obsahu (Diagnostic Content Validity Model). Soubor tvořilo 26 neonatologických sester – expertek. Za expertky byly považovány sestry pracující na neonatologii, které získaly minimálně 4 body dle modifikovaných Fehringových kritérií. Pro posouzení významnosti diagnostických prvků byl použit měřící nástroj obsahující 43 položek (diagnostických prvků) uvedených v klasifikačním systému NANDA-International. Výsledky: U ošetřovatelské diagnózy 00104 Neefektivní kojení za významné určující znaky (vážené skóre vyšší než 0,80) expertky považovaly: kojenec není schopen správně uchopit prs (0,82). Ze souvisejících faktorů dosáhlo váženého skóre vyšší než 0,80 celkem 3 prvky. U ošetřovatelské diagnózy 0106 Snaha zlepšit kojení váženého skóre vyšší než 0,80 nedosáhl žádný prvek. Závěr: Pro posouzení nespokojenosti nebo potíží matky a dítěte při procesu kojení na oddělení šestinedělí považují expertky za významné 4 prvky z 30, týkající se pouze ošetřovatelské diagnózy 00104 Neefektivní kojení., Background: Implementation of the International Classification of NANDA-I in neonatology care in the Czech Republic. Aim: The aim of the study was to validate the diagnostic elements of nursing diagnoses 00104 Ineffective Breastfeeding and 00106 readiness for inhanced breastfeeding selected file by neonatal nurses-experts and determine which diagnostic elements consider as the main, and which the minor are. Methods: For the content of validation was used Fehring diagnostic content of validity model (Diagnostic Content Validity Model). The sample consisted of 26 neonatal nurses-experts. For expert were considered nurses working in neonatology, which received at least 4 points modified by Fehring standard. To assess the significance of diagnostic elements was used measurement instrument containing 43 items (diagnostic elements) listed in the classification system, NANDA-International. Results: At nursing diagnosis 00104 Ineffective Breastfeeding, the main key features (score > 0, 80) experts considered: the infant isn´t unable to correctly to grasp the breast (0, 82). From the related factors achieve score 0, 80 in all 3 elements. At the nursing diagnosis 00106 Effort to improve breastfeeding weighted score of > 0, 80 reached no character. - The authors demonstrate the possibility of improving surgical results by the reduction of perioperative bleeding in thoracic surgery associated with extended resection procedures. We focused on patients in whom the expected perioperative blood loss was greater than 500 ml. The first group consisted of patients with lung cancer stage III A after neoadjuvant chemotherapy had been indicated to extend the resection procedure. The second group consisted of patients with chest wall and mediastinum tumors of various etiologies. The third group consisted of patients with post-inflammatory thoracic complications in whom combined decortication and pleurectomy was necessary. - Cíl: Cílem studie bylo vyhodnotit vztah mezi hodnotami indexu IgG a počtem oligoklonálních IgG pásů (OCB) v mozkomíšním moku (CSF) pacientů s roztroušenou sklerózou (SM). Materiál a metodika: Soubor 150 pacientů sestával ze 41 mužů a 109 žen (ve věku od 18 do 68 let, průměrný věk byl 36,6 ±10,1 let). U mozkomíšního moku odebraněho lumbální punkcí byla posouzena intratekální syntéza s využitím indexu IgG a stanovením OCB. Počet alkalických OCB v CSF byl stanoven metodou izoelektrické fokusace. K vyhodnocení statistické významnosti výsledků byla rovněž použita Pearsonova korelační analýza, x2 test homogenity, Mannův-Whitneyův test, párový t-test (parametrický) a jednovýběrový Wilcoxonův test (neparametrický). Výsledky: Pozitivní korelace mezi IgG indexem a počtem OCB nebyla nalezena. Ani Mannův-Whitneyův test neprokázal podstatný rozdíl v hodnotách indexu IgG u pacientů s OCB > 2 a < 2. Závěr Studie nepotvrdila korelaci mezi hodnotami IgG indexu a počtem OCB v CSF u pacientů s roztroušenou sklerózou., Objective: The aim of this study was to assess the correlation between IgG index values and the number of the oligoclonal IgG bands (OCB) in the cerebrospinal fluid (CSF) of patients with multiple sclerosis (MS). Material and Methods: The set of 150 patients consisted of 41 males and 109 females (aged 18–68, mean 36.6 ± 10.1 years). The CSF collected by a lumbar puncture was examined evaluating intrathecal synthesis using the IgG index and determining OCB. The number of alkaline OCB in the CSF was assessed using the method of isoelectric focusation. Pearson's correlation analysis, and homogeneity χ2 test , Mann-Whitney test, paired-sample t-test (parametric) and Wilcoxon signed-ranks test (nonparametric) were used to evaluate the statistical significance of the results. Results: No positive correlation between the IgG index and the number of OCB was found. Mann-Whitney test also did not demonstrate any significant difference of the IgG index values in patients both with the OCB number ≥ 2 and < 2. - Tuberkulózní postižení střev je v rozvinutých zemích vzácné onemocnění, které postihuje zejména imigranty a imunokompromitované pacienty. Klinický obraz může imitovat nitrobřišní infekce, nádorová onemocnění či Crohnovu chorobu. Diferenciální diagnostika Crohnovy choroby a břišní tuberkulózy může být pro klinika a patologa svízelná, v obou případech se jedná o chronický granulomatózní zánět s podobnou symptomatologií. Prezentujeme případ 53letého pacienta s perforační peritonitidou při tuberkulóze střeva, u kterého byla původně mylně diagnostikována Crohnova choroba. Průběh choroby, rozsah a povaha nálezu byly v každé fázi léčby příčinou kritických komplikací. S touto zkušeností předáváme i doporučení: břišní tuberkulózu zvažovat v diferenciálně diagnostické rozvaze každého chronického střevního onemocnění. Klíčová slova: střevní tuberkulóza − Crohnova choroba − kritické komplikace, Intestinal tuberculosis is a rare disease in developed countries, affecting mainly immigrants and immunocompromised patients. Clinical presentation may mimic many other abdominal diseases such as intra-abdominal infections, tumors, and Crohn´s disease. Differential diagnosis of Crohn´s disease and intestinal tuberculosis poses a dilemma to clinicians and pathologists as both are chronic granulomatous disorders with similar clinical features. We report the case of a 53 year-old man presenting with perforated intestinal tuberculosis in which the initial diagnostic work-up suggested Crohn´s disease. The severity of the findings resulted in critical complications during the course of treatment. This case points out the need for awareness of intestinal tuberculosis in the differential diagnosis of chronic intestinal disease. - Crohn's disease is a chronic immune-mediated intestinal inflammation targeted against a yet incompletely defined subset of commensal gut microbiota and occurs on the background of a genetic predisposition under the influence of environmental factors. Genome-wide association studies have identified about 70 genetic risk loci associated with Crohn's disease. The greatest risk for Crohn's disease represent polymorphisms affecting the CARD15 gene encoding nucleotide-binding oligomerization domain 2 (NOD2) which is an intracellular sensor for muramyl dipeptide, a peptidoglycan constituent of bacterial cell wall. The accumulated evidence suggests that gut microbiota represent an essential, perhaps a central factor in the induction and maintaining of Crohn's disease where dysregulation of normal co-evolved homeostatic relationships between intestinal microbiota and host mucosal immune system leads to intestinal inflammation. Taken together, these findings identify Crohn's disease as a syndrome of overlapping phenotypes that involves variable influences of genetic and environmental factors. A deeper understanding of different genetic abnormalities underlying Crohn's disease together with the identification of beneficial and harmful components of gut microbiota and their interactions are essential conditions for the categorization of Crohn's disease patients, which enable us to design more effective, preferably causative, individually tailored therapy. and L.
