Tato studie se vrací k nedávné kontroverzi ohledně obrany infanticidy, kterou v tomto časopise publikoval Miroslav Mitlőhner. V prvním oddíle upozorňuji na slabiny Mitlőhnerova článku. Ve druhém a třetím oddíle se obracím k nejpromyšlenější současné obhajobě infanticidy, totiž k argumentu australského bioetika Petera Singera. Druhý oddíl shrnuje Singerův popis současné lékařské praxe, která podle jeho názoru již opustila tradiční etiku rovné hodnoty lidských životů. Již tento vývoj sám o sobě motivuje etický revizionismus. Avšak jakoukoli revizi je třeba explicitně zdůvodnit. To je úkolem Singerova tzv. argumentu nahrazením (Replacement Argument), který je zkoumán ve třetím oddíle studie. Podle mého názoru toto čistě neosobní zdůvodnění infanticidy selhává. Ve čtvrtém oddíle tudíž takové zdůvodnění odmítám ve prospěch Dworkinovy distinkce mezi zkušenostními zájmy, jež mohou mít již novorozenci, a kritickými zájmy, jež se objevují až v pozdějším věku. Z toho dovozuji, že apelem k vlastním zájmům novorozence (např. abychom ulevili jeho utrpení) lze někdy zdůvodnit neonatální eutanazii. Nelze to ale učinit čistě neosobně. Jedinou výjimkou jsou ti novorozenci, kterým chybí schopnost pro jakoukoli kognitivní činnost, a kteří tak postrádají i zkušenostní zájmy. Je otevřenou otázkou, zda se jejich "život" vlastně liší od smrti, a zda bychom jejich usmrcením provedli infanticidu., The paper revisits the recent controversy over Dr. Mitlőhner’s defense of infanticide, published in this journal. In section 1, I point out the weaknesses of Mitlőhner’s paper. In sections 2 and 3 I turn to the most sophisticated defense of infanticide on offer today, that of Peter Singer’s. Section 2 sums up Singer’s description of the medical practice as already having abandoned the traditional ethic of equal value of all human lives, which motivates ethical revisionism. However, an explicit justification of a revision is necessary. This is the job of Singer’s Replacement Argument, examined in section 3. I argue that this justification of infanticide in completely impersonal terms fails. In section 4, I reject it in favor of Ronald Dworkin’s distinction between experiential interests, possessed by infants, and critical interests that develop later. Hence, neonatal euthanasia can sometimes be justified in terms of a newborn‘s own interests (presumably, to relieve its suffering), not in impersonal terms. The only exception is those infants that lack any capacity for cognitive activity whatsoever, and who thus lack even experiential interests. 95. Odvození a charakterizace buněčného modelu multilékové rezistence uroteliálního karcinomu

Hlavní stanovisko práce: Nově odvozená chemorezistentní buněčná linie karcinomu močového měchýře jako experimentální model pro studium mechanizmů mnohočetné terapeutické rezistence pokročilých nádorů močového měchýře. Cíl: Cílem práce bylo odvodit a charakterizovat novou progresivní chemorezistentní buněčnou linii uroteliálního karcinomu. Materiál a metody: Mateřská buněčná linie uroteliálního karcinomu BC44 byla odvozena z papilárně diferencované části pokročilého karcinomu močového měchýře (pT4 G3) a z ní ustanovená dceřiná linie BC44DoxoR byla získána selekcí rezistentních klonů po aplikaci zvyšujících se koncentrací doxorubicinu. Mikroskopická analýza chemorezistentního fenotypu byla fotograficky dokumentována po aplikaci jednotlivých cytostatik (doxorubicin, metotrexát, vinblastin, cisplatina a gemcitabin). Pro stanovení viability buněk v prostředí různých koncentrací cytostatik byl použit test funkce mitochondriálních dehydrogenáz. Ke kvantitativnímu vyhodnocení mitochondriální funkce byla využita metoda absorpční spektrofotometrie a výsledné naměřené hodnoty absorbancí byly zhodnoceny pomocí neparametrického Mann- -Whitney U testu pro hladinu významnosti α = 0,05. Výsledky: Podařilo se nám odvodit z mateřské buněčné linie karcinomu močového měchýře BC44 dceřinou buněčnou linii BC44DoxoR, která vykazuje mnohočetnou lékovou rezistenci vůči všem cytostatikům používaným ve standardní systémové chemoterapii uroteliálního karcinomu (doxorubicin, metotrexát, vinblastin, cisplatina a gemcitabin). Závěr: Nově ustanovená chemorezistentní nádorová buněčná linie BC44DoxoR představuje cenný modelový systém, jehož další molekulární analýza může přinést důležité nové poznatky pro pochopení terapeutické rezistence pokročilých nádorů močového měchýře, Major statement: New chemoresistant urothelial bladder cancer cell line as an experimental model for studying the mechanisms of multidrug resistance in advanced bladder cancer. Aim: The aim of the study was to establish and characterize a new multidrug resistant urothelial cancer cell line. Materials and methods: The parental cancer cell line BC44 was previously established from the large exophytic and better differentiated papillary part of a progressive tumor (pT4 G3) of a female patient. The daughter cell line BC44DoxoR has been derived by prolonged culture in increasing doxorubicin concentrations. Morphological response of parental and daughter cells after application of the individual drugs (doxorubicin, methotrexate, vinblastine, cisplatin, and gemcitabine) has been followed by phase-contrast microscopy. Cell viability has been determined using mitochondrial dehydrogenases assay and quantified by absorption spectrophotometry. Man-Whitney U Test (significance level α = 0.05) has been applied for statistical evaluation. Results: The derivative cell line BC44DoxoR exhibited chemoresistance to all cytostatics tested. To the best of our knowledge, this is the first report of such a multidrug resistant urothelial carcinoma cell line. Introduction: The type of the preparation junction is determined by a number of factors that need to be taken in consideration with CAD/CAM Fixed Prosthodontics: the material of which the construction will be made; the type and condition of the retainer teeth, of their periodontium and the occlusion; the design software, the CAM setting and the type of drills; the working protocol; the type of the cement and the method of cementation. Purpose: The aim of this publication is to describe the optimal preparation junctions for all-ceramic crown and bridge restorations made by CAM 5 - S 2 Impression, VHF. Materials and methods: Suitable are chamfer and shoulder preparation junction with rounded inner angle (width 1 - 1, 5 mm). Trimming of 1.5 – 2 mm dental tissues is necessary on the occlusal surface. The homothetic reduction of teeth is optimal. Results and discussion: The width depends on the size and vitality of the tooth. In stained teeth and those built with metal pins the removal of more tissues provides a greater volume needed to disguise the dark color. Vestibular preparation under the level of the gingiva is preferable to ensure optimal aesthetics. Preparation junction is determined also by the CAD/CAM technology - the type of drills and protocol of impression taking (classical or digital). The creation of a working model with TRIOS, 3Shape intraoral scanner is greatly facilitated by preparation junctions made above the gingival margin. Východiska: Příspěvek předkládá výsledky retrospektivní studie zaměřené na screening otoakustických emisí u novorozenců. Cíl: Hlavním cílem výzkumného šetření bylo zjistit incidenci sluchových vad u fyziologických a rizikových novorozenců ve vybraném období. Metodika: V rámci kvantitativního výzkumu byla použita metoda retrospektivní studie prostřednictvím záznamového archu, které autorům poskytlo novorozenecké oddělení. Pro sledování průběhu těhotenství, porodu, osobní a rodinné anamnézy byl využit informační portál vybraného zdravotnického zařízení. Výběrový soubor zahrnoval novorozence, u nichž bylo ve stanovém období provedeno vyšetření sluchových vad. Výsledná data byla vyhodnocena deskriptivní statistikou a prostřednictvím Pearsonova chí-kvadrátového testu. Výsledky: V roce 2014 bylo ve zdravotnickém zařízení hospitalizováno 1 516 novorozenců, screening otoakustických emisí byl proveden u 1 510 novorozenců. Vyšetřeno bylo 1 258 fyziologických novorozenců, přičemž sluchová vada byla odhalena u 5 (0,4 %) novorozenců. Ze skupiny novorozenců rizikových byla vada diagnostikována u 7 (2,78 %) novorozenců z 252. Byl statisticky potvrzen častější výskyt sluchových vad u této skupiny respondentů. Nejčastějším společným faktorem uváděným v anamnéze matky u novorozence se sluchovou vadou bylo onemocnění diabetes mellitus. 7 (58,33 %) novorozenců s poruchou sluchu mělo rodinnou anamnézu nezatíženou. Závěr: Incidence sluchových vad je u novorozenců rizikových i fyziologických. Pro správný vývoj dítěte je žá- doucí tyto vady odhalit v časném stádiu. V České republice však nadále chybí legislativní podpora ukládající zdravotnickým zařízením povinnost provádět novorozenecký screening sluchu celoplošně., Background: The article presents the results of retrospective study which is focused on otoacoustic emission screening of newborns. Aim: To analyse and compare physiological and risk newborns examinated of otoacoustic emission. Method: There was used the method of retrospective study through the record sheet and information portal in a chosen medical institution within quantitative research. File selection contained all newborns hospitalized in the medical institution in the year of 2014 and babies who took an examination of hearing impairments. Results: In the year of 2014 in the medical institution was born 1 516 newborns. The examination of otoacoustic emission was done of 1 510 newborns. From the number of 1 258 physiological newborns there were 5 (0,4 %) of them with hearing impairment. From the number of 252 risk newborns there were diagnosed 7 (2,78 %) newborns with hearing impairment.The conclusion is that the hearing impairment is more often in risk newborns. Diabetes mellitus was the risk factor in the hearing impairment. Conclusion: The incidence of hearing impairments occurs in risk newborns as well as physiological. It is necessary to discover hearing impairment in early stage for right child evolution. We still miss legislative support in the Czech Republic which should order the duty to carry out general screening of hearing of newborns in medical institutions., and Lucie Tobiášková, Helena Petržílková
