102. Perioperative safety process in practise (student´s contribution)

- Východiska: Příspěvek popisuje problematiku perioperačního bezpečnostního procesu (PBP) v praxi ve vztahu k regionu, ve kterém sestry pro perioperační péči pracují. Realizace šetření byla provedena ve 4 zdravotnických zařízeních východočeského a západočeského regionu. Cíl: Ve výzkumném šetření byly stanoveny 4 cíle: zjistit informovanost respondentek o PBP, zda a jak dlouho se na pracovištích respondentek používá PBP, jak respondentky vnímají přínosnost PBP pro pacienta a jak jsou respondentky spokojeny s nastavením PBP na jejich pracovišti. Metodika: Pro výzkum byl sestaven 27 položkový dotazník vlastní tvorby. Výsledná data byla vyhodnocována deskriptivní statistkou a neparametrickým Mannovým–Whitneyovým testem. Výzkumného šetření se zúčastnilo 61 respondentek ze zdravotnických zařízení východočeského a západočeského regionu, z toho 45 instrumentujících a obíhajících sester a 16 anesteziologických sester z gynekologických a centrálních operačních sálů 4 zdravotnických zařízení na území východočeského a západočeského regionu. Výběr respondentek byl záměrný, všechny souhlasily se zařazením do výzkumného šetření. Výsledky: Bylo zjištěno, že na pracovištích respondentek je využíván PBP ± 3 roky (průměrná hodnota 3,29, směrodatná odchylka 0,85). Informovanost respondentek o PBP je hodnocena jako výborná. Většina respondentek je spokojena s nastavením PBP na jejich pracovišti a přínosnost PBP pro pacienty většina z nich vní- má jako velmi vysokou. Závěry: PBP je nedílnou součástí každého operačního výkonu a je nutné, aby byl prováděn na všech pracovištích v souladu s rezortním bezpečnostním cílem., Background: e paper describes the problems of perioperative safety process (PBP) in practice in relation to the region in which perioperative care nurses work. Realization of research was performed in 4 health facilities in the territory of the East and the West Bohemian region. Aim: We established four goals in our research: to determine the level of awareness of respondents about PBP, whether and how long the responders used PBP in their workplaces, how perioperative care nurses perceive the utility of PBP for the patient and how responders are satisfied with setting of PBP in their workplace. Methods: We compiled 27 – items questionnaire for our research. e resulting data were evaluated by descriptive statistics and neparametrical Mann–Whitney test. 61 responders participated in this research. e research sample consisted of 45 perioperative care nurses and 16 anestethic nurses from gynaecological and central operating rooms of four health facilities in the territory of the East and the West Bohemian region. e respondents agreed with the research. Results: We found that on workplace of all respondents is used the perioperative safety process for ± 3 years (average 3.29, standard deviation 0.85). e awareness of respondents about perioperative safety process was evaluated as excellent. Most of respondents are satisfied with the setting of perioperative safety process on their workplace and their perceive the utility of perioperative safety process as very high. 106. Positive predictive value of cea and ca19-9 as tumor markers for recurrent colorectal cancer in cases where conventional work-up fail to localize disease

- Introduction: Routine surveillance of colorectal cancer includes serial measurements of CEA levels. Although not routinely indicated Ca 19-9 is also a tool for recurrence. When any of these serum markers is elevated during follow up, this could represent a recurrence. The management of elevated tumor marker levels include clinical exams, endoscopy and conventional imaging–ultrasound, CT, MRI. Objective: To evaluate the positive predictive value of CEA and Ca19-9 as tumor markers for recurrent colorectal cancer in cases where conventional imaging and endoscopic studies fail to localize disease. Materials and methods: A total of 75 patients with elevated CEA and/or Ca19-9 serum levels and negative endoscopic exam as well as negative abdominal CT and Chest X-ray were included in the study. CEA levels were tested in 50 patients. Ca 19-9 was tested in 65 patients. 34 of the patients had both markers tested. All patients underwent whole body 18F-FDG PET/CT. Patients with negative of equivocal PET scan were further followed up (10 to 24 months). Results: Based on the reference standard – the results from PET/CT, if positive and the results from follow-up in cases of negative or equivocal scans, the positive predictive value of Ca 19-9 was 84% and that of CEA -83%. There was no significant difference in the PPV of Ca19-9 and CEA. Conclusion: Elevated CEA and Ca 19-9 levels in patients under active surveillance after operation for colorectal cancer have high positive predictive value for recurrence, even in cases where conventional work-up – endoscopy and CT don’t localize disease., Yana Bocheva, Pavel Bochev, and Literatura

The article is particularly focused to the section 6 of the Act 373. This section prevents the group of women to use „specific health care” by the mandatory age limit. - Přehledový článek pojednávající na téma možných komplikací páskových operací běžně užívaných v léčbě stresové inkontinence moči. První metodou byla retropubická tahuprostá páska TVT, která s sebou nese zejména riziko poranění močového měchýře. V literatuře byly dále popsány komplikace typu závažného krvácení, poranění orgánů v malé pánvi, jako je střevo, nebo velkých cév. Další generace tahuprostých pásek se vyhýbá retropubickému prostoru vedením pásky skrz foramen obturatum. Tato generace má popsané riziko vzniku bolesti v třísle/tříslech. Avšak tato bolest většinou spontánně ustupuje, případně rychle odeznívá po podání nesteroidních antiflogistik. Poslední generace tahuprostých pásek jsou „minipásky“. Zavádí se z jednoho drobného řezu a jejich délka je menší než u předešlých generací. Rizikovost „minipásek“ spočívá v nedostatečné fixaci a tedy riziku selhání. Přestože se páskové metody používají celosvětově v klinické praxi již přes patnáct let, doposud nebyla vyvinuta metoda, která by byla jak naprosto bezpečná, tak úspěšná a jednoduchá k zvládnutí operatérem., This review focuses on possible complications during tape procedures carried out due to stress urinary incontinence. The first method was retropubic tension-free vaginal tape TVT that held mainly risk of bladder perforation. Further complications previously described in literature were severe bleeding and injuries of organs in lesser pelvis like bowel or great vessels. The next generation eliminated retropubic space by insertion through the obturator foramen. However another complication had been witnessed in presence of groin pain. Fortunately, this is usually transient and quickly passing either spontaneously or after use of non-steroid anti-inflammatory drugs. The last generation of tension-free vaginal tapes are “minislings” that are inserted from a single incision and are shorter in comparison with the previous generations. The risk of use of this “minislings” is in insufficient fixation and therefore failure of the procedure.
