Cieľ: V práci sme sa zamerali na vedomosti pacientov s astmou v oblastiach self-manažmentu tohto ochorenia a najmä na vplyv faktoru dĺžky trvania tohto ochorenia na úroveň vedomostí o self-manažmente. Zároveň sme zisťovali vzťah medzi úrovňou vedomostí o self-manažmente ochorenia a kontrolou astmy. Metodika: V tejto prierezovej štúdii bolo do súboru zaradených 297 respondentov. Na testovanie kontroly astmy a úrovne vedomostí pacientov sme použili valídne nástroje a to Dotazník vedomostí o self-manažmente astmy pre dospelých (Measuring asthma self-management knowledge in adults) a Test kontroly astmy (Astma Control Test ACTTM). Na štatistické spracovanie dát boli využité: Pearsonov Chi-kvadrat test nezávislosti a Spearmanov korelačný koeficient. Výsledky: Až 220 z 297 respondentov (74,07 %) preukázalo nedostatočné vedomosti, a to konkrétne v doméne Vedomosti o liečbe astmy. V oblasti Manažment bronchiálnej astmy malo nedostatočné vedomosti 191 respondentov (64,31 %). Početnosť správnych odpovedí zároveň naznačuje, že úroveň vedomostí v doménach self-manažmentu astmy, významných pre kontrolu ochorenia, bola vyššia v skupine pacientov diagnostikovaných v priemere jeden rok ako v skupine, ktorá trpí ochorením dlhšie (6-10 rokov), i keď rozdiel nie je štatisticky významný. Záver: Nedostatky vo vedomostiach astmatikov, vyššia frekvencia chýb v oblastiach self-manažmentu, vrátane self-monitoringu a horšia kontrola astmy pri ochorení diagnostikovanom viac ako 6 rokov naznačujú, že je potrebné v klinickej praxi venovať čas a pozornosť reedukácii, opakovanému posúdeniu vedomostí a praktických zručností (aplikácia metód self-manažmentu, inhalačnej techniky) v skupine už skôr diagnostikovaných astmatikov., Aim: This project is aimed to find out the patient knowledge with asthma in the area of self-management of this disease and particularly the influence of the length of the disease on the knowledge level about self-management. At the same time we tried to find out the relation between the knowledge level of the asthma self-management and control of asthma disease. Methods: The total number of 297 adult asthma patients were included in the cross sectional study. The valid instruments Asthma Control Test and Measuring asthma self management knowledge in adults were used for testing asthma control and the degree of patient knowledge in self management of asthma. The Chi-squared test and the Spearman´s rank correlation coefficient were used for statistical processing of data. Results: A total of 220 out of 279 respondents (74%) showed poor knowledge of selfmanagement of asthma, particularly in “Knowledge of asthma treatment” domain and 191 patients in “Management of bronchial asthma” domain (64%). Patients who had been diagnosed one year ago had better level of knowledge in selfmanagement of asthma than those who had been treated for asthma longer than six years (6-10 years). This difference was not significant, but it was related to those domains which has significant influence on asthma control (domains 2-5). Conclusion: Poor knowledge of asthma patients, higher frequency of errors in self management and worse control of asthma in patients with asthma longer than six years imply the need for better and repeated education, check of patient knowledge and skills for self-management, use of peak flow meter included in this group of patients., Michaela Dingová, Petronela Osacká, and Literatura
Určení prahu viability je výchozím bodem pro zvažování léčebného postupu. Kromě stupně zralosti plodu a jeho zdravotního stavu závisí viabilita na vývoji medicínské technologie. Je podmíněna také sociálně-ekonomickými a kulturními faktory. Hranice viability z hlediska možnosti narodit se živě a přežít leží mezi 22.-24. týdnem. Dohoda odborné komunity o poskytování aktivní péče vychází z pravděpodobnosti přežití a přežití bez závažného postižení. Ve vyspělých zemích je resuscitace a intenzivní péče od 26. gestačního týdne většinou povinná, protože děti narozené v tomto týdnu mají vysoké šance na přežití bez závažného postižení a dobrou kvalitu života. Období 22.-25. gestačního týdne je označováno jako šedá zóna, kdy výsledky péče jsou nejisté a předem nepredikovatelné. Závisí nejen na gestačním stáří, ale i na hmotnosti, pohlaví, četnosti těhotenství, místu porodu a indukci zralosti plic plodu kortikoidy. Jsou ovlivněny přístupem k péči, postoji lékařů i rodičů. Předpověď signifikantně mění přežití prvního týdne. Při rozhodování o způsobu péče je třeba vycházet z komplexního pojetí viability. Vytvoření standardů založených pouze na gestačním stáří, které často nejsme schopni přesně stanovit, by neodpovídalo medicínské i etické složitosti situace a bránilo v individuálním rozhodování., Determining the threshold of viability is the initial point in considering medical treatment. Viability depends - aside from the level of maturity of the fetus and its health condition – on the advancement of medical technology. Its quality is also based upon given socio-economical and cultural factors. The threshold of viability in regard of the possibility to be born alive and survive lies between the 22-24 weeks. An expert agreement on providing active care is based upon the probability of survival and survival without serious disability. Resuscitation and intensive care from the 26 weeks of gestation is usually mandatory in developed countries because infants born at this week have a very high chance of survival without severe disabilities and with good quality of life. The 22-25 week of gestation is known as the grey zone, during which the results of care are uncertain and unpredictable in advance. However, they depend not only on gestational age but on birth weight, sex, multiple gestation, place of delivery, and induction of lung maturity with corticosteroids. They are influenced by the caregivers approach and attitude. Surviving the first week changes the prediction significantly. It is essential to precede our decision-making according to a comprehensive concept of viability. Cíl: Zmapovat vliv preventivních strategií na vznik infekce močového traktu v souvislosti se zavedeným močovým katétrem. Metodika: Metodou praxe založené na důkazu bylo v elektronických databázích (PubMed, American Journal of Medicine, Virginie Henderson Library, Google Scholar, PubMed Central, Best Evidence for Nursing, Cochrane Collaboration) nalezeno 25 relevantních studií (meta-analýzy, systematické přehledy, randomizované kontrolované studie, prospektivní kohortové studie, průřezové studie, retrospektivní studie, studie případů a kontrol). Výsledky: Nebyly nalezeny silné důkazy, které by jednoznačně potvrdily nebo vyvrátily vliv používaných preventivních strategií (močový katétr potažený stříbrem, zvýšená hygiena rukou, stanovení optimální délky zavedení močového katétru) na snížení výskytu infekce močových cest u pacientů se zavedeným močovým katétrem. Závěr: Silné důkazy potvrzující vliv preventivních strategií nalezeny nebyly, avšak validní a relevantní výstupy a doporučení zahraničních studií mohou být přínosné v péči o katetrizované nemocné v podmínkách českého ošetřovatelství., Objective: To find out the influence of preventive strategies on the occurrence of catheter-associated urinary tract infection. Methods: Using the method of evidence based practice were found in electronic databases (PubMed, American Journal of Medicine, Virginie Henderson Library, Google Scholar, PubMed Central, Best Evidence for Nursing, Cochrane Collaboration) 25 relevant studies (meta-analysis, systematic reviews, randomized controlled trials, prospective cohort studies, cross-sectional studies, retrospective studies, case-control studies). Results: We did not found any strong evidence, which could clearly validate or refute the influence of prevention strategies used (silver-impregnated urinary catheter, intensive hand hygiene practices, determination of the optimal length of the inserted urinary catheter) on reducing the incidence of catheter-associated urinary tract infection. Conclusion: Strong evidence confirming the influence of preventive strategies were not found, however valid and relevant outcomes and recommendations of international studies may be useful in the care of catheterised patients in conditions of Czech nursing., Jana Bizoňová, Darja Jarošová, and Literatura
Subject of this work is critical review of applications of selected qualitative research methods used in in health policy research. Attenti - on is paid mainly to discourse ana - lysis and critical discourse analysis.
