Paraneoplastická limbická encefalitida je vzácné onemocnění náležící do skupiny paraneoplastických neurologických syndromů. V diagnostice hraje zásadní roli zobrazení mozku magnetickou rezonancí. Předkládáme kazuistiku 57letého muže, u něhož vyšetření magnetickou rezonancí jistě přispělo ke stanovení této diagnózy. Další významnou součástí diagnostického postupu je průkaz specifických, takzvaných antineuronálních autoprotilátek Prognóza choroby nebývá dobrá, je závislá především na možnostech suprese základního tumorózního onemocnění., Paraneoplastic limbic encephalitis is a rare disorder which belongs to a group of paraneoplastic neurologic syndromes. The diagnostic role of magnetic resonance imaging (MRI) is of utmost importance. We report a case of a 57-year-old man in whom MRI significantly contributed to establishing of proper diagnosis. Other important diagnostic test includes identification of specific, so called antineural auto antibodies. CT s vysokým rozlíšením (HRCT) sa stalo metódou voľby v zobrazovaní a diagnostike ochorení stredného ucha a prevodovej poruchy sluchu. Pomocou HRCT dokážeme detailne popísať anatómiu spánkovej kosti, patológiu stredného ucha (otitis média chronica, cholesteatóm a ich komplikácie) a traumatické zmeny. Magnetická rezonancia má naopak vysoké rozlíšenie mäkkých tkanív, preto ju využívame k diferenciálnej diagnostike mäkkotkanivovej masy stredného ucha zobrazenej na CT, k odlíšeniu reziduálneho ochorenia alebo recidívy v teréne pooperačných zmien a k zobrazeniu intrakraniálnych komplikácií akútnej a chronickej otitídy CT a MR sú komplementárnymi zobrazovacími metódami, pomáhajú stanoviť správnu diagnózu, ktorá je pre klinika dôležitá z hľadiska stratégie liečby. V publikácii uvádzame spektrum patológií stredného ucha, s ktorými sa pri zobrazení spánkovej kosti môžeme stretnúť a na ktoré treba myslieť., High resolution computed tomography (HRCT) is a modality of choice in diagnostic imaging of middle ear pathology and conductive hearing loss. It enables to image a detail anatomy of temporal bone, pathology of middle ear (e.g. chronic otitis média, cholesteatoma and their complications) and traumatic changes in this area. On the other hand, magnetic resonance imaging (MRI) enables a high resolution of soft tissue structures. It is used in imaging of soft tissue masses in middle ear cavity previously diagnosed on CT, differentiating residual disease or recurrence in postoperative ear, and in imaging of intracranial complications of acute and chronic otitis. CT and MRI are complementary imaging modalities in helping the clinicians to determine the proper strategy for patient treatment. Alzheimerova choroba (ACH) se řadí mezi závažná neurodegenerativní onemocnění provázená oxidačním stresem. Produkty radikálových reakcí mohou difundovat z primárních míst a být detekovány v cerebrospinální tekutině (CSF) či krvi. Tyto produkty představují potenciální biochemické markery pro diagnostiku ACH. Nejvíce pozornosti je zaměřeno na analýzu CSF, protože odráží patologické změny v mozkové tkáni u ACH. V CSF byly potvrzeny zvýšené hladiny oxidačních produktů lipidů i proteinů. Hladiny vitaminů jsou snížené, nicméně existují i práce, které nenašly rozdíly oproti kontrolám. Některé studie se zaměřily na detekci produktů oxidačního stresu v krvi u ACH, ale výsledky nejsou konzistentní. Část prací ukazuje na nárůst oxidačních produktů a snížené hladiny antioxidantů v plazmě. Nicméně jiné práce tyto výsledky nepotvrdily. Z hlediska hledání diagnostického biomarkeru pro ACH má význam se zaměřit na specifické produkty peroxidace lipidů, tzv. lipofuscinoidní pigmenty (LFP), v erytrocytech. Na základě fluorescenčních analýz LFP je možné najít specifický produkt v krvi u ACH., Alzheimer´s disease (AD) is a serious neurodegenerative disorder accompanied by oxidative stress. Products of free radical reactions diffuse from primary sites and can be detected in cerebrospinal fluid (CSF) and blood. Such products represent potential biochemical markers for diagnosis of AD. Most studies are focused on CSF since its composition reflects pathological changes in the brain in AD. Increased levels of lipid and protein oxidative products have been found in CSF. Levels of vitamins were reduced in CSF. However, there are studies which show no difference between AD and controls. There is research on free radical products in blood in AD but results are not consistent. Several studies show increased oxidative products and reduced antioxidant in plasma. Nevertheless, others did not confirm it. Considering the investigation of a diagnostic biomarker for AD, specific end-products of lipid peroxidation, so called lipofuscin-like pigments (LFP), in erythrocytes represent an important possibility. Úvod. Karcinom prsu je nejčastěji diagnostikovaný zhoubný nádor u žen. V České republice se vyskytuje u 20% žen, které onemocní zhoubným nádorem. Ročně onemocní na karcinom prsu přes 5000 žen a kolem 1900 žen na něj zemře. Od zavedení celoplošného screeningu nádorů prsu v České republice v roce 2002 se očekávalo zvýšení podílu nádorů nižších stadií a snížení úmrtnosti žen na karcinom prsu. Po zavedení screeningu se i přes stoupající incidenci karcinomu prsu úmrtnost nezvyšovala, spíše stagnovala nebo se mírně snížila. Cíl. Zhodnotit na souboru pacientek s karcinomem prsu zjištěným v rámci screeningového a diagnostického vyšetření, zda screening snižuje úmrtnost žen na karcinom prsu. Metoda. V období od ledna 2003 do prosince 2009 byl ve Fakultní nemocni Královské Vinohrady v Praze diagnostikován karcinom prsu u 466 žen. Celkem bylo u těchto žen zjištěno 476 karcinomů prsu. Do screeningové skupiny pacientek s diagnostikovaným zhoubným nádorem prsu bylo zařazeno 160 žen ve věkovém rozmezí 45-74 let se 161 zhoubným nádorem. Jedna pacientka měla v době stanovení diagnózy oboustranný nádor. Do diagnostické skupiny pacientek se zjištěným zhoubným nádorem prsu byla zařazena 181 pacientka. U této skupiny se prokázalo 186 zhoubných nádorů, neboť u pěti žen byl ve sledovaném období diagnostikovaný bilaterální karcinom prsu. Všechny pacientky absolvovaly mamografii, ultrazvuk a core-cut biopsii z prsu. U obou skupin pacientek byla hodnocena průměrná velikost zjištěných nádorů a statisticky byla zhodnocena úmrtnost žen na karcinom prsu. ýsledky. Průměrná velikost zhoubného nádoru prsu ve screeningové skupině byla 15,3 mm a v diagnostické skupině 25,4 mm. Během více jak 7letého sledování zemřely na karcinom prsu tři ženy ze screeningové skupiny (160 žen) a 13 žen z diagnostické skupiny (181 žena). Při porovnání screeningové a diagnostické skupiny byla prokázána statisticky významně nižší úmrtnost ve screeningové skupině než v diagnostické skupině. Závěr. V průběhu více jak 7letého sledování skupiny pacientek s karcinomem prsu bylo prokázáno, že zavedený screening nádorů prsu snižuje úmrtnost na karcinom prsu. Dosažené výsledky podporují pokračování screeningového programu nádoru prsu v České republice., Introduction. Breast cancer is the most frequently diagnosed major cancer among women. In the Czech Republic it is found in 20% of women suffering with cancer. Annually, about 5.000 women ill with breast cancer and around 1900 women die. Nationwide breast cancer screening has been introduced in 2002 in Czech Republic and there was expected that it should increase the proportion of lower stage cancers and reduce the mortality of women with breast cancer. The introduction of screening has shown that despite the rising incidence of breast cancer, mortality did not increase, rather stagnated or slightly decreased. Aim. To evaluate the group of patients with breast cancer identified in the screening and diagnostic examinations, whether screening can reduce the mortality of women of breast cancer. Method. During the period from January 2003 to December 2009 in the Faculty Hospital Královské Vinohrady in Prague there were total found 476 breast cancers in 466 women. In the screening group of patients diagnosed with breast cancer were enrolled 160 women with 161 breast cancers. One patient had at the time of diagnosis bilateral carcinoma. To the diagnostic group were included 181 women with breast cancer, with a total of 186 tumours. Bilateral breast cancer was diagnosed in 5 women in the period. Mammography, ultrasound and core-cut biopsy from breast tissue were used in all patients with the breast cancer. In both groups of patients was also evaluated the average size of diagnosed tumors and statisticly was evalueted the mortality of breast cancer. Results. The average size of a malignant tumor in the screening group was 15.3mms and 25.4 mm in the diagnostic group. During more than 7 years of monitoring, 3 women from the screening group (160 women) died of breast cancer. In the diagnostic group (181 women) 13 women died of breast cancer. A comparison of screening and diagnostic group was confirmed statistically significantly lower mortality rate in the screening group than in the diagnostic group. Conclusion. During more than 7 years old follow up of patients with breast cancer has been shown that breast cancer screening can reduce mortality of breast cancer. Zobrazenie sluchového nervu a vnútorného ucha ako aj vyšších centier sluchovej dráhy pred kochleárnou implantáciou pomocou MR a/alebo CT je v dnešnej dobe štandardom. Stav vnútorného ucha má zásadný vplyv na výsledok kochleárnej implantácie (Kí). U väčšiny kandidátov na Kí je hluchota spôsobená poruchou funkcie Cortiho orgánu v inak anatomicky diferencovanom uchu so zachovaným počtom gangliových buniek a vlákien sluchového nervu. Úlohou zobrazovacích metód je prispieť k určeniu príčiny hluchoty a identifikovať anatomické podmienky pre samotný chirurgický výkon. V článku sme sa zamerali na spektrum patologických nálezov spánkovej kosti, vnútorného ucha a sluchového nervu, s ktorými sa môžeme stretnúť a na ktoré pred kochleárnou implantáciou treba upozorniť. Predoperačné zistenie abnormalít na úrovni vnútorného a prípadne stredného ucha, dáva chirurgovi možnosť implantovať vhodné ucho, naplánovať chirurgický postup a vybrať správny typ elektródy na implantáciu., CT and/or MR imaging of acoustic nerve, inner ear and superior centers of acoustic pathway is a gold štandard before planned cochlear implantation. Condition of inner ear plays an important role in determining the outcome of cochlear implantation (CI). Most of the candidates for CI lost their hearing by impairment of the Corti organ, in otherwise anatomically normal ear with normál number of ganglion cells and fibers of acoustic nerve. Role of imaging is help to find the cause of deafness and identify anatómie conditions before surgical intervention. In this article we are trying to review different pathologic conditions of temporal bone, inner ear and acoustic nerve that are important to identify before cochlear implantation. Ke sledování vývoje zdravotního stavu pacientů po neurochirurgické intervenci a pacientů v neurointenzivní péči za účelem potřebných terapeutických korekcí přispívá též vyšetření likvoru. To by mělo být obecně dostupné, jeho výsledky by měly být rychle k dispozici a měly by s dostatečnou přesností informovat o případných změnách v centrálním nervovém systému těchto pacientů. Problémem ovšem je, že likvorový kompartment u neurochirurgických a neurointenzivistických pacientů obvykle představuje poměrně složitou oblast. Bývá totiž podstatně ovlivněn primárním postižením centrálního nervového systému a probíhajícími hojivými procesy, od kterých je potřebné odlišit případnou infekční či neinfekční komplikaci. Řešením této situace může být monitorování vývoje likvorového obrazu se zvláštním důrazem na specifikaci charakteru zánětlivého procesu v likvorovém kompartmentu. Zásadní postavení má cytologické vyšetření likvoru a určení energetických poměrů v předmětné lokalitě. Společné hodnocení těchto ukazatelů postavené na obecných imunologických principech umožňuje úspěšně diferencovat reparační procesy a úklidovou reakci od případných komplikací infekční či neinfekční etiologie v centrálním nervovém systému. Pozornost je věnována zejména podstatě, příčinám a likvorové detekci purulentního zánětlivého procesu a razantního zánětlivého procesu s oxidačním vzplanutím makrofágů. Zmíněny jsou ale též možnosti nástrojů základní likvorologie při odhalování poruchy cirkulace likvoru, destrukce tkáně centrálního nervového systému a krvácení do likvorových cest., Examination of the cerebrospinal fluid aids in monitoring neurosurgical and neurointensive care patients, which is necessary for guiding important therapeutic interventions. This should be generally available, should be available in short time and should provide reliable information about changes in the central nervous system of these patients. The CSF compartment in neurosurgical and neurointensive care patients is very intricate. It is usually influenced by the primary damage to the central nervous system and reparative processes, which need to be differentiated from the possible infectious or non-infectious complications. A solution to this complicated situation is repeated evaluation of the CSF, with emphasis on its inflammatory nature. A cytological examination and monitoring the turnover of energy in the CSF compartment play a crucial role in this diagnostic process. Combined evaluation of these indicators based on general immunological principles enables differentiation between reparation, or the so called “clean-up reaction”, from any infectious or non-infectious complications in the CNS. In this paper, we specifically focus on the nature, the causes and the detection of purulent inflammation and the detection of very intensive inflammatory process associated with oxidative burst of macrophages in the CSF.
