Cíl: Cílem šetření bylo zjistit četnost subjektivně pociťovaných potíží s polykací funkcí mezi klienty sociálních zařízení následné péče za pomoci zahraničního nástroje nazvaného Dotazník o přijímání potravy (Eating Assessment Tool, EAT-10). Dalším cílem bylo zjistit časovou zátěž spojenou s administrací tohoto nástroje v praxi. Metody: Nástroj EAT-10 byl přeložen do češtiny a v pretestu byla ověřena jeho srozumitelnost. Poté byl nástroj, pomocí rozhovoru, použit v 5 sociálních zařízeních následné péče. Před dotazováním byl proveden screening kognitivních funkcí za pomoci testu Mini-Cog, který sloužil k vyřazení klientů s nedostatečnou kognitivní funkcí z dalšího testování. Výsledky: Do studie bylo zařazeno 117 respondentů ve věku 65 let a výše, z nichž 104 úspěšně prošlo testem Mini-Cog. Více než polovina z nich (58) uvádí polykací potíže. K použití nástroje EAT-10 bylo u 1 respondenta potřeba cca 4–5 minut. Závěr: Klienti sociálních zařízení následné péče často subjektivně vnímali polýkání jako problematické. Nástroj EAT-10 lze použít při získávání základních údajů o polykání u těchto klientů., Aim: The aim of the study was to determine the frequency of subjectively perceived swallowing difficulties in clients of social institutions providing aft er-care, using a foreign instrument called Eating Assessment Tool (EAT-10). Another aim was to identify the amount of time required to administer the tool in practice. Methods: The tool EAT-10 was translated into Czech and its comprehensibility was verifi ed in a pre-test. Finally, it was employed in interviews in five social institutions providing aft er-care. Before the respondents were interviewed, they were screened for cognitive functions via the Mini-Cog test, which was used to exclude from further testing those respondents who had insufficient cognitive function. Results: 117 respondents aged 65 and above were enrolled in the study, 104 of whom were successful on the Mini-Cog test. More than half of them (58) reported swallowing difficulties. It took approximately 4–5 minutes to complete the EAT-10 tool with one patient. Conclusion: Subjectively perceived swallowing difficulties are a frequent problem in clients of social institutions providing aft er-care. Cíl: Cílem našeho příspěvku bylo zjistit vztah mezi konstrukty kvality života související se zdravím Health Related Quality of Life, používaný ve standardní zkratce HRQoL a subjektivní pohody Subjective well-being, používaný ve standardně zavedené zkratce SWB, u pacientů s chronickým onemocněním, konkrétně bronchiálním astmatem. Zaměřujeme se na diskriminační validitu obou konstruktů. Metodika: Výzkumný soubor tvořilo 316 pacientů, kteří byli vyšetřeni na plicní ambulanci ve Fakultní nemocnici Ostrava. HRQoL byla hodnocena prostřednictvím zkrácené verze Dotazníku kvality života astmatiků (Mini Asthma Quality of Life Questionnaire, Mini AQLQ), SWB prostřednictvím Indexu osobní pohody (Personal Wellbeing Index, PWI). Ke zjištění závislosti mezi vybranými proměnnými byl použit Pearsonův korelační koeficient a lineární regresní analýza. Diskriminační validitu jsme zkoumali prostřednictvím zkoumání rozdílů v HRQoL a SWB z hlediska vybraných klinických proměnných. Rozdíly v HRQoL a SWB byly zjišťovány vícefaktorovou analýzou rozptylu (ANOVA). Výsledky: Korelace mezi HRQoL (operacionalizované prostřednictvím Mini AQLQ) a SWB (operacionalizované prostřednictvím PWI) jsou pozitivní a středně silné. Percepce kontroly astmatu byla identifikovaná jako signifikantní prediktor obou konstruktů, avšak vytvářela vyšší míru variability v rámci HRQoL. Závěr: SWB a HRQoL představují související, avšak odlišné konstrukty. Výsledky podporují chápání HRQoL jako konstruktu založeném na větší míře klinických proměnných než je tomu u SWB., Aim: The goal of the study was to determine the relationship between the health-related quality of life (usually abbreviated as HRQoL) and subjective well-being (usually abbreviated as SWB) of patients with chronic disease, namely bronchial asthma. The focus was on discriminative validity of both constructs. Methods: The research sample consisted of 316 patients examined at the Pulmonary Clinic of the University Hospital Ostrava. HRQoL was assessed by a short version of the Asthma Quality of Life Questionnaire (Mini AQLQ) and SWB was assessed by the Personal Wellbeing Index (PWI). Pearson’s correlation coefficient and linear regression analysis were used for evaluation of dependencies between selected variables. Discriminative validity was explored by examination of differences between HRQoL and SWB relevant to selected clinical variables. Differences in HRQoL and SWB were detected by multi-factor analysis of variance (ANOVA). Results: The correlations between HRQoL (assessed by the Mini AQLQ) and SWB (assessed by the PWI) are positive and moderate. Perception of asthma control was identified as a significant predictor of both constructs. However, it produced greater variance in the context of HRQoL. Conclusion: SWB and HRQoL are related but distinct constructs. Cíl: Cílem výzkumu bylo popsat, jak vnímají a prožívají svou kvalitu života senioři s infekční tuberkulózou, jejíž léčba je dlouhodobá a součástí léčebných opatření je izolace ve zdravotnickém zařízení. Metody: Byla zvolena kvalitativní výzkumná metoda. Pro sběr dat byl použit nestandardizovaný rozhovor. Výzkumný soubor tvořilo 8 pacientů starších 65 let (s diagnózou infekční tuberkulóza) hospitalizovaných ve vybraných zdravotnických zařízeních oboru Pneumologie a ftizeologie a jeden rodinný příslušník pacienta. Rozhovory s respondenty byly realizovány v různém časovém období léčby tuberkulózy. Na základě kvalitativní analýzy dat byly vytvořeny 4 hlavní kategorie kvality života: 1. Prožívání nemoci v oblasti psychické, 2. Prožívání nemoci v oblasti fyzické, 3. Prožívání nemoci v oblasti sociální a 4. Životní hodnoty, názory. Výsledky: Z analýzy rozhovorů vyplynulo, že tuberkulóza je pro seniory stigmatizující chorobou, má negativní vliv na vlastní prožívání a vnímání kvality ţivota. Zhoršené vnímání kvality života je způsobeno více faktory, především skutečností, že mohli nakazit své blízké infekční chorobou. Mezi nejčastější emoce, které respondenti prožívali, patřily strach, úzkost, pocit zahanbení a viny, pocity osamělosti s beznadějí a pocity méněcennosti. Závěr: Problém geriatrické tuberkulózy je významný, neboť se jedná o potenciální zdroje tuberkulózní infekce ve zdravotnických zařízeních a v pobytových zařízeních sociální péče. Je proto důležité, aby si odborníci pečující o seniory uvědomovali existenci problémů při diagnostice a léčbě tuberkulózy., Aims: The aim of the research was to describe how the elderly with infectious tuberculosis, whose treatment is long-term matter and component of therapeutic measure is isolation in health care facility, perceive and experience their quality of life. Methods: A qualitative research method was chosen. For data collection was used non-standardized interview. The research file consisted of 8 patients over 65 years of age (diagnosed with infectious tuberculosis) hospitalized in selected health care facilities of pneumology and phthisiology and one family member of the patient. Interviews with respondents were conducted in different time periods of treatment of tuberculosis. Based on qualitative data analysis were created 4 major categories of quality of life: 1. Experiencing the illness in the area of psychic, 2. Experiencing the illness in the area of physical, 3. Experiencing the illness in the area of social and 4. Life values, beliefs. Results: The analysis of interviews revealed that tuberculosis is for the elderly stigmatizing disease, has a negative effect on their own experiencing and perception of quality of life. Deteriorated perception of quality of life is caused by several factors, primarily by the fact, that they could have infected their loved ones with infectious disease. The most common emotions experienced by respondents included fear, anxiety, feel of shame and guilt, feelings of loneliness with despair and feelings of inferiority. Conclusion: The problem of geriatric tuberculosis is significant because it is a potential source of tuberculous infection in health care facilities and in residential social care facilities. Přehled problematiky: Magnézium je čtvrtým nejčastějším kationtem v lidském organizmu a po draslíku druhým kationtem v intracelulárním prostoru. Účastní se jako kofaktor enzymů metabolických reakcí, tvorby a utilizace energie a je nezbytný k udržení elektrolytové rovnováhy. Tělo dospělého člověka obsahuje asi 0,043 % (535-730 mmol) magnézia, což je v přepočtu asi 22-30 g (60 % v kostěném skeletu, 40 % intracelulárně ve svalovině a měkkých tkáních, 1 % (7,5-10 mmol) je obsaženo v extracelulární tekutině). Denní potřeba magnézia se odhaduje na 10-20 mmol, tj. 200-400 mg. Ve vztahu k podané dávce pak platí, že čím vyšší podaná dávka, tím nižší procento rezorbce ze střeva. Z doporučené denní dávky 15 mmol (365 mg) se rezorbuje pouze okolo 1/3. Význam magnézia v těhotenství: Důsledkem manifestního nedostatku magnézia může být: 1. zvýšené riziko potratu - předčasný porod (inkompetence hrdla děložního, předčasný odtok vody plodové), 2. placentární insuficience a hypotrofizace plodu, 3. rozvoj gestózy u matky, 4. zvýšené riziko astmatu a ekzému u dětí. Magnézium sulfát, podávaný těhotným ohroženým předčasným porodem (23+0 -30+0), snižuje riziko dětské mozkové obrny u přežívajících novorozenců. Souhrn: Nedoporučuje se používat intravenózní magnézium sulfát v indikaci léčby hrozícího předčasného porodu déle jak 5-7 dní. Tato omezení se nijak nevztahují na perorální užívání magnézia., Objectives: Magnesium is the fourth most common cation in the human body, and the second cation to potassium in the intracellular space. It participates as a cofactor of enzymes in the metabolic reactions, creation and utilization of energy and is necessary for the maintenance of electrolyte balance. The body of an adult contains about 0.043% (535-730 mmol) of magnesium, which is the equivalent of about 22-30 g (60% in the bony skeleton, 40% intracellular in muscles and soft tissue, 1% (7.5-10 mmol) in extracellular fluid). The daily requirement of magnesium is estimated at 10 to 20 mmol, i.e. 200-400 mg. As for dosage, the higher the dose, the lower the percentage of absorption from the intestine. Of the recommended daily dose of 15 mmol (365 mg), only about a third is absorbed. The importance of magnesium in pregnancy: implications of a manifest lack of magnesium may include: 1. increased risk of miscarriage – preterm labor (cervical incompetence, premature rupture of membranes); 2. placental insufficiency and fetal hypotrophy; 3. development of maternal gestosis; 4. increased risk of asthma and eczema in neonates. Magnesium sulfate given to pregnant women with threatened preterm labor (23+0 – 30+0) reduces the risk of cerebral palsy in surviving infants. Summary: In cases of threatened preterm labor it is not recommended to administer magnesium sulfate intravenously for more than 5-7 days. Acenocoumarol is a vitamin K antagonist that is used for the treatment of acquired and congenital, both arterial and venous, thrombotic diseases. Its use is complicated by the narrow therapeutic range. Bleeding following oral anticoagulation, despite rare, remains the major complication. Most cases of hemorrhagic episodes usually require short hospitalization and transfusion, while surgical drainage of the hematoma is not recommended. However, in cases that conservative treatment isn’t successful, surgical intervention remains an option. Background. Nonunion of the lateral humeral condyle are of the complex pathology of the elbow joint, occurring relatively often and resulting in disability of children. The treatment of nonunion of the lateral humeral condyle of humerus with cubitus valgus remains controversial. Purpose of this report was improvement of the results of surgical treatment of the nonunion of the lateral humeral condyle of humerus with cubitus valgus of the lateral humeral condyle of humerus with cubitus valgus with use of differential approach to the surgical strategy. Material and methods. We were observing 28 children (17 boys and 11 girls) with nonunion and longstanding nonunion of lateral condyle of humerus, with various degrees of severity. There were used MRI and Xray investigations for differential study of the patients divided into 3 groups in relation to stability and nonstability of the nonunion of the lateral humeral condyle of humerus with cubitus valgus of the lateral humeral condyle of humerus with cubitus valgus. Results. All 28 patients lateral humeral condyle nonunions with cubitus valgus achieved union within sixty five days after operative procedure using Ilisarovs technique. The mean postoperative humerusulna angle was 6,0 degrees of cubitus valgus. All of reverse Tosteothomies healed uneventfully, and there was no loss of correction postoperatively. The mean duration of followup was 7 years. The overall results were excellent in 15(53,5%) patients, good in 11(39,3%) patients, and fair in 2(7,2%) patients. All 28 patients lateral humeral condyle nonunions with cubitus valgus achieved union within sixty five days after operative procedure using Ilisarovs technique. The significance of intracavitary, intraorgan, hypertension syndrome identification as a novel uniform (model) pattern of a pathological process is substantiated in the in the article based on currently available literature sources and clinical practice experience. A conformity analysis of the considered pathological syndromes and model pathological processes criteria is presented with reference to data on etiology, pathogenesis, anatomical and some ontogenetic suppositions and stipulations. Diabetes 1. typu (DM1) vzniká jako následek autoimunitní destrukce beta buněk pankreatu. Vliv diabetu na celkový výsledek těhotenství je poměrně dobře prostudován. Mnohem méně se toho však ví o vlivu diabetu matky na vyvíjející se imunitní systém plodu. Riziko rozvoje DM1 u dětí diabetických matek (pokud onemocněly před těhotenstvím) je nižší než např. u dětí diabetických otců. Tato diskrepance podporuje teorii, že by mohlo docházet k určitému navození imunotolerance u dětí DM1 matek. Z tohoto důvodu jsme si pro náš výzkum vybrali T regulační buňky (Tregs), které hrají důležitou roli v imunoregulaci a patogenezi mnoha autoimunitních onemocnění. Studie Tregs pupečníkové krve DM1 matek jsou navíc ojedinělé. Pomocí průtokové cytometrie jsme analyzovali zastoupení Tregs (definovaných jako CD45+ CD3+ CD4+ CD25+ CD127(low/-)) ve vzorcích pupečníkové krve od 17 matek s DM1, 17 s gestačním diabetem a 42 zdravých kontrol. Signifikantní rozdíl jsme nalezli jen v zastoupení Tregs z Th lymfocytů, kde v pupečníkové krvi zdravých matek činil tento poměr 9,25 % a u matek s DM1 6,73 % (p=0,043). Ostatní rozdíly nedosáhly signifikance. Pokud tedy dochází k navození specifické imunotolerance, je tato patrně podmíněna jinak než změnou počtu Tregs. Pokračujeme proto ve funkční analýze Tregs s cílem nalézt vysvětlení pro nižší výskyt diabetu u dětí DM1 matek ve srovnání s ostatními prvostupňovými příbuznými DM1 pacientů. Pochopení tohoto jevu by mohlo přispět např. i k vytvoření imunointervenční terapie DM1., Type 1 diabetes (T1D) develops due to autoimmune pancreatic beta cells destruction. The effect of diabetes on pregnancy outcome is well documented. However, much less is known about the effect of maternal diabetes on the developing foetal immune system. The risk of T1D development in a child of T1D mother who was diagnosed prior pregnancy is lower than e.g. in a child of T1D father. This discrepancy supports the theory that there is some immunoregulatory influence on her baby. For this reason we decided to study T regulatory cells (Tregs) which are important in immunoregulation and for autoimmune diseases pathogenesis. Moreover Tregs cord blood (CB) studies of T1D mothers are limited. Tregs defined as CD45+ CD3+ CD4+ CD25+ CD127(low/-) in CB samples were investigated in our study by flow cytometry and 17 T1D mothers, 17 gestation diabetes mothers and 42 healthy controls were enrolled. We found just that CB from babies of T1D mothers contained 6,73 % Tregs/total Th-lymfocytes in comparison to 9,25 % in controls (p = 0,043). Other differences were not significant. If an establishment of specific immunological tolerance in foetus of T1D mother really exists, it seems to be rather due to other factors than just to changes in Tregs count (which was moreover found to be decreased). We are currently focused on explanation why children of T1D mothers suffer from diabetes less frequently than other first degree relatives by using functional analysis of Tregs.
