Angiosarcoma is a soft tissue tumour with a dismal prognosis. We present a 74 year old male presenting with a non healing ulcer on the scalp. On histopathology a diagnosis of angiosarcoma was made. This article presents results of using proton therapy after operation of patient with brain meningioma. After operation brain meningiomas is known to not rarely recur due to certain localization and growth of tumor into main cerebral vessels, nerves and other main brain structures. This creates certain difficulties in operative access and surgical intervention, i.e. tumor resection. Práce prezentuje prenatální diagnostiku, možnosti terapie a následnou péči u plodů a novorozenců s kongenitálním chylothoraxem na podkladě zhodnocení 14 případů na naší klinice v letech 1993 až 2012. Povšechný hydrops plodu byl popsán ve 4 případech (29 %), v 10 případech (71 %) byla provedena odlehčovací amniodrenáž a ve 3 případech (21 %) prenatální odlehčovací punkce hrudníku. Spontánně bylo porozeno 11 plodů (78 %), hrudní drenáž byla nezbytná v průměru 9 dnů po porodu. Mortalita dosáhla v souboru 29 % (4 případy). V článku popisujeme vyšetřovací algoritmy, terapeutické prenatální i postnatální postupy a je zdůrazněna nutnost mezioborové spolupráce., The work presents prenatal diagnosis, treatment and care options in fetuses and neonates with congenital chylothorax based on a detailed evaluation of 14 cases recorded in our department between years 1993 and 2012. Generalized fetal hydrops was described in 29% of cases, therapeutic amnio drainage performed in 71% and prenatal thoracentesis in 21% of cases. 78% of fetuses were delivered spontaneously; thoracic drainage was necessary nine days after birth in average. Mortality reached 29%. An eight-month-old male child was admitted with weakness and swelling in the feet. Paraparesis and bilateral lower extremity edema were present in the neurological examination. Thoracic MRI showed an intradural intramedullary mass 61 × 11 mm in size in the T5–T10 levels. Laminotomy between the T5–T10 vertebrae was performed. A mass with smooth borders was separated from most of the neural tissue. In the postoperative MRI, we observed a contrast enhancing area, considered a residual fragment, only 5 × 4 mm in size. Histopathological properties were compatible with the intermixed subtype of ganglioneuroblastoma. Only a limited number of thoracic cord Ganglioneuroblastoma reports have been previously published. Although very rare in children and young adults, ganglioneuroblastoma should be included in the differential diagnosis of thoracic cord tumors. It is difficult to obtain a preoperative diagnosis with clinical features and radiological investigations. Diagnosis depends on histopathological examinations. Curative treatment should be in the form of a complete resection of the tumor. Hymenální atrézie - potažmo její důsledky - je vzácnou příčinou bolestí v podbřišku u dospívajících dívek, snadno zaměnitelnou za chirurgickou etiologii obtíží. Gynekologické vyšetření dívek a žen před chirurgickou intervencí je nezbytnou součástí diagnostického algoritmu, v případě atrézie hymen tímto lze v některých případech předejít abdominálnímu operačnímu řešení obtíží. Tato kazuistika demonstruje překvapivý nález v dutině břišní u 15leté dívky s bolestmi břicha, primárně chirurgem operované pro diagnózu „akutní apendicitida“. Kromě klinického obrazu jsme v tomto případě byli svědky i poměrně nezvyklého nálezu laboratorního - extrémně vysoké hodnoty CA 125., Hymenal atresia – and its results in fact - is the rare cause of pelvic or abdominal pain in pubescent girls, which could be changed for surgical cause of ailment easily. Gynecologic examination should be a part of a diagnostic algorithm in women and young girls before every surgical intervention - we can avoid an abdominal operation in case of hymenal atresia. We report the case of a 52year old man with severe acute pancreatitis. In this case report we discuss the undesirable effects of parenteral nutrition and the importance of nutritional support at patients with severe necrotizing pancreatitis. Severe acute pancreatitis is usually accompanied by systemic inflammatory response syndrome, which results in hypermetabolism with prominent protein catabolism. Providing nutrition to these patients is of paramount importance. An adequate nutritional support is crucial in patients with severe and complicated pancreatitis. Význam dnešního medicínského práva neustále stoupá, ale oblasti transplantologie není věnován dostatek pozornosti. Samotná nevědomost laické veřejnosti není tolik alarmující jako letitý nezájem a ignorace odborné veřejnosti našimi zákonodárci. Zákon č. 285/2002 Sb., o darování, odběrech a transplantacích tkání a orgánů a o změně některých zákonů nabyl, podle části osmé tohoto zákona, účinnosti dnem 1. září 2002. Od tohoto roku upozorňují lékaři, jak ústně tak i písemně, na velmi problematické ustanovení § 11 odst. 4, transplantačního zákona. Ani dnes, v roce 2011, však nedošlo k žádné změně v jeho znění, a to ani v podobě potenciálního řešení uzavřít mezinárodní smlouvu, které by alespoň z části mohlo přispět ke zlepšení situace. Vhodných orgánů k transplantacím není dostatek a na česká transplantační centra mohou být pak v případě odebrání orgánů od zemřelých cizinců podávány žaloby na porušení zákona, třebaže je v čekací listině několik urgentních čekatelů na transplantaci. O protizákonné jednání by se bohužel jednalo i v těch případech, kdy sama rodina zesnulého vznese dotaz, či požádá o možný odběr vhodných orgánů od svého zesnulého příbuzného, a tak jsou lékaři nuceni jejich přání odmítnout. Ve chvíli, kdy pak ve zdravotnickém zařízení čeká na složitý operační zákrok a záchranu života několik pacientů a lékařům se zároveň dostane svolení od příbuzných potenciálního dárce, dostávají se odborníci do situace, kdy by měli postupovat podle zákona, ovšem na úkor bioetiky a základních lidských práv a svobod. Takové počínání by bylo, dle mého názoru, nejen v rozporu s lékařskou etikou, ale zejména s nejvýše postavenými vnitrostátními zákony jako je Ústava České republiky a Listina základních práv a svobod. Na poli mezinárodního práva by se pak jednalo o přesah principů vyplývajících z Úmluvy na ochranu lidských práv a důstojnosti lidské bytosti v souvislosti s aplikací biologie a medicíny, tedy Úmluvy o lidských právech a biomedicíně, kterou je Česká republika od 1. října 2001 vázána. Ačkoliv je zcela nepochybné, že se transplantačním chirurgům vždy nepodaří zachránit život všem svým pacientům, je důležité, aby se lidé o medicínsko-právní problematiku začali, ve svém vlastním zájmu, více zajímat a nestaly se pouhým davem, jež slepě následuje některá ustanovení popírající etiku a základní lidská práva. Pro všechny tyto důvody se domnívám, že by bylo vhodné co nejdříve učinit kroky vedoucí ke změně transplantačního zákona. Na vybranou se zákonodárcům nabízejí dvě možná řešení, jak tuto kolizní situaci vyřešit. Prvním z nich, a dle mého názoru i vhodnějším, je zrušení ustanovení §11 odst. 4, o odběrech tkání a orgánů od mrtvých cizinců. Druhou možností by pak mohlo být uzavření mezinárodní smlouvy, která by speciálně danou problematiku upravovala. Transplantologie je velice perspektivním oborem, ale také citlivým a snadno zneužitelným v mnoha ohledech. Ať už se jedná o výše zmiňovanou problematiku odběru od mrtvých cizích státních příslušníků, nebo o velmi rozšířenou transplantační turistiku a obchodování s orgány, je nutné učinit mnohé změny a zabezpečení v legislativě. Zdravý člověk si jistě nepřipouští, že by se právě jeho mohla tato problematika jakkoliv dotknout. Je však nutné si uvědomit, že v takovýchto chvílích se nejedná pouze o něj samotného, a to ať už se vyskytne v roli čekatele, nebo případného dárce., Over the last years, the importance of the medical law has been increasing, however to the issue of transplantations among public has not been paid the sufficient attention. But this ignorance is not as alarming as a longtime lack of interest of Czech lawmakers. The Act No. 285/2002 – concerning a donation, removal and transplantation of the tissue and organs (Transplantation Act) – came into being on September 1, 2002. Since that time, physicians have referred to the contradictory provision of § 11, section 4 and also to the fact that this act is not seen as a contribution to a better clearance but rather as an increasing physicians’ uncertainty. Until now, lawmakers have not adjusted its wording, even the form of the international agreement, which would help us to solve this situation. There is no sufficient number of suitable organs and tissues for transplantations. In case of an organs removal from deceased foreigners, Czech Transplantation Centers are sued for law-breaking, although there are candidates for transplantation on a waiting list. It would be considered as an unlawful conduct even in those cases, when family ask for taking suitable organs from their deceased relatives, so that physicians are forced to reject their request. When patients wait for the surgical intervention and donor’s relatives agree with transplantation, medical facilities face the dilemma of following the law but at the expense of bioethics and human rights. This acting would be in a contradiction of the medical ethics and acts, such as the Constitution of Czech Republic and the Charter of Fundamental Rights and Basic Freedoms. In the context of the international law based on the Convention for the Protection of Human Rights and Dignity of the Human Being with regard to the Application of Biology and Medicine – Convention on Human Rights and Biomedicine, there would be an overlap of their principles which Czech Republic is obliged to follow from October 1, 2001. There is no doubt that physicians are not able to save each patient, but it is important to support people to be interested in medical and law issues. I presume that it would be appropriate to take action to change the Transplantation Act as soon as it is possible. From my point of view, there are two ways to solve this situation: to abandon the § 11, section 4 (perhaps the most acceptable), or to enter into the international treaty, which regulates this topic. Transplantology is a very promising field of medicine, but it is exploitable in many ways. Whether it is an issue of taking organs from deceased foreigner, or well-known “transplant tourism” and organ trade, it is necessary to have pillars in the legislative. V tomto textu se zabývám formami zavádění PBSP do individuální práce s klientem v rámci běžné klinicko-psychologické praxe. Reflektuji v něm svou zkušenost s metodou, kterou od roku 2001 praktikuji v její základní podobě, tedy formou pravidelných, dlouhodobých, malých skupin. Vývoj terapeutické technologie PBSP posledních let však umožnil její širší využití také v individuální terapii. Zatímco v prvních desetiletích vývoje metody postupoval terapeutický proces v průběhu sezení rychle a často dramaticky „od těla ke slovu“ (bezprostřední prací s pocity ve speciálních cvičeních), což vyžadovalo téměř bezpodmínečně velmi specificky pojaté prostředí skupiny, dnešní přístup sleduje pozvolnější, jemnější i bezpečnější směr – „od slova k tělu“ (od rozhovoru k tělovým projevům, které význam slova evokuje). K možnostem i atmosféře individuálního psychoterapeutického uspořádání se tak PBSP dostala mnohem blíže. Naléhavá otázka však zůstala: jak vložit do dyády terapeut – klient METODU? Metodu, která není jen funkcí terapeutova myšlení, o něž se klient nemusí v posledku mnoho zajímat, ale která naopak vyžaduje jeho aktivní spoluúčast a nemalou kompetenci, která má velmi nezvyklou vnější podobu, navíc patinu zvláštního slangu?, The topic of the paper is: how to set up PBSP into one to one session format within an ordinary clinical psychology practice. I reflect my own professional experience with the method I have been using for years in its basic group form. However, recent developents of PBSP technology have facilitated it‘s use also in individual psychoteprapy. In the first decades of PBSP developement, the process of therapy was running on ahaed fast, often dramatically, due to body work that had been carried out immediately in special exercises. The current approach has become much slower, subtle and safe and therefore it fits better for the individual setting conditions. However, the basic question stayed remained: how to introduce A METHOD into dyad therapist – client?? V souvislosti s ochranou lidských práv zaručených Úmluvou o ochraně lidských práv a základních svobod se v poslední době čím dál častěji diskutuje otázka, zda přístup k asistované reprodukci také spadá pod rámec ochrany poskytovaný Úmluvou. Příspěvek přináší přehled rozhodnutí Evropského soudu pro lidská práva, která se problematikou asistované reprodukce zabývaly. Na pozadí těchto rozhodnutí se příspěvek snaží analyzovat hranice rozšiřujícího pojetí práva na respektování rodinného a soukromého života podle čl. 8 Úmluvy a poukázat na skutečnost, že v oblasti řešení složitých biomedicínckých a bioetických otázek nemá Soud dostatečnou autoritu na to, aby se stal finálním arbitrem při posuzování hodnotových měřítek společnosti v jednotlivých evropských státech., In the last few decades decided European Court of Human Rights very problematic cases involving “right to access to assisted reproduction”. These cases are concerning very controversial ethical aspects of biomedical developments in the field of medical techniques of artificial procreation and reshape the content (meaning) of Article 8 of European Convention of Human Rights. This paper provides short overview of cases of European Court of Human Rights involving “right to access to assisted reproduction” and focuses especially on the latest developments in this field (case S.H. and Others v. Austria - Application no 57813/00) . Although the technology of medically assisted procreation had been available in Europe for several decades, many of the issues to which it gave rise, remained the subject of debate. There is no consensus towards the use of IVF treatment in general and even less clear are trends in some particular questions (f.e. allowing gamete donation).
