Spánek se významně podílí na celkovém zdraví a kvalitě života člověka, je důležitý nejen pro duševní a fyzickou regeneraci, ale také pro normální funkci metabolických pochodů, endokrinního a imunitního systému. V moderní společnosti dochází k postupnému zkracování délky spánku v důsledku celé řady faktorů. Epidemiologické studie v posledním desetiletí potvrdily, že spánek kratší než 7 hodin představuje rizikový faktor pro rozvoj obezity, diabetu mellitu 2. typu, hypertenze, kardiovaskulárních onemocnění, depresí a zvyšuje riziko mortality. Cílem této práce bylo zhodnotit délku spánku a posoudit vztah mezi délkou spánku a BMI u dospělé populace (n=478, 18–65 let). Metoda sběru dat zahrnovala 7denní záznam o délce spánku za den a antropometrická měření. Z výsledků vyplynulo, že průměrná délka spánku v souboru činila 7,45 (±0,91) hodin za den. 28 % dospělých osob přesto spalo méně než doporučených 7 hodin denně. Statisticky nejméně spali jedinci ve věku 40–49 let (průměr 7,2±0,80 hodin). Riziko obezity definované jako BMI ≥30 kg/m2 v souboru stoupalo se zkracující se délkou spánku: pro kategorie spánku [<6,3 h], [≥6,3 až <8,3 h] a [≥8,3 h] bylo riziko obezity na základě relativních četností vyjádřeno poměrem 2 : 1,5 : 1,2. Jedinci spící průměrně <6,3 hodiny vykazovali 1,3krát vyšší riziko obezity než ostatní jedinci spící déle i při zohlednění faktoru věku, tento závěr však není statisticky významný (OR=1,256; 95% IS: 0,631; 2,422). Z hlediska prevence obezity se dostatečný a kvalitní spánek jeví jako významný ochranný faktor, který by zejména v současné době rychlého životního tempa a při stoupajícím počtu obézních jedinců neměl být, vedle zdravé stravy a dostatečné pohybové aktivity, opomíjen., Sleep plays a significant role in overall health and in the quality of human life. In modern society there is a gradual shortening of the length of sleep due to a number of factors. In the last decade a number of epidemiological studies suggest that sleeping less than 7 hours a day increases the risk of developing obesity, type 2 diabetes mellitus, hypertension, cardiovascular diseases, and depression and is associated with premature mortality. The aim of this study was to evaluate sleep duration and to assess the relationship between sleep duration and BMI in the adult population (n=478, 18 to 65 years). The method of data collection consisted of a 7-day record of the length of the sleep per day and anthropometric measurements. The results showed that the average sleep duration was 7.45 (±0.91) hours per day. However, 28% of adults slept less than a recommended 7 hours a day. The individuals aged 40–49 years slept statistically the shortest time (mean 7.2±0.80 hours). The risk of obesity increased indirectly with duration of sleep in this population, for sleep categories [<6.3 h], [≥6.3 to <8.3 h] and [≥8.3 h] the risk of obesity defined as BMI ≥30 kg/m2 was 2 : 1.5 : 1.2 based on the relative frequencies of obese and non-obese participants. Individuals sleeping on average <6.3 hours were at 1.3 times higher risk of obesity than other individuals sleeping longer after adjusting for age, this conclusion was not statistically significant (OR=1.256, 95% CI: 0.631; 2.422). Few studies concerning the occurrence of cow’s milk allergy with the use of double-blind, placebo controlled food challenge test in adolescents and adult patients suffering from atopic dermatitis exist. Aim: To evaluate the occurrence of cow’s milk allergy in adolescents and adults suffering from atopic dermatitis. Method: Altogether 179 persons suffering from atopic dermatitis were included in the study: 51 men and 128 women entered the study with the average age of 26.2 (s.d. 9.5 years). Complete dermatological and allergological examinations were performed. Results: The positive results in specificIgE and in skin prick tests were recorded in 12% of patients. According to the open exposure tests and double-blind, placebo controlled food challenge tests these patients are only sensitized to cow’s milk without clinical symptoms of allergy. Double-blind, placebo controlled food challenge test confirmed food allergy to cow milk only in one patient (worsening of atopic dermatitis), the oral allergy syndrome was observed in another one patient, occurrence of this allergy was altogether 1.1%. The risk of high-grade lesions and carcinoma is known to correlate with specific high-risk HPV genotypes. The distribution of HPV types varies between countries and little is known about HPV distribution in our country. Therefore, the purpose of this study was to determine the range and frequency of HPV genotypes in studied group of women in Montenegro. HPV genotypes were determined using the method of enzyme restriction of PCR products amplified with group-specific primers MY09/MY11 and restricted with seven different restriction endonucleases. Out of the total number of women HPV infection was found in 1/5 of participants (20%). Genotyping performed in HPV DNA positive women shows that the HPV genotype 16 is dominant and present in more than 1/3 of the participants (36.8%). The second most frequent HPV infection is with HPV genotype 58 and it is found in 10.5% of participants. HPV 31 and HPV 6 infections are present in 7.9% of women, while infections with other genotypes were demonstrated individually by 2.6%. Cíl. Cílem práce bylo zjistit, zda je možné při použití úpravy CT obrazů hybridní iterativní technikou, označovanou iDose, snížit radiační zátěž při CT vyšetřování mozku u dětí a adolescentů o 30 %. Metoda. CT vyšetření mozku bylo provedeno na přístroji CT 64 (Philips Haelthcare) u 40 pacientů různých věkových kategorií. Získaná data byla rekonstruována s různými kernely a následně iterativní technikou iDose (Philips Healthcare, dále ID) různé intenzity (ID30%, ID50%, ID70%) a s přidanou volbou MR (multi-resolution). Takto rekonstruované skeny byly porovnány s obrazy z rutinního vyšetření s konvenční rekonstrukcí FBP (filtered back projection). Hodnocení výsledných obrazů bylo provedeno jednak subjektivně, jednak objektivně. Výsledky. Použití filtrů ID30%, ID50% ID70% sníží šum a potlačí artefakty v obrazech, avšak při intenzitě 70% již snižuje ostrost a zhoršuje podání detailů. Kompenzaci tohoto nežádoucího jevu je možné dosáhnout zapnutím volby MR při rekonstrukci. Kombinace "ostřejšího" kernelu UB a ID rekonstrukce s přidanou volbou MR byla nejlepší pro zlepšení detailu obrazu a celkového užitku pro diagnostiku. Lepších výsledků bylo dosaženo u dospělých, u dětských pacientů byl přínos menší. Po redukci dávky záření při CT vyšetření o 30 % došlo ke zhoršení kvality obrazů, které bylo kompenzováno užitím kombinované rekonstrukce ID70UA + MR nebo ID70UB + MR. Při subjektivním hodnocení výsledných obrazů byla variabilita mezi hodnotícími 1,1-1,3 bodů. Závěr. iDose umožňuje provádět CT vyšetření s redukcí radiační dávky, aniž by došlo ke snížení diagnostické hodnoty obrazů., Aim. To evaluate the possibility of 30% radiation dose reduction using an innovative hybrid iterative reconstruction technique called iDose on image quality of CT brain imagesin children, adolescents and adults. Methods. Brain CT was done in forty patients of various age, scanned on CT64 (Philips Healthcare). Obtained data were reconstructed using various kernels and consequently by the new iDose (Philips Healthcare, NL) hybrid iterative reconstruction prototype on 3 different iD levels (ID30, ID 50, ID70 and multi-resolution /MR/ option). The images were compared to conventional FBP (filtered back projection). The assessment was subjective and objective. Results. Analysis of the variance showed a decrease of noise corresponding to ID levels (Fig. 1) and no differences to MR option. The combination of the filter UA in combination with ID70 led to smoother images but with fewer details, as shown in Fig. 1. An improvement of image quality has been seen when using the MR option. This combination using the higher kernel filters UB showed an improvement of smaller details and higher impact in the adult group while was less pronounced for the children group (Fig. 1). The reduction of dose decreased the image quality in both FBP and ID images, however the ID70UAMR and ID70UBMR reconstructions have compensated for this deterioration. The inter-observer variability was found at 1.1 and 1.3 points respectively. Cílem této pilotní studie bylo srovnat výsledky ze dvou používaných nepřímých metod pro stanovení maximální spotřeby kyslíku (odhadu VO2max) a zhodnotit jejich využitelnost pro posuzování fyzické zdatnosti v rámci primárně preventivního vyšetření. K tomuto srovnání jsme použili Cooperův 12minutový běžecký test (CT 12) na běhátku a stupňovaný zátěžový test na bicyklovém ergometru (BE) do maxima. Naše výsledky ukázaly signifikantně nižší hodnoty odhadu VO2max z CT 12 na běhátku oproti stupňovanému zátěžovému testu na BE do maxima napříč různými skupinami populace. V celkovém souboru byly tyto hodnoty v průměru o 8,6 % (p<0,001) nižší. Rozdíly v odhadech VO2max se lišily svojí mírou v jednotlivých skupinách podle trénovanosti a převažujícího typu pohybové aktivity; největší rozdíly mezi oběma testy byly u cyklistů (-12,0 %), studentů medicíny (-10,9 %) a "nevyhraněných" (-10,4 %) a u těchto skupin dosáhly rovněž statistické významnosti (p<0,01). Podstatně nižší rozdíly byly naopak u běžců (-4,6 %, statisticky nevýznamné) a ve skupině triatlonistů prakticky žádné rozdíly nebyly. Výsledky a pozorování z tohoto pilotního testování naznačují, že CT 12 na běhátku je oproti stupňovanému testu na BE do maxima podstatně náchylnější k vlivům jako trénovanost, převládající typ pohybové aktivity, motivace apod. Po celkovém zhodnocení výsledků se nám pro vyšetření fyzické zdatnosti v rámci primárně preventivního vyšetření u běžné populace s průměrnou, ale i mírně nadprůměrnou fyzickou zdatností jako vhodnější jeví stupňovaný zátěžový test na BE do maxima., The aim of this pilot study was to compare the results of two commonly used indirect methods of assessing maximum oxygen uptake (VO2max estimation) and to assess the applicability of these two methods for physical fitness assessment of an individual as part of primary preventive examination. For this comparison we selected the Cooper run/walk test (CT 12) on a treadmill and an incremental maximum test on a bicycle ergometer (BE). Our results showed significantly lower VO2max estimations gained from CT 12 on a treadmill in comparison to incremental maximum testing on a BE in the population groups examined. In total the results were lower by about 8.6% on average (p<0.001). The differences varied in the groups according to how trained the subjects were and according to the prevailing type of their physical activity. The highest differences between the two tests appeared in the group of cyclists (-12.0%), medical students (-10.9%) and in the group of subjects without a preferred type of physical activity. In these groups the differences were also statistically significant (p<0.01). They were less substantial in the group of runners (-4.6%, statistically insignificant), and in the group of triathletes there were practically no differences. This suggests that a CT 12 on a treadmill is much more susceptible to influences such as training experience, prevailing type of physical activity and motivation compared with an incremental maximum test on a BE.
