Objectives: The aim of this study was to elucidate the role of dopamine receptor D2 / ankyrin repeat and protein kinase domain containing 1 (DRD2/ANKK1) TaqIA allelic polymorphism in the HPVinduced cervical carcinogenesis. Methods: 1. Effect on the risk of cervical precancer: After an 8year followup, out of 214 women with persisting highrisk HPV infection, 102 developed highgrade cervical dysplasia or cervical intraepithelial neoplasia (CIN) grade III, while 112 did not. The subjects were genotyped for the DRD2/ANKK1 TaqIA polymorphism by PCRRFLP, and the allelic distributions were compared between groups with and without highgrade dysplasia. 2. Prognostic value: Two hundred and thirty nine women with cervical precancer/cancer were followed for 5 years. Complete remission was achieved at 182 women. To assess the prognostic value of the TaqIA polymorphism, genotype frequencies were compared between patients reaching and not reaching complete remission. Results: The frequency of A1/A1+A1/A2 genotypes was higher among women who developed highgrade cervical dysplasia (OR: 1.87, 95% CI: 1.053.33; p=0.034) than in the other group. Occurrence of the A1 allele was more frequent among women who did not reach complete remission (OR: 2.00, 95% CI: 1.073.74; p=0.030) than in women with complete remission. Conclusions: This is the first report on the possible involvement of DRD2/ANKK1 TaqIA polymorphism in cervical carcinogenesis. The A1 allele seems to increase the risk of cervical precancer, and it may also be associated with a worse prognosis in women with HPVinduced cervical cancer. The risk of high-grade lesions and carcinoma is known to correlate with specific high-risk HPV genotypes. The distribution of HPV types varies between countries and little is known about HPV distribution in our country. Therefore, the purpose of this study was to determine the range and frequency of HPV genotypes in studied group of women in Montenegro. HPV genotypes were determined using the method of enzyme restriction of PCR products amplified with group-specific primers MY09/MY11 and restricted with seven different restriction endonucleases. Out of the total number of women HPV infection was found in 1/5 of participants (20%). Genotyping performed in HPV DNA positive women shows that the HPV genotype 16 is dominant and present in more than 1/3 of the participants (36.8%). The second most frequent HPV infection is with HPV genotype 58 and it is found in 10.5% of participants. HPV 31 and HPV 6 infections are present in 7.9% of women, while infections with other genotypes were demonstrated individually by 2.6%. Condylomata acuminata představují významný epidemiologický problém, a to zejména v teenagerovské populaci. Podstatou onemocnění je infekce tzv. lidským papilomavirem – HPV. Lidský papilomavirus neumíme léčit, proto terapie kondylomat většinou probíhá pouze formou lokální destrukce bradavic a nijak neřeší kauzální podstatu onemocnění. I samotné odstranění kondylomat, zejména při větším rozsahu onemocnění, může být obtížné a zdlouhavé, stejně tak jako i následné hojení pacienta, nezřídka komplikované časnými recidivami. Jakýkoliv nový preparát, který rozšíří možnosti terapie tohoto nepříjemného onemocnění, je velkým přínosem. Celkově působící imunomodulační preparát Isoprinosine je z tohoto pohledu velká naděje pro pacienty, a to zejména pro jeho systémové působení s efektem na všechny kožní a slizniční povrchy, pro jeho šetrnost vůči pacientovi, ale především pro jeho předpokládanou schopnost snížit procento recidiv. Na ambulanci onkogynekologické prevence při porodnicko-gynekologickém oddělení Městské nemocnice Ostrava a jejím přidruženém laserovém pracovišti jsme sledovali efekt léčby Isoprinosinem v klinické praxi a výsledky jsou velmi nadějné. Na základě našich pozorování se rovněž snažíme dát návod, jak lék používat, v jaké dávce a jak dlouho. Pro zajímavost uvádíme 1 kazuistiku i s fotodokumentací, v níž jsme léčbu Isoprinosinem využili s velmi dobrým efektem., Condylomata acuminata present a considerable epidemiological problem, particularly in a teenage population. The illness develops through the infection by a human papillomavirus- HPV. We are unable to treat the human papillomavirus, therefore the treatment of anogenital warts largely only consists in local destruction of warts and does not treat the cause of the illness. The removing of warts itself, in particular when the affected site is larger-sized, can be difficult and protracted, as well as subsequent healing of the patient, often complicated by recurrences. Any new preparation that broadens the possibilities of treatment of this obtrusive illness means a great benefit. In this regard the overall effect of the immunomodulation preparation Isoprinosine is a great hope for patients, mainly due to its systemic effect on all cutaneous and mucosal surfaces, its patient friendliness, but mainly due to its assumed ability to reduce the percent of recurrence. At the outpatients‘ department of oncogynecological prevention affiliated to the Department of Obstetrics and Gynecology of the Ostrava City Hospital and its Laser centre, we followed the effect of the treatment with Isoprinosine in clinical practice and the results are very encouraging. Based on our observations we also try to provide guidance on how to use the medicine, on its dosing and length of application.
