Cíl. Pseudomyxom peritonea je velmi vzácné semimaligní onemocnění. Záměrem našeho sdělení je pojednání o typickém nálezu, možnostech léčby a komplikacích. Metoda. Nedávné případy z našeho pracoviště ilustrují patognomické znaky nemoci v CT obrazu, obvyklý klinický nález, průběh onemocnění i svízelnost jeho léčby. Ve dvou našich kazuistikách je příčinou pseudomyxomu peritonea mucinózní cystadenom appendixu, v jednom appendikální low grade cystadenokarcinom, v posledním případě mucinózní cystadenokarcinom neznámého původu. U prvního pacienta se navíc objevila raritní komplikace - metastatická ložiska intrathorakálně. Závěr. Pseudomyxom peritonea je klinický syndrom, který je způsoben excesivní produkcí gelatinózního ascitu buňkami mucinózního cystadenomu či cystadenokarcinomu různého původu, nejčastěji se však jedná o tumory appendixu. Typický morfologický nález v zobrazovacích metodách určuje jejich významné místo v diagnostice, stagingu a monitorování výsledků léčby pseudomyxomu., Aim. Pseudomyxoma peritonei is an uncommon semimalignous disease. The aim of our statement is the discussion about the typical finding, the possibilities of treatment and complications. Method. The recent findings from our workplace illustrate pathological signs of the disease on CT-scan, usual clinical finding, the progression of the disease and the arduous of its treatment. In two of our cases is the cause of pseudomyxoma peritonei mucinous cystadenoma of the appendix, in one low grade mucinous cystadenocarcinoma and in the last case the mucinuos cystadenoma of unknown origin. There has appeared by the first patient a rare complication - intrathoracic implants lesions. Conclusion. Pseudomyxoma peritonei is a clinical syndrome that is caused by the excessive production of gelatinous ascites cells mucinous cystadenoma or cystadeno-karcinoma of different origin, but mostly these are the cases of appendix tumors. Multiple sclerosis (MS) is the most common cause of nontraumatic neurological disability in Europe and North America. Growth factor expression could participate in the repair process of the demyelinating disease. Among growth factors, brain derived neurotrophic factors (BDNF) has been demonstrated to play an important role in neuronal and axonal survival. In the central nervous system (CNS ), neurons are the main source of BDNF. Another potential source are activated astrocytes, which are present in inflamed areas in the CNS as shown in MS. In this study, total protein concentration (TPC) and BDNF levels in the cerebrospinal fluid (CS F) samples from the patients with MS (n = 48) and control subjects (n = 53) were measured using a Bio-Rad protein assay and enzyme linked immunosorbent assay (ELISA). No significant change in the CS F TPC of patients with MS was seen as compared to normal CS F. The presence of BDNF in the CS F samples was shown by Western blot. Using ELISA , it was shown that the level of BDNF in the MS CS F is higher than in normal CS F. It is concluded that BDNF is a constant component of human CS F. Kostní metastázy jsou častou příčinou morbidity i mortality u pacientů s kastračně rezistentním karcinomem prostaty. Dosud byla léčba omezena na docetaxel jako standardní první linie chemoterapie. We report the case of a 22-year old male, a self-confessed recreational drug user who developed cardiogenic shock because of severe destruction of the aortic valve by rapidly progressive aortic valve endocarditis. The disease progression was acute; in a matter of days, the clinical manifestations were life-threatening necessitating urgent aortic valve replacement surgery. Cultivation revealed Streptococcus viridans as the microbial agent. Subsequent recovery with antibiotic treatment was without complication. This case report shows that immediately performed transoesophageal echocardiography and early consultation with a cardiac surgeon has fundamental importance in diagnosis and management of acute infective endocarditis in haemodynamically instable patients., Malkia S. Cíl. Zhodnotit možnosti hybridního zobrazení a frekvenci používaných výkonů v detekci okultního muskuloskeletálního pyogenního zánětu jako příčiny sepse. Metoda. Retrospektivně byly zhodnocena hybridní zobrazení indikovaná během 3 let k detekci příčiny septického stavu nejasného původu, vyšetření byla vyhledána mezi všemi provedenými 18F-FDG-PET/CT a SPECT/CT. Výsledky. Bylo provedeno celkem 27 vyšetření 18F-FDG-PET/CT z indikace sepse nejasného původu, kdy v šesti případech byla nalezena spondylodiscitida, pyogenní artritida ve třech případech, svalový absces ve dvou případech a infekce endoprotézy ve čtyřech případech. Muskuloskeletální příčina sepse tvořila 55,5 % případů. Bylo provedeno celkem 16 vyšetření SPECT/CT, kdy v deseti případech byly použity techneciem značené fragmenty myších protilátek a v šesti případech leukocyty značené 99mTc-HMPAO. Ve dvou případech byla prokázána akutní spondylodiscitida, v jednom případě chronická spondylitida a v jednom případě mnohočetné abscesy svalů původem v infekci endoprotézy. Muskuloskeletální příčina tvořila 25 % nálezů. Závěr. Za dobu současného používání PET/CT a SPECT/CT v detekci okultních infekcí tvoří větší díl vyšetření pomocí 18F-FDG-PET/CT, neboť nabízí vícečetné výhody před zobrazeními leukocytárními., Aim. To asses a role and frequency of indications of hybrid imaging methods in the detection of occult pyogenous musculoskeletal inflammation as a cause of the septic state of unknown origin. Method. The retrospective evaluation of the hybrid imaging indicated due a sepsis of unknown origin during three years was performed. All procedures of 18F-FDG-PET/CT and the SPECT/CT using labeled leucocytes were enrolled into the assessment. Results. 27 procedures of 18F-FDG-PET/ CT were performed due to the searching of the cause in cases of the sepsis of unknown origin; the spondylodiscitis was found in 6 cases, pyogenous arthritis in 3 cases, muscular abscesses in 2 cases and 4 cases of pyogenous infection of implanted joint prosthesis. Muskuloskeletal inflammations formed 55,5% of all findings. 16 procedures of SPECT/CT were performed, 10 using 99mTc-labeled antigranulocyte antibodies, and 6 using 99mTc-HMPAO labeled leukocytes. In 2 cases, the acute spondylodiscitis was found, one case of chronic spon-dylitis and one case of soft tissue abscesses due to the infection of endoprosthesis were found. Muskuloskeletal inflammations formed 25% of all findings. Conclusion. Here we report the case of a 31-year-old man admitted to our hospital with echocardiografic and Cardiac Magnetic Resonance signs of myocarditis complicated by ventricular tachycardia, initially resolved with direct current shock. After the recurrence of ventricular tachycardia the patient was submitted to electrophysiological study revealing a re-entrant circuit at the level of the medium segment of interventricular septum, successfully treated with transcatheter ablation. This case highlights how the presence of recurrent ventricular arrhythmias at the onset of acute myocarditis, suspected or proven, could be associated with a pre-existing arrhythmogenic substrate, therefore these patients should be submitted to electrophysiological study in order to rule out the presence of arrhythmogenic focuses that can be treated with transcatheter ablation., Emanuele Cecchi, Serena Fatucchi, Elena Crudeli, Cristina Giglioli, and Literatura 15
Latitude gradients and secular trends in Europe and North America have been found in the male-female ratio at birth (M/F: male births divided by total births) which is expected to be 0.515. Annual national data for Czechoslovakia and the post-Czechoslovakian (Czech Republic and Slovakia) countries for male and female live births were obtained from the World Health Organisation and analysed with contingency tables. This study analysed 13,123,538 live births. An overall decreasing trend in M/F was found (p < 00001). No latitude gradient was noted. There was an overall deficit of 15,232 male births based on an M/F of 0.515. M/F is declining in this region, despite well developing economies that have resisted the worldwide slowdown. Východiska: 18F-FDG PET/CT je součástí vstupní diagnostiky a sledování léčebné odpovědi u pacientů se seminomem. Diseminace seminomu mimo spádové paraaortální lymfatické uzliny bývá nejčastější do plic, uzlin mediastina a nadklíčku, méně často do dalších orgánů (játra, mozek). Koincidence se sarkoidózou je v literatuře popisována jako možná komplikace diagnostiky i hodnocení léčby. Kazuistika: Naše kazuistika zahrnuje 2 pacienty s tumorem varlete, klasickým seminomem, pT1, s hladinou tumor markerů v normě. U obou byla na 18F-FDG PET/CT vyšetření patrná metabolicky vysoce aktivní lymfadenopatie mediastina a plicních hilů bez známek postižení retroperitonea. U prvního z pacientů (42 let) nebyla zpočátku histologická verifikace z mediastina z technických důvodů doporučena, pacient absolvoval onkologickou léčbu s efektem parciální regrese po 1. linii CHT a progrese při 2. linii. Poté následovala mediastinoskopie s histologickým nálezem granulomatózního zánětu, sarkoidózy. U druhého pacienta (45 let) bylo k histologické verifikaci nálezu v mediastinu přistoupeno již iniciálně, byla zjištěna sarkoidní granulomatóza. Na kontrolním 18F-FDG PET/CT byla zachycena ojedinělá patologická ložiska v RP i ve skeletu pánve a páteře. Tato ale hodnocena jako progrese sarkoidózy, progrese seminomu méně pravděpodobná, sledování pacienta dále pokračuje. Závěr: Jak je z vyšetření a sledování těchto pacientů patrné, při nálezu ne zcela typického obrazu diseminace tumoru je krom této diseminace nutné zvažovat i jiné příčiny patologického obrazu. V případě sarkoidózy je její relativně častější výskyt uváděn i spolu s karcinomy plic či lymfomy.
