Alzheimerova choroba (ACH) je nejčastější forma demence. Jedná se o degenerativní, nevyléčitelné, terminální onemocnění. Postihuje až 75 % nemocných na celém světě postižených demencí. Prevalence tohoto onemocnění je vysoká, etiologie stále není známá. Je sledována řada rizikových faktorů. Pilotní studie projektu Epidemiologie a genetika Alzheimerovy choroby s 334 případy a 102 kontrolami z psychiatrických léčeben přináší následující výsledky z dotazníkového šetření a stěru z bukální sliznice. Dotazník se skládá z otázek zaměřených na základní identifikační údaje, vzdělání, životní styl, vaskulární rizikové faktory, zájmy, zaměstnání, výskyt nemoci v rodině, výsledky Minimental State Examination (MMSE). Dále jsou uvedeny výsledky analýzy polymorfizmů genu pro angiotenzinkonvertázu (ACE). Ve výsledcích je naznačeno, že u pacientů s ACH se v anamnéze častěji vyskytuje kardiovaskulární onemocnění a úraz hlavy. Výsledky analýz vztahu polymorfizmu I/D genu pro ACE k Alzheimerově chorobě odpovídají dosud publikovaným výsledkům, tzn., že genotypy II a ID mají vztah k riziku ACH, i když zatím nebyl zjištěn statisticky významný rozdíl ve sledovaných skupinách respondentů., Alzheimer's disease (AD) is the most common form of dementia. The questionnaires contain questions about identification, education, lifestyle, vascular risk factors, hobbies, occupation, incidence of the disease in the family, results of Minimental State Examination (MMSE).Results of the analysis of the gene of I/D polymorphism of ACE are presented. Results suggest that persons with AD have often a cardiovascular disease and head injury in their case history. Cíl studie: Cílem studie bylo analyzovat životní styl studentů lékařské fakulty v dimenzi I. (rizikové chování – výživa, spánek, tělesná hmotnost, kouření, stres), dimenzi II. (volný čas, studium) a v dimenzi III. (sport a pohybové aktivity) a srovnat jednotlivé parametry u studentů prvního a pátého ročníku. Metodika: Výzkumu se zúčastnilo 90 studentů 1. ročníku a 90 studentů 5. ročníku, průměrný věk respondentů byl 19 a 23 let. K získání důležitých anamnestických dat byl používán předem sestavený dotazník. Výsledky: V 1. ročníku bylo signifikantně více studentů s BMI≤18,5 kg.m-2 (14,4 % vs. 2,2 % v 5. ročníku, p=0,003). V 5. ročníku bylo signifikantně více studentů s BMI mezi 25–30 kg.m-2 (11,1 % vs. 3,4 % v 1. ročníku, p=0,003). U studentek 5. ročníku byla zjištěna signifikantně vyšší hodnota BMI než u studentek 1. ročníku (medián 21,2 kg.m-2 vs. 20,1 kg.m-2, p=0,001), stejně jako u mužů studentů 5. ročníku (medián 23,7 kg.m-2 vs. 22,4 kg.m-2, p=0,029). V souvislosti se studiem na LF došlo ke zvýšení tělesné hmotnosti u 52,2 % studentů 5. ročníku a 23,9 % studentů 1. ročníku, p=0,002. Studenti 1. ročníku se cítí více ve stresu (78,9 % vs. 38,4 %, p<0,001), spí denně pouze 4–6 hodin (31,1 % vs. 17,8 %, p<0,001), méně studentů (17,8 %) užívá pravidelně léky (vs. 55,6 % 5. ročník, p<0,001). Studenti prvního ročníku mají méně volného času než studenti 5. ročníku (medián 14 hodin/týden vs. 20 hodin/týden, p<0,001) a významně méně cvičí pravidelně každý týden (41,1 % vs. 58,9 %, p=0,025). Signifikantně více studentů 5. ročníku pije kávu (81,1 % vs. 63,3 %, p=0,012), studenti častěji zažili v průběhu studia na LF záchvat úzkosti související se školou (81,1 % vs. 61,1 %, p=0,005), spí častěji pravidelně po návratu ze školy (43,3 % vs. 22,2 %, p=0,004). Více studentů 5. ročníku má vlastní zkušenost s užíváním antidepresiv (10,0 % vs. 1,1 %, tj. 1 student 1. ročníku, p=0,018). Závěr: Studenti lékařské fakulty mají vysoký výskyt některých ovlivnitelných rizikových faktorů kardiovaskulárních onemocnění., Aim: The study aimed at analyzing the lifestyle of medical students in dimensions I (risk behaviors – nutrition, sleep, body weight, smoking, stress), II (leisure time, studies) and III (sports and physical activities), and comparing individual parameters in first- and fifth-year students. Methods: 90 first-year students and 90 fifth-year students participated in the research, the average age of the respondents was 19 and 23 years. A questionnaire was used to obtain important anamnestic data. Results: There were significantly more students with BMI≤18.5 kg.m-2 in the first-year (14.4% vs. 2.2% in year 5, p=0.003). In the fifth-year, significantly more students had BMI between 25 and 30 kg.m-2 (11.1% vs. 3.4% in year 1, p=0.003). Both female and male fifth-year students had significantly higher BMI than their female and male first-year counterparts (median 21.2 kg.m-2 vs. 20.1 kg.m-2, p=0.001 and 23.7 kg.m-2 vs. 22.4 kg.m-2, p=0.029, respectively). A total of 52.2% of fifth-year students and 23.9% of first-year students (p=0.002) increased their body weight in association with medical studies. First-year students feel stressed more often (78.9% vs. 38.4%, p<0.001) and sleep for only 4–6 hours (31.1% vs. 17.8%, p<0.001) but take regular medicines less often (17.8% vs. 55.6%, p<0.001). They also have less leisure time (median 14 hours/week vs. 20 hours/week, p<0.001) and significantly less regular weekly physical activity (41.1% vs. 58.9%, p=0.025) than students in the fifth-year. Plicní aspergilóza patří mezi zánětlivá onemocnění způsobená houbami rodu Aspergillus. V závislosti na stavu imunitního systému a plicního parenchymu nemocného existují invazivní, semi-invazivní a neinvazivní formy onemocnění. V diagnostice tohoto onemocnění hrají hlavní úlohu prostý rentgenový snímek hrudníku a HRCT plic. Bisphosphonates (BP) are potent inhibitors of bone resorption used mainly in the treatment of metastatic bone disease and osteoporosis. By inhibiting bone resorption, they prevent complications as pathological fracture, pain, tumor-induced hypercalcemia. Even though patient’s benefit of BP therapy is huge, various side effects may develop. Bisphosphonate-related osteonecrosis of the jaws (BRONJ) is among the most serious ones. Oncologic patients receiving high doses of BP intravenously are at high risk of BRONJ development. BPs impair bone turnover leading to compromised bone healing which may result in the exposure of necrotic bone in the oral cavity frequently following tooth extraction or trauma of the oral mucosa. Frank bone exposure may be complicated by secondary infection leading to osteomyelitis development with various symptoms and radiological findings. In the management of BRONJ, conservative therapy aiming to reduce the symptoms plays the main role. In patients with extensive bone involvement resective surgery may lead to complete recovery, provided that the procedure is correctly indicated. Since the treatment of BRONJ is difficult, prevention is the main goal. Therefore in high risk patients dental preventive measures should be taken prior to bisphosphonate administration. Chování tvořící životní styl je formováno faktory environmentálními, psychosociálními i genetickými, které vzájemně interagují a mají potenciál modifikovat genetické riziko komplexních chorob ovlivněním genové exprese. Biologické cirkadiánní hodiny mají všichni obratlovci, včetně člověka. Jejich receptory jsou na oční sítnici, informace o střídání dne a noci jsou vedeny do centra v hypothalamu a odtud do epifýzy. Ta produkuje hormon melatonin (N-acetyl-5-metoxytryptamin), téměř výhradně v období tmavé části dne. Hormon je rychle vyplavován do krve a mozkomíšního moku a je bezprostředním nosičem informace o fázi cirkadiánního cyklu buňkám celého těla, které podřizují fyziologické funkce orgánů a organismu střídání dne a noci. Podle dosavadních výzkumů se melatonin účastní řady významných fyziologických funkcí: regulace spánku a cirkadiánních rytmů, nálady, imunitní, onkostatické a antioxidační aktivity. Syntéza melatoninu klesá s věkem. Umělé osvětlení večer nebo brzy ráno zkracuje tmavou část dne, také jen krátkodobé rozsvícení v noci redukuje množství melatoninu produkovaného během 24 hodin. Syntézu melatoninu významně narušuje letecký přesun přes několik časových zón. Zdravotní následky narušení biologických rytmů zahrnují poruchy spánku, zvýšený výskyt některých zhoubných nádorů, obezity, metabolického syndromu, diabetu, zhoršení projevů některých neurodegenerativních onemocnění. V primární prevenci je třeba omezovat noční směny a edukovat veřejnost o významu nočního spánku., All vertebrates, including humans, have a biological circadian clock. Their receptors are located on the retina, and information regarding the alternation of day and night are transferred to a centre in the hypothalamus and from there into the pineal gland which produces melatonin (N-acetyl-5-metoxytryptamin), almost exclusively during the hours of darkness. The hormone is quickly discharged into the bloodstream and cerebrospinal fluid into all cells. Such immediate information about the circadian phase of day/night changes allows to organs and organism to regulate their physiological functions. Current research suggests that melatonin is involved in several important physiological functions: sleep regulation and circadian rhythms, mood, immunity, oncostatic and antioxidant activities. Synthesis of melatonin reduces with age. Artificial light in the evenings and early mornings reduces the dark part of the day and even brief use of artificial light at night reduces the levels of melatonin produced over a 24-hour period. Air transport over a number of time-zones likewise significantly disrupts melatonin production. The health consequences of the disruption of biologcal rhythms include sleep disorders, higher frequency of some types of cancer, obesity, metabolic syndrome, diabetes and aggravation of some neurodegenerative symptoms. Riziko CNS progrese u vysoce agresivních lymfomů (lymfoblastový a Burkittův lymfom) dosahovalo v historických souborech až 30%, podařilo se jej však výrazně snížit pomocí intratekální profylaktické terapie a inkorporací systémových cytostatik prostupujících hematoencefalickou bariéru do léčebných protokolů. U ostatních agresivních lymfomů je riziko CNS progrese nižší (pod 10 % v recentních studiích), avšak u vybrané skupiny nemocných se může i zde pohybovat kolem 25–30 %. Bohužel, na rozdíl od lymfoblastového a Burkittova lymfomu se u jiných agresivních histologií nepodařilo profylaktické úspěchy zopakovat. Tento článek se zabývá incidencí, rizikovými faktory a možnostmi profylaxe CNS relapsu u imunokompetentních pacientů s difuzním velkobuněčným B-lymfomem, lymfomem z buněk pláště, periferními T-lymfomy a primárním mediastinálním B-lymfomem. Zmíněny jsou rovněž možnosti terapie CNS progrese, pokud nastane., The risk of CNS progression in highly aggressive lymphomas (lymphoblastic, Burkitt‘s) reached up to 30% in historical cohorts, but it was significantly diminished by intrathecal prophylactic treatment and by incorporation of systemic cytostatics penetrating blood-brain barrier into treatment protocols. In other aggressive lymphomas, the risk of CNS progression is lower (under 10% in recent studies), but in selected patients can even here reach 25–30 %. Unfortunately, unlike in lymphoblastic and Burkitt‘s lymphomas, prophylaxis does not seem to be particularly effective in other histologies. Hlavní příčiny morbidity a mortality u polycythemie vera (PV) a esenciální trombocytemie (ET) jsou trombóza a krvácení, progrese do myelofibrózy a přechod do akutní leukemie. Myelosupresivní léčba, především pak hydroxyurea, sice může četnost cévních komplikací omezit, ale u tohoto přípravku existují jisté obavy stran zvýšeného výskytu leukemické transformace. Léčba těchto poruch proto představuje náročný problém a abychom se vyhnuli neadekvátní expozici cytotoxickým léčivům na straně jedné či suboptimální léčbě na straně druhé, měli bychom dodržovat „risk-oriented” léčebnou strategii. Mezi známé rizikové faktory kardiovaskulárních příhod patří vyšší věk a předchozí trombóza, zatímco vliv nových biologických faktorů včetně leukocytózy a mutačního statutu, popř. mutační zátěže JAK2V617F je předmětem bádání. Nízkorizikoví pacienti s PV by měli být léčeni pouze flebotomií a aspirinem, zatímco vysoce rizikoví pacienti by měli rovněž podstoupit cytotoxickou léčbu. Co se týká léčby ET, u nízkorizikových pacientů jednoznačné indikace pro intervenci neexistují, nicméně vysoce rizikoví pacienti by měli podstoupit chemoterapii. U vybraných pacientů včetně pacientů s rezistencí nebo intolerancí na hydroxyureu se mohou uplatnit i jiné léčebné možnosti, jako např. interferon alfa či anagrelid., Major causes of morbidity and mortality in polycythemia vera (PV) and essential thrombocythemia (ET) are represented by thrombosis and bleeding, progression to myelofibrosis and transformation to acute leukemia. Myelosuppressive therapy, preferentially with hydroxyurea, can reduce the rate of vascular complications, but there is some concern about an increased rate of leukemic transformation with this agent. Therefore, the management of these disorders poses a significant challenge, and a risk-oriented therapeutic approach should be followed to avoid inappropriate exposure to cytotoxic drugs on one side or suboptimal treatment on the other. Established risk factors for cardiovascular events are represented by older age and previous thrombosis, while the impact of novel biological factors, including leukocytosis and JAK2V617F mutational status and/or mutational burden, is under investigation. Low-risk PV patients should be managed only with phlebotomy and aspirin, while high-risk patients should also receive cytotoxic therapy. Regarding the management of ET, there is no clear indication for intervention in low-risk patients, while high-risk patients should be managed with chemotherapy. Spánek se významně podílí na celkovém zdraví a kvalitě života člověka, je důležitý nejen pro duševní a fyzickou regeneraci, ale také pro normální funkci metabolických pochodů, endokrinního a imunitního systému. V moderní společnosti dochází k postupnému zkracování délky spánku v důsledku celé řady faktorů. Epidemiologické studie v posledním desetiletí potvrdily, že spánek kratší než 7 hodin představuje rizikový faktor pro rozvoj obezity, diabetu mellitu 2. typu, hypertenze, kardiovaskulárních onemocnění, depresí a zvyšuje riziko mortality. Cílem této práce bylo zhodnotit délku spánku a posoudit vztah mezi délkou spánku a BMI u dospělé populace (n=478, 18–65 let). Metoda sběru dat zahrnovala 7denní záznam o délce spánku za den a antropometrická měření. Z výsledků vyplynulo, že průměrná délka spánku v souboru činila 7,45 (±0,91) hodin za den. 28 % dospělých osob přesto spalo méně než doporučených 7 hodin denně. Statisticky nejméně spali jedinci ve věku 40–49 let (průměr 7,2±0,80 hodin). Riziko obezity definované jako BMI ≥30 kg/m2 v souboru stoupalo se zkracující se délkou spánku: pro kategorie spánku [<6,3 h], [≥6,3 až <8,3 h] a [≥8,3 h] bylo riziko obezity na základě relativních četností vyjádřeno poměrem 2 : 1,5 : 1,2. Jedinci spící průměrně <6,3 hodiny vykazovali 1,3krát vyšší riziko obezity než ostatní jedinci spící déle i při zohlednění faktoru věku, tento závěr však není statisticky významný (OR=1,256; 95% IS: 0,631; 2,422). In modern society there is a gradual shortening of the length of sleep due to a number of factors. In the last decade a number of epidemiological studies suggest that sleeping less than 7 hours a day increases the risk of developing obesity, type 2 diabetes mellitus, hypertension, cardiovascular diseases, and depression and is associated with premature mortality. The aim of this study was to evaluate sleep duration and to assess the relationship between sleep duration and BMI in the adult population (n=478, 18 to 65 years). The method of data collection consisted of a 7-day record of the length of the sleep per day and anthropometric measurements. The results showed that the average sleep duration was 7.45 (±0.91) hours per day. However, 28% of adults slept less than a recommended 7 hours a day. The individuals aged 40–49 years slept statistically the shortest time (mean 7.2±0.80 hours). The risk of obesity increased indirectly with duration of sleep in this population, for sleep categories [<6.3 h], [≥6.3 to <8.3 h] and [≥8.3 h] the risk of obesity defined as BMI ≥30 kg/m2 was 2 : 1.5 : 1.2 based on the relative frequencies of obese and non-obese participants. Infekce střevního traktu vyvolané C. difficile (CDI) jsou jednou z nejčastějších nozokomiálních nákaz. Toto onemocnění nejčastěji postihuje starší, imunodeficitní a polymorbidní pacienty. Mezi nejvýznamnější rizikové faktory vzniku onemocnění je řazeno užívání antibiotik, hospitalizace a anamnéza již proběhlé klostridiové enterokolitidy. I když se v evropských nemocnicích hypervirulentní typ Clostridium difficile vyskytuje, v České republice se jeho výskyt zatím nepotvrdil. Přestože v řadě evropských zemí již surveillance klostridiových kolitid funguje, tak v České republice souvislé sledování jednotlivých případů CDI chybí (hlášeny jsou pouze epidemie), což je spojeno s absencí validních informací o incidenci a struktuře ribotypů a následnou možností tlumení jejího výskytu. Záludnost Clostridium difficile spočívá jak v jeho sporulogenitě a dlouhodobém přežívání v prostředí, tak v jeho odolnosti vůči většině dezinfekčních prostředků a antimikrobiálních preparátů. V současné době máme metody k účinnému zabránění šíření onemocnění, ne však k jeho vzniku. V přehledové práci jsou obecně shrnuty dosavadní znalosti z oblasti epidemiologie, klinického obrazu, rizikových faktorů vzniku a rekurencí onemocnění a možnosti léčby., Intestinal infection caused by Clostridium difficile (CDI) is one of the most common nosocomial infections. Although a number of European countries have surveillance programmes on clostridial colitis, in the Czech Republic continuous monitoring data of individual cases of CDI are missing (reported are only epidemics), which is associated with the absence of valid information on the incidence and patterns of ribotypes and the possibility to combat it. The guile of Clostridium difficile is spore formation and survival after a long time in the environtment. C. difficile is resistant to most disinfectants and antimicrobials. Currently, we know methods for the effective prevention of the sprad, but not to the disease.
