To study efficiency of the combined treatment of advanced gastric cancer with inclusion Trastuzumab. Material: We present the intermediate analysis of the use of target therapy with Trastuzumab in patients with a HER2positive gastric cancer. Up to 01.10.2009y 118 patients have been tested for HER2 expression, and in 24 gastric cancer patients it is revealed HER2positive status of tumor. It is lead chemotherapy to all patients by the PLF regimen and Herceptin in doze 6 mg/kg once in 3 weeks (6 cycles). In control group in 26 patients it is lead only chemotherapy by the PLF regimen once in 3 weeks without addition Trastuzumab (6 cycles). Results: At the moment of the analysis of preliminary data, the median remission duration in compared groups has made 8.3 months, and 5.2 months, accordingly. This research paper presents the results of ovarian reserve estimation for 125 women with the Polycystic Ovary Syndrome (PCOS) who have undergone various methods of surgical treatment resection of the ovaries, thermokauterisation and drilling by laser (HoYag). Ovarian reserve was estimated according to the amount of antral follicles, level of follitropin and Müllerian inhibiting substance (MIS), also named antiMüllerian hormone (AMH). Blood flow in ovarian tissue was also examined after various methods of surgical treatment. Nádory hlavy a krku jsou celosvětově 6. nejčastějším zhoubným nádorem. Histologicky se ve většině případů jedná o spinocelulární karcinomy s lokalizací dutiny ústní, orofaryngu, laryngu, hypofaryngu a dutiny nosní. Hlavním rizikovým faktorem je kouření a alkohol. Nově se diskutuje souvislost s infekcí lidského papilomaviru (HPV 16), zejména v oblasti orofaryngu. Pacienti mající infekci HPV 16 mají 15–50× vyšší riziko pro vznik karcinomu. Dle výsledků zkoumání se jedná o rozdílnou formu karcinomu s výskytem u mladších pacientů, lepšího sociálního stavu, nejčastěji v oblasti orofaryngu a báze jazyka, s pokročilým postižením uzlin, s lepší prognózou proti HPV negativním tumorům. Základem léčby je chirurgie a radioterapie, chemoterapie a nově cílená léčba. Ve většině případů (nad 50 %) se jedná o lokálně pokročilé onemocnění zasahující oblast polykání, mluvení – výrazně omezující denní aktivity. Chemoradioterapie s cisplatinou je standardem pro pacienty s pokročilým neresekabilním karcinomem hlavy a krku. Ve studiích fáze III byla srovnávána samostatná radioterapie proti přidání cetuximabu k radioterapii. Výsledky odpovědi pro kombinaci s cetuximabem 74 % vs. 64 %, s mediánem přežití 49 vs. 29,3 měs. (Bonner, 2006). Cetuximab je monoklonální protilátka třídy IgG1, blokující vazbu endogenních EGFR ligand, což vede k inhibici funkce receptoru. EGFR je exprimován u nádorů epiteliálního původu. Patients with the HPV 16 infection have a 15–50-fold higher risk of developing carcinoma. According to research results, it is a distinct form of carcinoma occurring in younger patients, of a higher social status, most frequently in the area of the oropharynx and the base of tongue, with advanced lymph involvement, and with a better prognosis compared to HPV-negative tumours. The mainstay of treatment is surgery and radiotherapy, chemotherapy, and, more recently, targeted therapy. In most cases (over 50%), there is locally advanced disease involving the area of swallowing and speaking, thus significantly limiting daily activities. Chemoradiotherapy with cisplatin is the standard treatment for patients with advanced unresectable head and neck cancer. Jedinou naději na dlouhodobé přežití pacientů s kolorektálním karcinomem metastazujícím do jater je radikální chirurgická resekce jaterních metastáz. Systémová léčba zvyšuje pravděpodobnost úspěšného chirurgického odstránění jaterních metastáz. Cílem systémové léčby je dosažení co nějvětší léčebné odpovědi. V současné době se využívá kombinace systémové chemoterapie s cílenou léčbou. Kazuistika prezentuje pacienta, u kterého po chemoterapii FOLFOX v kombinaci s cetuximabem došlo ke kompletní klinické remisi onemocnění. Popisujeme kazuistiku velmi vzácného benigního střádavého onemocnění s projevy ve skeletu, které imitovalo metastatické postižení při pokročilém karcinomu ledviny. I velmi nepravděpodobná koincidence vzácných onemocnění může být v klinické praxi realitou a může vést k chybné diagnostice. Gaucherova choroba je onemocnění probíhající v různých formách od dětské – s možnými závažnými projevy a poruchami vývoje po dospělou s projevy lehčími či klinicky nepatrnými. Diagnostika a léčba metabolických a onkologických nemocí v posledních letech prošly výrazným vývojem. Karcinom ledviny má bohužel v ČR nejvyšší incidenci na světě. Jedná se o chemorezistentní a radiorezistentní onemocnění. Časná stadia jsou vyléčitelná chirurgicky, pokročilá a metastatická stadia jsou přes přínos nové cílené léčby v posledních letech v naprosté většině případů nevyléčitelná., The article describes a case of a patient with a very rare benign storage disease appearing in skelet that was imitating a metastatic spread of an advanced renal cell carcinoma. Even a very unlikely case of coincidence of rare diseases can be real in the clinical practice. Such a situation can lead to an erroneous diagnostic workup. The Gaucher disease is a disease with various possible forms. Some as the juvenile form can be serious with severe developmental limitations- and some as the adult form can rather be with lighter symptoms or inapparent at all. Plicní karcinomy jsou z onkologických onemocnění nejčastější příčinou smrti, nemalobuněčný plicní karcinom je nejčastějším histologickým typem plicního tumoru. Gefitinib je prostředkem biologické léčby pokročilého a metastatického nemalobuněčného plicního karcinomu u pacientů s aktivační mutací receptoru pro epidermální růstový faktor v první linii léčby, tzn. u chemonaivních pacientů. Lokální chemoprofylaxe neinvazivních nádorů močového měchýře je používána více než 40 let. Využíváno je několik agens, mezi nejčastější patří mitomycin C (MMC) a epirubicin. Názor na léčebná schémata není jednotný. Avšak na podkladě EBM je zřejmé, že adjuvantní instilace cytotoxických látek do močového měchýře prodlužuje dobu beznádorového intervalu.
